1.Effect of Herbs-partition Moxibustion on Cytokines in UC Rats

Objective to observe the effect of herbs-partition moxibustion oncytokines,skuch as IL-1,IL-6,TNF-in rats with ulcerative colitis.Method The animal models were created by immunological method. They were randomly divided into 5 groups. After treated by different methods,IL-1,IL-6 and TNF-in colon tissue of UC rats were detected with immunity technique.Result In control group,there was a little or no masculine cell of IL-1β in colon mucosa.The number of masculine cell of IL-1 in model group was much larger.Compared with model group,IL-1 masculine cells in mildness moxibustion group markedly decreased(P<0.05), and those in electro-acupuncture group and herbs-partition moxibustion group decreased markedly even more(P<0.01). The difference between the last two groups was evident(P<0.05). TNF-masculine cells are not or seldomly found in UC rats, but in model group, the number of them increased evidently, which were mainly cytoplasm of colon reticular layer macrophage. Compared with those in model group, the masculine cells decreased markedly(P<0.01), the number in mildness group and in herbs-partition moxibustion decreased even more(P<0.01). There were statistically significant differences between herbs-partition moxibustion group and electro-acupuncture group(P<0.05). Conclusion The possible mechanism of acupuncture and moxibustion was to reduce the expression of IL-I, IL-6 and TNF by inhibiting the activation of macrophage and thereby blocking their further activation.
2.RELATION OF SEVERITY OF LOW BACK PAIN TO TYPE OF PROLAPSED INTERVERTEBRAL DISC

In this study the relation between the extent and type of disc protrusion and the clinical severity of symptoms of low back pain was observed. Our series consisted of 300 consecutive cases of lumbar disc protrusion,with their diagnosis confirmed by intraoperative observation. Preoperative pain severity was assessed using a back pain scale that combined the symptoms of back pain and sciatica. Intraoperaative pathological findings of disc were documented and comparesd with preoperative symptoms and signs. The pathological changes of disc were classified as following four subtypes: bulging disc (85 cases), protruding disc (104 cases), extruded disc (72 cases) and sequestered disc (39 cases). Our results suggest that severity of pain in patients with herniated lumbar disc is related to position of herniation rather than magnitude and exent of herniation. Inflammed intraspinal tissues are significantly associated with low back pain and sciatica.

Cardiovascular disease caused by hypertension has become the first cause of death of residents in our country, the burden of disease has become more and more serious, how to manage hypertension has become an important public health issue in our country. In this review, the development and current situation of the community health management of hypertension at home and abroad are described and analyzed, the experience and practice worthy of learning are summarized to provide evidence for the prevention and control of hypertension in community.
4.Clinical research of the relationship between 5-lipoxygenase activating protein gene polymorphism and cerebral infarction susceptibility

Objective To investigate the expression of 5-lipoxygenase activating protein gene (ALOX5AP) polymorphism in the patients with cerebral infarction,and explore its relationship with cerebral infarction susceptibility.Methods Patients with cerebral infarction and healthy volunteers were selected for this study,whose venous blood was extracted and detected with polymerase chain reaction/restriction fragment length polymorphism (PCR-RFLP).Haplotype A (SG13S114T,SG13S89G,SG13S32A,SG13S25G),haplotype B (SG13S377A,SG13S114A,SG13S41A,SG13S35G),and their nucleotide polymorphism loci were observed.Results Single nucleotide polymorphism (SNP)-SG13S114,SNP-SG13S32 and HapA carrying rate were significantly different between patients with cerebral infarction and healthy volunteers (P <0.05).SNP-SG13S114 and SNP-SG13S32 were independent risk factors of cerebral infarction (OR > 1.0,P < 0.05).Conclusions The morbidity of cerebral infarction in Wenling City was influenced by SNP-SG13S114,SNP-SG13S32,and HapA carrying rate.
5.Design and clinical application of the instrument for percutaneous posterolateral lumbar foraminoplasty

ObjectiveTo introduce the design of the instrument for percutaneous posterolateral foraminoplasty and analyze long-term outcomes of percutaneous foraminoplasty and transforaminal endoscopic discectomy in the treatment of non-contained lumbar disc herniation.MethodsFifty-six patients with noncontained lumbar disc herniation confirmed by symptom,physical sign and concordant imaging underwent percutaneous foraminoplasty and transforaminal endoscopic discectomy,including 7 cases of L3-4,30 of L4-5 and 19 of L5S1.The visual analogue scales(VAS) of pre- and post-operative low back pain and sciatica were compared,and the Macnab scores were also evaluated.ResultsAll of the procedures were performed successfully,with mean operation time of 60 min(range,40-120 min),and a mean blood loss of 30 ml (range,20-50 ml).The follow-up time was more than 36 months.Postoperative VAS of low back pain and sciatica were significantly decreased compared with preoperative VAS (P<0.01).There were 44 cases of excellent,10of good,and 2 of fair according to Macnab score system,with total successful rate (excellent and good) up to 96.4%.Only 5 cases with L5S1 disc herniation were found complicated with sun-burn syndrome,which were relieved by pulsed electro-stimulant therapy for 1 week.ConclusionPercutaneous foraminoplasty and transforaminal endoscopic discectomy is an effective and safe minimally invasive treatment alternative for non-contained lumbar disc herniation.
6.Studies of fine needle aspiration cytology and histopathology of non-Hodgkin lymphoma in breasts

Objective:To investigate the clinic-cytology and histopathology of non-Hodgkin lymphoma (NHL) in breasts. Mothods:Twelve cases of NHL of the breast were studied by fine-needle aspiration biopsy (FNAC), histopathology and immunohistochemistry, and whose clinical data were analysed at the same time. Results:In 12 cases of NHL, 3 cases were T-cell NHL and 9 cases were B-cell NHL. Cytologically, the T-cell NHL cells were mostly arranging in diffuse patterns. The tumor cells were oval and pleomorphism. Some of them had distorted nucleus and thin nuclear envelope.The nucleus showed irregular course chromatin and visible nueleoic. Histopathologically, some of the tumor cells distributed around the blood vessels, and there was an obvious phenomenon of “blood-vessel-closing”. B-cell lymphoma cells were arranginy in diffuse pattern, and showed round and ellipse in shape with a clot and course granular chromatin and visible nueleoic and karyoknesis. Lymphoepithelia lesions were seen. Immunohistochemistry showed that CD3, CD43, and CD 45RO in T-cells NHL were positive.CD20, CD74 and CD79a in B-cells NHL were negative. CK, EMA, ER and PR in NHL were all negative. Conclusion:NHL of breast is extremely rare, and its definite diagnosis depends on various examination methods.
7.The use of unilateral external fixators in treatment of distal shaft fractures of humerus

Objective To retrospectively analyze the results of unilater al external fixators in the treatment of the distal third fractures of humeral s hafts. Methods From October 1997 to October 2002, 33 patients from 15y-70y (ave rage 31y) with the distal third fractures of humeral shafts were treated with un ilateral external fixators. They included 3 transverses fractures, 1 oblique fra cture, 9 spiral fractures and 20 comminuted fractures. 15 cases were treated wit h open reduction and fixation with external fixators and limited internal fixati on, 10 cases were treated with open reduction and fixation with external fixator s, and 9 cases were treated with closed reduction and fixation with external fix ators. 9 cases with radial nerve injury undewent nerve exploration. External fix ators were removed after bone healing. Duration of follow-up was 8 to 24 m (ave rage 18 m). Results The time of bone healing was 11 to 22 w (average 14 w). At t he latest follow-up, the function of injured radial nerves in 9 preoperative ca ses and 2 postoperative cases and the motion function of elbow were restored to normal. There were only 7 cases of superficial pin site infection which were cur ed with oral antibiotics and pin site care with mild disinfectants. Conclusion U nilateral external fixators can play special roles and have good results in the treatment of the distal third fractures of humeral shafts, combined with open re duction and limited internal fixation.
8.The application of ThinPrep cytology in the diagnosis of superficial tumors

Objective: To investigate the advantage of ThinPrep cytological test in tumor diagnosis by fine needle aspiration cytology. Methods: Using fine needle aspiration for smears,some of smears were made in conventional smears,the remains aspirating needles were washed with a transport washing solution.ThinPrep slides were made using the ThinPrep Processor. Results: In 84 cases FNA samples,49 cases were breast,29 cases were lymph node and 6 cases were others.Comparing with conventionally prepared specimens,ThinPrep processed samples showed the advantage in sensitivity or specificity.The lack of cellular debris,blood and exudates improve the ability to detect cellular abnormalities and speed up the time required for screening in ThinPrep processed samples.Rapid cell transfer from the sampling device to the transporing fluid containing the fixative ensure preservation of cell details with minimal artifactual changes. Conclusion: The use of Thinprep and by combination of FNAC and immunocytochemistry cytopathological diagnosis can be improved significantly.

10.Management of biliary complications following orthotopic liver transplantation

Objective To evaluate the management of biliary complications (BC) following orthotopic liver transplantation (OLT). Methods From Feb 1999 to Feb 2004, 236 cases underwent OLT with end-to-end choledocho-choledochostomy. Biliary anastomosis was performed by intermittent suture with T tube placement in 96 cases, without T tube in 39 cases, by continuous suture in posterior wall and intermittent suture in anterior wall and without T tube in 101 cases. Results Thirty-two (13.3%) patients developed BC, with incidences in group 1, 2 and 3 of 17.7%, 15.4% and 7.9%, respectively. The incidence of hepatic hilar and/or intrahepatic bile duct strictures was 8.3%, 2.6% and 1.0%, respectively. BC incidence in group 3 significantly decreased. Twenty patients with biliary stricture underwent endoscopic and/or radiological interventions, and stricture resolution was achieved in 90% of patients with anastomotic strictures and 60% of patients with hepatic hilar and/or intrahepatic strictures. Conclusions Modified biliary tract reconstruction technique contributes to the decrease of BC. Endoscopic and/or radiological interventions should be used for non-ischemic anastomotic biliary strictures or simple hepatic hilar strictures.
