1.Effect of Herbs-partition Moxibustion on Cytokines in UC Rats

Objective to observe the effect of herbs-partition moxibustion oncytokines,skuch as IL-1,IL-6,TNF-in rats with ulcerative colitis.Method The animal models were created by immunological method. They were randomly divided into 5 groups. After treated by different methods,IL-1,IL-6 and TNF-in colon tissue of UC rats were detected with immunity technique.Result In control group,there was a little or no masculine cell of IL-1β in colon mucosa.The number of masculine cell of IL-1 in model group was much larger.Compared with model group,IL-1 masculine cells in mildness moxibustion group markedly decreased(P<0.05), and those in electro-acupuncture group and herbs-partition moxibustion group decreased markedly even more(P<0.01). The difference between the last two groups was evident(P<0.05). TNF-masculine cells are not or seldomly found in UC rats, but in model group, the number of them increased evidently, which were mainly cytoplasm of colon reticular layer macrophage. Compared with those in model group, the masculine cells decreased markedly(P<0.01), the number in mildness group and in herbs-partition moxibustion decreased even more(P<0.01). There were statistically significant differences between herbs-partition moxibustion group and electro-acupuncture group(P<0.05). Conclusion The possible mechanism of acupuncture and moxibustion was to reduce the expression of IL-I, IL-6 and TNF by inhibiting the activation of macrophage and thereby blocking their further activation.
2.Design and clinical application of the instrument for percutaneous posterolateral lumbar foraminoplasty

ObjectiveTo introduce the design of the instrument for percutaneous posterolateral foraminoplasty and analyze long-term outcomes of percutaneous foraminoplasty and transforaminal endoscopic discectomy in the treatment of non-contained lumbar disc herniation.MethodsFifty-six patients with noncontained lumbar disc herniation confirmed by symptom,physical sign and concordant imaging underwent percutaneous foraminoplasty and transforaminal endoscopic discectomy,including 7 cases of L3-4,30 of L4-5 and 19 of L5S1.The visual analogue scales(VAS) of pre- and post-operative low back pain and sciatica were compared,and the Macnab scores were also evaluated.ResultsAll of the procedures were performed successfully,with mean operation time of 60 min(range,40-120 min),and a mean blood loss of 30 ml (range,20-50 ml).The follow-up time was more than 36 months.Postoperative VAS of low back pain and sciatica were significantly decreased compared with preoperative VAS (P<0.01).There were 44 cases of excellent,10of good,and 2 of fair according to Macnab score system,with total successful rate (excellent and good) up to 96.4%.Only 5 cases with L5S1 disc herniation were found complicated with sun-burn syndrome,which were relieved by pulsed electro-stimulant therapy for 1 week.ConclusionPercutaneous foraminoplasty and transforaminal endoscopic discectomy is an effective and safe minimally invasive treatment alternative for non-contained lumbar disc herniation.
3.Clinical research of the relationship between 5-lipoxygenase activating protein gene polymorphism and cerebral infarction susceptibility

Objective To investigate the expression of 5-lipoxygenase activating protein gene (ALOX5AP) polymorphism in the patients with cerebral infarction,and explore its relationship with cerebral infarction susceptibility.Methods Patients with cerebral infarction and healthy volunteers were selected for this study,whose venous blood was extracted and detected with polymerase chain reaction/restriction fragment length polymorphism (PCR-RFLP).Haplotype A (SG13S114T,SG13S89G,SG13S32A,SG13S25G),haplotype B (SG13S377A,SG13S114A,SG13S41A,SG13S35G),and their nucleotide polymorphism loci were observed.Results Single nucleotide polymorphism (SNP)-SG13S114,SNP-SG13S32 and HapA carrying rate were significantly different between patients with cerebral infarction and healthy volunteers (P <0.05).SNP-SG13S114 and SNP-SG13S32 were independent risk factors of cerebral infarction (OR > 1.0,P < 0.05).Conclusions The morbidity of cerebral infarction in Wenling City was influenced by SNP-SG13S114,SNP-SG13S32,and HapA carrying rate.
4.RELATION OF SEVERITY OF LOW BACK PAIN TO TYPE OF PROLAPSED INTERVERTEBRAL DISC

In this study the relation between the extent and type of disc protrusion and the clinical severity of symptoms of low back pain was observed. Our series consisted of 300 consecutive cases of lumbar disc protrusion,with their diagnosis confirmed by intraoperative observation. Preoperative pain severity was assessed using a back pain scale that combined the symptoms of back pain and sciatica. Intraoperaative pathological findings of disc were documented and comparesd with preoperative symptoms and signs. The pathological changes of disc were classified as following four subtypes: bulging disc (85 cases), protruding disc (104 cases), extruded disc (72 cases) and sequestered disc (39 cases). Our results suggest that severity of pain in patients with herniated lumbar disc is related to position of herniation rather than magnitude and exent of herniation. Inflammed intraspinal tissues are significantly associated with low back pain and sciatica.
5.Current situation and development of community health management of hypertension

In this review, the development and current situation of the community health management of hypertension at home and abroad are described and analyzed, the experience and practice worthy of learning are summarized to provide evidence for the prevention and control of hypertension in community.
6.Complications of the clavicle hook plate and corresponding managements

Object To analyze reasons of complications induced by the clavicle hook plate in treatment of acute distal clavicle fractures and acute acromioclavicular joint dislocations,and to investigate corresponding solutions.Methods Seventy nine clavicle hook plates were facilitated in the treatment of acuteb distal clavicle fractures (47 cases) and acute high grade acromioclavicular joint dislocations (32 cases) from May 2006 to May 2009.There were 51 males and 28 females,with an average age of 42.6 years(range,15 to 78 years).Seventy eight patients underwent plate removal operation.Forty patients agreed to accept the CT examination to evaluate the acromion erosion around the plates.Among them,7 patients received further CT examination 3 months after the removal surgery.The shoulder function was evaluated by the constant scores at the final follow-up.Results All patients were followed up for at least one year (range,12 to 30 months).The mean duration for retaining the hook plate was 8.3 months with the mean Constant scores 92 points in the acute distal clavicle group; 7.2 months with the mean Constant scores 95 points in the acute acromioclavicular joint dislocation group.There were 8 kinds,totally 105 complications happened in 78 patients (98.7%).The complications were classified into four groups: (1) Due to the specific working mechanism of the plate(88/105,83.8%);(2) Due to the iatrogenic errors(12/105,11.4 %);(3) Due to insufficiency design of the plate(3/105,2.9%);(4) Due to the etiology of the injury itself(2/105,1.9%).Conclusion The complication rate is unexpected higher.Most complications are unavoidable due to specific working mechanism of the plate.The patients should be well informed about this preoperatively in order to avoid the possible legal trouble.The iatrogenic errors can be avoided with proper indications and improved surgical techniques.The design of the plate needs to be improved,and the hook plate should be removed as early as possible.

8.Expression and clinical significance of Wnt10b gene in Hirschsprung disease

Objective To study the expression and clinical significance of Wnt10b gene in tissue samples of Hirschsprung disease (HD).Methods Sixty samples of stenotic intestines and 60 samples of normal intestines were collected from 60 patients with sporadic HD who were admitted to the Shengjing Hospital of China Medical University from 2003 to 2010.Tissue samples were stained by hematoxylin and eosin (HE),and then the expression of Wnt10b gene in the tissues was detected by immunohistochemical staining.The mRNA and protein expressions of Wnt10b in HD were detected by quantitative real-time polymerase chain reaction (qRT-PCR) and Western blot.All data were analyzed by two independent sample t test.Results The results of HE staining confirmed HD in the 60 patients,and the samples met the requirement of the study.The expressions of Wnt10b were strong positive in the plasma of intramuscular and submucosal cells in stenotic intestines,while weak positive or negative in normal intestines.The results of immunohistochemical staining showed that the percentages of positive proteins in the stenotic intestines and the normal intestines were 0.061% ± 0.014% and 0.006% ± 0.005%,respectively,with a significant difference (t =2.955,P < 0.05).The results of qRT-PCR showed that the relative quantification of mRNA of Wnt10b was 23.5 ± 1.6 in the stenotic intestines,which was significantly higher than 13.1 ± 1.7 in the normal intestines (t =1.687,P <0.05).High mRNA expressions of Wnt10b were observed in 45 patients with HD,and low mRNA expressions of Wnt10b were observed in 15 patients.The results of Western blot showed that the relative protein expression of Wnt10b was 35.2 ± 2.3 in the stenotic intestines,which was significantly higher than 19.1 ± 1.3 in the normal intestines (t =2.046,P < 0.05).High protein expressions of Wnt10b were observed in 43 patients,and low protein expressions of Wnt10b were observed in 17 patients.Conclusion There is a close relationship between the abnormal mRNA and protein expressions of Wnt10b in the intestines,which might play a role in the pathogensis congenital malformation of the alimentary tract.
9.The effects of advanced oxidation protein products in type 2 diabetic ocular muscles palsy

Objective To investigate the effects of advanced oxidation protein products (AOPP) in type 2 diabetic ocular muscles palsy (DOMP). Methods 58 DOMP patients and 50 type 2 diabetes patients were included in the research. Hemoglobin A1c (HbA1c), blood glucose (FPG), fasting insulin (FINS) and triglycerides (TG), total cholesterol (TC), high-density lipoprotein (HDL), low-density lipoprotein (LDL) were measured and recorded. Homeostasis model assessment was performed to evaluate the status of insulin resistance (HOMA-IR), basal insulin secretion (HOMA-β) and the insulin sensitivity index (ISI). Serum AOPP was measured by enzyme linked immunosorbent assay. Unconditional logistic regression model was used to evaluate the influencing factors of DOMP. Results The DOMP group showed higher levels of plasma AOPP, TG, LDL, FPG, FINS, HbA1c and HOMA-IR, but lower levels of HDL, HOMA-β and ISI than those of the T2DM group. Unconditional logistic regression analysis revealed that AOPP was an independent risk factors for DOMP (OR =3.01, P = 0.002). Conclusion AOPP may be involved in the pathogenesis of DOMP. AOPP could be a useful indicator for monitoring the development of DOMP and for evaluating its severity.
