1.Clinical value of chest CT in the diagnosis and management of tracheobronchial foreign bodies

Objective The purpose of this study was to investigate the value of chest CT in the evaluation ofchildren with suspected foreign body aspiration. Methods Chest CT was performed in 45consecutive children with suspected foreign body aspiration. The patients were examined with 16-slice Multidetector CT (MDCT) using 100 ~ 150 kV,30 ~ 50 mA, 1 mm section thickness. 1.2 pitch ratio, and 0.6 ~1.0 mm reconstruction interval. multiplanar reformatted (MPR) imaging were carried out after MDCT examinations. Results Chest CT revealed all foreign bodies in 42 of the 45 patients. Three patients with suspected foreign body aspiration did not show evidence of foreign body on CT,and they were treated with antibiotics for one week. These patients avoided unnecessary operations and recovered completely. The other 42 patients had evidence of foreign bodies in their bronchi. We designed the surgerical plan and selected appropriate foreign body forceps based on the CT scans. All of the foreign bodies were removed successfully, and no severe complications were observed. The location, shape, and volume of the foreign bodies found at surgery were consistent with the CT images. Conclusion The diagnosis of foreign body aspiration of the airwayin children can be accomplished by using chest CT. It is often useful in delineating the exact shape, location, volume and form of a bronchial foreign body and can help the surgeon plan for operative bronchoscopy and safe removal of the foreign body.
2.The early predictive indexes of curative effect of acute respiratory failure treated with noninvasive bilevel positive airway pressure ventilation

Objective To investigate the early predictive indexes of curative effect of acute respiratory failure(AFR) treated with noninvasive bilevel positive airway pressure(BiPAP) ventilation.Methods Sixty-eight patients with AFR were treated with BiPAP ventilation.According to the therapeutic outcome,patients were divided into success group and failure group,and the distributions of ages,acute physiology and chronic health evaluation(APACHEⅡ) score,consciousness level as well as arterial blood gas(ABG) measurement before and after the treatment with BiPAP ventilation were compared between the two groups.Results There was statistical difference in the ABG levels of pH and PaCO_2 after 1 hour's treatment with BiPAP ventilation(P0.05).Conclusion The levels of pH and PaCO_2 after 1 hour's treatment with BiPAP ventilation are valuable markers for curative effects in early stadge.If the levels of pH and PaCO_2 fail to improve,the treatment with intubation may be necessary.

5.Progress in treatment of chronic heart failure in Western medicine and treatment strategies in traditional Chinese medicine.

The concept of modern medicine in treating chronic heart failure (CHF) has changed markedly in recent years. To improve the quality of life and prolong life, the treatment goal is no longer just temporary improvement of symptoms, more importantly, is to prevent and delay the occurrence and development of ventricular remodeling. Long-term chronic over-activation of sympathetic system, renin-angiotensin-aldosterone system and other neuroendocrine factors promotes myocardial remodeling, increases myocardial injury and deteriorates cardiac function. Despite short-term use can significantly improve the blood flow dynamics, long-term use of beta-adrenergic receptor stimulators and phosphodiesterase inhibitors does not prolong life, but increases the rate of sudden death caused by cardiac arrhythmia. Angiotensin converting enzyme inhibitors and beta-blockers have become the preferred drugs in treating chronic heart failure. In fact, after long-term use, beta-blockers can significantly improve ventricular remodeling, enhance ventricular function and reduce the incidence of sudden death of patients with CHF. In traditional Chinese medicine practice, short-term use of drugs for warming yang and reinforcing qi can improve symptoms of CHF, but long-term use may have adverse effects, for these medicines can stimulate sympathetic system. Early treatment with medicines of cold and cool property may be more favorable to patients with CHF, except the advanced patients who need special intervention. Eliminating heat and nourishing yin may play more active role in controlling the occurrence and development of CHF. Drugs with good efficacy and value in treating CHF may be developed from the Chinese herbal medicines with eliminating heat and nourishing yin property.
6.Fluorescent carbon dots and the application in biomedicine.

As a new type of carbon nanomaterials, fluorescent carbon dots (fluorescent CDs) have many advantages when compared with the traditional fluorescent probes. They are photoluminescence stable and resistance to photo bleaching. Moreover, they are excellent in biocompatibility, low-toxic and easy to modify. All these above make them a promising optical image material as a probe in optical image. This article reviews structure, the common carbon sources, the preparation methods, and the light-emitting principles of the carbon dots. We also introduce the research progress of fluorescent carbon dots in biomedicine, and the problems need to be resolved in the study of fluorescent CDs.
7.Study of peritoneal micrometastasis of gastric cancer and its clinical significance

Objective To explore the significance of using cytologic and RT-PCR methods to examine(peritoneal) washings and peritoneal tissues of gastric cancer patients in prediction of peritoneal micrometastasis.Methods The peritoneal washings of 38 patients with gastric cancer and 5 patients with benign gastric(lesions) were collected and,at the same time,a small amount of omentum and peritoneum were removed for control.CEAmRNA expression of free cells in peritoneal washings were detected by RT-PCR method and(also) cytology of the washings were performed.Results The CEAmRNA expression rate of peritoneal washings and peritoneal tissues were 36.8%(14/38) and 39.5%(15/38)respectively.Both were more(sensitive) than that of cytologic examination 26.3%(10/38).TNM staging,depth of invasion,lymph node metastasis,and serosal involvement were related to the expression rate of CEAmRNA.Conclusions mRNA of CEA is more sensitive and specific than cytologic examination for detecting free cancer cells in peritoneal cavity.It is an effective method for detecting peritoneal micrometastases in gastric cancer patient.
8.Establishment of a mouse model of aorta dissection induced by β-aminopropionitrile drinking combined with angiotensin II infusion

Objective To establish a mouse model of aorta dissection (AD) by β-aminopropionitrile (BAPN) in drinking water + subcutaneously pumped angiotensin II (Ang II) infusion.Methods Forty 3-week-old C57B1/6J male mice were randomly divided into two groups.All animals received 0.1 g/kg/d BAPN in drinking water for 4 weeks.Then the BAPN drinking + saline infusion group and BAPN drinking + Ang II infusion group received continuous saline or Ang II (1,000 ng/kg/min) infusion, respectively, via subcutaneous osmotic minipump for 72 hour.The mice were restricted in a noninvasive computerized tail-cuff system and their arterial systolic blood pressure and heart rate were monitored.Autopsy was performed if a mouse died during the experiment.At the end of the experiment, mice were sacrificed by injection with an overdose of sodium pentobarbital and the aortas were harvested.The formation of aortic false lumen was observed by pathology using hematoxylin-eosin staining.Results The overall incidence of AD in the BAPN drinking administration +Ang II infusion group was 95%, whereas the incidence of AD in the BAPN drinking administration +saline infusion group was only 5%.The mortality from dissecting aneurysm rupture was 24% in the BAPN drinking administration +Ang II infusion group during the experiment.Pathological examination of the aortic cross-sections clearly showed the formation of blood-filled false lumens induced by Ang II.Conclusions A mouse model with high incidence of aortic dissection is successfully established.
9.Proteome Analysis of Hirschsprung Disease by Two-dimensional Electrophoresis

Objective To study the protein expression profiles in stegnotic and normal segment of Hirschspning disease (HD) and find the differentially expressed proteins. Methods Immobilized pH gradient two-dimensional polyacrylamide gel electrophoresis (2-DE) was used to isolate the proteins from stegnotic and normal segment of HD. After the samples were treated with silver staining,ImageMaster 2D Platinum analysis software was used to analyze the differentially expressed proteins. Results Repeatable 2-DE profiles were obtained. The mean matching rates of the stegnotic and normal mucosa were 78.1% and 86.7%,respectively. Totally, 103 spots of differentially expressed proteins were screened out between the stegnotic and the normal segment of HD. Conclusion Good reproduuibility and resolution could be obtained in the tissues of HD by applying immobilized pH gradient two-dimensional polyacrylamide gel electrophoresis.Screened differentially expressed proteins may provide the candidates of the markers for early detection of HD.
