1.Expression of psoriasin in actinic keratosis, Bowen's disease and squamous cell carcinoma and its significance

ObjectiveTo investigate the expression of psoriasin in actinic keratosis(AK),Bowen's disease and squamous cell carcinoma(SCC) tissues.MethodsImmunohistochemistry was carried out to quantify the expression of psoriasin protein in tissue specimens from the lesions of 20 patients with AK,25 patients with Bowen' s disease,21 patients with well differentiated SCC,and16 patients with poorly differentiated SCC,as well as from the skin of 18 normal human controls.ResultsThe expression rate of psoriasin was 11.1% in the control specimens,significantly different from that in the AK(95.0%),Bowen's disease (88.0%),well differentiated SCC (95.2%),and poorly differentiated SCC (92.3%) specimens (all P <0.01 ).Psoriasin was expressed in the cytoplasm of keratinocytes and dyskeratotic keratinocytes in stratum corneum and upper 1 to 3 layers of stratum spinosums in AK tissue,in the cytoplasm of keratinocytes as well as the membrane and cytoplasm of vacuolated cells in all layers of the epidermis in Bowen's disease tissue.In well differentiated SCC,cornified pearl and dyskeratotic keratinocytes located in the stratum comeum and all layers of the stratum spinosums stained positive for psoriasin,and in poorly differentiated SCC,psoriasin was distributed in the stratum corneum and upper 1 to 5 layers of the stratum spinosums,but absent in poorly differentiated squamous cells.The expression intensity of psoriasin in tissues sequentially increased from AK,Bowen's disease to well differentiated SCC.Though the expression intensity of psoriasin in poorly differentiated SCC decreased,but was still higher than that in normal skin tissue(P < 0.05).ConclusionPsoriasin is abnormally expressed in skin disorders with dysdifferentiation of squamous cells.
2.Occupational Infection among Supply Office Staff:Investigation and Prevention

OBJECTIVE To strengthen the self-protection consciousness of supply office staff to reduce the occupationol infection.METHODS From Sep 2006 to Aug 2007,97 supply office persons in 8 third-level hospitals and 2 second-level hospitals were carried on the related questionnaire survey.RESULTS 85.6% Of the staff were contaminated by blood,body fluid,secretion and the excreta,74.2% of them were contaminated for many times.Sharp instrument injunry occurcd among 77.3% of the staff,in which 14.4% were injured more than 5 times,About the knowledge of prevention,43.3% was familiar,47.4% only heared about them but not familiar and 9.3% never heared it.CONCLUSIONS The supply office staff is the high dangerous infection crowd,who are the suscepitable population of the sharp instrument injury.It is important to adopt the universal prevention and the standard preventive measures to reduce occupational infection occurrence.
3.Considerations on Ethics in the Process of Vitiligo Treatment

In this paper, the authors analyzed the clinical features and the psychological characteristics of pa-tients with vitiligo: nervousness and inferiority, urgent treatment, high expectation , disappointed feelings of the failtreatment. Aimed at these psychological problems, the medical ethics requirements in the process of diagnosis and treatment were proposed:respect for the patient, protect of the patient′s privacy, patient communication and regular follow-up and health education.
4.Efficacy of phacoemulsification with goniosynechialysis on acute angle-closure glaucoma and cataract complicated with extensive synechial angle closure

AIM: To assess the clinical efficacy of phacoemulsification, intraocular lens implantation with goniosynechialysis(PEI+GSL)for acute angle closure glaucoma(AACG)and cataract with extensive angle closure synechiae.
METHODS: A retrospective study, we studied 35 eyes of 32 patients with AACG and cataract in our hospital. The extent of anterior chamber angle-closure synechiae was defined as an eye with >180°. All patients underwent PEI+GSL and completed an ophthalmologic examination including vision, intraocular pressure(IOP), anterior chamber depth(ACD), angle-opening distance(AOD500), trabecular-iris space area(TISA500)were observed at 1d, 1wk, 1mo and 3mo after cataract surgery. The angle closure range and retinal nerve fiber layer(RNFL)thickness changes at postoperative 1mo and 3mo were observed, and recorded complications.
RESULTS: Postoperative 3mo BCVA(0.334±0.154)and IOP(14.63±3.59mmHg)were improved compared with preoperative(0.914±0.290, 42.54±8.06mmHg)(P<0.05). ACD(3.203±0.214mm), OCT angle parameters AOD500 and TISA500(0.308±0.014, 0.315±0.015mm, 0.134±0.013, 0.139±0.018mm2)were significantly increased compared with preoperation. The extent of angle closure with gonioscopy(72.32±28.33°)decreased compared preoperation(215.29°±30.66°), and RNFL thickness thinner than preoperation(P<0.001). Changes in AOD500 and TISA500 for both nasal and temporal were negatively correlated with IOP, but not with changes in ACD, and no significant complications occurred in the 3mo after surgery.
CONCLUSION: The treatment of PEI+GSL can improve vision, deeper ACD and effectively open ACA in the early stage, thus controlling IOP.
5.Effects of bicyclol on renal PAI-1 expression in rats with uniliteral ureteral obstruction.

OBJECTIVETo explore the protective effects of bicyclol against renal interstitial fibrosis and possible mechanisms of the protection.
METHODSEighty-one Sprague-Dawley (SD) rats were randomly assigned to a sham-operated group and UUO groups with and without bicyclol treatment. A rat model of renal interstitial fibrosis was prepared by unilateral ureteral obstruction (UUO). Renal tissues were examined by hematoxylin & eosin and Masson staining on 7, 14 and 21 days. Immunhistochemistry was used for determining plasminogen activator inhibitor-1(PAI-1) expression in the renal interstitium. PAI-1 mRNA expression in renal tissues was semi-quantitatively determined by reverse transcription-polymerase chain reaction (RT-PCR).
RESULTSThe relative areas of renal interstitial fibrosis in the bicyclol-treated UUO group 7, 14 and 21 days after operation were (9.6 ± 0.6)%, (16.8 ± 0.8)% and (33.6 ± 1.6)% respectively, which were significantly lower than those in the untreated UUO group [13.0 ± 0.7)%, (25.8 ± 1.5)% and (53.2 ± 2.5)% respectively] (P<0.05). The levels of protein and mRNA expression of PAI-1 in the bicyclol-treated UUO group decreased significantly compared with those in the untreated UUO group 7, 14 and 21 days after operation (P<0.05).
CONCLUSIONSBicyclol can alleviate renal interstitial injury and renal interstitial fibrosis caused by UUO in rats, possibly through a downregulation of renal PAI-1 expression.

6.Effects of NS398, an inhibitor of cyclooxygenase 2, on the growth and apoptosis of human squamous cell carcinoma cell line Tca8113

Objective To study the effect of NS398, an inhibitor of cyclooxygenase 2 (COX2), on the growth and apoptosis of human squamous cell carcinoma cell line Tca8113. Methods Cultured Tca8113 cells were incubated with NS398 (0, 6.25, 12.5, 25, 50, 100 μmol/L) for 24, 48 and 72 hours, respectively. Thereafter, MTT method, flow cytometry and transmission electron microscopy were applied to detect the proliferation, cell cycle and apoptosis of Tca8113 cells, respectively. Results The proliferation of Tca8113 cells was inhibited by NS398 in a dose- and time-dependent manner (both P<0.05). FCM analysis showed the appearance of a typical hypodiploid apoptotic (Sub-G1) peak, an increase in the percentage of cells at G0/G1 phase and a decrease in that at S and G2/M phases in NS398 ( 100 μmol/L) -treated Tca8113 cells. Moreover, the cell proliferation index was significantly downregulated by NS398 of 100 μmol/L from 41.03 to 24.33 (P<0.05). Under an electron microscope, morphological changes characteristic of apoptosis were observed in NS398-treated Tca8113 cells. Conclusion NS398, an inhibitor of COX2, could effectively inhibit the growth of Tca8113 cells in vitro by induction of apoptosis.
7.Evaluation of prediction of pathologic grade of regression to preoperative neoadjuvant chemotherapy in patients ;with resectable advanced gastric cancer using double contrast-enhanced ultrasound

Objective To discuss the value of double contrast-enhanced ultrasound(DCEUS) as a method to predict the pathologic grade of regression to preoperative neoadjuvant chemotherapy(NAC) in advanced gastric cancer(AGC) patients,the contrast parameters of gastric carcinoma were measured and its correlation with pathologic response degree was analyzed.Methods Fifty seven patients with endoscopic biopsy-proven AGC were considered for a complete resection of the lesion and had a DCEUS prior to and following XELOX pre-operative NAC therapy for 3 cycles.The arrival time (AT),time-to-peak (TTP), baseline intensity(BI) and peak intensity(PI) of the primary gastric tumor were measured.The enhanced intensity(EI)was defined as PI minus BI.The percentage of change of DCEUS parameters before and after NAC therapy and its correlation with phathologic grades of regression was calculated.Patients were divided into responder and nonreponder group according to different pathologic response grade.The differences of DCEUS parameters between two groups were compared.The diagnostic accuracy of DCEUS in prediction of benefit from preoperative NAC was represented by means of receive operating characteristic(ROC)curves. Results After NAC,the PI and EI values of local gastric cancer were significantly lower than before NAC. There were significant differences in PI and EI after NAC between the responder and nonresponder groups. Among the DCEUS parameters showed significant correlation with pathologic grade of regression,the correlation factor was highest in percentage of EI reduction of primary gastric tumor(ρ= -0.501 ,P =0.007).When the optimal cutoff value of EI reduction rate of gastric tumor determined was 27%,a sensitivity of 81 .8% and specificity of 66.7% were achieved.Conclusions DCEUS might be a novel, noninvasive,liable and potential method to select the benefit responder from the preoperative NAC in AGC patient.
8.In vivo homing of bone marrow mesenchymal stem cells transplanted through different ways in rats exposed to silica dust

BACKGROUND:Bone marrow mesenchymal stem cel s transplanted into rats exposed to silica dust can home to the injured lung, but the homing effects via different ways are stil unclear.
OBJECTIVE:To comparatively observe the distribution of bone marrow mesenchymal stem cel s transplanted via different ways into rats exposed to silica dust.
METHODS:Bone marrow mesenchymal stem cel s of donor rats were isolated through whole bone marrow adherent method and transfected by Lv-eGFP. Receptor rats were exposed to silica dust through windpipe injection and randomly divided into intravenous injection and intratracheal injection groups. Then, transfected bone marrow mesenchymal stem cel s were injected via the vein and trachea into acceptor rats. The acceptor rats were kil ed at weeks 1, 2, 3, 4 after transplantation to take the lung, heart, liver, spleen, kidney, and brain tissue that were made into frozen sections and observed under fluorescence microscopy. The intensity of green fluorescence (absorbance value) was analyzed using image analysis software.
RESULTS AND CONCLUSION:Strong, wide and lasting green fluorescence was both observed in the lung tissue of intravenous injection and intratracheal injection groups, which was especial y remarkable around the bronchus and blood vessels. The fluorescence intensities of both two groups were slightly decreased with time, but there was no difference between the two groups (P>0.05). The fluorescence in the other organs of both two groups was also observed at early stage. It was stronger and wider in the liver, spleen and heart, while fainter and less in the kidney and brain, and reduced with time in al the organs. Fluorescence could be observed few and faint only in the liver and spleen at late stage, and could hardly be seen in the brain. The fluorescence intensities of the liver, spleen, heart, kidney and brain had no significant difference between the two groups at the same time (P>0.05), but the fluorescence intensity in the brain at the 1st week showed significant difference between the two groups (P<0.05). These findings indicate that the intravenous injection and intratracheal injection of bone marrow mesenchymal stem cel s have similar homing effects in rats exposed to silica dust.
9.Peroral endoscopic myotomy for severe malnutrition secondary to achalasia in children:analysis of one case

Objective To understand the effectiveness of peroral endoscopic myotomy(POEM)for achalasia and secondary severe malnutrition in children and the effectiveness of nutrition support before and after POEM.Method Treatment of POEM,nutrition support(tube feeding by percutaneous endoscopic gastrostomy three months before operation)in one case were recorded and analyzed.Results POEM was successful without any complication such as perforation,bleeding,and pneumoderm.The child suffered from fever after surgery,which was dissolved after symptomatic treatment three days later.Chest pain was relieved after changing position.The diet was changed from liquid to semifluid,and then to normal diet.The child did not have dysphagia.During the 6-month follow-up,the patient had normal diets by oral route.The weight was 32 kg before achalasia and decreased to 18 kg after 9 months(at admission);it then increased to 29 kg after short-time(12 days)parenteral nutrition and long-time(3 months)enteral nutrition by percutaneous endoscopic gastrostomy,and then the patient received POEM.His weight was 30,31,31,29,and 31 kg 1,4,6,9,and 13 months,respectively,after POEM.Conclusion POEM can effectively cure achalasia in pediatric patients and improve the quality of life.Rational enteral nutrition can improve nutrition status and facilitate a successful POEM.
10.NB-UVB promotes melanogenesis and cell proliferation through downregulating the expression of miRNA-25 in human melanocytes

Objective To investigate the effect of narrow-band UVB (NB-UVB ) on melanocytes proliferation,melanin production,tyrosinase activities and miRNA-2 5 expression so as to explore the relationship between NB-UVB and miRNA-25 and the possible mechanism of NB-UVB for treatment of vitiligo.Methods Melanocytes cultured in vitro were treated with 40 mJ/cm2 dose of NB-UVB for 72 h,the effects of NB-UVB on cell proliferation,tyrosinase activity and melanin content were investigated.After NB-UVB stimulation for 1 2 h,the effect of NB-UVB on miRNA-2 5 expression in melanocytes was detected. Melanocytes were transfected with miRNA-2 5 mimics,miRNA-2 5 inhibitor and miRNA-2 5 mutant,respectively,the changes of cell proliferation, tyrosinase activitiy and melanin content were observed.Cell viability was detected using MTT method.Tyrosinase activities were measured with levodopa as the substrate.NaOH assay was used for the detection of melanin content. Real-time quantitative polymerase chain reaction (qRT-PCR)was used to detect miRNA-25 expression.Results After NB-UVB of 40 mJ/cm2 dose was used for the cells cultured for 72 h,the viability of melanocytes,tyrosinase activities,and melanin content were significantly increased (P<0.05 ).After NB-UVB stimulation for 12 h, miRNA-2 5 expression was significantly lower than that in the control group (P<0 .0 5 ).After knockdown of miRNA-2 5 ,the cell proliferation of melanocytes,tyrosinase activities and melanin content were increased,whereas overexpression of miRNA-2 5 decreased the cell proliferation, tyrosinase activities and melanin production.Overexpression of miRNA-2 5 partially inhibited the effect of NB-UVB on the treatment of melanocytes. Conclusion NB-UVB may promote cell proliferation,increase tyrosinase activities and melanin formation through partially downregulating the expression of miRNA-2 5 in melanocytes.
