1.Insulin resistance syndromes of monogenic mutations

Several insulin resistance syndromes, such as severe insulin resistance syndrome caused by mutations of insulin receptor gene and lipodystrophy caused by mutations of lamin gene, have been characterized clinically. The gene mutations, pathogenesis and clinical features of these monogenie diseases were reviewed in this article.
2.Research on the Application of Material Closed Loop Management in Medical Institutions Based on the Perspective of Management Accounting

Objective:Through the closed loop management mode,it realized the whole process of material information could be traced back,especially in the medical business level.Methods:Based on the requirement of management accounting,the integrated accounting and business information were in accordance with management accounting.Results and Conclusion:It set up different management nodes in the closed loop of the materials,such as medicine,sanitary materials and test reagents,so as to get the relevant information.The problems of the extensive management of materials could be solved to improve the efficiency of the management of hospital materials.
3.Research on the Application of Standard Cost in Blood Purification Institutions Based on Cost Behavior

Through collecting the economic operation information of the society-run blood purification institutionrs,the cost is divided into fixed cost,variable cost and mixed cost according to the relationship between the cost and the number of dialysis.It formulated the standard cost model of blood purification institutions,according to the county and city classification,the basic situation of the size and capacity of the organization were assumed.It mainly included standard labor costs,standard drug costs,standard health materials fees and standard professional equipment depreciation and other information,which was benefitial for industry decision-making and internal fine management.
4.Advance in mechanism research of the interaction between bone plant on osteoblast

The conformity between bone implant and bone tissue plays an important role in osseointegration.Protein molecules such as extracellular matrix proteins,cytoskeletal proteins,and attachment proteins,which produced by osteoblasts,promote the attachment of osteoblasts to bone implant and the surrounding ceils.In vitro studies show that nano-plated implant material bears excellent biocompatibility and superior physical and chemical features.Nano-scale material has the advantage of high surface roughness,wettability and electrochemistry features which erdaance the adherence and interaction of extracellular matrix proteins and promotes the attachmerit,proliferation and differentiation of osteoblasts.The expression of cytokines,such as prostaglandin E2(PGE2)and transforming growth factor-β(TGF-β)are also increased in nano-structured bone implants which in turn promote the new bone formation.Further studies need to be done to explain the mechanism of the effect of nano-tructured bone implant on osteoblasts.

[Summary] Various complications of type 2 diabetes mellitus (T2DM) are more harmful than T2DM itself.Cardiovascular diseases (CVD) are the major causes of diabetes-associated disability and mortality.Most studies have shown a close relationship between the lifestyles of T2DM patients and the development of CVD.Keeping a healthy lifestyle is essential in preventing and treating CVD for patients with T2DM.This article was tried to elucidate the roles of different components of diet,exercises,and sedentary behavior on diabetic CVD.

In recent years , active vitamin D is a hot research drugs because of its renal protective effect of independent regu -lation of anti-inflammatory effects outside calcium and phosphorus metabolism , regulation of apoptosis , mediated immunity and reduc-tion of proteinuria .Podocyte is the main target of active vitamin D based on the result of clinical and animal studies .In this article, we review the current literature on mechanism of active vitamin D and its analogues on the protection of podocytes about and give the clini -cal perspectives of activity vitamin D .
7.Study on the Relationship between Expressions of VEGF-C, VEGF-D and Metastasis in Papillary Carcinomas of Thyroid

Objective To explore the relationship between expressions of VEGF-C, VEGF-D and metastasis in papillary carcinoma of the thyroid. Methods Immunohistochemical method was used to detect the microvessel density(MVD) and expressions of VEGF-C and VEGF-D protein in 20 samples of nodular goiter and 115 samples with papillary carcinoma of the thyroid. Results The positive rates of VEGF-C and VEGF-D protein in nodular goiter were 15.0% and 20.0% respectively, and those in papillary carcinoma of the thyroid were 87.0% and 72.2%, respectively. The intratumour MVD in nodular goiter and papillary carcinoma of the thyroid was 15.2?6.7 and 21.5?8.5 respectively. The expressions of VEGF-C, VEGF-D protein and MVD in papillary carcinomas of the thyroid were significantly higher than those of nodular goiter. 8.Expression of p53,Cdk_4 and Survivin in gastric epithelial dysplasia with colonic and small intestinal metaplasia

Objective To evaluate the significance of p53,Cdk_4 and Survivin in gastric epithelial dysplasia with colonic and small intestinal metaplasia.Methods Immunohistochemical SP method was used to detect p53,Cdk_4,and Survivin in 44 cases of colonic intestinal metaplasia(27 low grade epithelial dysplasia,17 high grade epithelial dysplasia) and 51 cases of small intestinal metaplasia(42 low grade epithelial dysplasia,9 high grade epithelial dysplasia) by endoscope.Results The positive expression rates of p53 and Survivin had significant difference in two types of intestinal metaplasia and the difference was significant between low-grade and high-grade epithelial dysplasia.The expression of CDK_4 had significant difference in two types of intestinal metaplasia and had not difference between low-grade and high-grade epithelial dysplasia.Conclusion The expressions of p53,CdK_4 and Survivin are closely associated not only with the types of intestinal metaplasia but also with the different grades of epithelial dysplasia.The expression of p53,Cdk_4,and Survivin may be valuable markers in assessing the biological behavior of gastric epithelial dysplasia with intestinal metaplasia.
9.Cervical malignant tumors with lymph node enlargement:A clinicopathologic analysis of 887 cases

Objective To investigate clinicopathologic characteristics of malignant tumors initiated with cervical lymph nodes enlargement.Methods Clinical and pathological data of 887 cases with cervical malignant tumors kept in the college during January 1998 to December 2003 were analyzed for their histological patterns and primary site of metastatic carcinoma.Results Cervical cancer with lympb nodes metastasis was mostly seen at age of 51~60 years (182/837,21.74%), and much more common in men than in women. Major metastatic carcinomas in cervical lymph nodes originated from the nasopharynex, lung, thyroid, stomach and esophagus (591/671,88.08%), with 60.66%(407/671)of squamous cell carcinoma mostly from the nasopharynex, lung, or esophagus (380/407,93.37%). There was significant difference in the site, size, number, quality, appearance of lymph nodes, course of disease, and clinical manifestations between lymphoma and metastatic cancer.Conclusions Cervical metastatic carcinomas in Guangdong mainly originated from the nasopharynx. Some specific immunohistochemical markers, CT, MRI and ultrasonic technology played a very important role in diagnosis for lymphoma and determination of primary site of metastatic cancer.
10.Application of gastric tube insertion by left recumbent position for patients with exanimation

Objective To explore the practicability and clinical significance of gastric tube insertion by left recumbent position for patients with exanimation. Methods We divided 100 patients with exanimation who needed gastric tube insertion into the routien group and the left-recumbent-position group with 50 cases in each group randomly and adopted routine insertion method and insertion by left recumbent position.The success rate of tube insertion and incidence of complications were observed in the two groups.Results The one-time success rate of insertion was higher while the incidence of complication was lower in the left-recumbent-position group compared with the control group (P<0.05). Conclusion Gastric tube insertion by left recumbent position proved to be a desirable method in that it could increase the onetime success rate of insertion and decrease the complications.So it worth widely applying.
