1.Protective effect of constant magnetic field on ischemic-reperfusion brain

BACKGROUND: Magnetic therapy has a long history and is used in the treatment of various diseases. To study the protective function of constant magnetic field for ischemic cerebrovascular diseases may provide new clinical foundation for non-medicinal and non-traumatic treatment and develop a novel way for treatment with physical factor.OBJECTIVE: To observe the influence of constant magnetic field treatment on hemodynamics at macroscopic level, RBC membrane fluidity at subcellular level and antioxidase activity at molecular level, as well as NO and NO synthetase activity in rats with ischemic-reperfusion (IR) injury.DESIGN: A completely randomized design.SETTING: Biophysical Teaching and Research Department of Harbin Medical University.MATERIALS: This experiment was carried out at Biophysical Laboratory,Harbin Medical University, between May and November 2002. Forty healthy Wistar rats were adaptively raised for 1 week before randomized into 3 groups, namely, sham-operation group (n=10), model group (n=15)and magnetic therapy group (n=15).METHODS: ① IR model of middle cerebral artery (MCA) was established in rats in model group and magnetic therapy group, but MCA was only tied up without occlusion in sham-operation group. ② In magnetic therapy group, rat necks were exposed to 40 mT constant magnetic field instantly after ischemic injury for 30 minutes, once a day, while rats in sham-operation group and model group were not exposed to magnetic field.Three groups of rats were anaesthetized at postoperative 7 days for obtaining blood from eyeball, and cut off head for obtaining brain.MAIN OUTCOME MEASURES: ① Hemodynamic changes in three groups of rats. ② Changes of RBC membrane fluidity-related parameters.③ Changes of serum glutathione peroxidase and ceruloplasmin content.④ Changes of brain malonedialdehyde (MDA), NO, and NO synthetase activity indexes.RESULTS: Thirty rats were included and all entered the result analysis.① Hemodynamic parameters: Blood high-shearing, blood low-shearing viscosity, fibrinogen and HCT were remarkably higher in model group than in sham- operation group (P < 0.01), but obviously lower in magnetic therapy group than in model group (P < 0.01). ② RBC membrane fluidity: The fluorescence polarization, average microviscosity and aeolotropy were remarkably higher in model group than in sham-operation group (P < 0.01), and the above indexes were lower in magnetic therapy group than in model group (P < 0.05). ③ Serum glutathione peroxidase and ceruloplasmin contents in model group were remarkably lower than those in sham-operation group (P < 0.05), but were higher in magnetic therapy group than in model group (P < 0.01), and slightly higher than sham-operation group. ④ The brain Cu/Zn-SOD, MDA, NO, NO synthetase activities in model group were remarkably higher than in sham-operation group (P < 0.05 or 0.01),but antioxidase activity was remarkably lower than that in sham-operation group (P < 0.01); all parameters were proved to get better in magnetic therapy group after magnetic field treatment, and better than those in model group (P < 0.05, 0.01).CONCLUSION: Constant magnetic field exposure can remarkably improve rat hemodynamic property, increase RBC membrane fluidity and antioxidase activity, and reduce the content of MDA, NO, NO synthetase,thereby improving organic anti-oxidation capability and effectively preventing free radicals and NO-induced neural damage. It possesses certain function of protecting brain IR injury through holding up the pathophysiological development of brain ischemia injury.
2.CT EVALUATION OF INCREASED INTRACRANIAL PRESSURE WITHOUT LOCALIZING SIGNS

The CT findings in 200 cases of increased intracranial pressure without localizing signs were reported 132 out of the 200 cases (63.1%) were CT positive, among them there were 8 cases with brain metastatic cancer, 11 cerebral cysticercosis, 14 intracranial inflammatory lesions, 18 hydrocephalus with symmetrical ventricular enlargement, 66 mass lesions in the brain and 15 others. 77 out of the 209 cases (36.9%) were CT negative, among whom the diagnosis of the 61 cases coincided with that of BIH. It seems that our BIH cases were much different from those typical cases reported abroad, and the cause would be pending further investigation. It is to be noted that the following remarks, according to our study, would be associated with the positive findings in CT examination: 1. age, 60 years; 2. course of disease, within 6 months; 3. papilloedema and/or "soft" signs; and 4. local and severe abnomalities in EEG.
3.Imaging diagnosis of ameloblastomas of the jaw

Objective The purpose of the study was to characterize the X-ray and CT findings of ameloblastomas of the jaw.Meth-ods X-ray (n=9)and CT (n=20)findings of 20 patients with pathologically proven ameloblastomas of the jaw were obtained.Image features included location,size,shape,density of the lesion,and changes of adjacent bone were assessed.Results 1 9 cases were located in the mandible,including 10 cases in the molar-ramus region,four cases in the whole half of the mandible body,two cases in the whole half of the mandible body and the opposide mantum,one case in the whole mandible body,two cases in the anterior teeth region and mantum.One case was located in the molar region of the maxilla.Three cases were uniocular and 17 cases were multiocular in shape.All lesions caused expansile changes in the adjacent bone,including expanding towards the lip and bucca(n=15),the lingua(n=3),and expanding uniformly(n=2).The interruptions of the surrounding bone cortex were demonstrated in 1 9 cases.Conclusion There are some characteristic imaging find-ings for ameloblastomas of the jaw.Correct preoperation dignosis can be made on the basis of imaging findings.
4.Clinical application of transcatheter closure of secundum atrial septum defect with Amplatzer occluder in patients over 50 years of age

Objective To elucidate the efficacy and safety of transcatheter closure of secondary atrial septal defect (ASD) with Amplatzer septal occluder (ASO) in people over 50 years of age. Methods Retrospective comparison of 41 patients (14 males, 27 females) with ASD over 50 years of age ranging from 50 to 75(mean age 55.5?7.6, elder group) was conducted with 132 patients younger than 50 years(control group). After diagnosis of ASDs and evaluation of pulmonary artery systolic pressure and right atrial and ventricular dimensions by transthoracic echocardiography (TTE), all patients underwent transesophageal echocardiography (TEE) for complete assessment of ASD size, margins and anatomic relationship of the defect before closure of ASD. Each case was treated with ASO through the percutaneous transcatheter procedure under fluoroscopy and TTE or TEE. Early follow up(3 months) by echo was taken after the intervention. Results There was no difference of mean defect diameter measured by TEE and the balloon-stretched defect diameter of the ASDs between 2 groups. 5.CT and pathologic correlation of acute miliary pulmonary tuberculosis

Objective To elucidate the CT characteristics and pathology of acute miliary pulmonary tuberculosis (AMPT). Methods The CT features of AMPT in 25 cases were analyzed retrospectively, and the CT features in HIV-seronegative and HIV-seropositive patients were compared by 2-sided exact propability Chi-square test. Two lung specimens were inflated and fixed by Heitzman's method. HRCT scans, gross specimen section (80-150 μm) and histologic section (5 μm) were performed on dry lung specimens and CT-pathologic correlation was conducted. The distribution of micronodules in the secondary lobule on HRCT and pathology in one specimen was evaluated by Chi-square test. Results Twenty five patients with AMPT were included in this study, including 11 HIV-seropositive patients and 14 HIV-seronegative patients. HRCT showed diffuse micronodules randomly distributed throughout both lungs in 25 patients, and ground-glass opacity (17 patients)was the predominant complicated finding. Coalescence of nodules and consolidation in HIV-seropositive patients (5 and 6 patients) were markedly higher than that in HIV-seronegative patients (none). In lung specimens, most nodules located in the lung parenchyma between the central bronchovascular bundle and the perilobular structures (792 and 560 nodules), which located in the interlobular septum pathologically. The distribution of micronodules in the secondary lobule showed on HRCT (1060 nodules)and pathology(864 nodules) was not significantly difference(x2=2.814,P>0.05). HRCT showed ground-glass opacities when ARDS occured, which were pulmonary edema,inflammation and hyaline membrane on alveolar wall pathologically. Conclusions The HRCT characteristic of nodule distribution in AMPT is random. ARDS should be suspected when diffuse ground-glass opacities appear on HRCT.
6.Huwentoxin-Ⅰ: Antinociceptive Effects and Its Comparison with ω-Conotoxin-MVIIA on Acute Visceral Pain in Rats

The antinociceptive effect of epidural administration of huwentoxin-I was elucidated in a tonic visceral pain rat model produced by acute colon inflammation. The nociceptive behaviors were induced by perendoscopically injecting dilute formalin (50 μl) into the depth of the colonic wall in rats. Both ω-conotoxinMVIIA and morphine hydrochloride were given epidurally as positive control while saline as negative control.Similar to ω-conotoxin-MVIIA and hydrochloride morphine, the epidural administration of HWTX-Ⅰ significantly reduced the nociceptive responses in a dose-dependent manner in tonic visceral pain rat model ( P < 0.05). The suppression effects of both huwentoxin- Ⅰ and ω-conotoxin-MVIIA at 20 μg/kg were kept steady compared with the saline group and reached their maximum effects at the doses of 50 ~ 75 μg/kg within 1 hour when the nociception had been observed. It was also found that at the same doses, huwentoxin- Ⅰ was less effective in antinociception than ω-conotoxin-MVIIA. However, ω-conotoxin-MVIIA, but not huwentoxinⅠ , caused an obvious motor dysfunction at these doses. The action of morphine hydrochloride was initiated faster, but lasted for a shorter time than that of huwentoxin- Ⅰ and ω-conotoxin-MVIIA. Thus, huwentoxinⅠ , a potent blocker of neuronal N-type voltage-sensitive calcium channels, induced a remarkable dosedependent restrain effect similar to ω-conotoxin-MVIIA and morphine on the tonic visceral pain produced by colonic wall injection of formalin in conscious rats.
7.Value of 1H-MRS on SCA3/MJD diagnosis and clinical course

Objective:To investigate the value of proton magnetic resonance spectroscopy (1H-MRS) on the diagnosis of SCA3/MJD,and to calculate the correlation between 1H-MRS ratio and the clinical score.Methods:Sixteen patients with SCA3/MJD and 19 healthy volunteers were scanned with 1H-MRS.The data of N-acetyl aspartate,creatine,choline-containing compounds,myoinositol,NAA/Cr,Cho/Cr,and mI/Cr ratio were collected,which were grouped for comparative study.The onset patients with SCA3/MJD were evaluated with the International Cooperative Ataxia Rating Scale and Scale for the Assessment and Rating of Ataxia,the correlation between NAA/Cr,Cho/Cr or mI/Cr ratio and the clinical score was calculated.Results:The NAA/Cr in the pons and cerebellar dentate nucleus from the onset patients with SCA3/MJD was significantly reduced compared to that in the normal control group.The NAA/Cr in the cerebellar dentate nucleus of onset patients with SCA3/MJD was obviously correlated with ICARS.Conclusion:SCA3/MJD lesions are mainly located in the cerebellum and brainstem,where gray and white mater are also involved.The cerebellar dentate nucleus may be the earliest involved area.There is a correlation between the ICARS and the cerebellar lesion degree.The ICARS reflects the severity of clinical manifestations.1H-MRS is useful in the diagnosis of SCA3/MJD.
8.VocaI PoIyps and Precancerous Lesions Treated with CO2 Laser and ConventionaI LaryngeaI Microsurgery

Objective To compare the efficacy of CO2 laser and conventional laryngeal microsurgery in the treatment of benign lesions of vocal cord (polyp of vocal cord) and precancerous lesions (leukoplakia of vocal cord) of patients. Methods A total of 60 patients with vocal cord polyps were selected, and randomly divided into two groups, each with 30 patients. Thirty patients with vocal cord leukoplakia were selected and randomly divided into two groups, each with 15 patients. One group was performed by cold instruments for lesion resection (conventional group), the other by CO2 laser for removal of diseased tissue or mucosal ablative surgery (laser group). Two groups of patients were examined by laryngostroboscope, electronic laryngoscopy, GRABS, VHI subjective ratings and objective voice analysis before operation and one week, one month and three months after operation. ResuIts The outcomes of those with vocal cord polyps in early recovery (1 week) laser group were slightly worse than the conventional group. In later recovery (1~3 months), with electronic laryngoscopy inspection, laryngostroboscope mucosal wave observation, the analysis of subjective and objective data from the two groups showed no significantly differences. For patients with vocal cord leukoplakia in early recovery (1 week), laser group slightly worse than the conventional group, in later recovery (1~3 months), there were no obvious difference between the two groups in electronic laryngoscopy inspection, laryngostroboscope mucosal wave observation, the analysis of subjective and ob_jective data. Follow -up operations, the recurrence rates in laser group was significantly lower than conventional group. ConcIusion The treatment by CO2 laser can significantly improve their pronunciation quality for vocal cord polyps and vocal cord leukoplakia patients, it has a good therapeutic effect, especially the long-term effective of vo_cal cord leukoplakia is better than conventional operation.
9.Design and practice of medical service for the main venue of the 19th Hangzhou Asian Games

Objective:To clarify the medical security plan and its practical effects at the main venue of the 19th Hangzhou Asian Games.Methods:The plan described the medical security implementation plan of the main venue of the 19th Asian Games in Hangzhou (organizational management and operation mechanism, medical security operation system, standardized training and support processes, application of Asian Games intelligent first aid security system, and so on). And through high-facticity simulation verify the operation effect of the main venue's medical support (team mode and response time). Finally, the feasibility and effectiveness of the medical security program verified through the effect of medical services (quantity, disease types, transfers, etc.) during the Asian Games and Asian Paralympic Games.Results:The medical security team of the main venue of the 19th Hangzhou Asian Games included the management team and the operation team, of which consisted of 44 medical staffs (22 doctors and 22 nurses); and were deployed to 16 different security positions. It was also accompanied with 16 volunteers to assist medical staff to transport patients. In addition, a total of 300 medical observers were enrolled in audience area to assist rapid identification, intervention, and collaborative diagnosis and treatment with the medical team. Medical security teams were arranged to cover all the audience areas who can arrive at the patient's area within 2 minutes after receiving orders, transfer the patient to the ambulance’s site in 8 minutes, and transport the patient to a designated hospital within 10-20 minutes. During the Asian Games and Asian Paralympic Games, the medical-care team treated a total of 3 742 patients, including 9 patients transported from medical service sites of the audience area to the stadium infirmary, as well as 83 patients referred to the designated hospital.Conclusions:The medical security implementation plan for the main venues of the Asian Games provides good medical security for the Asian Games and Asian Paralympic Games. It also provides theoretical and practical experience for the development of the medical security system for international events in future.
10.Studies on the growth of rabbit skin fibroblasts on the surfaces of acellular dermal matrix.

The growth of fibroblasts on the acellular dermal matrix (ADM) was studied. The fibroblasts isolated from the skin of an adult New Zealand Rabbit were cultured in vitro and identified subsequently. After the cells were inoculated on the ADM as seeds, the adhesion rate and the growth ability were examined, and cellular morphology was assayed with DAPI fluorescent staining and Scanning electron microscope (SEM). The possibilities of applying ADM as cells carrier or deliverer in the field of transplantation were evaluated. The result revealed that pure fibroblasts were isolated through the specific method. Skin fibroblasts could adhere to ADM easily, and the adhesion rate was 96.78%, displaying no significant difference (P > 0.05) when compared with that rate of the control holes. The cells on the scaffolds and those on the control holes showed similar growth tendencies, but the activity of the former was lower (P < 0.01). The integral nucleus with blue fluorescence could be observed on the ADM under fluorescence microscope. The number of fibroblasts scaled up with the cultured time, The results of SEM showed that the state of cell was good and the fibroblasts were fused into a layer after being cultured for 5-10d. So rabbit fibroblasts can attach, survive, grow and proliferate on the ADM in a healthy way. It is entirely possible to use ADM as an appropriate scaffold material for the culture of fibroblasts and as a material for transplantations.
