1.Influence of Special Rectification on Antibiotics Application in Patients during Perioperative Period with Gynecologic Type ⅡIncision in Our Hospital

Objective:To explore the impact of special rectification on antibiotics application in the patients during perioperative period with gynecologic typeⅡincision in our hospital. Methods:The antibiotics type, use frequency, expense, administration time, dosage, treatment course and combined medication were retrospectively compared between April to September in 2011(before the recti-fication) and April to September in 2013 (after the rectification). Results: After the special rectification, the total DDDs was de-creased from 1 564. 5 to 419. 3, the expense for antibiotics and all drugs was respectively decreased from (297. 5 ± 47. 1) yuan to (108. 3 ± 29. 6) yuan and (806. 4 ± 63. 1)yuan to (498. 1 ± 70. 2) yuan, the proportion of antibiotics was decreased from (36. 9 ± 9.1) % to (21.7 ±6.8) %, and the treatment course was reduced from (3.7 ±1.4)d to (1.5 ±0.6)d(P<0.05). The rational rate of administration time, dosage and combined medication was respectively increased from 67. 7% to 98. 6%, 81. 5% to 100. 0%and 41. 1% to 80. 7%(P<0. 05). Conclusion:Special rectification can effectively improve the rational antibiotics use in gynecology department of our hospital during perioperative period of type Ⅱincision.
2.Treatment for ischemic necrosis of the femoral bead with autogenous spongy bone morselized and BMP implantation after arthroscopic core decompression

Objective To observe the clinical results of treatment for ischemic necrosis of the femoral head with autogenous spongy bone morselized and BMP implantation after arthroscopic core decompression.Methods Retrospectively analyzing the clinical data of 23 cases(30 hips),who were treated with autogenous cancerous bone morselized and BMP implantation after arthroscopic core decompression.Results 23 patients were followed up after discharge,follow-up time was 12 to 36 months,with an average of20.4 months.X-ray demonstrate good bone restoration in femoral head.In the observation period,femoral fractured or collapsed in 1 case of stageⅡ C、1 case of stage ⅢB,the original pain symptom aggravating,so artificial joint replacement were implemented.The other cases can maintain the results obtained in postoperative 6 months.According to the comprehensive evaluation.all the cases were 40~92scores after treatment.The rate of excellent and good results Was 76.7%.The results were better in stage Ⅰ(5/5),Stage Ⅱ(12/16),and Stage Ⅲ(6/9).Hip pain disappeared or lightening obviously,function improved obviously,Xray demonstrate femoral head contour smoothly and joint clearance normal.Conclusion Autogenous spongy bone morselized and BMP implantation after arthroscopic core decompression had good clinic curative effects in treatment of inchoate ischemic necrosis of the femoral head.
3.Self-supporting current situation and its influencing factors for the elderly in care institutions

Objective:To investigate the current situation and influencing factors of the cognition of self-supporting for the elderly in nursing institutions, in order to provide a reference basis for constructing an intervention system of self-supporting in nursing institutions and implementing self-supporting more effectively.Methods:This study was a cross-sectional study. From April to August 2023, 320 elderly people from 7 elderly care institutions in Changsha City were selected by convenience sampling method, and the general information questionnaire, the Barthel Index Scale, the Comprehension Social Support Scale, and the self-developed questionnaire on knowledge belief, and practice of self-supporting for the elderly in nursing institutions were used to investigate. Multiple linear regression was used to analyze the factors influencing self-supporting for older people in nursing facilities.Results:Totally 315 valid questionnaires were recovered including 95 males and 220 females, with the majority aged 80-89 years, being 159. The surveyed elderly people in nursing institutions scored (91.24 ± 11.92) points on the self-supporting knowledge questionnaire, and the mean score of the entries was (3.51 ± 0.46) points. The results of multiple linear regression showed that the total scores of activity frequency, the Barthel index of activities daily living and perceived social support were the main factors affecting self-supporting ( t = 5.02, - 6.67, 9.48, all P<0.05), which could explain 51.8% of the total variation. Conclusions:The self-supporting of the elderly in nursing institutions is at a high level, and relevant personnel can better exert the subjective initiative of the elderly in nursing institutions by publicizing and educating the elderly on self-supporting, organizing regular activities and strengthening social support construction.
4.Transcriptomic analysis of human papillomavirus type 16 immortalized cervical epithelial cell line H8 after malignant transformation

Objective:To investigate the related genes, signaling pathways and possible mechanisms of malignant transformation of human papillomavirus type 16 (HPV-16) immortalized cervical epithelial cell line H8.Methods:The malignant transformed H8 cell model was constructed, and the changes of cell invasion ability and cell migration ability of H8 cells after malignant transformation were detected by Transwell assay, and the changes of clone formation ability of H8 cells after malignant transformation were detected by plate clone formation assay. Total RNA was extracted from malignant transformed H8 cells and H8 cells, and the two groups of cells were sequenced by transcriptome using Illumina novaseq 6000 sequencing platform, differentially expressed genes (DEGs) were identified and analyzed, and Gene Ontology (GO) function enrichment analysis, Kyoto Encyclopedia of genes and genomes (KEGG) pathway enrichment analysis and protein-protein interaction were performed.Results:The invasion ability, migration ability and clone formation ability of malignant transformed H8 cells significantly increased as compared to H8 cells. A total of 203 differentially expressed genes were identified in H8 cells before and after malignant transformation, of which 98 were up-regulated and 105 down-regulated. GO enrichment analysis showed that DEGs were mainly involved in biological processes such as cellular processes, biological regulation, and metabolic processes. KEGG pathway enrichment analysis showed that DEGs were mainly enriched in alanine, aspartate and glutamate metabolic pathway, glycine, serine and threonine metabolism pathway, p53 signaling pathway and TGF-β signaling pathway, PI3K-Akt signaling pathway. PPI analysis screened 10 hub genes including DDIT3, TRIB3 and ASNS.Conclusions:Compared with H8 cells, malignant transformed H8 cells have a large number of differentially expressed genes and pathways at the transcriptional level, which could further provide new ideas for the mechanism of malignant transformation and carcinogenesis as well as finding new targets for the prevention of malignant transformation.
