Objective To observe the diuretic effect of Dextrans on primary nephritic syndrome. Methods In adoutble-blind placebo-controlled study,60 cases of primary nephritic syndrome patients were randomly devided into three groups and received by intravenous administration for 60 minutes for three days: (1)FU(1mg/kg) combination with as ham infusion;(2)FU(1mg/kg) combination with 50ml of 20% solution of albumin; (3)FU(1mg/ kg) combination with 250ml dextran. 2.Psychological effects of nursing interference on systemic lupus erythematosus patients after pregnancy termination

Objective To study the psychological features of systemic lupus erythematosus (SLE) women who underwent pregnancy termination and to investigate the effects of nursing interference on their psychological state.Methods Psychological nursing and symptomatic nursing were performed in 12 patients.The psychological state was evaluated by Self Rating Anxiety Scale (SAS)and Self-rating Depression Scale (SDS) before and after the interference.Results SAS and SDS score were remarkably reduced after nursing interference.Conclusions Nursing interference can reduce the negative effects caused by psychological state,reduce their psychological pressure,increase compliance to medical orders and promote recovery.
3.Comparison of arteriosclerosis degree between middle-aged and elderly patients with chronic kidney disease

Objective To compare the degree of atherosclerosis in middle-aged versus elderly patients with chronic kidney disease (CKD),and to explore the relationship between inflammatory molecules in venous blood and atherosclerosis.Methods Totally 87 elderly patients and 64 middleaged patients with CKD were selected.The vein fasting blood samples were taken in the morning.Serum levels of interleukin (IL)-18,IL-6,tumor necrosis factor (TNF-α),high-sensitivity C-reactive protein (hs-CRP) were measured by enzyme-linked immunosorbent assay (ELISA) and ankle-brachial pulse wave conduction velocity (baPWV) was detected.Results Inflammatory molecules such as IL-18,IL-6,TNF-α and hs-CRP levels in venous blood were increased with the severity of CKD in both two groups,and the levels of inflammatory molecules were higher in elderly group than in middle-aged group.There was a signifiant difference in the detection rate of patients with baPWV>1400 cm/s between elderly group and middle-aged group [30 cases (34.5%) vs.11 cases (17.2%),x2 =5.58,P<0.05].The prevalence of PWV>1400 cm/s was higher in patients with hyperlipidemia and hypertension than in patients with diabetes or heart disease (elderly group:100%,100% vs.45.5%,40%; middle-aged group:80%,100% vs.28.6%,33.3%,respectively; P<0.05).The inflammatory protein (IL-18,IL-6,TNF-α and hs-CRP) levels were significantly higher in patients with baPWV> 1400 cm/s than those with baPWV < 1400 cm/s (P<0.05).Conclusions The degree of atherosclerosis is more severe in elderly CKD patients than in middle-aged CKD patients,which needs to be taken seriously enough.
4.Piezoelectric Immunoglobulin Microarray Immunosensor

OBJECTIVE To develop new 2?5 type of piezoelectric immunoglobulin microarray immunosensor for determination of immunoglobulin.The energy converters are 10MHz AT-cut quartz crystals with gold-coated(electrodes).The monoclonal antibodies of immunoglobulin are immobilized onto the surfaces of crystals gold(electrodes) by staphylococcal protein A(SPA).METHODS The standard substance and serum were detected to find the detection time,temperature and specificity of the immunosensor.RESULTS The piezoelectric quartz(crystal) immunoglobulin microarray immunosensor could complete the detection in 15 min without nonspecific(response).Under the optimized conditions,the experimental results showed that the piezoelectric immunosensor had good(response) to immunoglobulin whose frequency shifts were linearly dependent on immunoglobulin (concentration) in different range.The piezoelectric microarray immunosensor had been used to detect(immunoglobulin) in serum,the analytical results given by this method were in satisfactory agreement with those given by traditional (immunoassay),with correlation coefficient of 0.94,0.94,0.94,0.92 and 0.91.(CONCLUSIONS) Piezoelectric microarray immunosensor for the determination of immunoglobulin is of high(sensitivity),high specificity,high analysis speed,unnecessary labelling,simple operation,real-time detection,(repeated) use,etc.It is suitable for (detecting) of immunoglobulin and should be used for clinical detection.
5.Detection of Mycobacterium tuberculosis by Fluorescence Chip of Molecular Beacon Probe

OBJECTIVE To have a research on the detection of Mycobacterium tuberculosis by fluorescence chip of molecular beacon probe.METHODS Fluorescence chip for the detection of M.tuberculosis was designed,based on the better bonding force and specificity of molecular beacon and target molecule.Hybridization efficiency was optimized,which included the concentration and purity of molecular beacon probe,hybridization temperature,hybridization time,the prescription of hybridization solution and the comparing of molecular beacon fluorescence signals of 4 molecules.RESULTS Molecular beacon fluorescence signal approached the highest when the final concentration of Mg was 5 mmol/L and the amplification efficiency of PCR approached the highest.Half extended molecular beacon probe could bind nice with chip when it was middle labeled with quench group.The results of 3 hybridization solutions were as follows: 0.15% SDS better than 0.2% SDS,10?SSC better than 5?SSC.Strong signal of hybridization reaction could be detected when the concentration of Cy3-BDH molecular beacon probe was 15 ?mol/L,the concentration of FAM-DABCLYE probe was 9 ?mol/L and the reaction time was 7 h.The efficiency of fluorescence quenching was higher when the molecule was consisted of Cy3-BDH,other than Cy3-DABCLYE.CONCLUSIONS The design of half extended molecular beacon probe tactfully makes use of the advantages of the high bonding force and specificity of molecular beacon probe on the chip technology.Fluorescence chip of molecular beacon for M.tuberculosis has satisfactory detecting rate,specificity and sensibility.
6.Effect of affected side upper limb pillow position on hemodynamic and safety of patients undergoing cerebellopontine angle tumor resection

Objective:To investigate the effect of upper limb pillow position on hemodynamic and safety of patients undergoing cerebellopontine angle tumor resection.Methods:Eighty-four patients receiving cerebellopontine angle tumor resection in our hospital from January 2016 to December 2019 were randomly divided into the experimental group (42 cases) and the control group (42 cases). Patients in the control group were placed in routine upper limb position, while patients in the experimental group were placed in upper limb pillow position. The data including systolic pressure, diastolic pressure, heart rate, saturation of blood oxygen were recorded on admission of operation room, completing placing body position, 30 minutes and 60 minutes after operation and after finishing the operation. The numbness/soreness of upper limbs and pressure injury rate was compared between the experimental group and the control group.Results:The rate of numbness/soreness of upper limbs were 2.4% (1/42) in the experimental group, 19.1%(8/42) in the control group, the differences were statistically significant ( χ2 value was 6.098, P<0.05). The stage 1 pressure injury were 2 cases in the experimental group, stage 1 and 2 pressure injury were 6 cases and 2 cases, respectively in the control group, the differences were statically significant ( Z value was 2.039, P<0.05). Conclusion:Upper limb pillow position of the operation side can reduce postoperative complication of patients undergoing cerebellopontine angle tumor resection, but will not increase the risk of abnormal hemodynamic fluctuation.
7.A comparative study of two methods to quantify Z scores of aortic root diameters measured in children by echocardiography

Objective To investigate the calculation method and its inlfuencing factors of Z scores in the aortic root diameters measured by echocardiography in children. Methods A total of 105 children with median age 19 months, who came to Shenzhen Children′s Hospital from March 2012 to October 2012 were included. The diameters of aortic ring (ARD) and aortic sinus (ASD) were measured by two dimension echocardiography, Z scores of ARD and ASD were calculated using two different normal reference values regression equation and mean square error derived from Shenzhen children′s hospital (C method) and Pettersen et al (P method). The regression equation from C method and body surface area (BSA) formula from P method were adopted to calculate Z scores of ARD and ASD (ZH method). The Z results of ARD and ASD calculated by those three methods were compared and were analyzed for their normality probability distributions. Results Z scores of ARD and ASD derived from C method were all showed as normal distribution (P=0.067 and 0.650). Z scores of ARD and ASD derived from P method were all showed as normal distribution (P=0.208 and 0.970). Z score of ARD derived from ZH method was showed as non-normal distribution (P=0.027), but Z score of ASD was normal distribution (P=0.430). There were no significant differences in ARD-Z calculated by C method (0.41±0.89), P method (0.23±0.85) and ZH method (0.36±0.94) (F=1.117, P=0.309). There were signiifcant differences in the Z scores of ASD calculated by C method (0.38±0.89), P method (0.58±0.71) and ZH method (0.36±0.84) (F=5.443, P=0.005). Z scores of ARD (r=0.917, P=0.000) and ASD (r=0.900, P=0.000) calculated by C method correlated well with that by P method. Conclusions Calculation method of BSA and normal reference values regression equation were the main influencing factors of Z score value in quantifying children aortic root diameters by echocardiography. For the clinical applications. The normal reference value should be used which is suitable for the Chinese children.
8.The Z scores value of cardiac structure by preoperative echocardiography in children with tetralogy of Fallot

Objective To evaluate the value and clinical application of cardiac structure Z scores in children with preoperative tetralogy of Fallot (TOF) by echocardiography. Methods Considering the result of 31 patients who underwent echocardiography in Shenzhen Children’s Hospital before TOF repair from January 2011 to September 2013, the median age was 10 months (range:3 days to 14 years). Other 31 normal children were selected as normal control group with age-and sex-matched, the median age was 10 months (range: 6 days to 13 years). Echocardiographic measurements included the diameter of right ventricular outflow tract (RVOT), pulmonary valve diameter (PVD), left pulmonary artery diameter (LPAD), right pulmonary artery diameter (RPAD), pulmonary valve peak velocity (PV-Vmax), tricuspid annulus diameter (TVD), mitral annulus diameter (MAD), left ventricular end-diastolic volume (LVEDV) and left ventricular mass (LVmass). Shenzhen Children’s Hospital pediatric echocardiography normal reference values and the regression equation are applied to obtain the forecast average, and using the Z-score formula into the standard value (Z=[M-y]/ MSE ). Results There was no significant difference in body surface area between the TOF group and control group (P>0.05). Compared with control group, the true value of RVOT reduced significantly in TOF group, its Z-score showed negative values increase (-4.87±1.86 vs 0.33±0.93, t=-13.90, P<0.05). PVD, LPAD and RPAD reduced significantly in TOF group, those Z scores showed a negative value increase (-4.15±2.93 vs 0.11±0.85, t=-7.76;-1.35±2.38 vs 0.66±1.17, t=-4.24;-1.47±2.03 vs 0.59±1.04, t=-4.94;all P<0.05). While PV-Vmax increased significantly, and it’s Z score showed a positive value increase (8.38±0.19 vs 0.24±0.98, t=29.50, P<0.05). TVD of TOF patients was slightly larger than the control group (P=0.10), but the Z score was significant greater than the control group (1.00±1.17 vs 0.29±0.52, t=3.06, P<0.05). MV-D, LVEDV and LVmass of TOF patients were slightly decreased than the control group (all P>0.05), but the Z scores of TOF were significantly reduced than the control group (0.25±0.78 vs 0.76±0.65, t=-2.75;-0.92±1.94 vs 0.03±1.01, t=-2.41;-0.83±1.59 vs 0.67±0.69, t=-4.71; all P<0.05). The Z scores of RVOT, PVD, LPAD and RPAD had no significant difference between transannular patch surgery and pulmonary valve-sparing repair patients (-4.89±2.03 vs -4.84±1.67, t=-0.08; -4.73±3.49 vs -3.34±1.75, t=-1.31; -1.88±2.54 vs -0.62±2.00, t=-1.49;-1.89±1.90 vs-0.84±2.15, t=-1.41;all P>0.05). For the Z scores of PV-Vmax, there were no significant difference between transannular patch surgery and pulmonary valve-sparing repair (8.58±1.20 vs 8.12±1.16, t=1.07, P>0.05). Conclusions The Z scores value of cardiac structure can be used in quantitative analysis of RVOT obstruction and development of left ventricular backward of TOF. The Z score of transannular patch surgery is more lower than pulmonary valve-sparing repair patients. The normalized Z score by body surface area correction is important for the preoperative diagnosis and the surgical planning.
9.Construction of anti-lipopolysaccharide single-chain phage antibody library in mice

Objective To construct an anti-LPS single chain phage antibody library in mice for further biomedical works. Methods Total RNA was extracted from splenic cells for reverse transcription after BALB/C mice had been immunized with pure LPS for 4 weeks. The designed primers were used to amplify the variable region genes of both heavy and light chain (VH,VL) with polymerase chain reaction. The VH and VL were then conjugated to form a single chain of variable fragment (ScFv) by a linker. The ScFv was cut by NotⅠ and SfiⅠ and then ligated into a pCANTAB5E phagemid vector which was digested with the same restriction enzymes. The ligated vector was then tranfected into the competent E.coli TG1 cells. Four TG1 clones were randomly selected to detect the exotic DNA. Results The titer of anti-LPS in murine sera was 1∶12 800. The concentration of total RNA was 12.3813 μg/ml. The length of the fragments were 340 bp for VH, 320 bp for VL and 800 bp for ScFv respectively. 1.9×107 clones were grown after transfection and one from the four randomly selected clones was identified to contain the exotic DNA. Conclusion A 4.75×106 murine anti-LPS single chain phage antibody library was successfully constructed.
10.Impact of supine recumbency duration on the complications after lumbar puncture in children

Objective To evaluate the post-puncture complications in children,and to evaluate the difference in the occurrence rate of headache and backache between patients who had 1 hour of supine recumbency and those who had 4 hours of supine recumbency,to provide evidence for the standardized lumbar puncture (LP) procedure of children.Methods Inpatients who were older than 3 and had diagnostic LP in the course of their treatment between Nov.2012 and Apr.2013 were enrolled.The LP was performed by the same investigator under the standardized institutional guideline for LP and the information of number of LP attempts,duration of LP and cerebrospinal fluid (CSF) drainage volume were recorded.After the procedure,all the children were randomly assigned into 2 groups:the test group had supine recumbency for 1 hour after LP,and the control group had it for 4 hours.For the first 5 days following LP,reports of headache and backache were recorded by another physician who didn't participate in this study.The difference in the occurrence rate of headache and backache between test group and control group was assessed by the chi-square test or Fisher's probabilities in 2 × 2 table.And a Logistic regression analysis was performed to assess the risk factors for the occurrence of the headache and backache.Results The overall frequency of headache was 4.6% (4/87 cases).The frequency of headache was not significantly different between the test group (2.4%) and control group (6.7%) (P =0.617).And the overall frequency of backache was 19.5 % (17/87 cases),and it was not significantly different between the 2 groups (test group:21.4%,control group:17.8 %,P =0.668).In a Logistic regression analysis,age (P =0.011,OR:6.884,95% CI:1.398-33.906) and the previous history of lumbar puncture (P =0.018,0R:0.126,95 % CI:0.026-0.618) were significant risk factors for the occurrence of backache.The risk of backache in children with more than 2 times of LP was decreased.And the risk was higher in the children older than 6 years than those younger than 6 years.Conclusions Headache and backache were the most frequent post-puncture complications in children.There was no difference between short duration (1 hour) of supine recumbency and long duration (4 hours) in preventing the occurrence of headache and backache after LP.
