1.Artesunate versus Quinine MAX for Malaria:Cost-minimization Analysis

OBJECTIVE:To evaluate the economic effects of artesunate(ART)versus Quinine MAX(QUI)in treating malaria.METHODS:A total of 64 patients with malaria were randomly assigned to receive ART or QUI for 7 days,with cost-minimization analysis conducted on the 2 therapeutic scheme.RESULTS:The average cost for ART vs.QUI scheme was 341 yuan vs.485 yuan,showing significant differences between the 2 schemes(P0.05).CONCLUSION:The artesunate is preferable compared with QUI and could be popularized in poverty-striken area such as Africa.

ObjiectiveTo investigate the clinical efficacy of Chinese medicineShuxin-Huoxue decoction on coronary heart disease and angina pectoris.MethodsThis study was a randomized controlled trial. 86 patients with coronary heart disease and angina pectoris and conformed to the indusive criteria were randomly divided into a control group and a treatment group, with 43 cases in each group. The control group was treated with conventional western medicine, the treatment group was treated with traditional Chinese medicineShuxin-Huoxue decoction. Serum homocysteine (Hcy) was measured by enzymatic cycling assay, high sensitive C reactive protein (hs-CRP) was measured by immunotubidimetric assay and left ventricular ejection fraction (LVEF), stroke volume (SV), cardiac output (CO), cardiac index (CI) were measured by color doppler echocardiography before and after treatment. The clinical effect of the two groups was evaluated.ResultsThe total effective rate of the treatment group and the control group was 90.7% (39/43) and 74.4% (32/43) respectively,there was significant difference (χ2=3.957,P=0.047). Followed up for 4 weeks, LVEF(65.7% ± 8.7% vs. 61.5% ± 8.4%,t=2.277), SV(86.9 ± 16.8 ml vs.79.2 ± 16.5 ml,t=2.144), CO(49.3 ± 9.1 ml/m2 vs. 44.7 ± 8.8 ml/m2,t=2.383), CI (3.7 ± 0.9 L/min?m2 vs. 3.3 ± 0.7 L/min?m2,t=2.301) of the treatment group were significantly improved after treatment than those of the control group (P<0.05), and the decrease of serum Hcy (8.7± 3.7mmol/L vs. 11.4± 3.5mmol/L,t=3.476), hs-CRP (2.7± 1.1 mg/L vs. 3.5± 1.3 mg/L,t=3.081) level of the treatment group were better than those of the control group(P<0.01).ConclusionTraditional Chinese medicineShuxin-Huoxue decoction has good therapeutic effect on coronary heart disease and angina pectoris.
4.Clinical retrospective analysis of the relationship between serum calcium levels and apathetic hyperthyroidism

The difference in serum calcium levels between patients with typical hyperthyroidism and apathetic hyperthyroidism was analyzed. Serum calcium levels in patients with apathetic hyperthyroidism were higher than those in patients with typical hyperthyroidism, while bone-specific alkaline phosphatase levels were lower. The atypical manifestation of apathetic hyperthyroidism may be due to the significant elevation of serum calcium level.
5.Effect of basic fibroblast growth factor on the proliferation and differentiation of human preadipocytes

BACKGROUND:Previous studies have shown that basic fibroblast growth factor (bFGF) can enhance fat survival rate by promoting vessel regeneration of transplanted fat.However,there is no clear conclusion addressing its effects on adipocytes.OBJECTIVE:To investigate the effect of bFGF on the proliferation and differentiation of human preadipocytes.DESIGN,TIME AND SETTING:Cytological in vitro comparison observation was conducted at the Medical College of Chinese People's Armed Police Force from March 2006 to December 2007.MATERIALS:Totally 10 samples of adipose tissue from patients undergoing abdominal liposuction were used in this study,including 1 male and 9 females,aged 21-45 years.METHODS:The human preadipocytes were obtained from human abdominal fat particles by isolation,culture and natural purity.The experiment was divided into bFGF and control groups.Cells in the control group were incubated in DMEM.Cells in the bFGF group were incubated in DMEM supplemented with 100 ng/L bFGF.MAIN OUTCOME MEASURES:Morphology of cultured cells was studied to determine the growth curve at 0,3,6,9,12,15 days following culture by inverted phase contrast microscope.Fat aggregation was observed in cells using oil red O staining.Absorbance value was examined using enzyme-linked immunosorbent assay.RESULTS:The preadipocytes represented the spindle-like character with strong proliferation.During 1 to 15 days,the preadipocytes proliferated into monolayers.Some cells became single bubble cells with lipid droplets deposited,close to maturity.The cells in bFGF group had no significant differences with the control group in cell morphology,but the amount of cells in bFGF group was 44% more than the control group.Following oil red O staining,lipid droplet was found in the preadipocytes,showing dark red.Following 15 days of incubation,the concentration of lipid droplet reached a peak.The absorbanca of bFGF group was increased 300% compared with the control group (P < 0.05).CONCLUSION:The bFGF not only promote the proliferation of human preadipocytes,but also induce the differentiation of preadipocytes into mature adipocytes.
6.The determination of organic acids in urine with gas chromatography-mass spectrometry and its application in clinical diagnosis

Objectivo To analyze urine organic acids in the urine using gas chromatography-mass spectrometry(GC/MS)for diagnosis of inherited metabolic diseases,especially for organic acids metabolic disorders.Methods 195 clinical urine samples from the patients with suspected organic acids metabolic disorders and 5 normal urine from adults were collected.After mixing some urine with intemal standards according to the concentration creatinine and adding hydroxylamine hydrochloride to mixture,the organic acids with hydroxyl group were oximated to the ketobodies.Organic acids were extracted twice with ethyl acetate and ethyl ether and derivatized with BSTFA-TMCS.An the organic acids were determined with Agilent GC/MS 6890/5973i with scan model.the mass-to-charge ratio range is 50-550 m/z,all data were nalyzed with Agilent GCMSD ChemStationG1701DA.We also investigated the linearity, accurate,precision.recovery and Carry-over by determining the internal standards in normal samples and positive organic acids in spiked control samples.Results More than one hundred kinds of organic acids in urine samples can be analyzed with this method.According to the two internal standards in normal urine samples,the minimal detection limit MMA and 2.PA was 2.5-2.8 μmol/L.Intra-and interassay coefficient of variation for MMA and 2-PA are both less than 10%.Pre-processing Interassay coefficient by sequential preparations of the same sample was 14%.The recoveries of the spiked samples were 95%-105%.Carryover analysis was less than 1%.All the parameters meet the requirement for clinical diagnosis.12 samples demonstrated positive including 6 cases of methylmalonic acidemia,1 case of propionic acidemia,3 cases of tyrosinemia Ⅰ,1 case of maple syrup urine disease and 1 cases of ketosis.Conclusions The method for the determination of organic acids in urine by GC/MS has been successfully established.It can be used for clinical screening and diagnosis for inherited genetic metabolic diseases.
7.Applicable value of real time interventional ultrasound guidance in family planning reproduction operation

Objective To determine the clinical value of real time interventional ultrasound guidance in family planning reproduction operation.Methods Under the guidance of ultrasound monitoring,522 cases with high risk and difficult uterine operation were undertaken in our department.Results The abdominal endoscopic contraceptive uterine operation under real time ultrasound monitoring was carried out for 522 cases in 4 years,with successful rates for high risk pregnancy as 287/289 cases,high risk troublesome withdrawal of contraceptive ring as 129/130 cases and puzzling uterine operation as 103/103 cases.The total successful rate reached 99.42%,without uterine rupture and other complications.Conclusion The former complex,blind and difficult uterine operations turn to be simple,safe and reliable under the guidance of real time ultrasound.
8.RGD and cell penetrating peptides co-modiifed paclitaxel loaded liposome inhibit the A549 cells

Objective To prepare RGD and TAT co-modified paclitaxel loaded liposome(RGD/TAT-LP-PTX)for A 549 cells targeting.Method The co-modified liposome was prepared by film-ultrasonic method. The appearance,particle size,Zeta potential were evaluated. The cellular uptake by A 549 cells in vitro was used to evaluate the targeting efficiency. The anti-proliferation efficiency of RGD/TAT-LP-PTX was evaluated by MTT assay. Results The particle diameter of the co-modified liposome was (118.5±11.4) nm with the Zeta potential of (21.58±2.42 )mV. The entrapment efficiency of PTX was 86.5%. The result demonstrated that the co-modified liposome uptaken by A 549 were 2.1, 2.8 times higher than that of TAT-LP and RGD-LP, respectively. The RGD/TAT-LP-PTX shows the highest anti-proliferation efficiency. Conclusion The co-modified liposome might serve as a promising tumor delivery system of antitumor drugs.
9.National Natural Science Foundation of China (NSFC) and the progress of basic research of pancreatitis in China

Objective To evaluate the funding and output of pancreatitis related projects supported by National Natural Science Foundation of China (NSFC),and provide suggestions for the funding strategy in the field of pancreatitis research.Methods Database of funded projects and completion reports of NSFC between 1990 and 2012 were retrieved.The number of grants,amount of grants,types,contents,and paper output of funded projects were respectively analyzed.Results From 2000 to 2012,114 projects involving pancreatitis were funded by NSFC,including 71 general programs (62.3%),8 regional programs,34 youth programs (29.8%),1 key program.The total fund during 2011-2012 reached 25.44 million RMB for pancreatitis research,which was 2.76 times higher than that of 2000-2010.According to PubMed search,93 SCI papers funded by NSFC were published during 2011-2012,which was 2.07 times more than that of 2000-2010.The researches mostly focused on molecular mechanism and therapeutic effect of acute pancreatitis,chronic pancreatitis and severe acute pancreatitis,which accounted for 61.4% of all projects.Conclusions The number of grants,amount of grants,and NSFC funded SCI paper output of pancreatitis research have grown steadily,and it indicates that long-term funding of NSFC has played a positive role in promoting the development of clinical and basic research in the field of pancreatitis.
10.The approach on methods of replacing tracheostomy tube after incision of trachea in 48 hours

Objective To explore the more secure and available methods in replacing traeheostomy tube.Methods Sixty patients with incision of trachea were divided into group A(29 eases)and group B (31 cases)randomly.The new method was used in group A with putting a catheter into the old tracheostomy tube before taking it out,and placing the new tracheostomy tube under the guide of catheter.While the traditional method was used in group B.The heart rate,SpO2,manipulation time,perioperative haemorrhage, and the condition of entering the false passage were recorded.Results The heart rate,SpO2 were no significant difference before and after manipulation in group A,but the heart rate increased and SpO2 decreased in group B(P<0.01 or<0.05).The manipulation time was(50.5±4.2) s in group A,and (84.9±5.3) s in group B(P<0.05).The perioperative haemorrhage >3 ml and the condition of entering the false passage in group A(2 Cases,o cage)were less than those in group B(15 cases.5 cases)(P<0.01 or<0.05).Conclusion The new method in replacing tracheostomy tube which use an input catheter is more security and more availability.
