1.Clinical Observation of Ebastine Combined with Chushi Zhiyang Ointment in the Treatment of Hand Keratin-izing Chapped Eczema

OBJECTIVE:To observe clinical efficacy and safety of Ebastine tablet combined with Chushi zhiyang ointment in the treatment of hand keratinizing chapped eczema. METHODS:135 cases of hand keratinizing chapped eczema were divided into control group A(45 cases),control group B(43 cases)and treatment group(47 cases)according to treatment regimen. Control group A was orally given Ebastine tablet,10 mg each time,qd;control group B was given Chushi zhiyang ointment alone,twice a day,morning and evening,applying thin layer of ointment on the affected area;treatment group was given same dose of Ebastine tablet orally and applied Chushi zhiyang ointment on the affected area. 3 groups received treatment for consecutive 4 weeks. Clinical efficacies of 3 groups were observed as well as the scores of pruritus,skin lesion area,keratinization,rhagades and VAS before and after treatment. The occurrence of ADR was compared among 3 groups. RESULTS:The total effective rate of treatment group was 68.09%,which was significantly higher than that of group A(42.22%)and control group B(16.28%),with statistical significance(P<0.05). There was no statistical significance in the scores of pruritus,skin lesion area,keratinization,rhagades and VAS among 3 groups before treatment(P>0.05). After treatment,above scores of 3 groups decreased significantly,and those of treatment group were significant-ly lower than those of control group A and B,with statistical significance(P<0.05). There was no statistical significance in the inci-dence of ADR among 3 groups(P>0.05). CONCLUSIONS:Ebastine tablet combined with Chushi zhiyang ointment is effective for hand keratinizing chapped eczema,and can significantly improve the skin of patients with good safety.
2.Drainage by puncture with 25G needles for primary retinal detachment

Objective To summarize the clinical outcome of direct trans-scleral drainage by puncture with 25G needles in the external-route surgeries for primary retinal detachment. Methods Fifty-four consecutive cases of primary retinal detachment were treated by external-route surgeries. The subretinal fluid of 23 cases was drained by direct scleral puncture with 25G needles (25GND group), the other 31 cases underwent the conventional scleral incision and drainage(CD group). Postoperative complications, the single-operation retinal reattachment rate and functional outcome were recorded.Results Subretinal fluid was adequately drained in all cases. there was no significant difference in the single-operation retinal reattachment rate between 25GND and CD groups. Objective To observe the effect of electrical stimulation of the pelvic floor muscles (PES) combined with acupuncture on urinary dysfunction in patients with spinal cord injury (SCI). Methods Sixty-two patients with SCI were randomly divided into a treatment group ( n =31 ) and a control group ( n =31 ).All subjects received routine bladder function training and a drinking water schedule for 8 weeks.In addition,the treatment group was given PES 30-40 min/d 6 times/week and acupuncture for 30 min/d,6 times/week for the 8 weeks.Residual volume of urine was evaluated using ultrasound and a daily voiding diary.Uurodynamic examinations and catheter extraction were used to assess the urinary function pre- and post-treatment. Results No significant difference was found between the two groups before treatment,but after 8 weeks of treatment the residual volume of urine detected by ultrasound and voiding frequency were significantly lower in the treatment group than in the control group.First desire to urinate and maximal urinary bladder capacities in the treatment group were also significant higher than in the control group.The efficacy of treatment was also sustained significantly longer in the treatment group. Conclusion PES combined with acupuncture therapy is more effective than routine bladder function training for SCI patients with urinary dysfunction.
4.Analysis on the characteristics and distress of symptoms in patients with bladder perfusion chemotherapy after resection of bladder cancer

Objective To discuss the extent and characteristics of symptoms and symptom distress, and find the individualized management for patients undergoing bladder perfusion chemotherapy after transurethral bladder resection of bladder tumor. Methods The general information questionnaire, M. D. Anderson Symptom Inventory ( MDASI) and additional entries were used to investigate 84 patients undergoing bladder perfusion chemotherapy after transurethral bladder resection of bladder tumor. After individualized treatment, scores of symptoms were observed. Results Symptom score of patients in this group was (3. 12 ± 1. 02), which was at the mild level. Symptoms subscale score entries sorted successively as poor appetite, fatigue, disturbed sleep, nausea, and vomiting. Symptom distress score was (2. 85 ± 2. 02), which was at the mild level. Distress subscale score in the order of entries were work, mood and general activity, enjoyment of life, walking and relationship. Added entries score in the order were burning sensation in the bladder, bladder irritation and gastrointestinal reactions. Scores of some symptoms were significantly lower than before ( P < 0. 05 ). Conclusions Symptom distress is prevalent in patients undergoing bladder perfusion chemotherapy after transurethral bladder resection of bladder tumor, prompting that the intervention should be individualized according to the patient′s symptom distress features to alleviate their symptoms distress.
5.CT characteristics of thoracoabdominal localized Castleman disease

Objective To assess the CT characteristics of thoracoabdominal localized Castleman disease (LCD) in 55 cases and correlate with histopathologic features. Methods Fifty-five patients with LCD proved histopathologically in thorax ( n = 25 ) and abdomen ( n = 30 ) were collected during past 20 years. The unenhanced and contrast-enhanced CT were performed in all patients. Two radiologists reviewed CT images and the CT findings were analyzed simultaneously. Results In 54 patients with hyaline-vascular type ( n = 50 ) and mixed type ( n = 4 ) localized CD, the lesion typically presented as solitary mass (90. 7% , 49 cases), with irregular or lobular or infiltrative margin ( 83.3% , 45 cases),central calcification (38. 9%, 21 cases), marked enhancement (100% , 54 cases), focal non-necrosis low attenuation areas (72. 2%, 39 cases), lymphadenopathy (70.4%, 38 cases) and dilated feeding vessels adjacent to the mass (96.3%, 52 cases). One lesion with plasma cell type localized CD presented as a mass with irregular margin, mild enhancement and central necrosis. Four morphologic patterns wereclassified on CT, including solitary mass with well-circumscribed margin (n =4), irregular or lobular margin ( n = 30), infiltrative or halo-like margin ( n = 16 ), and multiple coalescent maasses ( n = 5 ). Conclusion CT features of thoracoabdominal localized CD are closely related to the location and pathological type. LCD with hyaline-vascular and mixed type has typical CT characteristics, while LCD with plasma cell type has no typical CT findings.

Objective To investigate the value of peripheral blood soluble interleukin-2 receptor(sIL-2R),CD4+lymphocyte percentage/CD8+lymphocyte percentage ratio(hereinafter referred to as CD4+/CD8+)and tumor necrosis factor-α(TNF-α)in evaluating the efficacy of chemotherapy in elderly patients with newly treated active pulmonary tuberculosis.Methods A total of 102 elderly patients with newly treated active tu-berculosis admitted to the hospital from December 2019 to December 2022 were enrolled in the study as the observation group,and 102 healthy people aged 60 and older who underwent physical examination in the hos-pital during the same period were enrolled as the control group.The levels of sIL-2R,TNF-α and CD4+/CD8+in peripheral blood were compared between the two groups,and the correlations between sIL-2R,TNF-α and CD4+/CD8+were analyzed.The observation group was treated with 2HRZE/4HR anti-tuberculosis treatment regimen.The levels of sIL-2R,TNF-α and CD4+/CD8+in peripheral blood of patients with different efficacy before treatment,1 month and 6 months after treatment in the observation group were compared.The correla-tion between sIL-2R,CD4+/CD8+,TNF-α levels and therapeutic effect was analyzed.The receiver operating characteristic(ROC)curve was used to analyze the efficacy of indicators in evaluating the efficacy of chemo-therapy in elderly patients.Results The levels of sIL-2R and TNF-α in the observation group were higher than those in the control group,while CD4+/CD8+was lower than that in the control group,and the differ-ences were statistically significant(P<0.05).In the observation group,sIL-2R and TNF-α were negatively correlated with CD4+/CD8+(P<0.05),sIL-2R was positively correlated with TNF-α(P<0.05).After 1 month and 6 months of treatment,the levels of sIL-2R and TNF-α in patients with apparent efficacy were low-er than those in patients with efficacy,and the latter were lower than those in patients with no effect,while the CD4+/CD8+in patients with apparent efficacy was higher than that in patients with efficacy,and the latter was higher than that in patients with no efficacy,and the differences were statistically significant(P<0.05).The levels of sIL-2R and TNF-α were negatively correlated with the efficacy(P<0.05),and CD4+/CD8+was positively correlated with the efficacy(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of sIL-2R,CD4+/CD8+,and TNF-α used in combination to assess efficacy was significantly greater than the AUCs of the single indicators used in the assessment at each time point of treatment(P<0.05),and the AUC of the combination of the indicators was greater after 6 months of treatment than after 1 month of treatment(P<0.05).Conclusion The levels of sIL-2R,CD4+/CD8+and TNF-α are closely related to the ef-ficacy of chemotherapy in elderly patients with newly treated active pulmonary tuberculosis,and the combina-tion of the above indicators has certain reference value in evaluating the efficacy of chemotherapy in patients.
