1.Clinical Effect Observation of Bicyclol and Polyene Phosphatidylcholine in the Treatment of Liver Injury Induced by Hyperthyroidism

To observe the clinical efficacy and safety of bicyclol and polyene phosphatidylcholine in the treatment of liver injury induced by hyperthyroidism. Methods: Totally 112 cases of hyperthyroidism patients were randomly divided into two groups, and both groups were treated with antithyroid drug methimazole. The 56 patients in the treatment group were orally treated with bicyclol, and the other 56 patients in the control group received polyene phosphatidylcholine capsules orally. The changes of liver func-tion were compared before and after the 4-week treatment, including the level of alanine aminotransferase (ALT), aspartate amin-otransferase ( AST) , total bilirubin ( TBIL) and alkaline phosphatase ( ALP) . The efficacy and adverse reactions were also observed. Results:There was no difference in the levels of ALT, AST, TBIL and ALP in both groups before the treatment (P>0. 05), while the levels were all decreased significantly in both groups after the treatment (P<0. 05). Compared with those of the control group, ALT and AST levels of the treatment group were decreased significantly after the treatment (P<0. 05), and the total effective rate of the treatment group was higher than that of the control group with statistical significance (P <0.05), while no statistically significant difference was shown in the levels of TBIL and ALP (P>0. 05). No drug-related adverse events were shown during the treatment. Conclusion:Bicyclol is effective and reliable in the treatment of liver injury induced by hyperthyroidism, and the clinical efficacy is remarkable when compared with that of polyene phosphatidylcholine.
2.Analysis of drug treatment of the coronary embolism in Kawasaki disease

Objective To observe the efficacy of drug treatment of coronary aneurysm complicated with embolism in Kawasaki disease (KD). Methods The clinical data of eight KD children with coronary aneurysm and embolism were retrospectively analyzed. Results Eight KD children (six males and two females) at age of 0.25-5.2 years (mean=2.89) ,were diagnosed with gigantic coronary artery aneurysms. The diameter of aneurysm was around 8.3-13.8mm. Thrombosis appeared from 19 days to five months after coronary aneurysms formation. The onset manifestations included sudden chest pain and myocardial infarction with symptoms of heart failure in one case, shock in one case and no symptom in six cases. The maximum diameter of the thrombus was 2.8 mm×15.4 mm in the shape of funicular. Four cases had thrombus in the right coronary artery, two cases in the left coronary artery, and two cases in both sides. The patients underwent anticoagulant therapy taking heparin, urokinase, warfarin, aspirin and dipyridamole. Anticoagulant therapy was successful in 7 cases and the thrombus was completely dissolved in 7 days to more than 4 months. One case had recurrent coronary thrombosis after 4 months. One case died in 12 hours after thrombolysis. Conclusions Coronary thrombosis usually appears in the half year after the onset of KD coronary aneurysm. Thrombolysis therapy takes long time in Kawasaki patients. Breaking off of thrombus are rarely seen.
3.Comparison of Content of Eucalyptol in Artemisia argyi of 5 Different Planting Densities

OBJECTIVE:To compare the contents of eucalyptol in Artemisia argyi of 5 different planting densities. METH-ODS:Gas chromatography method was performed on the column of HP-5 capillary column,and cyclohexanone was selected as in-ternal standard,the carrier gas was nitrogen at a flow rate of 0.6 ml/min,the volume temperature was 240℃,the detector tempera-ture was 250 ℃,the split ratio was 10∶1,and the injection volume was 1 μl. RESULTS:The linear range of eucalyptol was 0.420 4-1.261 2 mg/ml(r=0.999 9);RSDs of precision,stability and reproducibility tests were no more than 0.92%;average recovery was 99.6%(RSD=1.4%,n=6). CONCLUSIONS:The method is simple,rapid,accurate,reliable and reproducible,and can be used for the content determination of eucalyptol in A. argyi. The optimal planting density of A. argyi was spaced 30 cm×40 cm.
4.The clinical observation of propofol with remifentanil anesthesia on the effection of upper gastrointestinal peristalsis in painless gastroscopy

Objective To discuss the application effect of joint remifentanil propofol anesthesia on upper gastrointestinal peristalsis in the painless gastroscopy.Methods 130 patients who taken painless gastroscopy were choosed,and using the random number table method,they were divided into the observation group and control group. Propofol anesthesia was used in the control group,joint remifentanil anesthesia was used in the observation group,and the anesthetic effect were recorded.Results In the observation group group,the excellent rate of anesthesia was 93.85%,the control group was 80.00%,there was statistically significant difference between the two groups (χ2 =5.481,P <0.05).The propofol dosage in the observation group was (1.73 ±0.24)mg/kg,patients awake time was (5.91 ±1.03)min,and departure from the hospital time was (14.58 ±2.79)min,while in the control group,the propofol dosage was (3.06 ±0.57)mg/kg,patients awake time was (9.44 ±1.87)min,and departure from the hospi-tal time was (22.05 ±4.86)min,there were statistically significant difference between the two groups (t =17.337, 13.330,10.747,all P <0.05).24 cases of observation group of patients with mild 36 cases,moderate,severe in 5 cases;23 cases of control group in patients with mild and moderate 27 cases,15 cases were severe,there was statistically significant difference between the two groups (χ2 =5.376,P <0.05).Conclusion Joint remifentanil propofol used in painless gastroscopy of anesthesia is better than only propofol anesthesia,and with little impact on patients with upper gastrointestinal system,it can reduce the dosage of propofol,shorten the patient awake time and time of departure from the hospital,so it is worth popularization and application in clinic.
5.Environmental Quality Evaluation of GAP Planting Bases of Artemisia argyi in Qichun County of Hubei Province

OBJECTIVE:To evaluate the environmental quality of planting bases of Artemisia argyi in Qichun county of Hubei province so as to provide scientific foundation for the construction of the planting bases of A. argyi which conforms to the require-ments of Good Agriculturing Practice (GAP). METHODS:By reference to national Ambient Air Quality Standards (GB 3095-1996),Water Quality Standards for Farm Irrigation(GB 5084-2005)and Soil Environment Quality Standards(GB 15618-1995), the ambient air,irrigation water and soil quality at 3 major producing areas of A. argyi,including Hongmenlou,Zhulinhu and Tongxin,were tested respectively. RESULTS:At the 3 major producing areas,the ambient air(including sulfur dioxide,nitrogen dioxide,total suspended particulates,fluoride,etc.)was up to primary standards in Ambient Air Quality Standards,irrigation wa-ter quality(including pH,chloride,sulfide,total arsenic,total mercury,lead,cadmium,fecal coligroup count,number of ascaris eggs,etc.)met the basic dry farming requirements in Water Quality Standards for Farm Irrigation,and soil quality(including pH, cadmium,mercury,lead,arsenic,chromium,copper,DDT,hexachlorocyclohexane,etc.) reached secondary standards in Soil Environment Quality Standards. CONCLUSIONS:The aforesaid results show that the planting bases meet GAP requirements in am-bient air,irrigation water and soil quality and are applicable producing areas of medicinal material A. argyi.
6.The research on the mechanism of CD40 RNAi mouse dendritic cells inhibits T lymphocyte proliferation

Objective To construct the mouse CD40 RNA interfering(RNAi) lentiviral vector and prepare dendritic cells (DCs) with low expression of CD40. And to explore the mechanism of inducing T lymphocyte incompetence by blocking CD40/CD40L costimulatory pathway. Methods Mouse myeloid DCs were cultured in selective medium containing necessary cytokines for DC growth in vitro. CD40 RNAi gene was transfected into DCs with lentiviral vector. The expression levels of CD40 mRNA and protein were assayed by real-time quantitative PCR and flow cytometry respectively. The influences on DCs stimulating the proliferation ability of T lymphocyte were observed through mixed lymphocyte culture(MLC). Results Myeloid DCs have been harvested from mouse through cell culture in vitro. A mouse CD40 RNAi ientiviral vector was built successfully. The lentiviral titer was 8×10~9 TU/ml. The CD40 mRNA inhibition rate after infection was significantly higher(P<0.05). The CD40 protein expression of DCs was significandy lower(P<0.05). The result of MLC demonstrated that the index of stimulation to T lymphocyte of CD40 RNAi transfected DC significantly decreased compared with non-transfected DC and empty plasmid transfected DC(P< 0.01). Conclusion Large quantity of myeloid DCs with typical histological configuration are obtained through in vitro culture in selective medium which may activate naive T lymphocyte to generate immune response. While DCs were infected by CD40 RNAi lentiviral vector, CD40 protein expression was inhibited significantly. The DCs could hamper the activation of allogeneic T lymphocyte by blocking CD40/CD40L cestimulatory pathway and induce T cell allergy. This will make the good foundation for studying the immune tolerance of CD40 RNAi.
7.Effects of blocking CD40/CD40L costimulatory pathway by RNA interference on the survival of mouse cardiac allograft

Objective To investigate the effect of blocking CD40/CD40L costimulatory pathway by the lentiviral vector-mediated RNA interference on the survival of mouse cardiac allograft. Methods Mouse bone marrow-derived dendritic cells (DCs) were infected by CD40-RNAi lentiviral vector in vitro, and tolerogenic DCs (Tol-DCs) with decreased CD40 expression were prepared. Fluorescence real-time quantitative PCR and flow cytometry were used to analyze the expression of CD40 mRNA and DC surface antigens CD40, CD11c, MHC Ⅱ before and after infection. Mouse model of heterotropic abdominal heart transplantation was established. Seven days prior to heart transplantation, Tol-DCs with decreased CD40 expression were transfused into recipient mice intravenously (lentivirus infected DC group). Control group and non-infected DC group were assigned simultaneously. The survival of cardiac allograft was monitored and pathological grade of acute rejection 7 days after heterotropic abdominal heart transplantation was determined. Results The transcription of CD40 mRNA of DCs was down-regulated significantly at 48 h after CD40-RNAi lentiviral vector infection, and the inhibition rate was 80. 9%. The expression of CD40 protein was also significantly decreased as compared with control group (40. 07% ± 4. 03% ) ( P < 0. 05 ).Compared to control group (8 ± 2 days) and non-infected DC group (9 ± 1 days), the survival time of cardiac allograft in CD40-RNAi lentivirus infected DC group (14 ± 4 days) was significantly prolonged (P< 0. 05 ), and the pathological grade of acute rejection decreased significantly ( P < 0. 05 ).Conclusion Blocking CD40/CD40L costimulatory pathway could hamper the activation of allogeneic T lymphocyte, inhibit the acute rejection and prolong the survival of mouse cardiac allograft.

9.A analysis of clinical value about endoscopic nipple modified resection in duodenal papillary adenoma resection

Objective: To analyze the clinical value of modified endoscopic nipple resection in duodenal papillary adenoma resection Methods: 40 patients with duodenal papillary adenoma were randomly divided into observation group (20 cases) and control group (20 cases)according to the random number table. The patients of control group were treated by conventional resection of papillary adenoma, while the patients of observation group were treated by modified endoscopic nipple resection. The resection rate and the implantation of stent between the two groups were compared. Besides, the occurrence rate of complication and postoperative follow-up between the two groups also were compared. Results: The complete resection rate of observation group (95.00%) was not significant difference with that of control group (90.00%), while en bloc resection rate of observation group (85.00%) significant higher than that of control group (55.00%) (x2=4.28, P<0.05).There was not statistically significant difference between the two groups about the implantation of stent (x2=0.36, P>0.05). In postoperative three months, the difference of recently complication between the two groups was significant (x2=6.14, P<0.05). While the difference of long-term complication between the two groups was no significant (x2=0.36, P>0.05) in more than postoperative three months. After the follow-up of one year, the recurrence rate of observation group (10.34%) was no significant with that of control group (14.29%)(x2=0.17, P>0.05). Conclusion: The modified endoscopic nipple resection can enhance the en bloc resection rate, and reduce the incidence of complications and the recurrence rate. Besides, it has many dominances including higher feasibility in clinical practices. Therefore, its promotion is worthy.
10.Discriminating schizophrenia with combination of multiple tests:a pilot study

Objective: The pilot study was to explore the possibility of discriminating schizophrenia from the normal with combination of multiple tests. Methods: Forty medication-naive (25 cases) or medication-free for at least three weeks (15 cases) patients with schizophrenia and 41 healthy normal controls matched with sex, age, headedness, and education, were recruited in the study. The Structured Clinical Interview (SCID) was used to render the DSM-Ⅳ diagnoses for schizophrenia or schizophreniform disorders. The intrahemispheric coherences between the parietal and frontal electrode pairs (left: F3/P3; right: F4/P4) and interhemispheric coherences between frontal leads (F3/F4) and parietal leads (P3/P4) in the wide-band alpha frequency and peak frequency of photic driving were calculated at rest and during 1Hz light stimulation, respectively. Tests of exploratory eye tracking movement test (ETM), Wisconsin Card Sorting Test (WCST), and Continuous Performance Test (CPT) were finished in the same working day. Stepwise discriminant function analyses were chosen to re-classify the patients and controls. Results:In the discriminant analyses, 61.7% of the subjects could be correctly classified into groups of patients or normal controls using the EEG coherence alone, 77.27% with ETM measures alone, and 70.6% when WCST and CPT parameters were combined. The correct discriminate rate increased to 81.5% when all the above variables were taken into consideration.Conclusion:In consideration of the heterogeneity of schizophrenia, it is suggested necessarily to combine multiple tests to make discrimination of schizophrenia.
