1.Variation of serum ghrelin and leptin in patients with functional dyspepsia

Objective To investigate the alteration of serum ghrelin and leptin in patients with functional dyspepsia (FD) and their clinical significance. Methods Sixty patients with FD [-30 patients with postprandial distress syndrome(PDS) and 30 patinets with epigastric pain syndrome(EPS)] were enrolled. Thirty healthy subjects were severed as controls. The serum ghrelin and leptin were measured by enzyme linked immunosorbent assay and radioimmunosorbent assay, respectively. Results The concentrations of serum ghrelin and leptin were both significantly lower in patients with FD than in controls (P<0.01). Meanwhile, the concentrations of serum ghrelin and leptin in PDS patients were significantly lower compared with controls (P<0.01) and EPS patients (P<0.01). There was no significant difference in levels of serum ghrelin and leptin between controls and EPS patients(P>0.05). Conclusions The alterations of serum ghrelin and leptin in PDS patients are responsible for their reduction in FD patients. The PDS may he associated with abnormalities in gastrointestinal motility. There is an interaction between serum ghrelin and leptin in development of FD. The detection of serum ghrelin or leptin is helpful in classification and treatment of FD.

Biliary atresia is the most common obstructive cholangiopathy in infants.Its etiology and pathogenesis still remains unclear.Hypothetical mechanisms include genetic predisposition,viral infection,chronic inflammation or autoimmune-mediated bile duct injury,and congenital malformations of vessels or biliary tracts.The key pathogenesis is related to viral infection and immunoreaction.This review overviewed the research progress in the pathogenesis of biliary atresia in the past few years.
3.A study of the relationship between intestinal subepithelial myofibroblasts and ulcerative colitis

Objective Intestinal subepithelial myofibroblasts(ISEMFs) exist throughout the gastrointestinal tract. Due to their ability to secrete cytokines, prostaglandins, growth factors, and extracellular matrix proteins, they are thought to play critical roles in inflammation, repair, fibrosis and neoplasia. The goal of this study was to investigate the relationship between ISEMFs and ulcerative colitis by establishing the method of ISEMFs detection from the biopsy specimens. Methods ISEMFs in colon biopsy specimens (15 from active ulcerative colitis patients and 20 from controls) were detected by immunohistochemistry with smooth muscle actin monoclonal antibody. Results ISEMFs existed in a subepithelial location of colon, were located in the basement of epithelial, and were oval or scaphoid in appearance. Objective To explore the association between DNA methyltrsansferase 3a (DNMT3a) expression and interleukin-4 (IL-4),interferon-gamma (IFN-γ) in mononuclear cells of colonic laminapropriamucosae in ulcerative colitis (UC) patients.Methods From November 2009 to March 2010,sixty colonic mucosa biopsy specimens through colon scopy were collected,specimens of active UC or normal controls were 30 in each group.The expression of DNMT3a,IL-4,and IFN-γ was detected with immunohistochemical method (SABC) in mononuclear cells of colonic laminapropriamucosae,and their relation with different degrees of colonic inflammatory response was analyzed.Results The positive expression rate of DNMT3a,IL-4,IFN-γ in mononuclear cells of colonic laminapropriamucosae in active UC group was significant higher than that in control group and there were statistically significant differences (x2 =16.596,P=0.000;x2 =42.857,P=0.000;x2 =6.667,P=0.024;).The expression of them was higher in sever inflammatory response than that in mild inflammatory response,and its expression intensity increased as inflammatory activity became more severe.There was positive correlation between IL-4,IFN-γ and DNMT3 expression in UC group (r=0.46,P=0.01 ;r=0.559,P=0.001).Conclusions The expression of DNMT3a,IL-4 and IFN-γ is associated with the degree of colonic inflammatory response.DNA methylation maybe involved in Th1/Th2 immune balance regulation in UC pathogenesis.
5.Application of low lateral drainage of bilateral alternating and turokinaseinstillation in treatment of spontaneous intraventricular hemorrhage

Objective To evaluate the clinical value of combined treatment for spontaneous intraventricular hemorrhage. Methods 26 patients with spontaneous intraventricular hemorrhage were conducted on the whole turn of low lateral drainage of bilateral urokinase infusion therapy, supplemented by lumbar puncture cerebrospinal fluid replacement. Results The total effective rate was 84.62% ,mortality rate was 15. 38%. Conclusion Bilateral lateral drainage of alternating low urokinase infusion,supplemented by lumbar puncture cerebrospinal fluid replacement was effective in treatment of spontaneous intraventricular hemorrhage. It could reduce mortality in patients with intraventricular hemorrhage and increase the patients' prognosis.
6.Investigation in the influencing factors of activities of daily living of patients after abdominal surgery

Objective To study the affecting factors of the activities of daily living in patients after abdominal surgery.Methods Using survey methodology,95 abdominal surgery patients were investigated br general condition questionnaire and Barthel index scales.The data were analyzed using descriptive analysis,t test,variance analysis and multiple regression analysis.Results The total number of valid return was 91.and the valid usable return rate was 95.8% .The affecting factors of recovery to ADL2 of abdominal surgery patients after operation were gender,age,education level,economic condition,type of illness,postoperation complications,operation times,nutrition risk scores and activities of daily living of preoperation,and the differences were significant.The affecting factors of recovery to ADL1 of abdominal surgery patients after operation were age,education level economic condition,marital status,type of illness,postoperation complications,nutrition risk scores and activities of daily living of preoperation,the differences wen significant.Conclusions The affecting factors of ADL of abdominal surgery patients after operation were gender,age,education level economic level,type of illness,postoperation complications,marital status,operation times,nutrition risk scores and activities of daily living of preoperation.
7.Endoscopic ultrasonography for ectopic pancreas in upper gastrointestinal tract

Objective To evaluate endoscopic ultrasonography (EUS) for diagnosis and treatment of ectopic pancreas in upper gastrointestinal tract.Methods Data of 52 patients with ectopic pancreas diagnosed by EUS and confirmed by pathological examination were reviewed.The feature of EUS image and diagnostic accuracy were retrospectively analyzed.Results Out of 52 patients, 41 (78.85% ) were pathologically diagnosed as having ectopic pancreas, 6 were stromal tumors, 4 were leiomyoma and 1 was nerve sheath tumor.All ectopic pancreases presented as protruding lesions, in which 37 were located at gastric antrum, 2 in duodenum, 1 in gastric body and 1 in gastric fundus.Under EUS, 1 case appeared as hyperechoic, 10 as isoechoic, 18 as hypoechoic and 12 as mixed echoic.The lesion involved mucosa in 2 cases, muscularis mu-cosae in 8, submucosa in 30 and muscularis propria in 1, in which 15 cases from submucosa underwent endoscopic resection.Bleeding occurred in 2 cases and no other complications occurred.There was one relapse during the 6 months of followed-up.Conclusion EUS is valuable and safe for the diagnosis of ectopic pancreas in upper gastrointestinal tract.Depth of lesions revealed by EUS can help further treatment.
8.A preliminary study on the relationship of insulin-like growth factor-Ⅰ receptor and the carcinogenesis of gastde cancer

Objective To study the relationship of insulin-like growth factor-Ⅰ receptor(IGF-ⅠR)and carcinogenesis of gastric cancer.Methods The expression of IGF-ⅠR was detected in 40 cases of resected gastric cancer tissues and adjacent normal gastric mucosa by immunohistochemistry.The expression of IGF-ⅠR in gastric cancer cell lines(MGC803 and SGC7901)was detected by Western Blot.siRNAs targeted to IGF-ⅠR were designed,synthesized and transfected into MGC803 cells,the changes of IGF-IR protein level were detected by Western Blot at 48 h after transfection,the cell proliferation was examined by MTT,and then the growth curve was obtained.Results The positive rate of IGF-IR expression in gastric cancer tissues was 75.5%,significantly higher than that of adjacent normal tissues (25%,P<0.01).The expression level of IGF-IR was related with TNM stage,lymphnode metastasis (P<0.05),but not related with sex,age,histological differentiation,invasion depth of gastric cancer (P>0.05).Intense expression of IGF-ⅠR was showed in gastric cancer cell lines.The inhibition ratio of IGF-ⅠR expression in sigNAl group were 89.80%±4.10%,the cell proliferation decreased to mininlunl level at the fifth day aftertransfection(by 29.0%±4.0%of mock-treated group),the cell number decreased by 21.15%±1.10%of mock treated group at the same time.Conclusions IGF-ⅠR is over-expressed in gastric cancer cells and can be effectively silenced by RNA interferes,therefore the growth of tmnor cell Was inhibited.Thus,it indicates that IGF-ⅠR may be a promising target for gene therapy of gastric cancer.
9.Effect of folate deficiency on experimental colitis in mice induced by dextran sodium sulfate

Objective To investigate whether folate deficiency cause high expression level of interferon gamma (IFN-γ) resulted from IFN-γ gene ( IFNG) hypomethylation and then promote the pathogenesis and development of ulcerative colitis (UC ) in a dextran sulfate sodium (DSS )-induced experimental colitis model in mice .Methods A total of 24 female BALB/c mice were divided into four groups ,six mice in each group , including folate deficient/DSS+ group , standard diet/DSS+ group , standard diet/DSS - group and folate deficient/DSS- group .At the beginning of the sixth week since fed , the mice of model groups were treated with 5% DSS to establish experimental colitis .By the end of the sixth week ,disease activity index (DAI) of colitis and histological changes were evaluated .The folate level of peripheral blood serum of mice were detected by enzyme-linked immunosorbent assay (ELISA ) . The expression of IFN-γ in colonic mucosa of mice was examined by immunohistochemistry . The methylation level of CpG island in the promoter region of IFNG was determined by methylation specific polymerase chain reaction (MSP) .The t test was used for measurement data .Chi square test was performed for comparison between groups of count data . Spearman correlation analysis was used for correlation analysis .Results The folate levels of peripheral blood serum of folate deficiency/DSS+ group and folate deficiency/DSS- group ((2 .70 ± 0 .19) and (2 .80 ± 0 .25)μg/L) were significantly lower than those of standard diet/DSS+ group and standard diet/DSS- group ((13 .62 ± 0 .38 ) and (13 .52 ± 0 .77)μg/L ,t= -63 .33、32 .27 ,both P< 0 .05) ,resepectively .The expression of IFN-γ in colonic mucosa of folate deficiency/DSS+ group and standard diet/DSS+ group were significantly higher than those of folate deficiency/DSS- group and standard diet/DSS- group (χ2 = 22 .18 ,P< 0 .05 ) . And the expression of IFN-γ in colonic mucosa of folate deficiency/DSS+ group was also higher than that of standard diet/DSS+ group (χ2 = 12 .00 ,P< 0 .05) .The expression level of IFN-γ of folate deficiency/DSS+ group and standard diet/DSS+ group was positively correlated with (r=0 .998、0 .953 ,both P<0 .01) .The folate levels of peripheral blood serum of folate deficiency/DSS+ group was negatively correlated with IFN-γexpression level and DAI (r= -0 .880 and -0 .926 ,both P<0 .05) .No abnormal methylation was detected in IFNG promoter CpG island in colonic mucosa tissues of mice of each group . Conclusion In the mice model of DSS induced acute experimental colitis ,folate deficiency may increace the expression of inflammatory factor IFN-γand enhance the inflammation activity of colonic mucosa .
10.Clinical analysis on tricuspid valve replacement for secondary tricuspid regurgitation late after left-sided valve surgery

Objective·To analyse the outcomes of tricuspid valve replacement (TVR) for secondary tricuspid regurgitation (STR) late after left-sided valve surgery during perioperative period and mid-term follow-up,investigate mechanisms of STR and surgical risk factors.Methods·A total of 85 consecutive patients who underwent the TVR surgery were analyzed.The perioperative and mid-term clinical outcomes were retrospectively investigated.The data were divided into bioprosthesis group (n=50) and mechanical prosthesis group (n=35) according to the prosthesis used,and divided into right anterolateral thoracotomy(RALT) group (n=51) and stemotomy(S) group (n=34) according to the surgical incision.Results·In-hospital mortality was 8.2% (7/85).There was no significant difference in the mortality with different choice of bioprosthetic or mechanical valve (4/50 vs 3/35,x2=0.009,P=1.000);while there was significant difference between S group and RALT group (6/34 vs 1/51,x2=6.642,P=0.015).Seven cases all died of right heart failure and severe low cardiac output syndrome.Five (5.9%) cases died in perioperative within 30 in-hospital days and 2 (2.4%) cases died after 30 in-hospital days.Seventy-four cases were followed up.With the follow-up of (31.5±23.1) months,there were 4 case of late deaths(5.4%),all of whom were mechanical prosthesis,of whom 3 died in cardiac related death and 1 died in later period bowel cancer.Seventy cases survived in New York Heart Association (NYHA) class Ⅰ-Ⅱ with no coagulated accident and redo-TVR.Conclusion·The perioperative and mid-term clinical outcomes are satisfied in timely and reasonable TVR with the standard follow-up for STR late after left-sided valve surgery.Right anterolateral incision is recommend for isolated TVR.
