1.Comparative study of the effect in the therapy of Hepatic Cysts by image-guided percutaneous catheter dralnage and sclerotherapy

Objective To compare the image guided effect between CT and Ultrasound for percutaneous catheter drainage and sclerotherapy in the treatment of hepatic cysts.Methods 26 patients with hepatic cysts underwent percutaneous catheter drainage and sclerotherapy by CT guided.87 cases underwent percutaneous catheter drainage and sclerotherapy by Ultrasound guided.Results 53 cysts underwent percutaneous puncture with 132 times by CT guided,and 5 cysts underwent sclerotherapy were failed(10.1%).231 cysts underwent percutaneous puncture with 267 times by Ultrasound guided,and 5 cysts underwent sclerotherapy were failed(2.1%).The failed rate of the two groups had statistifically significant difference(x2 =39.6875,P < 0.05).Conclusion In the image guided technique for percutaneous catheter drainage and sclerotherapy in the treatment of hepatic cysts,Ultrasound had more advantages,high accuracy,real time visual,high success rate,less damage,economy and convenience than CT.
2.Primary renal lymphoma of mucosa-associated lymphoid tissue: report of 2 cases and review of literatures

Background and purpose: Primary renal marginal zone B-cell lymphomas of mucosa-associated lymphoid tissue(MALT lymphoma) are rare tumors in the kidney.Till now,less than 50 cases have been reported all over the world.This article introduced 2 cases of MALT lymphoma based on the morphological and immunophenotype features,aimed to highlight this rare tumor in terms of clinical characteristics and pathology.Methods:2 cases of renal MALT lymphomas were treated in our hospital.Clinical history were collected,morphological analysis was evaluated based on the HE section,and immunohistochemistry were performed by CD20,CD79a,CD5,CD10,CD43,CD23,BCL10 and cyclinD1 antibodies.Results:2 female patients with the age of 48 and 55,respectively,both of them had a history of chronic pyelonephritis.Under the B ultrasonic and CT scanning,a lump in the kidney was found and whole nephrectomy was performed.In the macroscopic view,tumors were in the renal medulla with dark red color and ill-defined boundary.In the microscopic view,lymphoid cells infiltrate from the pelvis to the medulla of kidney,lymphoid cells mainly consisted of small lymphocytes,centrocyte-like cells,lymphoplasmacytoid and plasma cells.Reactive follicles and lymphoepithelial lesions also could be seen in the tumor.Immunohistochemistry showed that the tumor cells were CD20 and CD79a positive,CD43 was weak positive,but CD5,CD10,CD23,BCL10 and cyclinD1 were all negative.Conclusions:Primary renal MALT lymphomas are very rare diseases.According to the clinical manifestation,it is hard to differentiate from renal cell carcinoma.But the morphological features were consistent with the classic MALT lymphomas in other sites.Immunophenotype profiles were helpful for diagnosis.
3.The effect of bed exercise for patients with intracranial aneurysm undergoing neural interventional treatment

Objective To approach the effect of bed exercise for patients with intracranial aneurysm undergoing neural interventional treatment. Methods Ninety-six patients with intracranial aneurysm were divided into the test group(50 cases) and control group(46 cases).The control group received routine nursing, while the test group carried out bed exercise base on routine nursing. The incidence of complica-tions and changes of life quality were compared between the 2 groups. Results Lower incidence of com-plications took place in the test group than the control group(P < 0.05).But changes in pulse rate and blood were not significant (P > 0.05).The score of life quality in the physical domain and psychological domainwere not statistically different between the two groups (P < 0.01 ),but no evident difference was seen in the domain of social relationship and environmental domain (P > 0.05). Conclusions Bed exercise could al-leviated the anxiety and trepidation of patients with intracranial aneurysm,decrease the incidence of compli-cations such as body pain caused by absolute bed rest,thus improve patients' life quality.
4.The value of MCV,MCH in the screening of thalassemia

Objective To evaluate the value of the tests of mean corpuscular volume (MCV ) ,mean corpuscular hemoglobin (MCH) in the screening of thalassemia .Methods 603 cases were performed MCV and MCH tests and gene detection of thalasse-mia .The results of gene detection were used as reference standard .The sensitivity ,specificity ,accuracy ,positive predictive value and negative predictive value of single or combined results of MCV ,MCH tests were calculated .Results The sensitivity and specificity of single MCV test were 91 .2% and 76 .1% ,single MCH test were 91 .2% and 70 .7% .The sensitivity and specificity of parallel combined tests of MCV and MCH were 94 .8% and 61 .4% ,serial combined tests of MCV and MCH were 88 .4% and 75 .6% .Con-clusion The sensitivity of single MCV or MCH test in the screening of thalassemia is high .Parallel combined tests of MCV and MCH can improve the sensitivity ,and will be widely used in the in the screening of thalassemia .
5.Protective effect of COPP on hypoxia/reoxygenation injury of H9c2 myocytes

Aim To study the effect and molecular mechanism of pretreatment with COPP on hypoxia/reoxygenation injury of H9c2 myocytes.Methods H9c2 myocytes model of hypoxia/reoxygenation injury was established and H9c2 myocytes were given COPP pretreatment before hypoxia/reoxygenation.Treatment with Znpp and all-trans retinoic acid(ATRA)inhibited HO-1 and Nrf2-ARE respectively.The level of LDH and CK in cell supernatants were measured.HO-1mRNA expression was analyzed by RT-PCR.HO-1 and Nrf2 protein expressions were analyzed by Western blot.Results Compared with hypoxia/reoxygenation group,the level of LDH and CK in COPP pretreatment groups decreased significantly and the level of HO-1mRNA,HO-1 protein expression and Nrf2 protein expression in the nucleus significantly increased.Znpp abolished protective effect of COPP pretreatment.ATRA blocked the nuclear accumulation of Nrf2 and decreased HO-1 protein expression that COPP pretreatment induced.Conclusions COPP can induce HO-1 overexpression which has protective effect on hypoxia/reoxygenation injury of H9c2 myocytes.Its mechanism is related to Nrf2-ARE signaling pathway.
6.Treatment of lateral tibial plateau fractures involving posterolateral condyle with double plate fixation via combined anterolateral and posterolateral approaches

Objective To evaluate the treatment of lateral tibial plateau fractures involving the posterolateral condyle with double plate fixation via the combined anterolateral and posterolateral approaches.Methods From January 2013 to August 2015,12 patients with lateral tibial plateau fracture involving the posterolateral condyle were treated by double plate fixation via the combined anterolateral and posterolateral approaches.They were 8 males and 4 females,with an average age of 36.9 years (range,from 23 to 58 years).According to the Schatzker classification,we had 8 ones of type Ⅱ,3 ones of type Ⅴ and one of type Ⅵ.Firstly,the posterolateral condylar fractures were stabilized temporarily by the Kirschner wire after reduction through the posterolateral approach.Secondly,the lateral tibial plateau fractures were dealt with via the anterolateral approach,and fixated with a proximal tibial locking plate.Finally,a T-shaped plate for the distal radius was used to support the posterolateral condyle.The clinical and radiographic results were evaluated using the Hospital for Special Surgery (HSS) knee score and the Rasmussen Score,respectively.Results The average operative time was 143.3 min (range,from 110 to 210 min);the average intraoperative blood loss was 190.0 mL (range,from 100 to 300 mL).The follow-ups for the 12 cases ranged from 12 to 24 months (average,16.6 months).All the cases obtained clinical healing after 12 to 24 weeks (average,16 weeks).At the last follow-up,the HSS knee-scores ranged from 65 to 98 points (average,84.6 points),giving 7 excellent cases,3 good ones and 2 fair ones.The Rasmussen radiological scores ranged from 13 to 18 points (average,17.1 points),giving 8 excellent and 4 good cases.Conclusions The combined anterolateral and posterolateral approaches can offer excellent exposure for articular reduction and fixation with double plates,achieving satisfactory radiological and functional results in lateral tibial plateau fractures involving the posterolateral condyle.First reduction of the posterior lateral condylar fractures,followed by reduction and fixation of the anterolateral plateau fractures and finally fixation of the lateral condylar fractures,can lead to effectively improved knee function for the patients.
7.The occurrence of traumatic cavotid carernous fistla after fracture of basilaris cranii and its associated factors with outcome

Objective To analyze the occurrence of traumatic carotid cavemons fistula (TCCF) resulted from the fracture of basilaris cranii, in order to find out the related factors to outcomes and to discuss the approaches to improving prognosis.Method Data of 312 patients with the fracture of skull base complicatcd with TCCF con-firmed angiography from 1999 to 2005 were analyzed. These patients were classified into patients with disable and patients without disabed. The factors potentially impacting on outcomes were analyzed. Results The overall inci-dence of TCCF in 312 patients with fracture of basilaris cranii was 3.8% .The incideucs of TCCF occurred in pa-tients with the fracture of anterior fossa, middle fossa and posterior fossa accounted for 2.4%, 8.3 % and 1.7 %, respectively. Between two cohorts of patients, there were no difference in age, gender, number of embelization proce-dares performed and the time from injury to appearence of the first symptom except the differencc in time from ap-pearence of the first symptom to the intravascular embohzation performed (P>0.05). Conclusions A relatively high incidence of TCCF occurs in patients with middle fossa fractures, especially those with transverse or oblique fractures. Prompt diagnosis and intervention can not be emphasized in case of patients with TCCF, and non inva-sive techniques for the early detection of TCCF under certain circumstance after brain or facial trauma should be considered so as to avoid a miss in the early diagnosis of middle fossa fracture to ensure favourable outcomes.

Expressions of interleukin-6 (IL-6) and its receptor (gp80、gp130) in 8 patients with Graves' disease (GD) were compared with those in 8 euthyroid patients with nodular goiter or benign thyroid adenoma. The thyroid tissues of GD expressed significantly higher IL-6 mRNA, gp130 mRNA and IL-6 protein than those of the control group, suggesting that activated IL-6/gp130 signal pathway in the thyroid tissue may contribute to the pathogenesis of GD.
9.Design and verification of percutaneously puncturing apparatus for the vertebrae

BACKGROUND: Vertebroplasty or kyphoplasty is monitored under imaging apparatus. Standard entopic phase and standard lateral phase were obtained via repeatedly adjusting C-arm preoperatively. Puncturing angle was adjusted in experience, and time and X-ray exposure were significantly increased. OBJECTIVE: To design percutaneously puncturing apparatus for vertebrae in order to reduce X-ray exposure and shorten the operating time. DESIGN, TIME AND SETTING: A designing and verifying study was performed at the Department of Orthopaedics, the First Hospital Affiliated to Soochow University between October 2004 and March 2005. MATERIALS: Five samples of thoracic waist and lumbar vertebrae with soft tissues did not have abnormalities or defect. METHODS: CT scan and standard lateral image of 5 vertebrae specimen from T11 to L5 were got. Every "e" angle and "f" angle were measured and puncturing approaches were analyzed for kyphoplasty in thoracic and lumbar vertebrae. Percutaneously guiding instrument was only used on the left sides. The effect was analyzed and the exposure times were compared between the left and the right sides. MAIN OUTCOME MEASURES: Determination of puncturing approach, and X-ray exposure times between the left and the right sides. RESULTS: The entry point was at the superior-lateral 1/4 quadrant of the cross by a line passed the mid-point of two transverse process roots and a line passed the back edge of the superior articular process. In standard anterior posterior image, the entry points were at 2 o'clock on the left pedicle shadow and 10 o'clock on the right pedicle shadow respectively. The exposure times were 12.61±1.69 on the left and 23.19±1.60 on the right (P < 0.05).CONCLUSION: Ideal position can be arrived in puncturing according to the exact position, correct entry point and puncturing according to te exact position,correct entry point and puncturing angles.Application of percutaneously guiding instrument can sgnificantly diminish exposure time.
10.The significance of detecting serum DJ-1 protein combined with CA125 in epithelial ovarian tumors

Objective To explore the diagnostic value of detecting serum DJ-1 protein combined with CA125 for epi?thelial ovarian tumors. Methods Double antibody sandwich method and electrochemiluminescence immunoassay were used to determine the serum levels of DJ-1 protein and CA125 in 82 cases of epithelial ovarian tumors and 80 non-ovarian tumor cases (control group). The clinical significance of detecting serum DJ-1 protein combined with CA125 was analyzed. Results The expression levels of DJ-1 and CA125 were significantly higher in ovarian tumor group than those in the con?trol group (P<0.05). The critical value of serum DJ-1 was 6.800μg/L and 6.965μg/L in ovarian cancer group compared with the control group and non-ovarian tumor group. The sensitivity of combined detection of DJ-1 and CA125 was higher than that of any marker alone. Conclusion The detecting serum levels of DJ-1 combined with CA125 are helpful to the diagnosis of ovarian cancer, which can be a good marker of ovarian cancer and may improve the early diagnosis rate of ovarian cancer.
