1.Maternal caffeine consumption during pregnancy and the risk of low birth weight
Chinese Journal of General Practitioners 2017;16(8):633-636
Low birth weight is associated with newborn mortality and adult chronic diseases. Some studies found that naternal caffeine consunption during pregnancy was closely related with low birth weight.Resent research evidence indicates that caffeine may induce low birth weight by blocking adenosine receptors, inhibiting phosphodiesterase(PDE) and enhancing angiotensin Ⅱ (AT2) receptor gene expression to lower the placenta blood flow.We recommend that women should reduce caffeine intake during pregnancy.

Objective To quantitatively evaluate relative contribution of medical history, physical examination and laboratory investigation to diagnoses for medical outpatients. Methods In total,145 medical visitors to the outpatient department of Peking Union Medical College Hospital (PUMCH) during October 10 to 16,2008 were recruited and followed-up for 12 months. Results Nineteen of 145 visitors (13.1%) were lost during the period of follow-up and diagnoses were established for 86 of them (68.3%)finally with medical history and for 20 (15.9%) with physical examination or laboratory investigation, respectively.Confidence index of internists in their correct diagnosis increased to 7.3 with medical history and to 7.9 and 8.7 with physical examination and laboratory investigation in average, respectively. Conclusions Most visitors to internal medicine department could be diagnosed correctly with medical history only.On the basis of physical diagnosis, selection of adequate laboratory investigation for them is critical to improvement of clinical diagnosis.

Objective To evaluate diagnostic accuracy based on patient history and physical examinations in medical outpatients. Methods Totally, 145 consecutive patients visiting general internal medicine clinic at a university-affiliated teaching hospital during October 10 to 17, 2008 were recruited into the study and followed-up for 12 months. Results Eighteen of 145 patients ( 12.4% ) were lost to followup.Diagnosis was confirmed by follow-up for 45 ( 35.4% ) of those with medically unexplained symptoms (MUS).Sensitivity of physical diagnosis for those with MUS was 82.2 percent, with specificity of 95.1 percent, likelihood ratios of positive and negative results of 16.9 percent and 0.19 percent, its positive and negative prediction values of 90.2 percent and 90.7 percent, and overall accuracy of 90.6 percent, respectively. Conclusions MUS was common in medical clinical practice.Preliminary diagnosis for MUS based on patient history and physical examinations has been proved remarkably reliable.Carefully selected auxiliary laboratory evaluation combined with physical diagnosis is important for management of MUS.

Objective To investigate the effect of ultrasound- guided percutaneous injection of lauromacrogol in treating hepatic hemangiomas. Methods During the period from June 2010 to Jan. 2013 at authors’ hospital, a total of 32 patients with hepatic hemangiomas were treated with ultrasound-guided percutaneous injection of lauromacrogol. The largest lesion was 6.0 cm × 5.6 cm in size, and the smallest lesion was 3.0 cm × 2.6 cm in size. Before and after the treatment ultrasonography, hepatic functions and measurement of tumor size were performed, and the results were evaluated. Results A total of 54 times of puncturing were carried out for the 32 patients. Three times of puncturing were made in 2 cases , two times of puncturing in 18 cases, and single time of puncturing in 12 cases. Complete cure was achieved in 25 cases (78.1%) and effective result was obtained in 7 cases, with an effective rate of 100%. After the treatment, all patients complained of different degrees of pain at hepatic region, which were relieved spontaneously. Two patients developed fever (37.5 - 38.5℃), and after physical cooling the body temperature fell to normal within two days. One patient developed mild nausea and vomiting, which disappeared after symptomatic treatment. No severe complications occurred. Conclusion Ultrasound- guided percutaneous injection of lauromacrogol is a simple, safe and effective treatment for hepatic hemangiomas. Besides, this technique has no serious complications.

Objective To investigate the impacts of soluble dietary fiber (SDF) on gastric emptying, postprandial blood glucose and insulin in patients with type 2 diabetes mellitus (T2DM).Methods Totally 30 T2DM patients and 10 healthy subjects (control group) were randomized to receive SDF-free liquid (500 ml,2092 kJ) and isocalorie SDF liquid (at β-glucan 7.5 g,500 ml,2092 kJ) on two separate days based on a Cross-over order.Gastric emptying was monitored by ultrasomography at intervals of 30 min for 2 hours.Fasting and postprandial blood samples were collected at intervals of 30-60 min for 180 min to determine blood glucose and insulin. Results The proximal gastric emptying was delayed in SDF both in T2DM group (P =0.001)and control group (P =0.037).SDF significantly decreased the area under the curve of postprandial glucose (P =0.001) and insulin (P =0.001) in T2DM group.Postprandial glucose (r=-0.547,P =0.047) and insulin (r =-0.444,P =0.030) had negative correlation with distal emptying of SDF in T2DM patients. The distal gastric emptying was delayed significantly in T2DM patients with HbAlc≥6.5% (n =13,P =0.021)by SDF.Conclusions SDF can improve the postprandial glucose level, which may be related with the delayed gastric emptying.T2DM patients with higher average blood glucose has remarkably delayed gastric emptying after the administration of SDF.

Objective To study the role of hydrogen sulfide (H2S) in the effect of SB203580 on proliferation and apoptosis of hepatic stellate cells and the effects of H2S on expressions of collagenⅠand collagenⅢmRNA in hepatic stel-late cells. Methods There were five groups of HSC-T6 cells in this study including control group (DMEM medium contain-ing10%fetal bovine serum), dimethyl sulfoxide (DMSO) group, sodium hydrosulfide (NaHS)group,SB203580 (SB)group and SB+NaHS group. MTT method was used to detect the cell proliferation and inhibition rate of HSC-T6 cells treated by SB203580 and H2S. The apoptotic rate of HSC-T6 cells was detected by flow cytometry with annexin V-FITC/PI double staining. RT-PCR was used to detect the expressions of collagenⅠand collagenⅢmRNA in HSC-T6. Results The apop-totic rate of HSC-T6 cells was significantly higher in SB group and SB+NaHS group than that of control group, and the rate was significantly higher in SB+NaHS group than that of SB group (P<0.05). There was no significant difference in the apop-totic rate of HSC-T6 cells between DMSO and NaHS groups than that of control group. The expressions of collagenⅠand col-lagenⅢmRNA were found in five groups of cells. There was a higher expression of collagenⅠand collagenⅢmRNA in NaHS group than that of control group (P<0.05). The expressions of collagenⅠand collagenⅢmRNA were significantly lower in SB group and SB+NaHS group than those of control group and NaHS group (P<0.05). Conclusion H2S activated P38MAPK signal pathway. And P38MAPK was specifically blocked by SB203580 in HSC-T6 cells, which inhibited the cell proliferation stimulated by H2S and promoted the apoptosis.
