1.Analysis on Rationality of Vancomysin Use in Our Hospital

OBJECTIVE:To evaluate the rationality of vancomysin use in the hospital where the authors work.METH-ODS:50patients taking vancomysin between May.2005and Nov.2005were studied.The pathogenic examination,monitoring of drug concentration in blood,indices of renal function,combined use of drugs as well as adverse effects in these patients were reviewed and analyzed statistically.RESULTS:On the whole vancomysin use in our hospital was safe.CONCLUSION:Vancomysin use will be made safer by monitoring drug concentration in blood,individualizing drug administration,and timely correcting the aggravating factors of kidney injury.
2.Osteotomy of mandible ascending ramus by “a cover of box” manner provides a surgical approach to large tumors in the parapharyngeal space

Objective:To explore an improved osteotomy of mandible ascending ramus for the surgical approach to large tumors in parapharyngeal space.Methods:According to the operation of parotid gland, masseter muscle was cut near the border of mandibular angle till subperiosteum dissection to mandibular notch, vertical osteotomy outside mandibular foramen to 1.0 cm under mandibular notch, vertical to posterior border of ascending ramus, tumor was exposed and removed, bone plate was repositioned and fixed with titanium. Results:Tumors were completely removed in this way in 3 patients without complications. Conclusion:This surgical approach is suitable for the surgical removal of parapharyngeal interstitial giantism tumor.
3.Medical students’demand for knowledge about healthcare-associated infection

Objective To investigate medical students’demand for knowledge about healthcare-associated infec-tion(HAI),explore the opportunity and method of setting up curriculum about nosocomiology.Methods 447 medi-cal students who were about to start clinical practice were investigated through questionnaires.Results 434 (97.09%) questionnaires were available.152 (35.02%)medical students had access to knowledge about HAI,148 (34.10%) knew or learnt six-step hand-washing method,80 students understood six-step hand-washing method through media and society (n=65,81 .25%)or school curriculum (n=15,18.75%).95.62% of students hoped to set up curric-ulum about nosocomiology,354 (81 .57%)students preferred this discipline to be set up before clinical probation;55.07% of students hoped this course should be set up as a practice course,44.93% thought that theory and prac-tice should be combined.Occupational precaution measures as well as methods of disinfection and isolation were the most interesting knowledge among them.Conclusion Nosocomiology should be an obligatory course in medical col-leges,the semester before clinical practice is the best time for this course,the curriculum should combine theory with practice.
4.Recent progress on diagnosis and treatment of benign symmetric lipomatosis

Objective:To introduce recent progress on diagnosis and treatment of benign symmetric lipomatosis(BSL).Method:Detailed clinical data of 6 patients with BSL were reviewed and analyzed.We present a summary of the clinical symptoms,physical sign,diagnosis and therapeutic methods of BSL. And related literatures were discussed together.Result:All of 6 patients have excessive subcutaneous fat deposit predominantly around neck.One patients had upper extremity localizations. Six patients had the complication of left ventricular diastolic function changes,glucose intolerance or diabetes mellitus, chronic hepatopathy, hyperuricemia and sleep apnea syndrome in one or more. One patients with several symptoms occur simultaneously, another one female patient was accompanied by all symptoms but chronic hepatopathy. Five male patient were alcohol abusers. Tatal neck lipentomy and abstinence from alcohol were performed on 5 patients. One patient refused treatment.During a follow-up of 3 months to 4 years, one patients was relapsed again,and no recurrence was seen in another 4 patients. All pathological results were nonencapsulated fat. Conclusion:BSL is a lipodystrophy caused by diffuse fatty tissue, symmetry deposition in the neck and shoulder subcutaneous fascia space or deep fascial space .The highest incidence favors to middle-aged man who is alcoholist. Chronic alcohol addiction and typical clinical symptoms help to diagnosis BSL. Lipectomy represents a successful procedure in treating BSL.
5.The feasibility investigation of experimental animal model of cerebral alveolar echinococcosis disease of sheep

Objective To build experimental animal model of cerebral alveolar echinococcosis disease of sheep,in order to study of human alveolar hydatid disease of the brain.Methods Experiment animal models of ten Xinjiang big-tail sheep were performed by, direct skull puncture,intracerebral inoculation of echinococcus multilocularis.MRI was used to observe the growth status of cerebral alveolar echinococcosis disease of sheep after 8 months,and morphological and pathological characteristics after autopsy were ana-lysed.Results 4 sheep models (40%)were successful built which were confirmed by pathology and MRI.On MRI,4 cases all were single lesion,on T2 WI there was multiple follicles bubbles under the background of low signal in one case,and low signal in other three cases.Under the microscope,a large number of lymphocytes,eosinophils and plasma cells infiltrated the lesion area,around which small blood vessels were blocked and had inflammatory reaction were showed.Conclusion The method using artificial inocula-tion rat alveolar echinococcosis to establish experimental model of cerebral alveolar echinococcosis disease of sheep has the character-istic of feasibility,simplicity and repeatability.
6.The alkaline phosphatase analysis of 40 children with cerebral palsy

ObjectiveTo explore the relations between the alkaline phosphatase(ALP) and the type of cerebral palsy. MethodsThe ALP level of 40 children with different types of cerebral palsy were examined with the Olympus AU-600 apparatus. ResultsThe ALP value of the children with spastic cerebral palsy is normal. The ALP value of the children with athetoid cerebral palsy is higher than spastic cerebral palsy(P<0.001).Conclusions The ALP value can help us to identify the type of cerebral palsy.
7.Correlation between Current and Period in Effect of Block Myoelectricity Fixed Botulinum Toxin A Injection in Cerebral Palsy

ObjectiveTo investigate the correlation between current and therapeutic effect of block myoelectricity fixed Botulinum Toxin A(BTX-A) injection in cerebral palsy.Methods15 children with spastic cerebral palsy were treated with BTX-A block injection under block myoelectricity fixing.The dose of BTX-A was identified with the weight of the child and the Modified Ashworth Scale.The correlation between current and period in effect was analyzed.ResultsThere was negative correlation between current and period in effect(r=-0.769,P=0.01).ConclusionThe BTX-A should be injected where the fixed current is less.
8.Optimal Dose of Botulinum Toxin A to Release Spasticity of Triceps Surae in Children with Cerebral Palsy

Objective To determine the coefficient in the formula of the optimal dose of botulinum toxin A(BTX-A) to treat spastic cerebral palsy(CP).Methods58 CP children(male 38,female 20,mean age 59.3±24.6 months,range 27 to 144 months) were divided into A,B,C,D and E five groups according to used coefficient and treated by BTX-A with different doses to release the spasticity.ResultsAccording to the improvement of motor function,the BTX-A dose of the group D was optimal.According to the effective endurance,the BTX-A dose of the group C was optimal.The optimal coefficient was 1.8.ConclusionThe formula of the optimal BTX-A dose injected in triceps surae is dose =(1+1.8×MAS score)×body weight.
9.Optimal Dose of Botulinum Toxic A in Treatment of Spastic Cerebral Palsy

ObjectiveTo investigate the optimal dose of botulinum toxic A (BTX-A) block to relax spasticity in cerebral palsy.Methods61 children (27~144 months old) with spastic cerebral palsy were treated by BTX-A block and divided into A, B, C, D and E groups according to the dose coefficient. The dose of BTX-A block was 30~160 IU (average 69.0±32.4 IU) according to the weight and the score of Modified Ashworth Scale.Results55 BTX-A blocks were successful. The muscles tone turned below and the function were improved. The effective rate was 90.16%. The effective time ranged from 8 weeks to 32 weeks, average 17.56±8.30 weeks. The effective time of the group C (2≤dose-coefficient<3) was the longest in the five groups.ConclusionThe optimal dose of BTX-A block in triceps surae should be counted as follow: dose(IU)=(score of Modified Ashworth Scale+2.5)×weight(kg).
