1.Characteristics of Consultation-Liaison Psychiatry in a Psychiatric Hospital in Beijing

Objective:To explore the characteristics of consultation-liaison psychiatry service in general hospital.Methods:Totaly 541 consecutive inpatients from general hospitals were enrolled,who were referred to the consultation-liaison psychiatry service of a psychiatric hospital during a one-year period.The data was analyzed,including demographic characteristics,reasons for referral,referral department,psychiatric diagnosis according to criteria in the 4th edition of the Diagnostic and Statistical Manual of Mental Disorders(DSM-IV),and psychiatric management suggestions.Results:The rate of psychiatric consultation in the hospital which applied for the most consultations was 1.0%.The patients with psychiatric history(22.6%)were most likely to be referred.The principal reasons for psychiatric referral were psychotic episode(31.8%),depression(13.1%),and anxiety disorder(10.9%).According to DSM-IV,cognitive disorders(32.2%),depressive disorder(17.7%),anxiety disorder(13.1%),schizophrenia and other psychotic disorders(9.8%)were the major psychiatric diagnoses.The psychiatrists prescribed antipsychotics in 72.8% patients,gave nursing and behavior suggestions in 39.2% patients,gave following-up suggestion in 38.1% patients,and provided suicide prevention in 10.2% patients.Conclusion:It is important to expand the consultation-liaison psychiatry service in general hospital.And it is necessary to improve the physicians' ability in recognition and treatment of psychiatric disorder in patient with physical illness.
2.Podocalyxin expression in renal tissues and correlation with the number of urinary podocytes in children with Henoch-Sch(o)nlein purpura nephritis

Objective:To analyze the podocalyxin (PCX) expression in the kidney and the number of urinary podocytes in different pathological grades of Henoch-Sch(o)nlein purpura nephritis (HSPN),and to determine whether the number of urinary podocytes reflects the renal damage in HSPN.Methods:Fifty-six children diagnosed with HSPN in our hospital were enrolled in the study and classified into 4 groups by renal pathology:grade Ⅱ (Ⅱa+Ⅱb) (n=10),grade Ⅲ (Ⅲa+Ⅲb) (n=21),grade Ⅳ (n=16),and grade Ⅴ (n=9).Four kidney autopsy specimens without histomorphologic lesions and 8 urine samples from healthy children served as controls.With immunofluorescence assay,the PCX expression in 4 normal renal tissues and in the renal tissues of the 56 HSPN children was detected and quantitatively analyzed.Positive rate and the number of urinary podocytes were detected in the 8 healthy children and 56 HSPN children.Results:In the renal tissues of the normal control group and grade Ⅱ (Ⅱa+Ⅱb) HSPN group,the PCX expression was complete.The percentage of the PCX positive area out of the total glomerular area in the renal tissues of 2 groups had no significant difference (P>0.05).In the renal tissues of grade Ⅲ (Ⅲa+Ⅲb),Ⅳ,and Ⅴ HSPN groups,the PCX expression showed various degrees of loss,decreasing in turn from grade Ⅱ (Ⅱa+Ⅱb),Ⅲ (Ⅲa+Ⅲb),Ⅳ to Ⅴ,with significant differences between each group (P<0.01).For HSPN with grade Ⅲ (Ⅲa+Ⅲb) or higher,positive PCX expression was found in the urine,suggesting the presence of enough podocytes in the urine.The percentage of fluorescence positive area out of the total glomerular area of PCX in the renal tissues was negatively correlated with the total number of urinary podocytes (r=-0.637,P<0.01).Conclusion:Podocyte injury plays a certain role in the pathological progression of HSPN.The urinary detection ofpodocytes can reflect the degrees of pathological damage in HSPN.
3.Magnetic Solid Phase Extraction with Fe3O4-Grafted Nitrogen-Doped Graphene for Determination of Four Organochlorine Contaminants in Environmental Water Samples

Fe3O4-grafted nitrogen-doped graphene (Fe3O4/N-G) nanomaterials were synthesized by chemical co-precipitation method, and its adsorption properties were discussed preliminarily.It was demonstrated that the adsorption of parachlormetaxylenol on Fe3O4/N-G was not limited to uniform monolayer adsorption and the adsorption kinetic followed the pseudo-second-order kinetic mode.Then, an ultrasound-assisted magnetic solid-phase extraction with Fe3O4/N-G as the magnetic adsorbent has been developed for the determination of four compounds including triclosan, chloroxylenol, hexachlorobenzene and 2,2′,4,4′,5,5′-hexachlorobiphenyl in environmental water samples, in combination with gas chromatography coupled to tandem mass spectrometry.Several factors related to extraction efficiencies, such as the amount of adsorbent, extraction time, sample pH and desorption conditions were investigated.The proposed preparation procedure was as follows: 6.0 mg of Fe3O4/N-G was dispersed into 100 mL of water sample under ultrasound.After 15 s, the Fe3O4/N-G carrying four compounds was separated from the water sample by an external magnetic field.Then, the targets were eluted from Fe3O4/N-G with 3 mL of ethanol and 2 mL of dichloromethane, sequentially.Finally, the eluent was dried under a mild stream of nitrogen and reconstituted with methanol and dichloromethane (1∶1, V/V) for the subsequent GC-MS/MS analysis.Under the optimized condition, an excellent linearity was observed in the range of 0.1-10 ng/L for the four compounds, with the correlation coefficients ranging from 0.9983 to 0.9999.The limits of detections (S/N=3) ranged from 0.05 to 0.6 ng/L and the limits of quantity (S/N=10) ranged from 0.2 to 2.4 ng/L.The mean recoveries at three spiked levels ranged from 68.2% to 99.6%.The relative standard deviations (RSDs) of intraday and interday were in the range of 3.3%-6.9% and 3.4%-9.4% (n=6), respectively.The proposed method was demonstrated to be simple and feasible for the trace analysis of antimicrobial agents and organochlorine contaminants in environmental water samples.
4.Colonoscopy surveillance in colorectal cancer after surgery

Objective To evaluate the colonoscopy surveillance in colorectal cancer patients after surgical removal of the tumor.Methods From June 1986 to June 2007,2762 asymptomatic patients who had underwent operation for colorectal cancer were put into colonoscopy surveillance.They had the first examination 3-6 months after the operation,and were re-examined once a year thereafter for 3 years.The follow-up findings were compared with those from the 218 symptomatic patients who had colorectal cancer surgery from September 1981 to May 1986.Results In 2762 asymptomatic patients,48 cases of multiple primary cancer were detected,in which 39 cases(1.4%) were found at one examination and 9 cases(0.3%)at different examination.The TNM staging of these lesions included stage Ⅰ in 6,stage Ⅱ in 31 and stage Ⅲ in 11.During the surveillance,583 cases(21.1%) of adenoma were detected and endoscopically resected,in which 17(3.2%) were invasive early cancer and 58(9.9%) were high grade dysplasia.In 218 patients with symptoms,29 cases(13.3%) of adenoma and 27 cases( 12.4%) of cancer were detected,including 4 cases of stage Ⅰ cancer,6 of stage Ⅱ and 16 of stage Ⅲ.Conclusion Colonoscopy surveillance in colorectal cancer patients after surgery is important in finding precancerous lesion and early stage cancer,and is recommended in all patients.
5.Management Situation of Cross Infection in Outpatient Service of Stomatology in Hospital and Private Clinic: An Investigation and Analysis

OBJECTIVE To investigate the management situation of cross infection in outpatient service of stomatology in hospital and clinic in Zhanjiang city and conduct effective measure to control hospital infection.METHODS In our investigation,we selected 105 hospitals and private clinics to inspect their management situation of cross (infection) via dental(instrument).RESULTS The management in the hospitals of the city was quite qualified but the some private dental clinics were terrible.The main problems were as follows: no effective disinfection for dental instrument and the bad environment in procedure room,especially the worse sterilization situation of the dental handpieces.Most of the doctors in private clinics were short of knowledge in the sterilization for dental(instruments.) CONCLUSIONS There is no time to delay for the health administration department to normalize the management of the private dental clinics.It is essential to train the medical staffs,and implement measures for dental instrument disinfection management in private dental clinics.
