1.Contrast in respiratory mechanics and blood gas between bronchial blocker and double lumen tube in one lung ventilation

Objective To compare the influences between bronchial blocker (BB) and double lumen tube (DLT) in respiratory mechanics and blood gas in one lung ventilation. Methods Thirty-six patients undergoing pulmonary lobectomy or radical operation of esophageal carcinoma were randomized into group BB (17 cases) and group DLT (19 cases). In group BB,BB was intubated under the guidance of a fibreoptic bronchoscope after intubation of single lumen tube; while in group DLT, the position of the tube was confirmed using fibreoptic bronchoscopy after intubation. The peek airway pressure (Pmax) and pulmonary dynamic compliance (CLdyn) during double lung ventilation and one lung ventilation were recorded in recumbent position. Blood samples were taken from, radial artery 20 min after one lung ventilation for blood gas analysis,partial pressure of oxygen in artery(PaO2), partial pressure of carbon dioxide in artery(PaCO2), saturation of arterial blood oxygen (SaO2) were recorded. Results No significant difference was found in Pmax and CLdyn between two groups during double lung ventilation,but Pmax was significantly lower in group BB [(21.00 ± 3.59) cm H2O,1 cm H2O = 0.098 kPa] than that in group DLT [(26.89 ±4.21) cm H2O] (P<0.05), and CLdyn was greater in group BB [(28.10 ±6.24) ml/cm H2O] than that in group DLT [(24.00±7.13) ml/cm H2O ] (P < 0.05); there were no significant differences in PaO2, PaCO2, SaO2 betweentwo groups during one lung ventilation (P> 0.05). Conclusion The influence to respiratory mechanics in one lung ventilation is smaller using BB than using DLT, BB has blocker well applicable value in clinics.
2.Assessing the degree of necrotic femoral head, and association of blood supply with pathlogical changes:study protocol for a diagnostic animal trial

BACKGROUND: The imaging methods of evaluating the volume of necrotic femoral head are based on Kerboul angle and necrosis index revealed on X-ray, as well as CT and MRI three-dimensional reconstruction, but the accuracy of each method is different, and none is widely accepted. Additionally, the association of the volum and degree of necrotic femoral head with the status of blood supply to the femoral head remain unclear. OBJECTIVE: To evaluate the accuracy of each imaing method for assessing different stages of femoral head necrosis, and to confirm a convenient method used for measuring the volum of necrotic femoral head. METHODS: This diagnostic in vivo animal trial was finished in the Affiliated Zhongshan Hospital of Dalian University, Liaoning Province, China. The canine models of steroid-induced necrosis of femoral head were prepared, and received CT and MRI examinations, followed by three-dimensional reconstruction combined with Micro-CT to make accuratediagnosis; the degree of necrotic femoral head and its association with the status of blood supply were evaluated throughangiography and histopathological examination. All above outcome measures were compared with the Association Research Circulation Osseous to testify the consistency of this precise method with traditional methods. The experiment follows the national guidelines for the Care and Use of Laboratory Animals, and Consensus author guidelines on animal ethics and welfare by the international Association for Veterinary Editors (IAVE). The article was prepared in accordance with the Animal Research: Reporting of In Vivo Experiments Guidelines (ARRIVE Guidelines). RESULTS AND CONCLUSION: The aim of the study is to obtain a accurate measurement method by detecting different stages of osteonecrosis using different imaging techniques, which enables surgeons make exact diagnosis and clinical stage of osteonecrosis, thus desiging standardized treatment program.
3.Manual reduction for external fixation of distal radius fractures

BACKGROUND:Manual reduction for external fixation of distal radius fractures can achieve good outcomes.
OBJECTIVE:To carry out the bibliometric analysis on manual reduction for external fixation of distal radius fractures.
METHODS:Literatures concerning reduction for external fixation of distal radius fractures were retrieved in CNKI database from 2003 to 2012. The keywords were“external fixation;distal radius fractures”. Duplicate and irrelevant articles were eliminated, and a total of 408 articles were retrieved. A bibliometric analysis of these 408 articles was performed in terms of publishing time and number, subject categories, source journals, research institutions, times cited and download frequency.
RESULTS AND CONCLUSION:Literatures concerning manual reduction for external fixation of distal radius fractures exhibit an upward trend in number. In 2012, there were 85 relevant articles. In the past 10 years, 18 relevant articles have been published in Chinese Journal of Traditional Medical Traumatology&Orthopedics, which is the most. There are not many relevant studies in various research units, only 1-7 articles published per unit, and five articles are funded.
4.Clinical followed-up study of greater trochanter bone flap with the lateral femoral circumflex artery reconstruct femoral head

Objective To analyze retrospectively the clinical efficacy that the application with vascularized greater trochanter to reconstruct the collapse of femoral head necrosis.Methods Followed-up the data of 21 patients who underwent reconstruction of the femoral head from January,2008 to December,2012,by this we made the clinical and radiological assess,Harris hip scoring system was used to evaluate the situation of hip function.All patients were followed up regularly for X-ray film (after 3 months,6 months and then be reviewed once a year),by the film we assessed the bone healing and repair of the femoral head,and determined whether had the progress in phases.The clinical survival was decided by receiving or not the arthroplasty.Results All 21 patients were followed up for 24-72 months,with an average of 47 months.The postoperative Harris hip score was 82.8 ± 7.1 points,compared with the preoperative,the score improved significantly (average 52.4 ± 4.3 points for preoperative),the difference was statistically significant (P < 0.05).Based on Ficat stage for osteonecrosis,8 patients in this group had progress after 24 to 48 months (mean 33 months),whose femoral heads got collapse worsened.The radiographic success rate was 61.9%.Six cases received arthroplasty in 18 to 48 months (mean 34 months) due to progressive collapse of the femoral head or severe hip pain,or both.FicatⅢ contained 25% (4 hips),and Ⅳ had 40% (2 hips),the clinical survival rate was 71.4%.Conclusion The application with vascularized greate.r trochanter transfer coverage to rebuild the collapse of the femoral head in patients with osteonecrosis is a good way.It' s worthy to be applied.
5.The iliac periosteal flap with ascending branch of lateral femoral circumflex artery for the treatment of Legg-Calvé-Perthes disease

Objective To investigate the clinical effect of the iliac periosteal flap with ascending branch of lateral femoral circumflex artery for the treatment of Legg-Calvé-Perthes disease.Methods Followed-up 18 patients with Legg-Calvé-Perthes disease who had undergone the treatment of iliac periosteal flap with ascending branch of lateral femoral circumflex artery from January,2008 to May,2012 in our hospital.Minimum follow-up time was from 24 to 60 months,and the average time was 38 months.The followed-up period ended in May,2014.Assessed the effect of surgery by X-ray,clinical examination and Harris scoring system.Results Sixteen patients had good healing with iliac periosteal flap according to X-ray show.Their epiphyseal height recovered and the femoral head rounded gradually.Two cases became flat and collapse.The result of excellent and good rate was 88.9% by Mose method combined with the results of X-ray.The average Harris hip score improved from 72.3 ± 4.9 points preoperatively to 91.3 ± 2.4 points postoperatively.The difference of the result was statistically significant (P < 0.05).Conclusion The transfer of iliac periosteal flap with ascending branch of lateral femoral circumflex artery is an effective treatment for Legg-Calvé-Perthes disease.
6.Ultrasound-guided lauromacrogol sclerotherapy of simple liver cyst:analysis of its efficacy and safety

Objective To compare the efficacy and safety of lauromacrogol injection sclerotherapy with ethanol injection sclerotherapy in treating simple liver cysts. Methods A total of 166 patients with simple liver cyst were randomly divided into the lauromacrogol group (study group, n=86) and the absolute alcohol group (control group, n=80). Under ultrasonographic guidance, puncture aspiration of liver cyst was carried out in all patients, which was followed by injection of lauromacrogol for patients in the study group or injection of ethanol for patients in the control group. The therapeutic effect and the side-effect were evaluated. The results were compared between the two groups. Results No serious complications such as bleeding or infection occurred in both groups. During the therapeutic course , 45 patients (56.3%) in the control group felt pain to some degree and 23 patients (28.8%) developed drunk-like symptoms, while no patient in the study group felt any obvious discomfort. One week after sclerotherapy , 20 patients (25%) in the control group complained of distending pain on the right upper abdomen, while only 9 patients (10.5%) in the study group complained of pain, and the difference was statistically significant (χ2= 6.073, P < 0.05). Six months after the treatment, the cure rate of the study group and the control group was 95.7%and 93.5%respectively, and the difference between the two groups was no significant (P > 0.05). Conclusion For the treatment of liver cysts, lauromacrogol injection is safe and effective. Therefore, this technique should be recommended in clinical practice.
7.Optimization of Ethanol Extraction Technology for Xiaoluo Granules by Orthogonal Experiment

Objective:To optimize the ethanol extraction technology for Xiaoluo granules. Methods:The extraction technology for the ethanol dissolvable ingredients in 10 kinds of medicinal materials was optimized by orthogonal experiment. The weighted sum of ba-icalin content and the dry extract rate as the index, 4 influencing factors with 3 levels each were optimized by L9 (34 ) orthogonal table. Results:The optimal ethanol extraction conditions were as follows:the medicinal material was soaked by 10-fold 50% ethanol solution for 45min, and then extracted by heating reflux for 3 times with 1. 5h each time. Conclusion: The method can effectively extract the ethanol dissolvable ingredients in 10 kinds of medicinal materials, which can be used as the ethanol extraction technology for Xiaoluo granules.
8.Analysis of the incidence of hospitalized children with Henoch-Sch(o)nlein purpura and Henoch-Sch(o)nlein purpura nephritis

Objective To analyze the incidence of hospitalized children with Henoch-Sch(o)nle in purpura (HSP) and Henoch-Sch(o)nlein purpura nephritis (HSPN) from 2009 to 2012,and to characterize the epidemiology of HSP and HSPN in Jiangxi province.Methods Inpatients of Jiangxi Children's Hospital with the diagnosis of HSP or HSPN were recruited during 2009 to 2012.The basic messages in the home page of medical records,such as the admission year,sex,age,area coming from were collected.Chi-square test was used for statistical analysis.Results A total of 2516 HSP pediatric patients were included in this study.Of whom,412 cases were diagnosed in 2009,568 cases in 2010,750 cases in 2011,786 cases in 2012.Among them,renal damage as the presenting symptom in 110 cases in 2009,148 cases in 2010,198 cases in 2011,and 196 cases in 2012.The average incidence of HSPN was 25.91% (652/2516).The morbidity of HSPN were similar in different admission years (x2=0.62,P>0.05).In HSP patients,the ratio between male and female was 1.59∶1; the peak age was 4 to 9 year-old (65.50%,1648/2516).The morbidity of HSPN was similar in both girls and boy patients group (x2=0.14,P>0.05).The morbidity of HSPN was 18.76% (163/869) in patients younger than 6 years old,which was lower than 26.85%(359/1337) in patients between age 6 to 11 years old,and the morbidity of HSPN was 41.94% (130/310) in patients older than 11years old,the difference was significant (x2=65.24,P<0.01).The morbidity of HSP in the month of year was different,the peak time was between October and December,which as 40.74%(1025/2516).The morbidity of HSPN was 30.23%(208/688) during January to March,higher than in other period of the year (x2=9.87,P<0.05).In the last four years,there were 824(32.75%) hospitalized patients of HSP in Nanchang district,1692 cases (67.25%) in other areas in Jiangxi province.The morbidity of HSPN in Nanchang district was 17.35% (143/824),compared with the average level 25.91%(652/2516),the difference was significant (x2=25.08,P<0.01).Conclusion The number of children diagnosed with HSP is progressively increased from 2009 to 2012 in Jiangxi province.The peak period for HSP is between October and December,and the peak age is 4 to 9 year-old.The incidence of HSPN between January to March group is higher than in other periodof the year.The morbidity of HSPN is increased with age.
9.Attachment and proliferation of bone marrow mesenchymal stem cells cultured on porous tantalum rod

BACKGROUND:Porous tantalum rods, possessing high porosity and highly similar elastic modulus to the trabecular bone, not only can provide effective mechanical support for the femoral head, but also can enhance revascularization in the areas of necrosis, reduce stress shielding, and provide guarantee for bone ingrowth in necrotic area.
OBJECTIVE:To investigate the attachment and proliferation of bone marrow mesenchymal stem cells on the surface of porous tantalum rod.
METHODS:The purified bone marrow mesenchymal stem cells which were separated from canines were seeded on the surface of porous tantalum rod by the celldensity of 1.5×109/L. cellattachment and proliferation was observed under phase microscope and scanning electron microscope at 5, 10 and 15 days under co-cultured condition.
RESULTS AND CONCLUSION:(1) Inverted microscope:Within 1-5 days after co-culture, the cells began to proliferate, and less cells were found near the tantalum rod, but more cells far from the rod. At 6-10 days after co-culture, the cells increased significantly and gradual y migrated to the rod, and even some cells attached to the edges of the rod. After 14 days, the cells were interconnected to form a film, packing the surrounding and dents of the tantalum rod. cells were visible to agglomerate and cellcalcification occurred. (2) Scanning electron microscope:At 5 days after co-culture, no celladhesion appeared on the the tantalum rod surface. At 10 days after co-culture, the cells scattered on the surface of tantalum rod, but there was no interconnection between them. After 15 days, cells were polygon shaped and connected into pieces. It could be seen that the cells secreted large amounts of col agen fibers, and the cells that were fusiform and polygon shaped were surrounded by a great amount of extracellular matrice. Bone marrow mesenchymal stem cells appear to have good adhesion and proliferation capability on the surface of tantalum rod.
10.Apoptostic effect of arsenic trioxide on human coronary smooth muscle cells in vitro

Objective To investigate the effect of arsenic trioxide on human coronary smooth muscle cells(HCSMCs). Method After the subculture of HCASMCs, cells were divided into 4 groups: control group and three arsenic trioxide (4.0 μmol/L) groups as the arsenic agent co-cultured with cells for 12,24 and 48 h. Flow cytometry and TUNEL were used for counting the ntmnber of cell apoptosis. RT-PCR was used to detect the expression of E2F-1 mRNA and Western blot was used to measure the levels of Bcl-2 ond Bax. Results Arsenic trioxide inhibited the proliferation of HCASMCs. When arsenic trioxide was 4.0 μmol/L, the living cells were ( 8.44 ±0.10) × 105/mL,signifiantly than that of control group (16.44 ± 1.34) × 105/mL, P < 0.05. This inhibition of proliferation cycle was produced by affecting S and G2-M phase, which did not appear in the control group. Apoptotic cells increased from 16.0% ± 3.1% to 38.7% ± 2.7% (P < 0.05) detected by TUNEL. The arsenic agnet decreased the Bcl-2 level and increased Bax. The expression of E2F-1 mRNA was decreased in 4.0 μmol/L arsenic trioxide group. Conclusions Arsenic trioxide exerts the apoptotic effect on human coronary smooth muscle cells.
