1.Application of Coronary Artery Disease-Reporting and Data System in risk factor analysis of patients with coronary artery disease

Objective:To investigate the application of Coronary Artery Disease-Reporting and Data System (CAD-RADS) in the diagnosis of coronary atherosclerotic heart disease(CAD) and its risk factors,and to clarify the effective strength of different risk factors in the diagnosis of CAD by using CAD-RADS.Methods:All the data of 266 patients,who were initially suspected with CAD and underwent CT angiography,were collected and diagnosed by using CAD-RADS and were divided into CAD group(n=174) and non-CAD group(n=69).The informations of age,gender,hypertension,hyperlipemia,diabetes,smoking,serum uric acid (UA) levels,and plasma fibrinogen (FIB) levels of the patients in two groups were analyzed;single factor analysis and multivariate Logstic regression analysis were used to analyze the risk factors of CAD diagnosed by CAD-RADS.Results:Compared with non-CAD group,the ratios of male,hypertension,diabetes,smoking of the patients in CAD group were increased (P<0.05),and the age and the level of UA of the patients in CAD group were also increased (P<0.05).The Logistic regression analysis results showed that age and diabetes were the independent risk factors for CAD diagnosed by CAD-RADS.Conclusion:There are many independent risk factors for CAD diagnosed by CAD-RADS,and age and diabetes are the most correlated risk factors for CAD.
2.Clinic features and echocardiographic evaluation of patients with idopathic submitral left ventricular aneurysm

Objective To study cinical features of patients with idopathic submitral left ventricular aneurysm(ISLVA) and evaluate their global and segmental systolic function as well as diastolic function through echocardiography.Methods Clinical features of eight patients with ISLVA were analysed retrospectively.Standard 2-dimentional and real-time 3-dimentional echocardiography were performed in all the eight cases and other twenty subjects with normal left ventricular(LV) function (defined as control group).Results Two patients were diagnosed as ISLVA due to ventricular arrythmia and the other six case sowing to congestive heart failure.Coronary angiography was normal in all patients.Apical systolic murmur was audible in 6 cases.Four patients received mitral valve surgical repair,one underwent aneurysm resection and radiofrequency ablation,implantable cardioverter defibrillator was implanted in one case,one was only treated by medication.One case died.Patients with ISLVA demonstrated significanly larger left atrium(LA) and LV diameter (both end-diastolic and end systolic),thinner LV posterior wall,and lower LV ejection fraction (LVEF) than controls (P <0.05).Indexes of the LV 17 segments time-volume curves including the time to minimal systolic volume(Tmsv) 16-SD,Tmsv 12 SD,Tmsv 6-SD,Tmsv 16-Dif,Tmsv 12-Dif,Tmsv 6-Dif,Tmsv 16-SD%,Tmsv 12-SD%,Tmsv 6-SD%,Tmsv 16-Dif%,Tmsv 12-Dif%,Tmsv 6-Dif% were significantly higher in patients with ISLVA than those in controls(all P <0.05).All patients with ISLVA showed mitral regurgitation and decreased LV diastolic function in varying degrees,five patients accompanied by elevated LA pressure.Conclusions Clinical features of patients with ISLVA are nonspecific.Echocardiography can evaluate systematically their functional and structural abnormalities.

4.Diffusion tensor imaging map of anterior cruciate ligament contrasted with MRI in healthy adults.

OBJECTIVE:
To measure anterior cruciate ligament (ACL) by diffusion tensor imaging (DTI), combined with ACL half quantitative measurement magnetic resonance imaging (MRI) method as the contrast, and to preliminarily investigate the feasibility of DTI for ACL.
METHODS:
The ACLs of 31 healthy volunteers were scanned with ordinary MRI and DTI. At ordinary MRI map, sagittal ACL-tibial angle, coronal ACL-tibial angle, Blumensaat line-ACL angle, angle of inclination of the intercondylar roof, and ACL-tibial insertion site were measured. As for DTI analysis of ACL, ACL was divide to 5 portions, namely 1(st), 2(nd), 3(rd), 4(th), 5(th), and all fractional anisotropy (FA) and apparent diffusion coefficient (ADC) values of every portion were recorded and repeated.
RESULTS:
The sagittal ACL-tibial angle, coronal ACL-tibial angle, Blumensaat line-ACL angle, angle of inclination of the intercondylar roof, and ACL-tibial insertion site were 53.00°±2.46°, 52.42°±5.15°, 12.67°±5.71°, 39.41°±2.64°, (24.90±2.84)%, respectively. FA values of different portions were 0.611±0.042, 0. 618±0.051, 0.596±0.045, 0.566±0.059, and 0.497±0.072. ADC values of 1(st)-5(th) portion were (1.068±0.216), (1.128±0.268), (1.189±0.197), (1.455±0.423), and (1.779±0.384)× 10(-3) mm(2)/s. The correlation coefficient of sagittal ACL-tibial angle and the FA value of 2(nd) and 3(rd) portion was -0.568 and -0.429. The correlation coefficient of Blumensaat line-ACL angle and the FA value of 1(st) and 4(th) portion was -0.507 and -0.633. The correlation coefficient of ACL-tibial insertion site and the FA value of 4(th) portion was -0.593, all with statistical significance. FA and ADC values of all portions in both team's ACL didn't have significant difference (P>0.05), but had obvious correlation.
CONCLUSION
DTI can be used to effectively evaluate the orientation and connection of ACL, having good contrast virtue with ACL half quantitative MRI measurement. It may provide more profound ACL information for clinicians, and it is of great significance for the further research and large sample data base of ACL pathology.

5.Comparison of diagnostic values between magnetic susceptibility weighted imaging and routine MRI in patients with cerebral amyloid angiopathy-related hemorrhage

Objective:To analyze the imaging features of magnetic susceptibility-weighted imaging (SWI) in the patients with cerebral amyloid angiopathy-related hemorrhage (CAAH),and to clarify the diagnostic value of SWI for CAAH.Methods:A total of 68 patients presumptively diagnosed as CAAH were collected,and their imaging data of routine MRI and SWI were collected and analyzed.The detection rates of hemorrhage focus of the CAAH patients were compared between two kinds of imaging examination.The consistency of detection of CAAH by routine MRI and SWI was analyzed.The imaging features of SWI and the risk of focus hemorrhage in the patients with CAAH were analyzed by multivariate Logistic regression analysis.Results:Sixty-one patients were confirmed as CAAH by pathologic diagnosis,and 53 patients were confirmed as CAAH by routine MRI;the detection rate was 86.89%;59 cases of hemorrhage focus were confirmed by SWI and the detection rate was 96.72%.The number of lesions detected by SWI was more than that of routine MRI (P<0.05).The consistency of detection of CAAH by routine MRI and SWI was poor,and the value of Kappa was 0.3666.The patchy high signal and multiple clear edge low signal area were the relative imaging features of CAAH with SWI in the patients with CAAH analyzed by multivariate Logistic regression analysis (OR=3.895,P=0.025;OR=3.124,P=0.029).Conclusion:SWI can efficiently detect the hemorrhage focus in the patients with CAAH and the diagnostic value is better than routine MRI.
6.Experimental study on antitumor effects of extracts from Actinidia argutaor.

OBJECTIVETo observe the inhibitory effect of extract from Actinidia argutaor on in vivo and in vitro carcinomata, and explore its mechanism.
METHODThe in vivo S180 and H22 model were used to observe the effect of A. argutaor on inhibitory rate of carcinomata, organ relative weight of spleen and thymus gland and the release of tumor necrosis factor. A549 cells were exposed to extract from A. argutaor with different concentrations for 24, 48, 72 hours. MTT assay was used to evaluate the inhibiting effects of extract from A. argutaor on the proliferation of the cells. Flow cytometry was applied to detect cell cycle.
RESULTThe inhibitory effects of the extracts on in vivo and in vitro carcinomata were observed, the inhibitory rate for S180 and H22 line was 33.32% and 34.62% respectively. The extracts could inhibit the proliferation of A549 cells during G0-G1 period and significantly decrease the cell ratio of S stage.
CONCLUSIONThe extracts from A. argutaor showed a good antineoplastic activity.

7.Effect of different compatibility of Wuji Wan on bile excretion of representative ingredient.

OBJECTIVETo research the representative ingredient excretion in bile using the different compatibilities of Wuji Wan, in order to indicate the regularity of compatibility.
METHODL9 (3(4)) orthogonal design table was used, in addition 9 simples, altogether 18 compatibilities. After duodenal administration bile at different time spot was collected for LC-MS detection.
RESULTThe excretion of Evodiae Fructus was negative correlated with that of berberine and palmatine in bile. The excretion of Paeoniae Radix Alba was positive correlated with that of berberine and palmatine in bile. The excretion of Coptidis Rhizoma, Paeoniae Radix Alba was negative correlated with that of evodiamine, rutaecarpine; meanwhile, the most superior proportion was also caculated which promote the representative ingredient excrete from bile.
CONCLUSIONEvodiae fructus suppresses representative ingredients of Coptidis Rhizoma excrete through bile. Paeoniae Radix Alba promote suppresses representative ingredients of Coptidis Rhizoma excrete through bile. Coptidis Rhizoma, paeoniae Radix Alba suppresses representative ingredients of Evodiae Fructus excrete through bile. Coptidis Rhizoma, Evodiae Fructus suppresses representative ingredients of Paeoniae Radix Alba excrete through bile.

OBJECTIVETo observe the impact on absorption of berberine and palmatine in different compatibilities of Wuji pill by the perfused rat intestine-liver preparation.
METHODUse L9 (3(4)) orthogonal design table, establish the perfused rat intestine-liver preparation, the twelve Wuji pill compatibilities duodenal administrated, collect the perfusate at different times points for LC-MS detection, calculate the absorbed score, Ka.
RESULTEvodiae Fructus and the absorption score, Ka of berberine and palmatine are inverse correlated. The most superior portion which promote the absorption is Coptidis Rhizoma-Evodiae Fructus-Paeoniae Radix Alba 3:1:3.
CONCLUSIONEvodiae Fructus suppressed the absorption of berberine and palmatine. With the different portion the absorption also have big different.

9.Influence of jinlingzi powder with different compatibility on activity of cytochrome P1 A2 from rat liver microsomes.

To illustrate the compability rule of Jinlingizi powder, by investigating the effects of Jinlingzi Powder with different compatibility on the enzymatic activity of cytochrome P1 A2 (CYP1A2) from rat liver microsome. The different compability of Jinlingizi powder is designed, based on the orthogonal array L9 (3(4)). In vitro test, rat liver microsomes incubation system is applied to detect the 50% inhibitory concentraton of Jinlingzi powder with different compatibility to cytochrome P1A2 (CYP1A2) enzyme. In vivo experiments, rats is treated orally with the different compability of Jinlingizi powder for 5 days, then be injected with probe drug phenacetin. The biosample from liver tissue is obtained by microdialysis probe, then analysisd by HPLC. The concentration-time data are modulated by software WinNonlin. IC50 data show no significant inhibitory activty to cytochrome P1 A2. Acetaminophen and phenacetin PK parameters indicate that the different compability of Jinlingizi powder can modulate the CYP 1A2 mediated metabolism, which is associate with the compatibility of Jinlingzi powder.

10.Effect of PI3Kδ inhibitor CAL-101 on myeloma cell lines and preliminary study of synergistic effects with other new drugs.

OBJECTIVETo investigate the proliferation inhibitory role and mechanism of PI3Kδ inhibitor CAL-101 on multiple myeloma (MM) cells, and to provide new therapeutic options for MM treatment.
METHODSMM cell lines U266 and RPMI8226 cells were treated with various concentrations of CAL-101. MTT assay and CalcuSyn software were performed to determine the inhibitory effect of CAL-101 and the synergistic effect with PCI- 32765, SAHA (suberoylanilide hydroxamic acid), BTZ (Bortezomib) on MM cells. The protein expression level of p-AKT, p-ERK, AKT, ERK and PI3Kδ processed by CAL-101 were analyzed by Western blot.
RESULTSCAL-101 at concentration of 15, 20, 25, 30 and 40 μmol/L could induce significant dose-dependent proliferation inhibition on U266 cells after treatment for 48 hours. The cell proliferation inhibition rates were (33.54 ± 1.23)%, (41.72 ± 1.78)%, (53.67 ± 2.01)%, (68.97 ± 2.11)% and (79.25 ± 1.92)%, respectively. Similar results were found in RPMI8226 cell line. Western blots showed high expression level of p-AKT, p-ERK, AKT, ERK and PI3Kδ in cell lines and MM primary cells. p-AKT and p-ERK protein expression levels were down-regulated significantly by CAL-101 treatment. Synergistic effect has been verified between CAL-101 and PCI-32765, SAHA and Bortezomib in U266 cell line, and PCI-32765, Bortezomib in RPMI8226 cell line with CI values less than 1.
CONCLUSIONCAL-101 could inhibit proliferation of MM cell lines. High levels of p-AKT, p-ERK, AKT, ERK and PI3Kδ protein expression were observed in both cell lines and primary cells. Down-regulation of p-AKT and p-ERK probably related with the mechanism of CAL-101 in MM cell proliferation inhibition. CAL-101 has significant synergistic effect with PCI-32765, SAHA and BTZ.
