1.The clinical study on the relationship between silent cerebral ischemia and cognitive impairment in inpa-tiens older than sixty with nonvalvula atrial fibrillation

Objectives The aim of this study was to study the relationship between silent cerebral ischemia (SCI) and mild cognitive impairment(MCI)in inpatients with nonvalvula atrial fibrillation (NAF) and those with sinus rhythm (SR) older than sixty. Methods Ninety-eight inpatients were enrolled from November 2014 to November 2015 in Hui?zhou Municipal Central Hospital: 45 patients with NAF and 53 with SR. All general clinical data was collected. The Montreal Cognitive Assessment (MoCA) and brain MRI were used to assess the cognitive performance of patients within 1 weeks of admission. Results The MoCA scores were lower in patients with NAF than in those with SR[16(10,20) vs. 20 (15,23),P=0.006]. At least one foci of SCI was present in patients 80%with NAF and 52.8%with SR (NAF vs. SR, P=0.018).The rates of SCIs in the cortex/subcortex were higher in the NAF group than in the SR group (25.2%vs. 12.4%, P=0.017). Multivariate analysis showed that MoCA score were related to the degree of education in NAF patients and were related to age and education level in SR patients. Conclusions NAF is an independent risk factor for cognitive impair?ment in NAF patient and associated with the SCI present. Educational level is a factor affecting the cognitive function with elderly patients.
2.Advances in the genetics of spinal muscular atrophy in childhood

Spinal muscular atrophy in childhood is one of the most common neuromuscular disorders with an autosomal recessive mode of inheritance. The main pathogenesis is homozygous loss and small intragenic mutations of the SMN1. SMN2 ,as a very identical copy of SMN1, modulates the disease severity. The functional difference between both genes is a variance of single nucleotide within the coding region, causing silent mutation obviously to decrease SMN2 transcripts. Histone deacetylase inhibitors have been shown better perspective in clinical trials.
3.Transportation of Schizandrin B in Caco-2 Cell Model

Objective:To investigate the effects of schizandrin B on P- glycoprotein by a classical in vitro cell model. Methods:Caco-2 cell model was used as the carrier,and rhodamine 123 and cyclosporin A were employed as the P-gp substrates, the transmembrane transportation of schizandrin B,rodamine 123 and cyclosporin A were detected by HPLC and a liquid scintillation counting assay,and the apparent permeability coefficient and permeability directional ratio were calculated. Results:The bidirectional transportation rates of schizandrin B(20 μg·ml -1 ,40 μg·ml -1 and 80μg·ml-1 )were similar,and showed non-selective difference. Schizandrin B(20-160 μg ·ml -1 )significantly inhibited the BL → AP directional transportation of rhodamine 123 and cyclosporine A in Caco-2 cell model(P < 0. 05) in a concentration-dependent manner. Conclusion:Schizandrin B is a P-gp inhibitor,while it isn’t a P-gp substrate.

5.Inhibitory Effects of Deoxyschizandrin on P-glycoprotein in Vitro and in Vivo

Objective To investigate the influence of deoxyschizandrin (Deo) on P-glycoprotein (P-gp).Methods The effect of P-gp on Deo (20,40,80 μg·mL-1) was studied in the Caco-2 cell model in vitro,and the apparent permeability coefficient (Papp) of Deo (20-160 μg·mL-1) on a P-gp substrate,rhodamine123 or cyclosporine A,was calculated.Healthy male Sprague-Dawley rats were randomly divided into five groups:blank control group,verapamil group,low-,medium-and high-dose Deo group (8 rats in each group).Rats in the low-,medium-and high-dose Deo group were intragastrically administered once daily with Deo at 8,16 and 32 mg·kg-1 for 3 consecutive days,while rats similarly received gavagewith verapamil (4 mg·kg-1) in the verapamil group and equal volume of purified water in the blank control group.Thirty minutes after the rats were treated with their respective drugs,rhodamine123 (5 mg· kg-1) was orally administrated.Then the pharmacokinetic profiles of rhodamine 123 were analyzed to evaluate the inhibitory ability of Deo on P-gp in vivo.Results The bidirectional transport rates of Deo (20,40,80 μg·mL-1) were similar,with non-selectivity.Deo (20-160 pg·mL-1)significantly inhibited the basolateral→apical(BL→AP) directional transports of rhodamine 123 and cyclosporine A in Caco-2 cell model (P < 0.05) in a concentration-dependent manner.And Deo (8-32 mg· kg-1) also dose-dependently decreased the peak concentrations (Cm.) and the area under the plasma concentration-time curve (AUC0-t) of Rho123.Conclusion Deo can inhibit P-gp in vitro and in vivo,but it is not a P-gp substrate.
6.Necessity,principle and procedures of clinical trials optimization in TCM

Optimization of Traditional Chinese Medicine (TCM) clinical trials as philosophy, process and method, is extremely necessary and can be applied in different levels. Optimization of TCM clinical trials should follow some principles and procedures, and be executed in two ways. Optimization of treatment plan should be supported by the research basis and literature experience, and be perfected by research group and experts argumentation. Optimization of design plan is modified and accomplished by expert discussion and repeated consulting multi-disciplinary experts. Specific research objective, exact target population definition, proper design plan selection, valid calculation of sample size, precise and appropriate main outcome measures, data management system, medical ethics and statistical analysis are the focuses.
7.Optimization of diagnosis standard in traditional Chinese medicine clinical research

The principle of TCM diagnosis standard is communicational convenience, and the international universal standard recommended by WHO should also be paid attention to. When there’s no corresponding standard, inferior standard is available, but self made standard is generally not accepted. It’s suggested to apply the same standard for one program. The standards include international standard, regional standard, domestic standard, industrial standard, provincial standard and enterprise standard, the hierarchies and levels of these standards reduce successively. It’s thought that medical standard consists of international standard (issued by WHO or set by international academic conference), domestic standard (set by government authorities, national academic organization and conference), diagnosis standard set by provincial academic organization and textbook standard by universities and colleges. A lot of literature network database, as well as ICD-10, therapy guide, professional periodicals, professional society website, and books could be used for searching. TCM clinical trials also cover syndrome diagnosis standard. The conclusion standard and exclusion standard setting and influence of improper standard setting is also discussed in this article.
8.Optimization of Treatment Regimen in Traditional Chinese Medicine Clinical Research

TCM clinical investigators should attach importance to buildup of research basis in order to ensure the feasibility and normalization of clinical regimen,definite curative effect of treatment regimen and enhance consistency among different researchers and repeatability of clinical regimen.Syndrome,therapeutic method and prescription should be consistent,the addition or subtraction of the medicinal or the dosage should be reasonable.Reduce the use of rare or invaluable medicinal to avoid the restrictions of herb resource,drug cost and animal protection.When transforming decoction to capsule,granule,dripping pill and compound granule,the preliminary test in clinic to recur the curative effect of the decoction is necessary.The application of a single medicinal,the single target area of a medicinal or active ingredient of a single medicinal in a prescription should be based on the preliminary work experience.Meanwhile the county-level producing area of the processed medicinal should be explicit,in order to ensure the quality and efficacy of the processed medicinal.This article also discussed the key point of treatment regimen optimization of acupuncture and massage.
9.Optimization of quality control of TCM clinical research

10.Influence of comprehensive nursing intervention on postoperative pain of patients undergoing general surgery

Objective To investigate the effect of comprehensive nursing intervention on postoperative pain of patients undergoing general surgery.Methods 100 patients undergoing general surgery with postoperative pain were divided into the observation group and the control group with 50 patients in each group.The control group received routine nursing,and the observation group received comprehensive nursing intervention.The nursing effect was compared between two groups.Results The number of cases using the PCA and analgesics in the observation group was fewer than those in the control group,and there were significant difference between them.The cases of pain level Ⅰ in the observation group were more than those in the control group,and the cases of pain level Ⅲ in the observation group were fewer than those in the control group,and there were significant difference between them.The satisfaction degree of the observation group were more than those in the control group,and there were significant differences between them.Conclusions The effect of comprehensive nursing intervention in cases with postoperative pain undergoing general surgery is obvious,so it is worth being used.
