Objecltive To intensify the coordination and nursing in the operation room in order to prevent surgical site infection after gastrointestinal operation. Methods 337 medical history of patients received gastrointestinal operation and third rate healing from 1999 to 2006 were collected. A series of intensified measures were applied to surgical site infection from 2003 gradually, including invocation of new surgical handwashing method, modified skin disinfection manner, adoption of degreasing with ethanol first before disinfection with iodophor, placement of incision protector and clean bag for incision protection after entering abdomen, changing to use new gastrointestinal anastomofic thimerosal, standardization of operation order and clean manage-ment in operation room. The incidence rate of surgical site infection after gastrointestinal operation of patients from 1999 to 2002 and from 2003 to 2006 underwent χ2 test. Results The incidence rate of surgical site in-fection after gastrointestinal operation greatly decreased after adoption of intensified nursing intervention, Signifi-cant difference existed in rate of patients with third rate healing between the year 1999 to 2002 and 2003 to 2006. Conclusions Modified nursing intervention for surgical incision after gastrointestinal operation can de-crease incision infection rate evidently.

Objective Minimally invasive technique was used in the treatment of pectus carinatum. Indication, techni-cal details and perioperative complication are discussed in this paper. Methods From March 2008 to January 2009, 6 patients with pectus carinatum underwent minimal invasive operation by using the principle of Nuss procedure in which pressure applied through a curved steel bar that was placed subcutaneously anterior to the sternum, via lateral thoracic incisions. Preoperative CT scan were performed and Hailer index was used as operative indication. Results The average age of patients was (13.6 ± 1.94) years (range, 10 - 16 years). They were all males. The mean operation time was (104.17 ± 15.63) minutes. The av-erage blood loss was (5.17±2.56) ml. The mean hospital stay was (5.67±0.82) days. Postoperative follow-up was 1 -11 months. Displacement of the stablizer was the only complication that was corrected by further fixation procedure. COnClusion The minimally invasive technique for the treatment of peetus earinatum is safe and effective with minimal complications. It can be selected as an alternative techniques and the appropriate age for this procedure is 10-16 years.

Objective Base on the local pathology, the early diagnosis and a reasonable treatment for congenital esophageal stenosis (CES) were discussed. Methods Forty two cases of congenital esophageal stenosis during 1980~2000 were reviewed retrospectively, including age, vomitus and frequency, nutrition and development, diagnostic method used and operative pathological findings, time of normal feeding and time of nutritional restoration. Results According to local pathology, there were 26 cases of tracheobronchial remnant, 11 fibromuscular stenosis, 3 membranous diaphragm, and 2 ectopic gastric mucosa. Membranous diaphragm excision gave the best result without postoperative dilatation. The patients were followed up for 4~10 years, Normal feeding was restored in 1 month in 78 6%, 9 5% in 6 months, and 11 9% in 1 year. Conclusion Persistent vomiting of undigested food and failure in gaining weight are the chief complaints of congenital esophageal stenosis. The confirmatory diagnosis depends on barium swallowing and esophagoscopy. A good result can be achieved by follows local resection and necessary postoperative dilatation.

Objective To analyze the effect of Botulinum toxin-A (BTX-A) block on the upper extremity spasticity in children with cerebral palsy. Methods From January 2004~December 2011, 47 children with spastic hemiplegic cerebral palsy were divided into control group (n=25) and experimental group (n=22). The control group was administered by occupational therapy. The experimental group was treated by BTX-A block in addition. The dose of BTX-A block was identified with the body weight of the child and the Modified Ashworth Scale (MAS). The dose of injection ranged from 30 IU to 110 IU, average (50.7±12.7) IU. Results There was no significant difference between 2 groups in scores of MAS and Simple Test for Evaluating Hand Function (STEF) before treatment (P>0.05). The MAS score reduced in the experimental group 1 month after treatment (P<0.01), and was lower in the experimental group than in the control group (P<0.05). The score of STEF increased in both groups (P<0.05), and it was higher in the experimental group than in the control group (P<0.01). Conclusion The BTX-A block can decline the upper extremity muscle tone in children with cerebral palsy efficiently, and help to improve the upper extremity function.
