1.Comparison of arteriosclerosis degree between middle-aged and elderly patients with chronic kidney disease

Objective To compare the degree of atherosclerosis in middle-aged versus elderly patients with chronic kidney disease (CKD),and to explore the relationship between inflammatory molecules in venous blood and atherosclerosis.Methods Totally 87 elderly patients and 64 middleaged patients with CKD were selected.The vein fasting blood samples were taken in the morning.Serum levels of interleukin (IL)-18,IL-6,tumor necrosis factor (TNF-α),high-sensitivity C-reactive protein (hs-CRP) were measured by enzyme-linked immunosorbent assay (ELISA) and ankle-brachial pulse wave conduction velocity (baPWV) was detected.Results Inflammatory molecules such as IL-18,IL-6,TNF-α and hs-CRP levels in venous blood were increased with the severity of CKD in both two groups,and the levels of inflammatory molecules were higher in elderly group than in middle-aged group.There was a signifiant difference in the detection rate of patients with baPWV>1400 cm/s between elderly group and middle-aged group [30 cases (34.5%) vs.11 cases (17.2%),x2 =5.58,P<0.05].The prevalence of PWV>1400 cm/s was higher in patients with hyperlipidemia and hypertension than in patients with diabetes or heart disease (elderly group:100%,100% vs.45.5%,40%; middle-aged group:80%,100% vs.28.6%,33.3%,respectively; P<0.05).The inflammatory protein (IL-18,IL-6,TNF-α and hs-CRP) levels were significantly higher in patients with baPWV> 1400 cm/s than those with baPWV < 1400 cm/s (P<0.05).Conclusions The degree of atherosclerosis is more severe in elderly CKD patients than in middle-aged CKD patients,which needs to be taken seriously enough.
2.Optimization of Ultrahigh Pressure Extraction of Forsythoside from Forsythia Suspensa by Box-Behnken Experimental Design

Objective: To optimize the ultrahigh pressure extraction ( UPE) process for forsythoside from Forsythia suspensa by Box-Behnken experimental design. Methods:On the basis of single factor screening, a three-factor and three-level Box-Behnken ex-perimental design was employed with liquid/material ratio( X1 ) , extraction pressure ( X2 ) and extraction time ( X3 ) as the independent variables. The response variable was the extraction yield of forsythoside. Results:The optimal UPE conditions for forsythoside were as follows:the ratio of solvent to material (ml·g-1) was 70, the extraction pressure was 151 MPa, and the extraction time was 114 s. With the optimal extraction process, the extraction yield of forsythoside was 13. 15 mg·g-1 . Conclusion:As a novel extraction tech-nology for Chinese herbal medicines, UPE procedure has higher extraction yield, lower extraction temperature, shorter extraction time and less power consumption, which provides a brand-new method for the extraction of forsythoside from Forsythia suspensa.

Objective To evaluate the clinical performance of INNOVANCE D-Dimer,and provide information for clinical application.Methods 402 cases of sodium citrate anticoagulant blood were tested with INNOVANCE assay and PLUS assay on CA7000 analyzer to measure plasma D-Dimer levels.VIDAS-30 immunology analyzer was also used to validate the two assays.4 patients with elevated D-Dimer were monitored continuously during 5 days using INNOVANCE assay and PLUS assay respectively,then the consistency of trend between 2 assays was analyzed.Plasma specimens added with hemoglobin,bilimbin and triglyceride were used to verify the anti-interference capability of INNOVANCE D-Dimer assay.Results In 402 specimens,the result ranges of INNOVANCE D-Dimer and PLUS D-Dimer were [2.15 (0.33,8.63)]mg/L FEU and [325.50 (123.75,974.00)] μ,g/L DDU,respectively.The consistency between two assays was poor (Z =-17.375,P =0.000),especially the results in the range of PLUS D-Dimer (201-300) μg/L DDU and (301-400) μg/L DDU,the coincidence rates were only 25% and 15%,respectively; the coincidence rate was up to 85% during PLUS D-Dimer (500-600) μg/L DDU; the coincidence rate was close to 100% when PLUS D-Dimer over 700 μg/L DDU.Totally 47 of 402 cases were unmatched between two assays.Verified by VIDAS 30,83.0% (39/47) was false negative for PLUS assay,4.3% (2/47) was false negative for INNOVANCE assay,12.7% (6/47) was false positive for PLUS assay.There were 5 false positives and 39 false negative for PLUS assay,totally 45 cases; Two false negative for INNOVANCE assay.Four patients with elevated D-Dimer were monitored and the results showed similar trend between 2 assays.For INNOVANCE assay,the capacity of anti-interference to free bilirubin,unconjugated bilirubin,hemoglobin,and triglyceride was up to 217 μmol/L,337 μmol/L,41.04 g/L,18.35 mmol/L,respectively.Conclusions INNOVANCE assay can markedly reduce false negative results of D-Dimer compared with PLUS assay.INNOVANCE D-Dimer has good performance on anti-interference to jaundice,hemolysis and lipemia samples.

6.Exploring the Pathogenesis of Lung Cancer from Xuanfu and Its Corresponding Syndrome Differentiation and Treatment Based on YE Tianshi's"Acrid"Theory

[Objective]To explore the pathogenesis of lung cancer from the perspective of Xuanfu,and to analyze the theoretical basis and clinical significance of YE Tianshi's acrid theory in the treatment of lung cancer.[Methods]According to the theory of Xuanfu,this paper discussed its influence on the occurrence,development and metastasis of lung cancer from three aspects:the loss of opening and closing of Xuanfu,the loss of"Xuanfu-collaterals"and the loss of"Xuanfu Qi-liquid".By reading YE Tianshi's A Guide to Clinical Practice with Medical Records,YE Tianshi's"acrid"theory and its differentiation and treatment experiences for cancer were summarized,and demonstrated it with a medical record.[Results]Xuanfu opening and closing loss,collaterals loss,Qi and liquid loss,the body's healthy Qi deficiency,Qi stagnation,blood stasis,phlegm and toxin interaction,led to the occurrence and development of lung cancer;YE Tianshi was good at using acrid drugs in the treatment of cancer,and advocated that acrid drugs could dispel wind,move Qi,dissolve phlegm,break concretions and tonify deficiency.In the elderly patients mentioned in the medical records,lung cancer recurred due to deficiency of Qi and stagnation of Xuanfu,phlegm and blood stasis.Therefore,the treatment of acridity was given,and lung cancer was alleviated.[Conclusion]YE Tianshi's"acrid"theory is suitable for different stages of lung cancer treatment by regulating the opening and closing of Xuanfu,which is worthy of clinical promotion.
7.Imaging features of the initial chest high resolution CT scan in juvenile patient with coronavirus disease 2019

The coronavirus disease 2019 (COVID-19) was confirmed by nucleic acid test in 15 juvenile patients aged 4-17 years in Hangzhou Xixi Hospital from January 24 to February 10, 2020. The clinical data and the initial chest high resolution CT (HRCT) findings were retrospectively analyzed.Among 15 cases, 8 patients with a mean age of (6.5±2.3) years (4-11 year) had normal HRCT manifestations (mild disease), while 7 patients with a mean age of (11.9±3.0) years (7-17 years) showed abnormal manifestations (moderate disease). In the 7 moderate cases the CT findings included local thickening of bronchial wall in 2 cases, single lesion in 1 case, bilateral pulmonary lesions in 4 cases. There were total 11 lesions, 6 of which were ground glass and slightly high-density nodules, 5 of which were speckled ground glass and 2 with a little consolidation. There were 10 subpleural lesions, including 7 in the lower lobe of the two lungs, and 1 non-subpleural lesion; the thickened local bronchioles were observed in 3 cases. None of them had hilum and mediastinal lymph node enlargement or pleural effusion.
8.Immunological pathology mechanism research of cervical cancer based on tumor related macrophage

AIM:To analyze the mechanism of tumor-associated macrophage (TAMs) in the development of cervical cancer and to investigate its correlation with Th1/Th2 and Th17/CD4+ CD25+ Foxp3 + Treg.METHODS:Twenty seven cases of cervical cancer and 53 cases of cervical intraepithelial neoplasias (including 22 cases of CIN Ⅱ and 31 cases of CIN Ⅲ) were selected as subjects.The venous blood of patients before treatment was extracted to detect Th1/Th2 and Th17/CD4 + CD25 + Foxp3 + Treg with flow cytometry,and detect serum IFN-γ,IL-4,IL-17A,IL-17F,TGF-β1 and IL-10 levels with ELISA Kits.Furthermore,pathological tissues were extracted during operation,and its TAMsCD68 expression was detected by immunohistochemistry technique.RESULTS:The Th1/Th2 and Th17/CD4 + CD25 + Foxp3 + Treg of cervical cancer were both lower than those of CIN Ⅲ,and those of CIN Ⅲ were both lower than CIN Ⅱ,the difference between groups had statistical significance (P < 0.05).The serum IFN-γ,IL-4,IL-17A,IL-17F,TGF-β1 and IL-10 levels of cervical cancer were all higher than those of CIN Ⅲ,and those of CIN Ⅲ were all higher than CIN Ⅱ,the difference between groups had statistical significance (P < 0.05).The TAMsCD68 expression level of cervical cancer was higher than that of CIN Ⅲ,and that of CIN Ⅲ was lower than CIN Ⅱ,the difference between groups had statistical significance (P < 0.05).The correlation analysis results showed TAMsCD68 expression level had negative correlations with Th1/Th2,Th17/CD4+CD25+ Foxp3+ Treg,and serum IL-17A level,whereas presented positive correlations with serum IL-10 and IL-4 level (P < 0.05).CONCLUSION:TAMs is closely related with Th1/Th2 and Th17/CD4 + CD25 + Foxp3 + Treg in cervical cancer,and possibly is mediating the occurrence and development of cervical cancer through influencing the balance of these two systems.
9.Proximal gastric cancer: lymph node metastatic patterns according to different T stages dictate surgical approach.

BACKGROUNDAs a common form of gastric cancer migration, lymph node metastasis largely affects the surgical treatment and prognosis of gastric cancer. Surgery is the fundamental curative option for gastric cancer that varies depending on different stages. The study aimed to compare the clinicopathological characteristics and lymph node metastatic patterns in patients of proximal gastric cancer with different T stages and investigate a reasonable radical gastrectomy approach in terms of the range of lymphadenectomy for proximal gastric cancer.
METHODSIn our retrospective study, the data of 328 patients of proximal gastric cancer with different T stages were analyzed. By comparing the differences of lymph node metastatic rate and ratio, we investigated the clinicopathological characteristics and metastatic patterns of lymph nodes. Also, we were especially interested in the differences in survival rates between patients with and without No. 5 and 6 group metastasis with the same TNM stage.
RESULTSThe overall lymph node metastatic rate and ratio of advanced proximal gastric cancer were 73.4% and 23.3%, respectively. The tumors of different T stages were statistically significant in size and differentiation degree (P < 0.05), multivariate analysis showed that the depth of tumor invasion was an independent risk factor for lymph node metastasis in proximal gastric cancer (RR, 12.025; 95% CI, 2.326 to 62.157; P = 0.003). The overall survival rate of patients with No. 5, 6 group lymph node metastasis and those without was significantly different, but the differences in survival rates between patients with and without No. 5 and 6 group metastasis with the same TNM stage were not statistically significant.
CONCLUSIONSDifferent T stages in proximal gastric cancer showed different patterns and characteristics of lymph node metastasis. D2 lymphadenectomy in patients with early gastric cancer had little survival benefit because metastasis to level 2 nodes was rare. Therefore the range of the lymph node dissection in radical gastrectomy for early gastric cancer was considered reasonable. Moreover, to meet the requirements of the lymph node dissection, total gastrectomy plus D2 lymphadenectomy or more are supposed to be applied for the advanced proximal gastric cancer patients. Precise T staging largely determines the range of gastrectomy and lymphadenectomy.
