Objective To study the mental status and its influencing factors of patients with rheumatic disease. Methods Three hundreds and eighty-one patients with rheumatic diseases were assessed with the self-rating anxiety scale, self-rating depressive scale, Eysenck Personality Questionnaire symptom checklist-90, and demographic characteristics questionnaire. The comparison among groups were tested by t tests, Spearman's correlation analysis. Results Patients with anxiety symptoms accounted for 35.96%, and pre-valence rate of depression was 48.03%. The gender (r=0.87, OR=2.34) and duration (r=0.006, OR=1.006) were the risk factors and the C4 (r=-2.63, OR=0.07) was the protective factor. In patients with systemic lupus erythematosus, depression was related to education (Z=2.228, P=0.026), disease duration (Z=-2.015,P=0.044), IgG level (t=-2.266, P=0.027), T cell subsets CD8 (Z=1.981, P=0.047) and CD3 (Z=1.967, P=0.049). Anxiety was related to education (Z=2.452, P=0.014), duration (Z=-2.054, P=0.04), complement C3 (t=2.035, P=0.045), C4 (Z=-2.204, P=0.028) and IgG level (t=-2.126, P=0.036). The value of CRP(Z=-1.984, P=0.047) and IgM (t=2.243, P=0.028) of rheumatoid arthritis patients were higher in the depression group. The depression group was different in disease duration (Z=-2.631, P=0.009), gender ( χ2=4.415, P=0.042), ESR (Z=2.17, P=0.023) and CD3 cell (Z=2.023, P=0.043) in Sj?gren's syndrome patients. Correlation analysis showed positive correlation between the score of SDS, SAS and the P value (r=0.21, P=0.01) (r=0.14, P=0.007) N dimension (r=0.52, P=0.01) (r=0.55, P=0.007) of EPQ. There were negative correlation between the score of SDS, SAS and the E dimension (r=-0.23, P=0.01) (r=-0.14, P=0.006) of EPQ. Conclusion The prevalence of depres-sion and anxiety is higher in patients with depressive and anxious symptoms. They are in older age, female gender, poor education, longer duration, lower complement and lower T cell subsets when compared with those patients without depression and anxiety. Patients who have an introverted, instable, nervous temperament are more likely to be depressed and anxious. Psychological assessment intervention and therapy should be carried out as early as possible.

Objective To study PFSDQ-M in evaluating meal-induced hypoxemia in patients with chronic obstructive pulmonary disease. Methods PFSDQ-M scores were collected from 63 COPD patients;49 of them were performed spirometry and meal-related SpO2 monitoring. On the basis of different lung function parameters, data analysis was used to assess the relationship between PFSDQ-M and meal-induced hypoxemia. Results The SpO2 fluctuation during eating period was of significant difference in COPD patients with 3 respective levels of pulmonary function parameters (P

Objective To study on the thermic effect of food (TEF) of patients with chronic obstructive pulmonary disease(COPD).Methods Basal energy expenditure (BEE) of 12 male cases and resting energy expenditure (REE) of 11 male cases of COPD were detected by indirect calorimetry with metabolic cart. There were no significant difference in sex, age, height and body weight between two groups' e.g:TEF=REE-BEE.Results BEE and REE were 1114?208kcal/d,1433?136kcal/d respectively, the latter was higher 28% than BEE.Conclusion TEF increases significantly which may contributes to malnutrition and non-response to nutritional support in patients with COPD.

Objective To analyze the clinical features of chronic obstructive pulmanory disease(COPD)with different results of bronchodilator reversibility test. Methods A total of 47 cases of COPD were selected and given bronchodilator reversibility test and lung fuction who were divided into two groups according to the bronchodilator reversibility test.We compared these two groups with sex, age, smoke history, body mass index, levels of IgE,eosinophil and lung function. Results The positive group had a shorter history of smoking and a higher level of IgE.The amelioration of FVC,FEV1,FRC,MEF 75% and MEF 50% was higher in the positive group than the negative(P

Objective To study the immune enhancing effect of arginine on patients with chronic obstructive pulmonary disease (COPD) complicated with malnutrition. Methods Thirty cases with COPD accompanied by malnutrition received nutritional support of total calorie intake of 1 5 times of resting energy expenditure (REE).Group A(n=12) received oral administration of 30g L-arginine per day as supplementation;Group B(n=18) were control subjects. Results The nutritional parameters such as TSF(triceps skinfold) thickness,PAlb(prealbumin),CHI(creatine height index) were markedly improved in Group A than Group B,TNF-?,CD 8 decreased and IL-2,NO(nitric oxide),IgG and CD 4 increased significantly in Group A(P

Objective To monitor pulse oxygen saturation(SpO_2)continuously in patients with chronic obstructive pulmonary disease (COPD) so as to study its clinical implication. Methods SpO_2, thoracic and abdominal respiratory activity was continually recorded during meal time by PSG in 19 patients with COPD. Body mass index and lung function were measured respectively. Results SpO_2 was lower after meal(P

Objective To validate near-infrared cerebral topography (NCT) as a practical toolin tracing the regional hemodynamic changes during normal ischemic stroke model of rat. Method Middle cerebral artery occlusion (MCAO) and photosensitizer induced intracranial infarction model of rat were established. The geometric shape and infarction area were measured by NCT, functional magnetic resonance imaging (fMRI), and TTC stained anatomical imaging techniques. Result In photosensitizer induced infarction model, the correlation between anatomical infarct area and NCT image area for infarct focus were r= 0.897 for 24 h group (P<0.05) and r=0.906 for 2 months group (P<0.01),respectively. The correlation between anatomical infarction area and NCT image area for infarct focus were r=0.820 for normothermia group (P<0.05) and r= 0.851 for hypothermia group (P<0.05), respectively. The correlation between fMRI and NCT image area for infarction focus were r= 0.874 for normothermia group (P<0.05) and r= 0.782 for hypothermia group (P<0.05),respectively. Conclusion Measurement with NCT for infarction focus matched well with fMRI and anatomic sample in rats. NCT technique might be a practical tool for short-term prediction of stroke and the rehabilitation after stroke in real time.

Chinese Journal of Internal Medicine 2010;49(4):325-327
Objective To investigate the expression of Dickkopf-3 (Dkk-3) in esophageal cancer and normal esophageal tissue and the relationship between Dkk-3 expression and the biological behaveior of esophageal cancer. Methods Immunohistochemical method of S-P was used to examine Dkk-3 expression in 69 cases of esophageal squamous cell carcinoma and 5 cases of normal esophageal tissue with non-tumor tissue microarray and the results were analyzed and correlated with their clinical and pathological features. Results Positive Dkk-3 expression was observed in 65.7% (44/67) of the esophageal squamous cell carcinoma cases, but only one of the five cases with normal esophageal tissue showed positive microvascular expression of Dkk-3. In cases with positive expression of Dkk-3 significant differences were found in fiber membrane infiltration, depth of invasion, lymph node metastasis and TNM staging (P < 0. 05), while no significant differences were found in the age, gender and pathological grading (P > 0. 05). Conclusions The upregnlation of Dkk-3 expression in esophageal squamous cell carcinoma may contribute to tumor invasion and metastasis.
