1.Clinical efficacy of preoperative application of hydrochloride tirofiban in patients with acute coronary syndrome by percutaneous coronary intervention

ObjectiveTo observe the clinical efficacy and application value of preoperative application of hydrochloride tirofiban in patients with acute coronary syndrome(ACS) by percutaneous coronary intervention (PCI).Methods60 ACS patients underwent PIC surgery were randomly divided into observation group and control group.All patients underwent preoperative and postoperative regular routine anti-angina and acute myocardial infarction treatment,the observation group was given the hydrochloride tirofiban in 1h before surgery of emergency PCI,the control group received conventional therapy.The bleeding and thrombocytopenia occurrence during hospitalization of the two groups were observed,before surgery and 1 month after surgery the cardiac function of the two groups were compared,1 month after surgery the occurrence of adverse cardiovascular events were recorded.ResultsThe bleeding incidence of observation group was 6.7%,the control group was 23.3%,the difference between two groups was significant ( x2 =15.594,P < 0.01 ) ; The thrombocytopenia occurrence of observation group was 3.3%,the control group was 10.0%,the difference between two groups was significant( x2 =13.366,P < 0.01 ).1 month after surgery the LVEF,CO and E/A of the two groups increased significantly,compared with the prior-treatment,the difference was significant (P < 0.05 ),and the difference was significant between the two groups( P < 0.05 ) ;The occurrence of adverse cardiovascular events of observation group was 33.3%,the control group was 66.7%,the difference between two groups was significant(x2 =8.924,P < 0.01 ).Conclusion The hydrochloride tirofiban is a safe and effective anti-platelet drugs.Preoperative application of hydrochloride tirofiban in patients with ACS by PCI can reduce the occurrence of cardiovascular events.
2.Clinical observation of insulin pump therapy in type 2 diabetes

Objective To explore the clinical effect of the two different administration style of insulin pump to treat type 2 diabetes.Methods 118 patients with type 2 diabetes were selected and randomly divided into the experimental group,which 59 cases of the patients were treated with insulin pump (CSII group) and the control group,which 59 cases of the patients with subcutaneous injecting of insulin.The treat ment effects were observed.Results The blood glucose level of the two groups of patients was significantly decreased after treatment,the CSII group spend shorter time [(4.5 ± 1.3 ) d vs (7.2 ± 2.3 ) d],used a significantly less dose of insulin and has a lower incidence of hypoglycemia than the MDII group in the course of decreasing the blood glucose to aimed level(all P <0.05 ).Conclusion Insulin pump is safe and more effective than subcutaneous injecting insulin in treating type 2 diabetes,and has no significant adverse reactions,worthy of clinical application.
3.Plasma markers of endothelial function in cardio-cerebrovascular diseases

Endothelial cells can be activated, and develop dysfunction and damage under the pathological state. Endothelial dysfunction plays a key role in the processes of cardio-cerebrovascular diseases, and can be detected in peripheral blood. The concentration of von Willebrand factor (vWF) predicts the risk of coronary heart disease and stroke; the concentration of soluble thrombomodulin is associated with the prognoses of coronary heart disease and stroke; soluble intercellular ashesion rnolecule-1 (ICAM-1) predicts the risks of carotid athero-sclerosis, coronary heart disease, peripheral arterial disease, and diabetes mellitus; the levels of plasma soluble E-selectin in patients with carotid atherosclerosis, coronary artery disease, and diabetes mellitus are increased; the increased endothelin-1 levels predicts the elevated risk of congestive heart failure. The injection of endothelial progenitor cells is promising to become a novel approach for the treatment of cardio-cerebrovascular diseases.
4.Convergence of Visceral and Somatic Nociceptive Inputs onto Hippocampal Units in Cats

Ten cats lightly anesthetized and D-tubocurarine immobilized were employed.The responses of a same unit to the train stimuli of square wave applied to the saphenous nerve,the greater splanchic nerve,and the two nerves simultaneously were observed.Among the 80 units recorded on the 10 cats,43 went through all the experimental procedures,33 out of the 43 could be activated by splanchic inputs.Convergence of the splanchic and saphenous inputs occurred to 26(79%) out of the 33.These 26 units could respond to the noxious stimuli on the saphenous and greater splanchic nerves in turn or simultaneously.Furthermore,they could be divided into 3 types;15 units showed similar pattern and duration of discharge only when the greater splanchic nerve was stimulated,6 showed similar discharge only when the saphenous nerve was stimulated,and 5 showed smaller responses when either of the two nerves was stimulated.When the interval between stimulations was shortened to 5 seconds,37(86%) out of the 43 units showed a decrease in discharge,which was apparently an after-inhibitory effect.The results indicate that the inputs from visceral and somatic C or A-delta afferent fibers can converge onto the units of the dorsal hippocampal CA1 area,and evoke blockage and inhibition of these units.
5.Gene cloning, expression, and purification of recombinant human heat shock protein 70D

Objective: To study the cloning, recombinant expression and purification of human HSP70D. Methods: The full length code region of HSP70D was amplified by RT PCR from HeLa cells. Then the PCR product was inserted into pET 24a(+), by NdeⅠ/XhoⅠ double digestion. The recombinant expression vector pET24a HSP70D was transformed into E. coli strain BL21. Positive clones were determined by NdeⅠ/XhoⅠ digestion and sequence analysis, which was used in the expression and purification of HSP70D protein. The engineered bacteria were collected after fermentation. The recombinant HSP70D was purified by using ethanol precipitation, anion exchanger, hydrophobicity interaction chromatography, and finally high resolution gel filtration chromatography. Results: The expression level of HSP70D was over 20% of total protein in the engineered bacteria. After purification, electrophoretically pure (more than 95%) rhHSP70D was obtained. Conclusion: This study setup a satisfied down stream process which can be used for production of large quantity of recombinant HSP70D.
6.Bacteria colonized and antibiotic resistance analysis on nongonococcal urethritis in men

Objective To investigate the composition and drug resistance of pathogenic bacteria of nongonococcal urethritis in men,in order to give some references for clinical treatment.Methods The pathogens were cultured by Urethral secretions of nongonococcal urethritis in men and drug susceptibility test followed.Results Coagulase negative staphylococcus and mycoplasma urealytium were major pathogens of nongonococcal urethritis in men;The constituent ratio of meticillin resistance staphylococcus was above 40%.There was significant difference that the constituent ratio of meticillin resistance coagulase negative staphylococcus compared with specimen in the same period.Meticillin susceptible coagulase negative staphylococcus and meticillin susceptible Staphylococcus aureus were sensitive to cephalosporins and quinolones,excluding penicillins.The antimicrobial susceptibility of meticillin resistance staphylococcus was just opposite completely;Mycoplasma urealytium and Mycoplasma hominis were high sensitivity to josamycin、doxycyline and minocycline.Conclusion The colonization bacterium of nongonococcal urethritis in men are primarily consist of coagulase negative staphylococcus and mycoplasma urealytium.It is worthy of further researching correlation between coagulase negative staphylococcus and nongonococcal urethritis,it also suggested that antibiotics on should be rationally choosed on therapy of nongonococcal urethritis.
7.Serological Studies on Biejia Jian Decoction in Counteracting Hepatic Fibrosis

[Objective] To investigate the effect of Biejia Jian Decoction on serum transforming growth factor ?1 (TGF-pl) , tumor necrosis factor a (TNF-?), lipid peroxidation ( LPO) and superoxide dismutase ( SOD) in patients with hepatic fibrosis (HF) induced by chronic hepatitis B. Its effect was compared with lamivudine. [Methods] Fifty-three cases of HF were randomized to two groups: group A (n = 27) was treated with Biejia Jian Decoction and group B ( n -26) with lamivudine for oral use. Twenty volunteers served as the normal controls (group C) . Serum levels of TGF-?1, TNF-?, LPO and SOD were observed before and after treatment. [Results] After treatment, serum levels of TGF-?1 and TNF-a were decreased markedly in group A (P 0.05) . Serum LPO level was lower and SOD level higher in groups A and B than those in group C before treatment ( P 0.05) but the level in group B differed from that of groups A and C (P 0.05) but the level in group B did not decrease and differed from that of groups A and C ( P

9.Immune adjuvant-like effect of recombinant human heat shock protein 70D

Objective: To determine the adjuvant activity of recombinant human HSP70D protein. Methods: Dendritic cells were cultured in vitro , rhHSP70D was added into the culture medium, the cytokine secretion and maturation of dendritic cells(DC) stimulated by rhHSP70D were measured; C57BL/6 mice were immunized with rhHSP70D OVA 257 264 complexs reconstituted in vitro , and the protective antigen specific immune response against tumor challenge were evaluated in the immunized mice. Results: rhHSP70D stimulated human DC to secrete IL 1?, IL 12 p70 and TNF ?, up regulated the HLA DR,CD86 and CD40 expression on DC and improved the antigen presentation function of DC; After immunization with the hybrid of rhHSP70D and OVA 257 264 , the mice were able to resist the challenge of E. G7 OVA, which was a tumor cell line expressing model antigen OVA. Conclusion: rhHSP70D can induce maturation of DC, immunization of rhHSP70D peptide complexs can induce antigen specific immune response, suggesting that rhHSP70D may be a new adjuvant in immunotherapy of the cancer and infection diseases.
10.Correlation between Carroll Upper Extremities Functional Test and Fine Motor Domain of Gesell Developmental Scale in Children with Cerebral Palsy

Objective To investigate if Carroll Upper Extremities Functional Test (UEFT) is a better rating scale for evaluating fine mo-tor function in children with cerebral palsy. Methods From January, 2011 to January, 2014, 104 children with cerebral palsy were divided in-to non-hemiplegic palsy group (n=76) and hemiplegic palsy group (n=28), they were assessed by two certified valuers in one week. Their fine motor function was evaluated with UEFT and the Gesell Developmental Scale. The scores of UEFT were compared respectively with the developmental age (DA) and developmental quotient (DQ) in fine motor domain of Gesell Developmental Scale. At the same time, the UEFT score of the affected limb was compared with of the unaffected limb in children with hemiplegic palsy. Results In the non-hemiplegic palsy group, the UEFT score strongly correlated with DA (r=0.752, P<0.001) and moderately correlated with DQ (r=0.456, P<0.001). In the hemiplegic palsy group, the UEFT score of the unaffected limb strongly correlated with DA (r=0.722, P<0.001), and a moderate correlation was found in the affected limb (r=0.591, P<0.001). The UEFT scores of both sides did not correlated with DQ (r<0.186, P>0.05). The UEFT score of unaffected limb was significantly higher than that of the affected limb (Z=-4.708, P<0.001). Conclusion Both of UEFT and the Ge-sell Developmental Scale could be used clinically in assessment of fine motor function in children with cerebral palsy. The UEFT was better in evaluating children with hemiplegic palsy.
