1.Correlation study of type 2 diabetes mellitus and serum carbohydrate antigen 19-9 level

Objective To observe the changes of carbohydrate antigen 19-9 (CA19-9) in type 2 diabetes mellitus (T2DM) patients,and determine the positive boundary value of CA19-9 for differentiating digestive tract malignant tumor and T2DM.Methods The clinical data of 300 cases were retrospectively analyzed.The patients were divided into 3 groups:group A with 100 T2DM patients,group B with 80 tumor patients and group C with 120 non-tumor and non-diabetic mellitus normal people.Chemiluminescence was used to determine the serum CA19-9 level,and the results of the 3 groups were analyzed.Results The serum CA 19-9 levels in group A and group B were significantly higher than that in group C [(21.38 ± 18.23),(32.21 ± 19.00) kU/L vs.(11.21 ± 10.08) kU/L],and there were statistical differences (P <0.05).Furthermore the serum CA 19-9 level in group B was significantly higher than that in group A,and there was statistical difference (P < 0.05).The serum CA19-9 level was associated with fasting plasma glucose (r =0.527,u =5.144,P < 0.01),2 h postprandial blood glucose (r =0.617,u =6.044,P < 0.01),fasting insulin (r =0.427,u =4.144,P < 0.01),and glycosylated hemoglobin (r =0.507,u =4.881,P < 0.01),while the serum CA19-9 level was not associated with serum creatinine and courses of T2DM.CA19-9 ROC curve was used to determine 45.0 kU/L as the positive boundary value between the group A and group B.Conclusions The serum CA 19-9 level of T2DM patients is higher than that of normal people,but lower than that of patients with digestive tract malignant tumor.The serum CA 19-9 level is associated with blood glucose and glycosylated hemoglobin.The 45.0 kU/L as serum CA19-9 level positive boundary value can significantly improve the diagnosis value of digestive tract malignant tumor.
2.Relationship Between Endometriosis and Menstruation as well as Gestational Status

To explore the relationship between endometriosis with menstruation as well as gestational status. [Methods] Case- group comparative trial was applied to investigate the menstruation as well as gestational status in 91 cases of endometriosis and 67 cases of non - endometriosis in the same period. [ Results ] Single - factor analysis showed that primary dysmenorrhea and secondary dysmenorrhea were the risk factors of endometriosis. Age of primigravida and primiparity at 24 or less, times of pregnancy over 2 and the application of contraceptive device were the possibly protective factors. Unconditional logistic multivariate regression analysis showed that primary dysmenorrhea and secondary dysmenorrhea were the risk factors of endometriosis, and there was a positive interaction between the age of primigravida and the application of contraceptive device. [Conclusion] Primary dysmenorrhea was the risk factor of endometriosis.

4.Therapeutic Observation of Acupoint Application plus ModifiedZhi Sou Powder for Post-cold Cough

Objective To observe the clinical efficacy of acupoint application plus modifiedZhi Sou powder in treating post-cold cough.Method A total of 120 patients with post-cold cough were randomized into group A, group B, and group C, 40 cases in each group. Group A was intervened by acupoint application plus modifiedZhi Sou powder, group B by oral administration ofZhi Sou powder, and group C by oral administration of compound Dextromethorphan syrup. The three groups were intervened once a day, 7 days as a treatment course. The clinical efficacies were compared after 2 treatment courses, and the adverse reactions during the study were observed.Result The total effective rate was 87.5% in group A, 67.5% in group B and 60.0% in group C. The total effective rate in group A was significantly different from that in group B and group C (P<0.05). The adverse reaction rate was 7.5% in group A, 5.0% in group B and 72.5% in group C. The adverse reaction rates in group A and B were significantly different from the rate in group C (P<0.01).Conclusion Acupoint application plusZhi Sou powder is an effective approach in treating post-cold cough, and it’s more effective than ordinary drug and produces less adverse reactions.
5.Reliability and Validity of Morisky Questionnaire in Measurement of the Compliance with Hypertensive Medications

Objective To evaluate the reliability and validity of Morisky questionnaire in measurement of the compliance with hypertensive medications. Methods Seventy-four eligible subjects were selected and asked to fill a Morisky questionnaire about their adherence in taking antihypertensive drugs during the past 1 month; nineteen of them received two surveys at 3~7 day intervals. The internal consistency (Cronbach ? coefficients),test-retest reliability (interclass correlation coefficients),construct validity and convergent validity were calculated. Results The Cronbach ? coefficient of the questionnaire was 0.749 and the interclass correlation coefficients between the items and the total scores of the questionnaires ranged from 0.71 to 1.00. Factor analysis yielded one factor with an accumulated contribution rate of 57.3%,and the loading values of all items on this factor were above 0.6. 6.Function of vascular endothelial growth factor in ovarian hyperstimulation syndrome

Objective To evaluate the function of vascular endothelial growth factor (VEGF) and other clinical indexes to forcast ovarian hyperstimulation syndrome (OHSS). Method Collecting the serum and follicular fluid (FF) of 42 cases in the in vitro fertilization and embryo transfer (IVF-ET) cycles and then analysis the relationship of the clinical and detected indexes between the OHSS (10 cases) and the non-OHSS (32 cases). Results The concentration of VEGF in FF?E 2 on the day of human chorionic gonadotropin (hCG) injection, basal luteinizing hormone (LH) and the number of ovum retried much higher in the OHSS than in the non-OHSS. Conclusions The concentration of VEGF in FF of OHSS cases is higher than that of controls, supporting the role of VEGF as a mediator of OHSS. Therefore VEGF in FF is a forcast index of OHSS.
7.Obesity parameters and obstructive sleep apnea-hypopnea syndrome in aircrew

Objective To investigate the relationship between obesity parameters and obstructive sleep apnea-hypopnea syndrome (OSAHS) in aircrew. Methods A questionnaire survey in aircrew members was performed. Outcome measurement included body height, body weight, neck circumference ( NC), waist circumference (WC), and body mass index (BMI). Those with snoring during sleep and/or Epworth sleepiness scale (ESS)score ≥ 9 were screened with pulse oxygen saturation test during overnight sleep. Those with oxygen saturation decrease index ≥ 10 times/h and suspected OSAHS were tested by polysomnography (PSG). The objects were then assigned to three groups: the OSAHS group,the snore group and the normal group. The relationship between obesity parameters and OSAHS was analyzed. Results There were 399 ( 37. 54% ) overweight, 36 ( 3. 39% ) obesity, 130 ( 12. 23% ) increased NC, and 354 (33.30%)increased WC in 1063 aircrew members (OSAHS group > snore group > normal group; P<O. 05). The mean value of BMI, NC and WC in the three groups were dittos. Multiple logistic regression analysis showed that overweight, obesity and increased WC were significantly associated with snoring (P <0.05) and snoring enlarged NC was significantly correlated with OSAHS (P< 0.05). Conclusion Increased BMI, NC and WC may be risk factors of OSAHS among aircrew. Effective prevention and treatment of OSAHS should be needed.
8.The clinical observation of the repair of pharyngeal mucosa and neck skin defects with acellular dermal matrix.

OBJECTIVE:
To evaluate the clinical effect of acellular dermal matrix (ADM) on the repair of different tissue defects in pharyngeal and laryngeal cancer.
METHOD:
Twenty-three cases of pharyngeal and laryngeal cancer were analyzed retrospectively, including 16 patients with pharyngeal cancer, and 7 patients with laryngeal cancer. After surgery, the patients appeared two types of tissue defects respectively: hypopharynx tissue defect and neck skin tissue defect. These defects were repaired with ADM, and the clinical effects were observed.
RESULT:
Sixteen cases of hypopharynx tissue defect and 7 cases of neck skin tissue defect were repaired with ADM. One case in hypopharynx tissue defect group and 2 cases in neck skin tissue defect group undergone pharyngeal fistula postoperatively.
CONCLUSION
The repair of tissue defects in pharyngeal and laryngeal cancer with ADM had some advantages, such as good histocompatibility, low immune rejection, wide material sources and simple method. There were some differences in clinical effects between hypopharynx cavity defect and neck skin tissue defects, so the emphasis of the need to pay attention to is different in the clinical.

9.Comparison between stapled and traditional suture closure total laryngectomy.

OBJECTIVE:
To compare the clinical results between stapled and traditional suture closure total laryngectomy.
METHOD:
Fifty-three cases of laryngeal cancer with total laryngectomy were divided into 2 groups: 32 cases with traditional suture closure total laryngectomy (group A) and 21 cases with stapled total laryngectomy group (group B). Compare two groups in pharyngeal fistula, postoperative bleeding, dysphagia, and nasal regurgitation.
RESULT:
There was no difference of postoperative pharyngeal fistula between group A and B (P>0. 05). The incidence of dysphagia, nasal regurgitation, and postoperative bleeding in group B were 23. 8%, 14. 3% and 14. 3% respectively, which were obviously higher than that in group A (P<0. 05).
CONCLUSION
Stapled total laryngectomy has the advantages of reducing the operative time and simplifying the operation with some disadvantages such as higher standards of operation indications, higher incidence of dysphagia, higher nasal regurgitation, higher postoperative bleeding, and poor medical economic profit. Traditional suture closure total laryngectomy is recommended in clinical practice.
