1.Effects of Interleukin-1 Receptor Antagonist Gene Polymorphism in Cardiovascular Events of Patients with Coronary Arterial Disease

Objective To test the relationship between interleukin-1 receptor antagonist(IL-1Ra)gene polymorphisms and the incidence of coronary artery disease and the effect of IL-1Ra gene polymorphisms in prediction cardiovascular events.Methods Polymerase chain reaction was used in a series of 220 patients of coronary arterial disease(CAD)and a control group of 100 cases.A prospective 12 months follow-up was conducted in the CAD patients and cardiovascular events were recorded.Serum levels of total cholesterol,triglyceride,low density lipoprotein cholesterol,fibrinogen,hsCRP,and body mass index were measured.Results There was no significant difference between CAD patients and the control subjects in IL-1Ra gene polymorphisms.However,the IL-1Ra gene polymorphism was significantly higher among subjects in CAD patients without cardiovascular events than those with cardiovascular events.Conclusion It suggests that IL-1Ra gene polymorphisms can not predict the incident of coronary heart disease but might possess some protection for the patients of coronary arterial disease from cardiovascular events.
2.Therapeutic effect analysis of three internal fixation methods in the treatment of intertrochanteric femur fracture

[Objective] To discuss the therapeutic effect of different methods in the treatment of intertrochanteric femur fracture. [Methods]Totally 118 middle aged or old patients with intertrochanteric femur fracture were fixed with dynamic hip screw (DHS), Gamma nail or PFNA.[Results]Totally 112 cases were followed up, and the average time was 16 months, in those 45 patients were fixed with DHS: 29 were excellent, 11 cases were good, and 5 were poor, and the excellent to good rate was 88.9%; 36 patients were treated with Gamma nail, and the excellent, good, poor results were 28, 2, 1, respectively, and the excellent to good rate was 96.8%; 36 patients were fixed with PFNA, and the excellent, good, poor results were 29, 6, 1, respectively. The differences were significant (P=0.02, analyzed by Logistic regression with SPSS software). The excellent and good superiority of Gamma nail and PFNA were 5.1552 and 2.3726 times compared to DHS, and there was significant differences between the groups.[Conclusion]Surgical treatment of intertrochanteric femur fracture can improve the functional recovery and reduce the complication incidence, and PFNA fixing method can shorten the operative time, reduce the bleeding and the therapeutic effect is reliable, and suitable for old patients combined with osteoporosis.
3.Effects of different delivery methods on maternal and fetal complications during labor

Objective To investigate the effects of 3 different delivery methods on maternal and fetal complications during labor and determine better mode of delivery. Methods Sixty pregnant women with ASA class Ⅰ or Ⅱ were randomly divided into three groups with 20 cases each: the cesarean section group (group Ⅰ), the pain relief group (group Ⅱ) and normal delivery group (group Ⅲ). In group Ⅰ and Ⅱ, an epidural catheter was placed through L_(2-3) interval. 0.5% ropivacaine was injected via the epidural catheter for continuous epidural anesthesia in group Ⅰ. After a loading dose of 8-10 mL 0.125% ropivacaine+2 μg/mL fentany, the patient-controlled epidural analgesia (PCEA) during labor was started with bolus 5 mL, lockout interval 20 min and back ground infusion rate 6-8 mL/h in group Ⅱ. The level of block was controlled below T_(10). BP, HR and RR were continuously monitored during labor. Blood samples were taken from umbilical vein at delivery for blood gas analysis and determination of lactate concentration. Neonates were assessed by Apgar score and neurological and adaptive capacity score (NACS). Maternal postoperative complications were recorded. Results There was no statistical differences in plasma pH, PCO_2, PO_2, SO_2, Apgar score and NACS among 3 groups. The umbilical vein blood lactate concentration was significantly increased in group Ⅱ and Ⅲ compared to the group Ⅰ, while blood lactate concentration in group Ⅱ was lower than that in the group Ⅲ (P<0.05). Base excess (BE) was significantly higher in group Ⅲ than in group Ⅰ. Compared with group Ⅱ and Ⅲ, maternal postoperative complications such as urinary retention, postpartum hemorrhage and so on were increased significantly in group Ⅰ (P<0.05). PCEA provided satisfactory effect in group Ⅱ. Conclusions Epidural labor analgesia with 0.125% ropivacaine+2 μg/mL fentany can provide comparative and effective analgesia for pain relief during labor without adverse effects on the mothers and fetuses and may reduce the postoperative complications. It is a safer and more effective technique in pain relief during labor.
4.Experimental study of heme oxygenase-1 gene transfer prevents chronic allograft nephropathy

Objective To investigate the effects of HO-1 on renal allografts after HO-1 gene transfection in rat chronic allograft nephropathy model. Methods Twenty F344 and twenty-six Lewis rats were included in this experiment. They were divided into 3 groups at random. Six Lewis rats were in pseudo-operation group, 10 Lewis were in empty carrier group (transfected with adenovirus) and 10 Lewis were in the gene transfection group (transfected with Ad-HO-1 adenovirus). The expression of HO-1 protein was detected by WB at the 1st ,2nd,3rd and 4th week. The content of creatinine in blood was assayed at the 4th,8th, 12th and the 16th week. The weight of rat, the value of patholiogical changes and the expression of ?-SMA,TGF-?1 and PDGF-B were analysized at the 16th week. Results The weight of rats in 3 groups had not any changes at 16th week. The content of creatinine in blood of gene transfection group were lower than those in the empty carrier group. The expression of HO-1 protein were very high in the 1st and 2nd week and decreased at 3rd and 4th week. The Banff value of kidney in the gene transfer group was better than that of the empty carrier group at the 16th week. The level of ?-SMA, TGF-?1 and PDGF-B in the gene transfection group were significantly lower than those in the empty carrier group. Conclusions Ad-HO-1 could efficiently transfer HO-1 gene into rat donor kidney. The prevention of chronic allograft nephropathy may have relationship with the decreasing expression of ?-SMA, TGF-?and PDGF-B.
5.Transfection of Ad-HO-1 on renal tubular epithelial cell HK-2 and its effect on PBMC proliferation

Objective To observe the cytotoxicity of Ad-HO-1 and its capacity of mediating the expression of HO-1 in cultured HK-2 cells. Methods The HK-2 cells were infected by Ad-HO-1 with 100, 200 and 400 MOI for 3 h, followed by the green fluorescence observation 2 days later. The live cells ratio was detected 2 and 4 days later. The expression of HO-1 and GFP in HK-2 were detected under laser scan confocal microscope. The expression of HO-1 was detectee by Western blot analysis. 3H-TdR was used to assay the ability of HO-2 cells infected with Ad-HO-1 to inhibit the PBMC proliferation. Results Over 90% HK-2 cells expressed green fluorescence after infected by Ad-HO-1 (MOI 200) for 2 days. The live cell ratio at 2 and 4 days had not any difference from those of the control group. The expressions of HO-1 and GFP in the Ad-HO-1 transfected HK-2 cells were determined under laser scan confocal microscope. The locations of these two proteins were the same. HK-2 cells infected by Ad-HO-1 had protein immune blot strap combined with HO-1 antibody. When the MOI of Adv-HO-1 was 0.01, 0.1, 1 and 10, the proliferative capacity of PBMC was inhibited significantly. Conclusion The stable expression of HO-1 mediated by Ad-HO-1 in cultured tubular epithelial cells can inhibit the cell proliferation of PBMC.

7.Discriminating schizophrenia with combination of multiple tests:a pilot study

Objective: The pilot study was to explore the possibility of discriminating schizophrenia from the normal with combination of multiple tests. Methods: Forty medication-naive (25 cases) or medication-free for at least three weeks (15 cases) patients with schizophrenia and 41 healthy normal controls matched with sex, age, headedness, and education, were recruited in the study. The Structured Clinical Interview (SCID) was used to render the DSM-Ⅳ diagnoses for schizophrenia or schizophreniform disorders. The intrahemispheric coherences between the parietal and frontal electrode pairs (left: F3/P3; right: F4/P4) and interhemispheric coherences between frontal leads (F3/F4) and parietal leads (P3/P4) in the wide-band alpha frequency and peak frequency of photic driving were calculated at rest and during 1Hz light stimulation, respectively. Tests of exploratory eye tracking movement test (ETM), Wisconsin Card Sorting Test (WCST), and Continuous Performance Test (CPT) were finished in the same working day. Stepwise discriminant function analyses were chosen to re-classify the patients and controls. Results:In the discriminant analyses, 61.7% of the subjects could be correctly classified into groups of patients or normal controls using the EEG coherence alone, 77.27% with ETM measures alone, and 70.6% when WCST and CPT parameters were combined. The correct discriminate rate increased to 81.5% when all the above variables were taken into consideration.Conclusion:In consideration of the heterogeneity of schizophrenia, it is suggested necessarily to combine multiple tests to make discrimination of schizophrenia.

OBJECTIVE:To improve the efficiency and accuracy in database management of drug storehouse.METH?ODS:The computerized database management pattern of drug storehouse in the hospital where the authors work was intro?duced as follows:the account books of storehouse in-and out-record as well as invoice payments were established in Excel by computers,and these drug storehouse data were managed with the support of‘Military Health No.1’system.RESULTS&CONCLUSION:The established pattern can fill in the gap in‘Military Health No.1’system and improve the database man?agement of drug storehouse in hospital.
9.Effect of acute hypervolemic hemodilution on cerebral O_2 supply/consumption and energy metabolism during general anesthesia combined with epidural block in adults

Objective To investigate the effects of acute hypervolemic hemodilution (AHH) on cerebral O2 supply/consumption and energy metabolism in patients undergoing elective surgery under general anesthesia combined with continuous epidural block. Methods Fourteen ASA Ⅰ or Ⅱ patients of both sexes (9 male, 5 female) aged 44-62 yrs weighing 55-70 kg undergoing elective surgery under general anesthesia combined with epidural block were enrolled in this study. Lactated Ringer's solution 6-8 ml?kg-1 was infused before anesthesia. After correct placement of epidural catheter was confirmed radial artery was cannulated for BP monitoring and blood sampling. BP, EGG, SpO2 and PET CO2 were continuously monitored during anesthesia. General anesthesia was induced with fentanyl, midazolam, propofol and rocuronium and maintained with iaoflurane and intermittent i. v. boluses of vecuronium. The patients were intubated and mechanically ventilated. PET CO2 was maintained at 30-35 mm Hg. Epidural block was produced by a mixture of 1.33 % lidocaine + 0.167 % dicaine. Right internal jugular vein was retrogradely cannulated and advanced cephalad until jugular bulb for blood sampling. AHH was conducted by infusing 6% HES 20 ml?kg-1 at 50 ml?min-1 after induction of general anesthesia. Blood samples were taken from artery (a) and jugular venous bulb (jv) simultaneously before AHH (T0) at 15 min (T1) and 120 min (T2) after AHH was accomplished for blood gas analysis and determination of Hb, glucose and lactate concentrations. Cerebral O2 extraction rate (CERO2), glucose extraction rate (CMRglu) , arterial-jugular bulb venous O2 content difference (D a-jvO2) and arterial-jugular bulb venous lactate difference (VADL) were calculated. Results At the end of AHH Hct and Hb decreased by 20% as compared to the baseline values before AHH. S jvO2 was significantly higher, D a-jvO2 smaller and CERO2 lower at T1 than at T0. There were no significant differences in BP, HR, CMRglu and VADL at all time points. Conclusion AHH can increase cerebral O2 supply and has no significant effect on cerebral glucose metabolism during operation performed under general anesthesia combined with epidural block.
