Objective To investigate the clinical value of medial approach in the treatment of posterior tibial plateau fractures.Methods 60 cases with posterior tibial plateau fractures were randomly divided into observation group and control group.The observation group was operated by medial approach,and the control group was operated by medial approach.Results The operation time of the observation group was (138.4 ±18.2)min,which of the con-trol group was (119.2 ±15.8)min,the difference between the two groups was statistically significant(t =4.363,P <0.01).The excellent and good rate of rehabilitation in the observation group was 96.7%,which was significantly higher than 83.3% of the control group (Z =2.237,P <0.05).The fracture healing time of the observation group was (14.5 ±3.3)weeks,which was significantly shorter than (17.9 ±3.6)weeks of the control group (t =3.813, P <0.01).The hospitalization time and incidence rate of complications had no statistically significant differences between the two groups (P >0.05).Conclusion The medial approach in the treatment of the posterior tibial plateau fractures has relatively small injury,reliable fixation,and good rehabilitation.It is one of surgical approaches which worthy of clinical consideration.

OBJECTIVEThis study aims to do the following: construct a three-dimensional finite element model of an labial inverted impacted maxillary central incisor and its supporting tissues, analyze stress distribution in the periodontal tissue when various tractions are exerted, and provide references for treating impacted maxillary central incisor.

METHODSA three-dimensional finite element model oflabial inverted impacted maxillary central incisor and its periodontal tissues was established using Mimics 10.01 and Ansys 14.0 software based on original cone beam computed tomography (CBCT) data. Various traction values (20, 30, 40, 50, 60, and 70 g) were exerted on the incisal margin in the direction perpendicular to the impacted tooth. Different Von Mises stress values were determined.

RESULTSStress distribution on the periodontal ligament increased with traction size. When 30 g traction was exerted on the labial inverted impacted maxillary central incisor, the Von Mises stress was 24 919.0 Pa, which was within the range of the optimum force and close to its maximum value.

CONCLUSIONThe optimum traction for early orthodontic treatment of labial inverted impacted maxillary central incisor is nearly 30 g.

BACKGROUND:Confirming the reliability and validity of the My jump 2 application for measuring lower limb vertical stiffness may offer the possibility of it as an alternative to the Kistler three-dimensional force platform for measuring lower limb stiffness. OBJECTIVE:To verify the reliability and validity of the My Jump 2 application in measuring lower limb vertical stiffness of college students. METHODS:The drop jump data of the participants were collected through the Kistler three-dimensional force platform and the My Jump 2 application,and the vertical stiffness of the participants'lower limb vertical stiffness was calculated.The intraclass correlation coefficient was used to analyze the data measured by the My Jump 2 application and the Kistler three-dimensional force platform,attempting to verify the reliability of the My Jump 2 application.The bias and average between the two devices were drawn into a Bland-Altman diagram to verify the consistency between the two test methods.Finally,the test-retest reliability of the My Jump 2 applications at 30 cm and 40 cm was analyzed using the Cronbach's alpha(α)and coefficient of variation.Pearson product-moment correlation was used to analyze the correlation of My Jump 2 applications. RESULTS AND CONCLUSION:My Jump 2 application has high reliability and validity when measuring the vertical stiffness of the lower limb.At the same time,due to its advantages of low cost,convenient portability and field testing for large samples,it can be used as an alternative to the Kistler three-dimensional force platform to test the vertical stiffness of the lower limb in college students and similar populations.

OBJECTIVETo screen potential mutation and explore the underlying mechanism for a consanguineous pedigree featuring pyruvate kinase (PK) deficiency.

METHODSThe red blood cell pyruvate kinase activities of all family members were detected. All the exons and intron-exon boundaries of the PKLR gene for the proband were amplified and analyzed by direct sequencing. Restriction endonuclease enzymes were used to identify the presence of mutations of all family members.

RESULTSThe pyruvate kinase activities were 5.89 U/g Hb in the proband, 3.45, 6.54, 8.87, 7.89, 9.32 U/g Hb in his younger sister, father, mother, grandmother and elder aunt, respectively. The homozygous missense mutation of T>C transition at position 941 in exon 7 of PKLR gene resulted to a Ile314Thr substitution in the proband, and mutant alleles were identified at the level of RNA transcript by cDNA sequence analysis. His younger sister was also homozygous for Ile314Thr. Heterozygosity for Ile314Thr was confirmed in his grandmother, parents and elder aunt.

CONCLUSIONIle314Thr homozygous missense mutation in exon 7 of PKLR is the molecular mechanism of pyruvate kinase deficiency in this family.
