Objective To investigate the effect of sevoflurane pretreatment on the expression of tight junction protein Occludin and ZO-1 in the lung following ischemia-reperfusion (I/R) injury in rats. Methods Ninty-six adult male Wistar rats weighing 250-350 g were randomly divided into 4 groups ( n = 24 each): group Ⅰ sham operation (group S);group Ⅱ I/R; group Ⅲ sevoflurane (group Sevo) and group Ⅳ Sevo + I/R. The animals were anesthetized, tracheostomized and mechanically ventilated. Right femoral vein and left carotid artery were cannulated for BP monitoring, blood sampling and drug and fluid administration. Lung I/R was induced by clamping left pulmonary hilum for 45 min followed by 120 min reperfusion in group Ⅱ and Ⅳ. In group Ⅲ the animals inhaled 2.2 % sevoflurane for 30 min. In group Ⅳ the animals inhaled 2.2 % sevoflurane for 30 min before lung ischemia. Six animals were killed at the end of 45 min ischemia (T1), and 60 and 120 min of reperfusion (T2, T3 ). The lungs were immediately removed for determination of W/D lung weight ratio and the expression of Occludin and ZO-1 protein (by Western blot). Tissues of lung were obtained for observation of histopathology with light microscope. Lung permeability index (LPI) was calculated. Another 6 animals were killed at 120 min of reperfusion for lung lavage. Results I/R significantly increased W/D ratio and LPI and significantly reduced Occludin and ZO-1 protein expression in the lung tissue as compared with group C and Sev. Sevoflurane pretreatment significantly attenuated the I/R-induced changes. The pathological damage to the lung tissue was significantly less in group SP than in I/R. Conclusion Sevoflurane pretreatment may protect the lungs from I/R injury by up-regulating the expression of Occludin and ZO-1.

Objective To study clinical factors about thrombosis in coronary stents.Methods We reviewed 571 patients who underwent percutaneous coronary intervention(PCI) and thrombosis in coronary stents happened in 5 patients.The clinical data and angiography results and anti-thrombus treatment about 5 patients have been studied.Results Thrombosis in stents had been happened in 6 lesions of 5 patients with acte coronary syndrom.3 lesions were C type.Drug-eluting stents had been deposited in 4 lesions.The cause of thrombosis in stent may be imcomplete stent expansion in 1 lesion and imcomplete apposition in 2 lesions and imcomplete treatment of anti-thrombu in 2 patients.Conclusion Our results show that thrombosis intrastent probably correlate with following factors:(1) Acue coronary syndrom.(2) Long lesion,imcomplete stent expansion, imcomplete apposition and drug-eluting stents.(3) Imcomplete treatment of anti-thrombus.

Objective To evaluate the clinical effect of CYPHER TM (rapamycin) stent in treatment of left anterior descending (LAD) long lesion. Methods From October 2002 to April 2003, 65 patients with coronary heart disease (CAD) were treated with CYPHER TM (Cordis) stent and 65 stents were planted in 53 LAD lesions. Among the patients with LAD lesions, 42 cases were long lesions (≥20 mm). The length of lesion was from 20 mm to 50 mm (28.2?8.8 mm), the degree of stenosis before procedure was 88.9%?8.5%, and the vessel diameter before procedure was 3.0?0.9 mm. Three lesions were chronic total occlusion (CTO) lesion. Two lesions were in-stent restenosis. 12 lesions over 35 mm long were treated with two overlapped stents to cover the whole lesion. 30 lesions were treated with only one stent. We observed the procedure success and complication rates. The occurance of angina, myocardial infarction, sudden death, and the revascularization rate during the in-hospital and 6 months follow up. Results Immediate procedure success rate was 100% with residual stenosis of 5.0%?4.8%. All the lesions were covered fully by stents and the coronary flow reached TIMI grade three. No new dissection was observed in the optimal of stents. Only one patient was diagnosed as myocardial infarction after procedure and in-stent thrombolysis was proved by coronary artery angiography (CAG). During the 6 months follow-up, the symptom of angina was disappeared in 38 patients and alleviated in 4 patients. Eight patients received CAG in 3-6 months and no restenosis was observed. Conclusion Drug eluting stent (CYPHER TM) implantation is a safe and efficient therapy in treating patients with LAD long lesion, and short term follow-up results were better than the general coronary stents.

Two cases of nevus of Ota resistant to multiple sessions of Q-switched laser treatment are reported.Both of the patients were females,who developed beige or blue patches on the left side of the face (zygomatic region,cheek)several months after birth.No conjunctiva or sclera was involved.The lesions spread to the left retroauricular region and posterior neck during pregnancy in one patient.Histopathological examination of the lesions showed scattered melanocytes and melanophages in lower dermis and subcutaneous adipose tissue as well as apparent proliferation of collagen.Immunostaining for CD34 showed no abnormality in the distribution of vascular endothelial cells in these tissues.No obvious improvement was observed in either of the patient after more than 10 sessions(12 sessions in 1 case,and 13 sessions in the other case)of treatments with Q-switched alexandrite(755 nm)or Q-switched Nd:YAG laser(1064 nm).The immediate response of lesions to laser irradiation was unusual.Purpura or errhysis was absent in the lesions after repetitive irradiation with high-energy laser,but both were visible in the adjacent normal skin even after irradiation with low-energy laser.

Liver transplantation is the best treatment for end-stage liver disease.Because of the severe shortage of donor sources,most of the patients died while waiting for liver grafts.Artificial liver support system can improve the liver function in a short time,and help patients to pass the waiting periods.Artificial liver support system takes place of composition,detoxification and metabolism function of liver,stabilizes the physiological and biochemical index of liver,relieves the burden of liver and helps patients to prepare for the liver transplantation.With the wide application of artificial liver support system,new types of the artificial liver support systems gradually conquered the defects of the old types,but they still have their own defects.This review concludes the merits and demerits of artificial liver systems,its clinical application and the problems so as to help the treatment of end-stage liver disease.

Objective To analyze the skin barrier function in patients with facial steroid dermatitis.Methods Ninety-five patients with facial steroid dermatitis were enrolled into this study along with 25 healthy volunteers as controls.Transepidermal water loss(TEWL)and skin hydration were measured in 8 facial regions.including lesional skin in forehead,nasal tip,submaxilla,both cheeks and angulus oris,and non-lesional skin in the left posteior ear.Results Compared with the healthy group,the patients with steroid dermatitis had a significant higher value of TEWL on lower mandible,both angulus oris and cheeks(t:4.90,2.60,2.57,2.54,3.77,respectively,P＜0.01),while a significant lower level of skin hydration was noted on both angulus otis,right cheek and forehead(t=3.27,3.81,2.02,2.78,respectively,P＜0.05).Among the 8 test sites in both patients and controls,TEWL value decreased in the following order:submaxilla and both angulus oris＞both cheeks and forehead＞non lesional skin on the posteroir ear,and the decrease in skin water content was highest in submaxilla and both angnlus oris,followed by cheeks and forehead,and finally by left posterior ear.Conclusions In patients with facial steroid dermatitis,skin barrier function is disturbed on submaxilla,both angnlus oris and cheeks,and TEWL value is found to be higher in perioral area,while cheeks have a lower level of skin hydration.

AIM: To establish a quantitative method of determining thymol and carvacrol in Shushi Ganmao Ling(dripping pill)(Herba Moslae,Herba Artemisiae Annuae,Radix Scutellariae,etc.). METHODS: The GC method was used with HP-17 quartz capillary column(HP 50% PhMe Silicone,10 m?0.25 mm?0.25 ?m).The initial temperature of chromatographic column was at 130 ℃ for 14 min,then elevated to 220 ℃ with the rate of 20 ℃/min,and retention for 36 min. RESULTS: The linearities of thymol and carvacrol were respective in the range of 12.51 ?g/mL-125.1 ?g/mL(r=0.999 8) and 21.52 ?g/mL~215.2 ?g/mL(r=0.999 7).The average recoveries were 97.9%(RSD was 1.69%) and 100.6%(RSD was 2.27%).RSD of repeatability were 1.88% and 1.60%,respectively. CONCLUSION: The method is reliable,accurate and specific.It can be used for quality control of Shushi Ganmao Ling(dripping Pill).

Objective To explore the establishment methods of animal models of adult growth hormone deficiency, and to provide a good model for experimental research and treatment for abnormal bone metabolism caused by growth hor-mone deficiency.Methods The methods of establishment of animal models of adult growth hormone deficiency were re-viewed and evaluated refering to literature.Results There were three methods including spontaneous lack-of, pituitecto-mized and gene knockout can establish animal models of adult growth hormone deficiency.Conclusions Hypophysecto-mized animal models are inexpensive and easy to create, but not suitable for studying the relationship between growth hor-mone and bone metabolism.Spontaneous lack-of and gene knockout models are specific growth hormone deficiency and of great research significance in exploring the relationship between growth hormone and bone metabolism.

Objective: To observe the clinical efficacy of combined tuina manipulations with Qi Zheng Qing Peng paste for herniation of the lumbar intervertebral disc. Method: All 568 cases were randomly allocated into an observation group (189 cases), control Ⅰ group (187 cases) and control Ⅱ group (192 cases). Combined tuina with Qi Zheng Qing Peng paste, combined tuina with diclofenac diethylamine emulsions and tuina alone were adopted in the above three groups respectively. The efficacies in the three groups were compared after 4 weeks of treatment. Results: The total effective rates in the observation group, control Ⅰ group and control Ⅱ group were 89.9%, 86.6% and 84.9% respectively, showing a significant difference (P ＜0.05) . Conclusion: Combined tuina manipulations with Qi Zheng Qing Peng paste works well for herniation of the lumbar intervertebral disc.

Objective: To discuss the significance of neoadjuvant chemotherapy followed by surgery in the treatment of local advanced esophageal cancer. Methods:A total of 272 cases of local advanced esophageal cancer were studied in retrospect. Out of the 272 cases, 112 were treated with neoadjuvant chemotherapy followed by surgery (CT-S), whereas the remaining 160 cases underwent surgical treatment (S) only. Complications and survival state after surgery were compared. Results: The rate of complications after surgery was as follows: CT-S: 34.8% (39/112); S: 29.4% (47/160), P=0.50. The five-year survival rate was 35.7% and 29.4%, respectively, P<0.05. The CT-S patients were divided into partial remission (PR) and stable disease (SD)/progressive disease (PD) groups according to the effect of the chemotherapy. The five-year survival rate was 38.5% and 30.1%, respectively, P<0.01. Conclusion: Neoadjuvant chemotherapy is available for local advanced esophageal cancer. Postoperative complications are not increased by chemotherapy, and the survival rate for local advanced esophageal cancer is improved by neoadjuvant chemotherapy. PR has better prognosis compared with SD/PD.
