1.Clinical observation of insulin pump therapy in type 2 diabetes

Objective To explore the clinical effect of the two different administration style of insulin pump to treat type 2 diabetes.Methods 118 patients with type 2 diabetes were selected and randomly divided into the experimental group,which 59 cases of the patients were treated with insulin pump (CSII group) and the control group,which 59 cases of the patients with subcutaneous injecting of insulin.The treat ment effects were observed.Results The blood glucose level of the two groups of patients was significantly decreased after treatment,the CSII group spend shorter time [(4.5 ± 1.3 ) d vs (7.2 ± 2.3 ) d],used a significantly less dose of insulin and has a lower incidence of hypoglycemia than the MDII group in the course of decreasing the blood glucose to aimed level(all P <0.05 ).Conclusion Insulin pump is safe and more effective than subcutaneous injecting insulin in treating type 2 diabetes,and has no significant adverse reactions,worthy of clinical application.
2.Serum Th1/Th2 cytokine levels and their clinical significance in patients with alcoholic liver cirrhosis

Objective To investigate the relationship of serum Th1/Th2 cytokine levels with liver function and their clinical significance in patients with alcoholic liver cirrhosis (ALC).Methods Thirty-four ALC patients,13 patients with viral liver cirrhosis (VLC),and 17 healthy controls were included in the study.Serum levels of Th1 cytokines (tumor necrosis factor (TNF)α,interferon (IFN)γ,and inter-leukin (IL)-12)and Th2 cytokines (IL-10,IL-4,IL-6,and IL-13)were measured by cytometric bead array,and their correlations with Child-Pugh classification,model of end-stage liver disease (MELD)score,and biochemical parameters of liver function were ana-lyzed.Comparison between multiple groups was made by Kruskal-Wallis H test,and comparison between two groups was made by Mann-Whitney U test.Correlation analysis was performed by Spearman rank test.Results The Th1 (TNFαand IFNγ)/Th2 (IL-10,IL-4, and IL-13)cytokine levels in ALC group were significantly lower than those in healthy control group (P<0.05),and these indices de-creased nonsignificantly as the Child-Pugh class increased.Compared with VLC patients,ALC patients had significantly increased Th1/Th2 cytokine levels except IL-4 level (P<0.05).In ALC patients,TNFα,IL-10,and IL-13 levels were positively correlated with pro-thrombin time activity (P<0.05)but negatively correlated with total bilirubin and MELD score (P<0.05);especially,the levels of IL-10,IL-13,IL-4,TNFα,and IFNγwere negatively correlated with daily alcohol consumption (P<0.05).Conclusion The expression of Th1/Th2 cytokines is significantly reduced in ALC patients and is correlated with alcohol intake and liver function damage.Monitoring se-rum Th1/Th2 cytokine levels in ALC patients is helpful for clinical treatment and prognostic evaluation.

By taking the evaluations made by scholars such as Wang Yongyan, et al. on status of TCM academic research as the introduction, various influencing factors in acupuncture school development are separately discussed, and the overall trend and existential state are described in modern acupuncture academic area. By displaying the opinions of acupuncture schools in recent years, the relationship among specific therapy, school and schools is analyzed to explain the similarities and differences of school and schools as well as their respective characteristics. It is proposed that the obstacles that lay before acupuncture school researchers are not the rights and wrongs of school and schools, but the evaluation criterion and development patterns that is appropriate for acupuncture research and development.

4.Comparison of clinical effect of three different methods in treatment of premature

Objective To compare the clinical effect of three different methods in treatment of premature.Methods 150 premature patients were randomly divided into three groups,50 cases for each group,received treatments with ritodrine,magnesium sulfate plus salbutamol,or magnesium sulfate alone respectively. The tensity of uterine contraction, the interval between drug administrations and the relief of uterine contraction, the accumulative drug using time, the elongated pregnancy durations were observed for the dertermination of effectiveness. Results The group treated with ritodrine, magnesium sulfate plus salbutamol or magnesium sulfate were effective in 94%, 78%,70% of patients of each group, and it showed that ritodrine was more effective than the combination and magnesium sulfate. The intervals of drug using and contraction relief were statistically different among three groups, (2. 16 ±0. 91)h,(4. 14 ± 1.07)h,(5.12 ± 1.45) h for each(all P <0.05).The cumulative drug using times were(7. 04 ±1. 16) d, (7.46 ± 1.20) d, (7.06 ± 2.78) d for three groups and proved not to be statistically significant ( all P >0. 05 ). The elongated pregnancy durations were (22.08 ± 7.32 ) d, ( 17.42 ± 5.85 ) d and ( 11.82 ± 5.04 ) d, and significantly different in three groups ( all P < 0. 05 ). Conclusion The treatment of premature with ritodrine or the combination of magnesium sulfate and salbutamol was superior to using magnesium sulfate alone.
5.Study on serum cancer antigens 125 expression in different types of ovarian endometriosis and related factors

Objective To investigate serum cancer antigens (CA) 125 expression in different types of ovarian endometriosis and related factors.Methods A total of 182 patients with ovarian endometriosis was collected,then they were divided into two groups:ovarian endometriosis combined with other parts of lesions (A group,88 cases),simple ovarian endometriosis group (B group,94 cases).And selected 100 healthy physical check-up women from the hospital outpatient department were as control group.The serum level of CA125 was detected.The patients' information including body shape,menstrual factors,reproductive activities and disease staging were collected.Chi-square criterion and multivariate binary Logistic regression analysis were completed.Results The serum levels of CA125 in A group,B group and control group were (48.64 ± 33.18),(41.35 ± 23.46),(22.16 ± 13.73) kU/L,respectively,and the positive expression rate of three groups were 64.77% (57/88),52.13% (49/94),2.00% (2/100),respectively,there were significant differences (P < 0.05).The multivariate binary Logistic regression analysis result showed that age of 35-49years,reforming American fertility society of Ⅲ-Ⅳ stage,pelvic adhesion and menorrhalgia were the important factors of positive expression of CA125.Conclusion The serum level of CA125 can be used as the evaluation of the diagnosis and treatment of ovarian endometriosis.
6.The results analysis of the rapid detection of HPV genetype in 282 cervical cancer precursor cases

Objective To evaluate the clinical values of human papilloma virus(HPV)testing in screening for women of early cervical cancer lesion.Methods 282 cases of cervical screening of patients had received cervical exfoliated cells collection,at the same time for a liquid thin-layer cytology test(TCT)and HPV genotyping detection.Abnormal results received histologic examina-tion,the results regarded as the diagnostic gold standard by pathological examination.Results In HPV DNA positive group(126 cases),by liquid based cytology,NILM,ASCUS,LSIL,HSIL and SCC were found in 53,22,32,18,1 cases,respectively.His-topathologic examination positive rate was 42.86%.In HPV DNA negative group(156 cases),there were 133 cases with NILM,16 cases with ASCUS,and 7 cases with LSIL.Histopathologic examination positive rate was 11.54%.Two groups of patients with pathologic positive rate compared,the difference was statistically significant(P <0.01 ).High risk type HPV detection rate in-creased with the increased degree of cervical lesions.Conclusion Women of early cervical cancer and precancerous lesions in the screening line HPV detection has important significance.
7.Content Measurement of Bulb Total Alkaloid and Saponin in Zhejiang Fritillariae and Thuubergii

[Objective] To mensurate and compare with the mensuration of alkaloid and saponin in the flower,pedicel and bulb of fritillariae thuubergii,make full use of all parts of fritillariae thuubergi.i[Method]To mensurate the mensuration of alkaloid and saponin in the three parts of fritillariae with the twophase titration method and gravimetric method.[Result]The mensuration of alkaloid in the flowers of fritillariae thuubergii is 0.182%,while it is 0.047% and 0.341% in the pedicel and bulb separately,and the mensuration of saponin in the flowers of fritillariae thuubergii is 5.112%,while it is 8.840% and 1.724% in the pedicel and bulb separately.[Conclution] The difference of major component in the fritillariae thuubergii offers the foundation to make full use of the flowers,pedicel and bulb.
8.A SAS macro for testing differences among three or more independent groups using Kruskal-Wallis and Nemenyi tests.

As a nonparametric method, the Kruskal-Wallis test is widely used to compare three or more independent groups when an ordinal or interval level of data is available, especially when the assumptions of analysis of variance (ANOVA) are not met. If the Kruskal-Wallis statistic is statistically significant, Nemenyi test is an alternative method for further pairwise multiple comparisons to locate the source of significance. Unfortunately, most popular statistical packages do not integrate the Nemenyi test, which is not easy to be calculated by hand. We described the theory and applications of the Kruskal-Wallis and Nemenyi tests, and presented a flexible SAS macro to implement the two tests. The SAS macro was demonstrated by two examples from our cohort study in occupational epidemiology. It provides a useful tool for SAS users to test the differences among three or more independent groups using a nonparametric method.
9.A exploration of the optimization for dispensing speed of automatic dispensing machine in outpatient pharmacy

Objective:To optimize the storage placement of drug in automatic dispensing machine so as to increase the dispensing speed and work efficiency.Methods: The drugs were dispensed on the up and down side of the center of axle wire from high to low according to the dispensed frequency. The application regularity of various drugs was analyzed by using a number of prescription information and the relevant drugs were stored in nearest tank aroundthe drug outlet as the principle of minimum time algorithm. Through optimizing the storage placement of automatic dispensing machine and applying average dispensing time as evaluation index, the change of average dispensing time between before and after 3 months of the optimization of storage placement was analyzed.Results: After the storage placement was optimized, average dispensing time was shortened 2.0s. And the difference of the time between before and after optimization was statistically significant (t=11.92,P<0.05). Conclusion: Through applying minimum time algorithm, the optimized storage placement of automatic dispensing machine shortens the dispensing time of prescription and increases work efficiency.
10.Efficacy of Mycobacterium Bovis in Combination with Antituberculosis Drugs for Initially Treated Smear Positive Pulmonary Tuberculosis(TB)in Senile Patients

OBJECTIVE:To evaluate the clinical efficacy and adverse effects of mycobacterium bovis in combination with antituberculosis drugs for initially treated smear positive pulmonary tuberculosis(TB)in senile patients.METHODS:A total of 121 initially treated smear positive TB cases were assigned to receive 2HReZE/4HRe alone(control group,n=61)or in combination with Mycobacterium bovis(22.5 ?g,once in two weeks for 6 months started at the end of the second week,treatment group,n=60).RESULTS:At the end of the first and second month since treatment,the sputum negative conversion rates showed significant differences between the treatment group and the control group(63.9% and 86.9% respectively in treatment group vs.30.0% and 58.3%,respectively in control group)(P0.05).CONCLUSION:As compared with antituberculosis drugs alone,the concomitant use of mycobacterium bovis and antituberculosis drugs shortened the time for the sputum negative conversion,focal absorption on X-ray chest film and clinical symptom improvement in initially treated smear positive senile TB patients,and mycobacterium bovis is a satisfactory immunotherapeutic agent for TB and it can be used in combination with antituberculosis drugs for smear positive senile TB patients.
