METHODS:The status quo of prescription-checking pharmacists in China was analyzed and some concrete suggestions on how to cultivate qualified prescription-checking pharmacists were made.RESULTS:The phenomena of low educational level and low professional title,lack of professional knowledge and medical knowledge,lack of staff members were widespread in today's Chinese prescription-checking pharmacist professionals.CONCLUSION:The rational drug use can only be realized through being fully aware of the importance of prescription-checking pharmacists,enhancing whose admittance qualifications,working out professional standards for whom,clearly defining their responsibilities,strengthening their cultivation and increasing their numbers.
3.Anesthetic Management of Non-cardiac Surgery In Patients With Cardiac Disease

Cases of using vasoactive drug in group E (69.23%) were more than group C (56%) and group G (47.36%.).Conclusions It is important for patients with severe cardiac disease to maintain organs oxygen supply during non-cardiac surgery. Steady hemodynamic might be gainned by using vasoactive drugs carefully.

BACKGROUND: Soluble dispersive mixture of domestic chicken collagen type Ⅱ (SmCC Ⅱ ) has been proven to be safe and effective for rheumatoid arthritis (RA) treatment while there are some similar articular cartilage degradation and autoimmune pathogenesis between osteoarthritis (OA) and RA, so it is worth studying whether SmCC Ⅱ is effective for the precaution or treatment of OA.OBJECTIVE: To observe the prophylactic and therapeutic effects of SmCC Ⅱ on rat OA and analyze the concomitant matrix metalloproteinase (MMPs) and tissue protease changes in osteoarthritic rats.DESIGN: Randomized and controlled observation.SETTING: Department of Clinical Pharmacy, Sixth People's Hospital, Shanghai Jiao Tong University.MATERIALS: Totally 258 rats of clean grade and either gender, aged 6 weeks were involved in this experiment. SmCC Ⅱ (white powder, Batch No.00031004) was provided by Professor Ren Geng-Fu from Shanghai Engineering Institute of Herbal Biomedicine. The 20 mg/L and 80 mg/L solution of SmCC Ⅱ effective component were prepared before use.While 0.25% excipient (mannitol, product of Jiangsu Tianyuan Medical Co., Ltd) solution was prepared by 200 g/L mannitol dissolved in sterile saline solution.METHODS: The study was carried out in the Whole Animal Laboratory and Experimental Center, Sixth People's Hospital,Shanghai Jiao Tong University between March 2003 and February 2006. ①Prophylactic study: Totally 132 rats were randomized into 5 groups: OA group (n =36): treated with sterile saline solution orally by a 16G syringe, 1mL/d; Low- and high-dose SmCC Ⅱ groups (n =24, respectively): treated with 20 mg/L and 80 mg/L SmCC Ⅱ solution orally, 1 mL/d;Excipient group (n =24): treated with 2.5g/L mannitol, 1 mL/d. OA models were surgically induced in these 4 groups by Hilth's method; Normal group (n =24): Rats without operation were treated with sterile saline solution orally, 1 mL/d. All the rats were administrated on the day of modeling. ②Therapeutic study: Totally 126 rats were randomized into 5 groups, grouping, administration and rat number in different groups were the same as those in prophylactic study,respectively, except for the n =30 in OA group. In addition, all the rats were adminstrated for 8 weeks since 6 weeks after operation. The knee joints of right hind limb of all the rats were taken off after 1-week treatment. And sample was cut open along coronal incision for use. ③Morphological study of articular cartilage was conducted by HE staining and Mankin score was calculated to evaluate the severity of arthritis, immunohistochemical studies of matrix metalloproteinase (MMP)-13, MMP-9 and cathepsin K (Cath K) were carried out by ABC method in situ and the percentage of positive-stained chondrocytes were calculated while the mRNA level for MMP-13, MMP-9, Cath K and the tissue inhibitor of metalloproteinase 1 (TIMP1) were determined by semi-quantitative reverse transcription-polymerase chain reaction (RT-PCR) method.MAIN OUTCOME MEASURES: ①The morphological changes of articular cartilage in different groups in prophylactic or therapeutic study.②The level of MMP-13, MMP-9, Cath K and their corresponding mRNA levels.RESULTS: All the 258 rats were involved in the final analysis. ①Prophylactic study: Apparent degeneration appeared in the rats of OA group and the mankin score in OA group was higher than that in high- or low- SmCC Ⅱ groups [(6.44±0.81), (2.75±0.85), (2.70±0.81) points respectively, P < 0.05], the mRNA and positive-stained chondrocyte percentage of MMP-13, MMP-9 and Cath K in OA group was higher than that in high- or low-dose SmCC Ⅱ group, respectively (P <0.05-0.01) while the TIMP1mRNA level in OA group was not significantly higher than that in high or low SmCC Ⅱ group (P> 0.05). 6.Technical improvements in radical retropubic prostatectomy (report of 32 cases)

Objective To evaluate the effect of technical improvement of radical retropubic prostatectomy (RRP) for patients with localized prostatic cancer. Methods RRP was performed in 32 cases (mean age,68 years) of prostatic cancer at clinical T 1 and T 2 stages.Technical improvements included extensive lymph node clearance,protection of the neurovascular bundle,suture and ligation of the dorsal vein complex,preservation of the distal prostatic urethra,and intussusception of the bladder neck. Results The mean operative time was 3.5 h.The mean blood loss was 450 ml;blood transfusion was conducted in 17 cases.Postoperative pathologic findings revealed tumor confined disease in 30 cases,positive surgical margin in 1, and lymph node metastasis in 1.All the patients survived and were followed up for a mean of 22 months (range 8 to 48 months),with PSA

Objective To investigate the influence of electrocardiogram(ECG)-controlled dose modulation on exposure dosage and image quality of cardiac CT imaging in a cardiac phantom with simulated fluctuant heart rate.Methods The basal heart rate of the cardiac pulsating phantom was set as 60 bpm.the experimental situations were divided as 6 groups according to different heart rates.The cardiac imaging was performed on the cardiac phantom when the ECG-controlled dose modulation was firstly turned off.The exposure dosage of each scan sequence was documented.The standard deviation of the CT values of the phantom was measured on the central slice after coronal reformation of the raw data.The quality of 2D and 3D images were scored.Thell cardiac imaging was performed when ECG modulation was on and set as four groups according to different modulation parameters.All the data were documented as before.The results from the five groups with and without ECG modulation current were analyzed bv F test and comparative rank sum test using the statistical software SPSS 10.0.Results Statistical analysis showed no significant difference(P>0.05)between the SNR of images(SD value was 27.78 and 26.30)from the groups that full mA output at wide reconstruction phase(69%~99%)when the heart rate was fluctuant(≥7.5 bpm).There was also no significant difference(P>0.05)between the quality of the 2D and 3D images.But there was a significant difference(P<0.01)between the SNR of images(SD value was 26.78 and 29.90)that full mA was only used at 85%reconstruction phase when the heart rate Was fluctuant(≥7.5 bpm).The exposure dosage was remarkably reduced when the ECG modulated current was on than when it Was off under fluctuant heart rate.Furthermore.there were significant difierence(P<0.01)among the difierent ECG modulated current parameter groups.The exposure dosage can be reduced by 44.7%under the situation that the heart rate was fluctuant.Whell the fluctuation of the heart rate was≤12.5 bpm,there wag no obvious relationship between the fluctuation of the heart rate and the exposure dosage (the variation was from 0.1 to 1.1 mSv),but if the heart rate fluctuation was>12.5 bpm,the exposure dosage would increase obviously (from 0.6 to 1.7 mSv).Conclusion For cardiac imaging with 16-slice row CT,the application of ECG modulated current can effectively reduce the exposure dosage without compromising the image quality even if heart rate was fluctuant.
8.Functional MR imaging of kidneys in patients with lupus nephritis

Objective To evaluate the functional magnetic resonance (MR) imaging in the assessment of renal involvement and pathological changes in patients with lupus nephritis (LN).Methods Seventeen patients with LN and 10 healthy controls underwent coronal echo-planar diffusion-weighted (DW) MR imaging and blood oxygen level dependent (BOLD) MR imaging of the kidneys with a single breath-hold time of 16 s.The apparent diffusion coefficient (ADC) and R2* value of the kidneys were calculated with high b values (b=500 s/mm2).The correlation between the renal injury variables and the ADCs or R2* values was evaluated.Results The mean ADC value of kidneys in patients with LN was (2.43+0.24)×10-3 mm2/s,the mean R2* values of the renal cortex and medulla were (11.72+2.35)/s and (13.07+2.35)/s respectively,which were all significantly lower than those in volunteers (P=0.045,P=0.048and P=0.001,respectively).In the patients with LN,the mean ADC values were positively correlated with estimated glomerular filtration rate (eGFR) (r=0.558,P<0.05).There was a negative correlation between the ADC values of the right kidneys and pathological chronic indexes (r=-0.493,P<0.05).Moreover,the R2*values of the renal medulla were negatively correlated with 24 hours proteinuria,serum creatinine,pathological active indexes.The patients were assigned to group A (class Ⅲ,Ⅳ,Ⅴ,n=8) and group B class Ⅴ + Ⅲ and Ⅴ + Ⅳ,n=9).The tubulointerstitial lesions in group B were more severe than those in group A,while the mean ADC values and R2* values of the renal cortex in group B were lower as compared to group A.Conclusion DW MR imaging and BOLD MR imaging may be used to non-invasively monitor the disease activity and evaluate the efficacy in lupus nephritis.
9.Clinical Observation of Acupuncture plus Medicine and Function Training for Post-stroke Shoulder-hand Syndrome

Objective To observe the clinical efficacy of acupuncture plus medicine and function training in treating post-stroke shoulder-hand syndrome. Method Sixty patients with post-stroke shoulder-hand syndrome were randomized into a treatment group of 30 cases and a control group of 30 cases. The treatment group was intervened by acupuncture plus medicine and functional training, while the control group was only by functional training. Before and after intervention, the hand swelling degree, Visual Analogue Scale (VAS), shoulder joint motion scale, Fugl-Meyer Assessment (FMA) scale were used to measure the swelling and pain of hand, motion of shoulder, and motor function of upper limb. Result After intervention, the swelling and pain of hand, motion range of shoulder and motor function of upper limb were significantly improved in both groups (P<0.01), and the improvements in the treatment group were more significant than that in the control group (P<0.05). Conclusion Acupuncture plus medicine and function training can enhance the efficacy in treating post-stroke shoulder-hand syndrome, superior to pure function training.
10.Implication of plasma pentraxin 3 in peripheral arterial disease of hemodialysis patients

Objective To clarify the role of pentraxin 3 (PTX3) in the development of peripheral arterial disease (PAD) in maintenance hemodialysis (MHD) patients. Methods One hundred and sixteen patients undergone MHD therapy in our center for more than 3 months were enrolled in the study. Clinical data were collected for analysis. Ankle-brachial index (ABI) was used to estimate the presence of PAD. Patients were divided into PAD group (ABI<0.9) and nonestimate the association of PAD with PTX3 as well as other potential risk factors. Results The incidence of PAD was 18.1% (21/116). Plasma level of PTX3 was significantly higher in PAD patients than that in non-PAD patients [(5.55 ±2.63) μg/L vs (2.32 ±1.29)μg/L, P<0.01].Univariate analysis showed ABI values were negatively correlated with plasma PTX3 levels (r =-0.548, P<0.01), high-sensitivity C-reactive protein (hsCRP), age, blood glucose and triglyceride. ROC curve of PAD revealed that area under curve (AUC) of PTX3 was 0.901 (P<0.01). With the cut-off value of PTX3 as 4.06 μg/L, the diagnostic sensitivity and specificity in PAD were 81.0% and 91.5%. ROC curve of PAD showed that AUC of hsCRP was 0.640 (P<0.05). With the cut-off value of hsCRP as 3.33 mg/L, the diagnostic sensitivity and specificity in PAD were 57.1% and 56.8%. Using Logistic regression, plasma PTX3 was found to be associated with PAD (0R=9.755, 95%CI:2.359-19.354, P=0.001). Conclusions The PAD incidence of MHD patients in our center is 18.1%. Plasma PTX3 level is significantly correlated with the presence of PAD in MHD patients. The sensitivity and specificity of PTX3 are higher than those of hsCRP for PAD diagnosis.
