1.Effect of castration on the heart function in postinfarction congestive heart failure model in male rabbits

Objective To investigate the effect of castration on heart function in male rabbits postinfarction congestive heart failure model. Methods 32 male 4-5 months old New Zealand rabbits were randomly assigned to ligation of first branch of left coronary artery or sham operation (sham group, n=8). 3 weeks later 20 survived MI model rabbits were randomly subjected to castration (castration group, n=10) or sham operation (MI group, n=10). Serum FT, DHEAS levels and LVEF were checked at week 0, 3 and 15. Heart diameter and left ventricular wall thickness were measured at week 15. Results The serum FT and DHEAS levels decreased significantly at week 3. In MI group and castration group serum androgen level was lower and heart weight, ventricular diameters, ventricular wall thickness corrected by body weight were higher than that in sham group at week 15. The androgen level and body weight of castration group were lower than that in MI group. But LVEF in castration group was not higher than MI group. Conclusion There was no benefit effect of castration on heart function in male rabbits postinfarction congestive heart failure model.
2.Effects of low dose androgen treatment on serum androgen levels in male adult rabbits postinfarction CHF model

ObjectiveTo investigate the effects of low dose testosterone undecanonate (TU) and dehydroepiandrosterone (DHEA) on serum androgen levels in male adult rabbits postinfarction congestive heart failure (CHF) model.Methods48 male 4~5 months old New Zealand rabbits were randomly assigned to ligation of first branch of left coronary artery (n=40) or sham operation (n=8). 3 weeks later, 30 survived model rabbits were randomly treated with TU 3 mg/kg per 2 weeks intramuscular injection (TU group, n=10), DHEA 3 mg/kg/d oral (DHEA group, n=10) or placebo (placebo group, n=10) for 3 months. Serum androgen changes were recorded at 0th, 3rd and 15th week.ResultsAt 3rd week after peration, left ventricular ejection fraction of model animals was 47.6±4.5% lower than that of the sham group (66.7±5.8%) (P<0.05). Serum free testosterone level of model animals returned to normal ranges after low dose TU or DHEA treatment, otherwise DHEAS level was higher than the sham group (P<0.05).ConclusionThe doses used in this research reached physiological dosage to male adult rabbits in postinfarction CHF model.
3.Influencing factors for anxiety and depression in patients with dysphagia after stroke

Objective :
To investigate the influencing factors for depression and anxiety in patients with dysphagia after stroke.
Methods:
From May 2017 to June 2018,patients with post-stroke dysphagia in the Department of Rehabilitation Medicine of Zhejiang Hospital were recruited to collect their height,weight,education level,serum nutritional indicators and feeding patterns. Self-Rating Anxiety Scale and Self-Rating Depression Scale were employed to assess the incidence of anxiety and depression. Multivariate logistic regression model was used to analyze the influencing factors for depression and anxiety.
Results :
Among 96 patients enrolled,43 suffered from anxiety and depression,with the incidence of 44.79%. The results of multivariate logistic regression analysis showed that normal albumin(OR=0.208,95%CI:0.054-0.800)was a protective factor for anxiety,while indwelling gastric tube(OR=5.789,95%CI:1.654-20.260)was a risk factor. Normal transferrin(OR=0.189,95%CI:0.042-0.860)was a protective factor for depression,while indwelling gastric tube(OR=13.977,95%CI:1.472-132.667)was a risk factor.
Conclusion
Normal albumin and transferrin are the protective factors for anxiety and depression in patients with dysphagia after stroke,and indwelling gastric tube is the risk factor for anxiety or depression.
4.Effect of Electroacupuncture on the Expression GLUT4 of Skeletal Muscle in Type-2 Diabetic Rats

[Objective]To observe the effect of electroacupuncture on the expression of glucose transporter 4(GLUT4)of skeletal muscle in rats with type-2 diabetes mellitus,and to elucidate the mechanism of electroacupuncture on blood glucose regulation.[Methods]Twenty-four rats with type-2 diabetes were randomly divided into blank control group,exercise intervention group and electroacupuncture group,with 8 rats in each group.Rats in blank control group did normal activities.After 1 week of adaptation,rats in exercise intervention group began to run on the treadmill for 2 weeks.The rats in electroacupuncture group were adapted for 1 week,and then the electroacupuncture was placed at the Zhusanli acupoint on both sides with continuous wave stimulation for 2 weeks.Fasting blood glucose and insulin resistance index(IRI)were detected before and after the treatment,and the GLUT4 level of rat muscle cells was detected after the treatment.[Results]Compared with before intervention,the fasting blood glucose level of rats with type-2 diabetes in exercise intervention group and electroacupuncture group reduced significantly after intervention(P<0.001,P=0.028),and the IRI was reduced significantly(P<0.001,P<0.001).The differences of fasting blood glucose level and IRI were significant among the three groups after intervention(P<0.001).The difference of level of GLUT4 was also significant among the three groups after intervention(P<0.001).[Conclusion]Electroacupuncture stimulation can significantly decrease the level of blood glucose and IRI,and regulate the level of GLUT4 in skeletal muscle,which may be an important mechanism of regulation.
5.Clinical features and prognosis of head and neck cancer in patients with esophagus cancer and triple primary carcinoma

Objective To investigate the clinical features and prognostic characteristics of head and neck cancer in patients with esophagus cancer and triple primary carcinoma(TPC).Methods A total of 30 patients with head and neck cancer with esophagus cancer TPC were collected in Zhejiang Cancer Hospital from January 2007 to December 2016.The distribution of cancer kinds and the incidence of synchronous and metachronous cancer were described.The clinical characteristics and prognosis were also compared in synchronous and metachronous cancer.The influence of number of hospitalization and different treatments on the survival time were analyzed.Results The TPC of "laryngeal pharynx + esophagus + lung" and "laryngeal pharynx + esophagus + oropharynx" had the highest incidence,that was 20.0% in 30 patients (6/30).The second type was "laryngeal pharynx + esophagus + larynx".Fifteen cases were synchronous cancer and other 15 cases were metachronous cancer.The rate of surgery was 73.3% (11/15),and the number of hospitalization who more or equal than 5 was 73.3% (11/15) in the synchronous cancer.While the rate of surgery was 33.3% (5/15),and the number of hospitalization who more or equal than 5 was 33.3% (5/15) in the metachronous cancer.There were significant differences between synchronous and metachronous cancer (x2 =4.661,4.661,all P < 0.05).The 1-year,3-year and 5-year survival rates were 39.9%,19.9% and 0.0% in patients with synchronous cancer.The mean survival time was (18.4 ± 6.2)months.In contrast,the survival rates were 78.7%,77.8% and 59.1% in metachronous cancer.The mean survival time was (122.2 ± 17.2) months.There were significant differences between the two groups (survival rate:x2 =10.934,P =0.001;mean survival time:t =3.201,P =0.003).The survival rate of the number of hospitalization more than or equal to 5 times had significant difference compared with those less than 5 times (x2 =10.574,P =0.001).There was no statistically significant difference in the improvement of OS between single operation,chemotherapy and target treatment (P > 0.05).Conclusion Head and neck cancer in patients with esophagus cancer TPC can still has a high survival rate through active combined modality therapies,especially in metachronous carcinoma.
6.The application of multifunctional inflatable pelvis and hip-joint fixator (MIPHF) in damage control in patients with pelvic fracture

Objective To discuss the safety and the application of the self-designed multifunctional inflatable pelvis and hip-joint fixator (MIPHF) in damage control in pelvic fracture patients.Methods The MIPHF was subjected to pressure test and quality inspection.From September 2016 to June 2017,61 pelvic-fracture patients were treated with our self-designed MIPHF as pre-hospital first-aid care according to the concept of damage control orthopedics (DCO) (MIPHF group).The control group consisted of 69 pelvic-fracture patients who had not received pre-hospital first-aid care with the self-designed MIPHF from December 2015 to August 2016.There were no statistically significant differences between the two groups in gender,age,types of pelvic fracture,and preoperative injure severity score (ISS).The study compared the two groups for the case fatality rate,volume of blood transfused during surgeries,early complication rates,fracture reduction (Matta standards),and long-term efficacy (Cole scores).Results The pressure test showed that the MIPHF had a good fixation effect on the pelvis.And the quality inspection showed that the material used for the MIPHF was in line with national standards and the safety was guaranteed.The MIPHF group had 1 death (1.6%) and the control group had 8 deaths (11.6%),which was a significant difference (x2=4.979,P=0.026).All survival patients in both groups were followed up.The MIPHF group (61 cases) received 3.0 to 18.0 months follow-up,with an average of 9.0 months.And the control group (69 cases) had 18.0 to 30.0 months follow-up,with an average of 21.9 months.In the MIPHF group,23 cases were treated conservatively,and 37 cases were treated with surgery.Among them,3 cases were fixed with external fixator,20 cases with anterior open reduction and internal fixation,9 cases with posterior open reduction and internal fixation,and 5 cases with combined anterior and posterior fixation.The timing of surgery was 1 to 20 days after injury,with an average of 4.1 days.The volume of blood transfused in the MIPHF group during surgery was 200 to 1500 ml,with an average of 628.6 ml.In the control group,27 patients were treated conservatively,and 42 patients were treated with surgery.Among them,2 cases were fixed with external fixator,24 cases with anterior open reduction and internal fixation,10 cases with posterior open reduction and internal fixation,and 6 cases with combined anterior and posterior fixation.The timing of surgery was 1 to 15 days after injury,with an average of 3.l days.The volume of blood transfused in the control group during surgery was 200 to 4000 ml,with an average of 1 707.1 ml.There was a significant difference between the two groups in intraoperative blood transfusion(Z=-2.330,P=0.020).The MIPHF group had 10 (16.4%) cases of early serious complications and the control group had 22 (31.9%) cases,which had a significant difference (x2=4.187,P=0.041).According to the criteria proposed by Matta et al.,the good rate of results for treating fractures was 82.0% in the MIPHF group and 60.9% in the control group,which got a significant difference (x2=6.967,P=0.008).The MIPHF group and the control group also differed significantly in their mean long-term Cole scores (27.2±4.0 versus 25.1 ±5.6,t=2.457,P=0.015).Conclusion MIPHF,which reflects the DCO concept,may be recommended as pre-hospital first-aid care for patients with pelvic fracture because it can lessen bleeding and prevent secondary pelvic injury,thus reduce case fatality rate and the incidence of complications.It can also improve the success rate of treating pelvic fracture,which will positively affect long-term outcomes.
7.Application of continuous nursing in discharged patients after percutaneous minimal invasive bladder ;fistulization

Objective To explore the effect of continuous nursing on patients with spinal cord injury neurogenic bladder who were discharged from the hospital after spercutaneous minimal invasive bladder fistulization. Methods Totally 43 patients with spinal cord injury and underwent percutaneous minimal invasive bladder fistulization were divided into the control group and the observation group by random digital table method, with 23 cases in each group. Patients in the control group received routine nursing, while patients in the observation group received continuous nursing based on the routine nursing. The level of colostomy adaptation and the incidence of complications related to catheter in two groups were observed. Results Six months after nursing intervention, the score of colostomy adaptation level in the observation group was significantly superior to that in the control group (P<0.01); in the intervention group, the incidence of urinary tract infection, skin infection around the stoma;the incidence of complications such as catheter slippage, catheter blockage, catheter breakage, were all lower than that in the control group ( P<0. 05 ) . Conclusions The continuous nursing intervention can improve the level of colostomy adaptation and reduce the incidence of catheter related complications.
8.Different human parvovirus B19 subgroup circulation in Tibet and Han population in China

Objective To investigate the genetic diversity differences of human parvoviruse B19 and parvovirus 4 (PRVA4) in Tibet and Han population in China.Methods Phylogenetic analysis was performed on genome fragments of B19 or PARV4 obtained from the blood samples of Tibet and Han population in China by using a PCR followed by sequencing.Results Ten partial VP1 fragments of B19 (2 from Tibet,3 from Sichuan,5 from Zhejiang) and 10 partial ORF1 fragments of PAV4 (2 from Tibet,2 from Sichuan,1 Yunnan,5 from Zhejiang) were obtained.Phylogenetic analysis indicated that different B19 subgroup circulates in Tibet and Han population although they belong to the same 1A subtype.While the gene evolution of PAV4 is very conserved between the Tibet and Han population in China.Conclusion These studies on genetic diversity of B19 in different Chinese population provide a way for detection and prevention of B19 human parvovirus infection.
9.Value of ultrasound-guided fine needle aspiration cytology in the assessment of ovarian neoplasms and retroperitoneal metastatic lymph nodes

Objective To evaluate the diagnostic value of ultrasound-guided fine needle aspiration cytology(US-FNAC)in the assessment of radiologically detected ovarian neoplasms and retroperitoneal metastatic lymph nodes. Methods FNAC was performed under ultrasound guidance on 126 patients suspected of ovarian neoplasms and retroperitoneal metastatic lymph nodes. Cytologic examination was performed after staining smears with the haematoxylin and eosin method. Clinical data were retrieved from the medical records and all cytological specimens were reviewed. In these cases, the cytologic findings were correlated with histology of the primary tumor and were compared with surgical pathology. Results Satisfactory sampling was obtained in 86.2% of punctures, and cytological diagnosis was made in 85.7% cases. The size of the lymph nodes punctured was less than 20 mm in 93.5% cases, with the sensitivity of 81.6%、86.2%, specificity of 95.8%、100.0%, positive predictive value of 98.3%、100.0%, negative predictive value of 63.9%、33.3%, and accuracy of 85.3%、87.1%. Seven patients presented slight abdominal discomfort, and relieved without clinical treatment. Conclusions The fine needle aspiration technique has excellent positive predictive value and low morbidity. US-FNAC, as the valuable investigation, is not only useful in the diagnosis of ovarian masses and lymph nodes but can also help in choosing appropriate management. From our experience, US-FNAC can be added in follow-up of selected patients in whom the cytological identification of such masses and nodes is significant for the patient′s treatment.
