Objective To evaluate the safety of laparoscopic cholecystectomy (LC) for patients with hepatic cirrhosis. Methods We retrospectively analyzed the clinical data of 72 patients with hepatic cirrhosis (Child-Pugh’s grads A or B). The patients received LC between July 2002 and March 2007. Under general anesthesia, the operation was preformed with four trocars and an intra-abdominal pressure of 8 to 12 mm Hg. Results LC was completed in 71 of the patients (antegrade in 67 and retrograde in 4), 6 of them received subtotal cholecystectomy. One patient was converted to open procedure because of dense adhesion at the Calot triangle. The operation time was 35 to 105 minutes (mean, 51 minutes); blood loss ranged from 5 to 60 ml with a mean of 12 ml. The patients were discharged 3 to 11 days (mean, 5.6 days) after the operation and were followed up for 3 to 18 months (mean, 12 months). After the operation, 7 patients developed ascites, and 1 had hemorrhage at the puncture site. During the follow-up, all patients were free of biliary symptoms, no residual or recurrent liver stone was found. Conclusions LC is safe for patients with liver cirrhosis, and should be the first choice for Child-Pugh’s grads A or B patients. Proper preoperative preparation and intra-and postoperative treatments are critical for the surgical outcomes.
4. Prospective cohort study on the relationship between dietary intake of soy food and total cancer mortality risk in urban Shanghai
Tumor 2017;37(4):350-358
Objective: To investigate the relationship between soy food intake and total cancer mortality risk. Methods: In the Shanghai Men's Health Study (2002-2013) and the Shanghai Women's Health Study (1997-2013), the food frequency questionnaires were used to collect information concerning soy food intake. Hazard ratio (HR) and 95% confidence interval (CI) for the association of soy food intake and total cancer mortality risk were derived using the COX proportional hazards model with adjustment of potential confounders. Results: During average 9.2 years for men and 14.7 years for women of follow-up, 2 122 cancer deaths were identified from 59 857 male participants, while 2 580 cancer deaths were identified from 73 044 female participants. Compared with the lowest quartile of soy food consumption, the HR s of the highest quartile intake group were 0.99 (95% CI: 0.87-1.12) and 0.96 (95% CI: 0.85-1.08) for the risk of total cancer mortality in men and women, respectively. It suggested that the association of soy food intake with total cancer mortality risk was not statistically significant. The further analyses showed the relationship between dietary soy isoflavones intake and total cancer mortality risk was not statistically significant in both men and women, the corresponding HR s were 1.06 (95% CI: 0.93-1.19) and 0.98 (95% CI: 0.88-1.10). Conclusion: Dietary intake of soy food may be not associated with the decreased risk of total cancer mortality.

Objective: To discuss the effect of volatile organic compounds to the potential of embryonic development. From the angle of the laminar flow process, we found ways to control VOCs to ensure that the potential of embryonic development. Methods:To collect the air from the outdoor, laboratory hundred and thousand grade level central corridor three VOCs for testing;we used raw, medium effect filter to replace the original filter and got different results. Results:The indoor VOCs were higher than outdoor;Changes in concentrations of VOCs can affect embryonic development potential. The effect of replaced filter is bigger. After a period of time in a laminar flow system, it gradually getting better and finally change more than before. Conclusion:The embryo laboratory has volatile organic compounds VOCs. VOCs have larger influence on embryonic development potential and can be effectively controlled.

Objective To explore the possible negative interference of circulating cardiac troponin Ⅰ(cTnI) autoantibody on the immunoassay of cTnI in five commonly used cTnI detection systems. Methods Thirteen patients with positive cTnI autoantibodies in their serum samples were firstly screened and selected from 121acute myocardial infarction (AMI) patients using ELISA assay. The serum cTnI values and their recovery rates were then carefully measured and analyzed. Results cTnI values in these 13 samples showed amazing difference in the five detection systems, demonstrating various degrees of pseudo-drop, or even false-negative. One sample with low recovery was detected in Access-2 system. One sample with low recovery as well as one sample with moderate recovery were detected in Architect i2000 (Abbott). Two samples with moderate recovery and one sample with low recovery were detected in Axsym(Abbott). Three samples with moderate recovery and two samples with low recovery were detected in Dimension X Pand (Dade Behring)and one sample with moderate recovery together with four samples with low recovery were detected in Vidas (Biomerieux). And the serum levels of autoantibodies (A450) positively correlated with the degrees of their negative interference for the detection of cTnI. The R2 and P values on each system were 0. 841 (P <0. 01)vs Access-2, 0. 808 (P < 0. 01) vs Architect i2000 (Abbott), 0. 772 (P < 0. 01) vs Axsym (Abbott), 0. 707 (P < 0. 01) vs Dimension X Pand (Dade Behring) and 0. 424 (P < 0. 05) vs Vidas (Biomerieux), respectively. Conclusion Circulating autoantibodies of cTnI can induce considerable negative interference in all the 5 commonly used cTnI detection systems, which might then lead to incorrect judgments of the obtained results of cTnI in daily clinical work.

Objective:To prepare the ocular thermosensitive gel of atropine sulfate and study its in vitro release. Methods: The gel formula was optimized by central composite design response surface methodology. The influences of the amounts of poloxamer 407 (P407) and poloxamer 188(P188) on gelling temperature before and after the dilution with simulated tear fluid were investigated. A membraneless dissolution model was used to determine the gel erosion and in vitro release. Results:The optimized gel formula was as follows:23% P407 and 5% P188. The deviations between the measured values and predicted values were all lower than 5%. The in vitro release experiment showed that the gel erosion and the drug release fitted zero-order kinetics equations with promising correlation, indicating a dissolution-controlled release mechanism. Conclusion:The optimization of the ocular thermosensitive gel of atropine sul-fate can be achieved by central composite design response surface methodology with good estimation. The thermosensitive gel with sus-tained drug release property meets the design requirements.
