1.Cast post-core and porcelain-fused-metal combined crowns restore separated mandibular molar residual crowns and roots in 42 cases

BACKGROUND:A large number of studies have shown that cast metal post-core has many advantages that it is close to the canal wal has high strength and can shape the core outside the root according to the needs. The cast metal post-core also has good retention, resistance and wel effect on clinical repair of dental defects. OBJECTIVE: To observe the clinical effect of Co-Cr aloy cast post-core and porcelain fused metal combined crowns in restoration of the separated mandibular molar residual crowns and roots. METHODS:Totaly 28 patients (42 teeth) who had the separated mandibular molar residual crowns and roots were enroled, including 10 males and 18 females, aged 18-32 years. After perfect root canal therapy and part of teeth with crown lengthening, teeth were repaired with Co-Cr aloy cast post-core and porcelain fused metal combined crowns. The patients were folowed up for 2 years. The repair effect was evaluated by combining clinical manifestations with X-ray examination. RESULTS AND CONCLUSION: During 2 years of folow-up, 26 cases were repaired successfuly, 2 cases failed, including 1 case of periapical periodontitis and 1 case of periodontitis. These results demonstrate that the cast post-core and porcelain-fused-metal combined crown has a good result on the residual root and crown of the separated mandibular molar.
2.Nerve growth factor and leukemia inhibitory factor upregulate the expression of IL-4 and IL-5 in lymphocytes of asthmatic rats

AIM:To investigate the role of nerve growth factor (NGF) and leukemia inhibitory factor (LIF) in the expression of IL-4 and IL-5 in the splenic lymphocytes of asthmatic rats. METHODS:Sixteen Sprague-Dawley rats were randomly divided into two groups:control group (n = 8) and asthma group (n = 8). The asthmatic model was set up by intraperitoneal injection of 100 mg ovalbumin and 100 mg aluminum hydroxide,and receiving nebulization aspiration of 1% ovalbumin solution 2 weeks later. The splenic lymphocytes from the control rats and asthmatic rats were isolated and cultured with exogenous NGF or LIF at different doses. The mRNA expression of IL-4 and IL-5 in the splenic lymphocytes was respectively detected by reverse transcription-polymerase chain reaction (RT-PCR). Results: 12 herbs in Jianpixiaoshi capsule can be distinguished by Microsccopic distinguish, Atractylodes macrocephala, Glycyrrhiza uralensis, Panax ginseng, Citrus reticulate, Ammomum villosum, Crataegus pinnatifida and Codonopsis pilosula can be distinguished respectively by TLC.Conclusion: The method is accurate and simple, and can be used for distinguishing Jianpixiaoshi Capsule.

5.Transumbilical laparoendoscopic single-site resection of postperitoneal paraganglioma: report of 5 cases

ObjectiveTo discuss the safety and feasibility of transumbilical laparoendoscopic single-site resection of postperitoneal paraganglioma. MethodsFive patients with postperitoneal paraganglioma accepted transumbilical laparoendoscopic single-site surgery from October 2010 to October 2011.Four patients were male and 1 patient was female aged from 24 to 52 years with the average age of 38 years.Three patients presented with paroxysmal hypertension,1 patient with persistent hypertension,and the other with no symptom.The tumors were located on left postperitoneum in 3 cases (2 adjacent to left renal vessels,the other para-aortic),and right postperitoneum in 2 cases (1 adjacent to right renal vessels,the other between inferior vena cava and aorta).The diameter of tumors ranged from 2.5 cm to 6.0 cm.or-blocker was given for preoperational preparation.After 2 -4 weeks,transumbilical laparoendoscopic single-site resection of tumors were administered.Transparacolic route was selected in 4 cases and the other was operated through mesentery. ResultsAll the operations were successful.None was converted to conventional laparoscopic surgery or open surgery.The operative time ranged from 80 to 190 min,the mean was 150 min.The blood loss ranged from 50 to 150 ml,the average blood loss was 85 ml. No operation-related complications ocurred.Four cases with hypertension returned to normal blood pressure.All cases recovered uneventfully and discharged in 3 to 6 d after operation.Postoperational pathologic result proved the diagnosis of paraganglioma.The wound healed very well and the scars were occult.Follow-up was performed in all cases for 3 to 15 months.No recurrence was found.ConclusionTransumbilical laparoendoscopic single-site resection of postperitoneal paraganglioma could be safe and effective,and has good cosmetic effect.
6.Astaxanthin inhibits sodium azide-induced cytotoxicity in hepatocyte L-02 cells probably by H+ transferring function.

This study is to investigate the protective effect of astaxanthin against injured hepatocyte L-02 cells induced by sodium azide (NaN3) and reveal the possible mechanisms. Hepatocyte L-02 cells were exposed to 100 mmol.L-1 NaN3 with various concentrations of astaxanthin pre-incubated, then the cell viability was measured by MTT method; The level of reactive oxygen species (ROS) was determined by DCFH-DA method; The changes of mitochondrial membrane potential (MMP) and apoptosis ratio were detected by JC-1 method and Annexin V-FITC/PI double stain method, respectively. Results showed that after cells were exposed to 100 mmol.L-1 NaN3 for 3 hours, the cell viability significantly decreased; ROS level and the percentage of late phase apoptosis increased obviously; MMP was also declined. When cells were pretreated with astaxanthin, the cell damage and late phase apoptosis ratio reduced and MMP was maintained. However, the level of ROS showed insignificant decrease (P>0.05). The beneficial concentration of astaxanthin in improving cell viability and MMP was not in a dose dependent manner and the most effective of which was 0.10 nmol.L-1 (P<0.01). In order to reveal its possible non-antioxidant mechanism, mitochondrial membrane was imitated and H+ transferring function of astaxanthin was also detected by bilayer lipid membrane (BLM) method. Results showed that 2.0% astaxanthin could transfer H+ efficiently. These suggested the mechanisms of astaxanthin in protection of hepatocyte L-02 cells not via its ROS quenching capability but via its H+ transferring function, which improved the mitochondrial function and had the sequence biology effects.
7.Von Hippel-Lindau syndrome-2B accompanied with leukocytoclastic vasculitis: a case report and lirerature review

Objective To explore clinicopathologic features,diagnosis,treatment and prognosis of von Hippel-Lindau (VHL) type 2B mixed cutaneous leukocytoclastic vasculitis. Methods A 22-Year-Old Man who presented with constitutional symptoms,severe hypertension,and purpuric lesions over the knees.Clinical features,histopathological,IHC and anti-angiogenesis therapy of this case with VHL syndrome-2B mixed cutaneous leukocytoclastic vasculitis was studied based on the available follow-up data.He underwent laparoscopic adrenalectomy and nephron sparing nephrectomy.Clinical evaluation included ophthalmologic examination as well as imaging exams and endocrinal hormone test for tumors markers ; molecular analysis consisted of PCR amplification of the complete VHL gene coding sequence (three exons) and automated nucleotide sequencing. Results 24 h urine Norepinephrine was 295.84 μg and octreotide scan was positive.The VHL-2B patient suffered from leukocytoclastic vasculitis,a retinal hole,pancroatic tumors (endocrine tumor and microcystic cystadenoma),bilateral pheochromocyoma,clear renal cell carcinoma,renal cysts,hepatic hemangioma and epididymal cyst.The patient developed 5 different tumors related to VHL within a period of 6 years.The cutaneous vasculitis persisted despite treatment with high-dose systemic corticosteroids,but rapidly resolved after treatment with phenoxybenzamine and removal of bilateral pheochromocytomas.Tumor cells of resected PHEO samples stained positive for CgA and S-100,but negative for Melan-A.,and with less than 1% Ki-67. Conclusions Cutaneous leukocytoclastic vasculitis is one of paraneoplastic syndrome of pheochromocytoma.Because new lesions may develop during the patient's lifetime,regular clinical inspection is recommended in order to check up the development of any new lesions.
8.Effects of remifentanil preconditioning on intestinal ischemia-reperfusion injury in rats:the rela-tionship with opioid receptors

Objective To evaluate the effect of remifentanil preconditioning ( RP ) on intestinal is?chemia?reperfusion ( I∕R) injury in rats and its relationship with opioid receptors. Methods Seventy?two Sprague?Dawley rats, aged 6-7 weeks, weighing 250-280 g, were randomly divided to 9 groups ( n=8 each): sham operation group (S), intestinal I∕R group (group I∕R), RP group, different opioid receptor antagonists groups (N, BNI and CTOP groups), and opioid receptor antagonists + RP groups (N+RP, BNI+RP and CTOP+RP groups) . Intestinal I∕R was produced by clamping the superior mesenteric artery for 1 h followed by 2 h reperfusion in all the groups except group S. RP was induced by 3 cycles of 5 min infusion of remifentanil 0?2 μg·kg-1 ·min -1 followed by 5 min infusion of normal saline before ischemia. Naltrindole (δ?receptor antagonist, 5 mg∕kg) , nor?binaltorphimine (κ?receptor antagonist, 5 mg∕kg) and CTOP (μ?receptor antagonist, 1 mg∕kg) were administered before RP. At 2 h of reperfusion, blood sam?ples were collected from the cardiac apex for determination of serum diamine oxidase ( DAO) activity. Intes? tinal tissues were then removed for microscopic examination. Intestinal damage was assessed and scored ac?cording to Chiu. Apoptosis in intestinal mucosal epithelial cells was detected using TUNEL assay, and ap?optosis index was calculated. The expression of activated caspase?3 in intestinal mucosal epithelial cells was measured by Western blot. Results Compared with group S, the serum DAO activity, Chiu′s score, and apoptosis index were significantly increased, and the expression of activated caspase?3 was up?regulated in I∕R and RP groups ( P<0?05) . Compared with group I∕R, the serum DAO activity, Chiu′s score, and ap?optosis index were significantly decreased, and the expression of activated caspase?3 was down?regulated in RP, BNI+RP and CTOP groups (P<0?05), and no significant change was found in the parameters men?tioned above in N, N+RP, BNI and CTOP+RP groups (P>0?05). Compared with group RP, the serum DAO activity, Chiu′s score, and apoptosis index were significantly increased, and the expression of activa?ted caspase?3 was up?regulated in N+RP and CTOP+RP groups ( P<0?05) , and no significant change was found in the parameters mentioned above in group BNI+RP ( P>0?05) . Conclusion RP can mitigate in?testinal I∕R injury in rats, and the mechanism is related to the anti?apoptotic effect mediated by activation ofδ?and μ?opioid receptors, but not κ?opioid receptors.
9.A case matched study on laparoscopic versus open pancreaticoduodenectomy

Objective To compare the clinical outcomes of total laparoscopic pancreatoduodenectomy (TLPD) and open pancreaticoduodenectomy (OPD).Methods From August 2013 to December 2014,137 pancreaticoduodenectomies (PDs) were performed at the Department of General Surgery of our hospital,of which 19 TLPDs were performed at the Department of Minimally Invasive Surgery (the Experimental group).At the same time,another cohort of 19 OPDs were matched for age,gender,body mass index (BMI),ASA score and tumor site and they formed the control group.The clinical data in the perioperative period were evaluated,and the short-term clinical outcomes were compared.Results Operation time in the experimental group was significantly longer than the control group [(407.8 ± 146.5)min vs (263.3 ± 65.3) rmin,P < 0.05].The mean intraoperative blood loss [(309.7 ± 151.2)ml vs (509.4 ± 309.9)ml],mean intensive care time after surgery [(47.5 ±16.8)h vs (68.1 ± 19.1)h],mean postoperative time to pass flatus [(3.5 ± 1.1) d vs (4.3 ± 1.1) d],mean postoperative hospitalization stay [(8.8 ± 2.1) d vs (10.8 ± 2.3)d] and mean incision length [(5.1 ± 0.9)cm vs (14.4 ± 1.3)cm] in the experimental group were significantly different from the control group (P < 0.05).There were no significant differences on the intraoperative R0 resection rates,intraoperative pathology,tumor size [(20.6 ± 9.6) mm vs (25.9 ± 10.2)mm],number of lymph node dissected [(17.7 ± 6.5) vs (19.4 ± 5.6)],complication rates,recurrence rates and mortality between the two groups (P > 0.05).Conclusions TLPD had comparable safety and therapeutic outcome when compared with OPD.Moreover,TLPD has the advantages of less bleeding,smaller wounds and faster postoperative recovery.TLPD requires specialized appliances and equipments,better surgeon experience and patient selection to achieve a high success rate.
10.The expression, purification and identification of recombinant SARS S1 subunit expressed in E. Coli

Objective:To study the course and mechanism of the immune response to SARS virus. Methods:The recombinant SARS virus S1 subunit was expressed in E. Coli according to the results of bioinformatics analysis. After purification, the recombinant S1 protein was identified by 6 serum samples of recovered SARS patients and 6 serum samples of health donors, which were collected before out-break of SARS. Results:Sequencing analysis confirmed that the recombinant protein has the same sequence of natural SARS virus S1 subunit. The recombinant S1 protein could react with all the samples from recovered SARS patients but not the control samples from healthy donors according to the results of Western blot. Conclusion:The recombinant SARS virus S1 subunit may provide a good tool for the research of immune response to SARS virus and the producing of recombinant vaccine to prevent people from SARS.
