1.Paraneoplastic neurological syndrome with positive SOX-1, GABABR, and VGCC antibodies: A case report and literature review

Objective Paraneoplastic neurological syndrome is a rare immune-mediated disease induced by underlying tumors, and early identification of special subtypes of paraneoplastic syndrome and specific autoantibodies can guide efficient tumor screening, thereby promoting timely antitumor treatment and prolonging survival time. Methods A retrospective analysis was performed for the diagnosis and treatment of a case of paraneoplastic neurological syndrome with multiple clinical phenotypes and multiple autoantibodies who was admittd to Department of Neurology, Weifang People’s Hospital, and a systmatical review was performed for related articles. Results The patient presented with unsteady gait and mental and behavioral disorders in the early stage and was initially diagnosed with paraneoplastic cerebellar degeneration and paraneoplastic limbic encephalitis with positive anti-SOX-1 and anti-GABABR antibodies. Due to abnormal electrophysiological results and positive P/Q-type VGCC antibodies, the patient was considered to have Lambert-Eaton myasthenic syndrome, and all evidence suggested that the patient had occult small cell lung cancer. Finally thoracoscopic biopsy confirmed that lymph node enlargement on PET-CT was in accordance with the pathological changes of small cell lung cancer. The patient received both antitumor treatment and immunotherapy and had a survival time of 19 months. Conclusion The detection of anti-SOX1 antibodies should promote the screening for small cell lung cancer. In patients with paraneoplastic limbic encephalitis and paraneoplastic neuromuscular syndromes mediated by cell surface antibodies, immunotherapy should be initiated in parallel with antitumor treatment, and such patients tend to have a good prognosis.
2.Application value of CT- based three-dimensional digital navigation technology in interventional microwave ablation of lung cancer

Objective To explore the application value of computed tomography (CT)-based three-dimensional digital navigation technology in microwave ablation of lung cancer. Methods A total of 92 patients with lung cancer in our hospital were retrospectively analyzed, and randomized to receive microwave ablation with the three-dimensional digital navigation technology or traditional CT-guided microwave ablation (three-dimensional navigation group and traditional group). The two groups were paired according to tumor location, size (the maximum diameter difference of less than or equal to 2 mm), and microwave ablation conditions, for a total of 46 pairs. The operation time, times of microwave ablation needle puncture, CT dose index, incidence rates of intraoperative complications, and postoperative lesion control were compared between the two groups. Results The operation time [(30.07 ± 6.36) min vs (47.20 ± 9.65) min], times of puncture [(1.72 ± 0.69) times vs (7.13 ± 3.00) times], CT dose index [(11.16 ± 2.20) mGy vs (26.67 ± 8.72) mGy], and incidence of intraoperative complications (10.87% vs 34.78%) in the three-dimensional navigation group were lower than those in the traditional group, and the response rate was higher in the three-dimensional navigation group than in the traditional group (93.48% vs 71.74%) (P < 0.05). Conclusion CT-guided three-dimensional digital navigation technology for microwave ablation of lung cancer makes the operation of interventional puncture more accurate and safe.
3.Research Progress of Mesenchymal Stem Cells and Their Exosomes on Tumors.

In China, malignant tumor is the main cause of death in both urban and rural areas. Mesenchymal stem cells (MSCs) have multidirectional differentiation potential, self-renewal ability and good immunomodulatory properties. Exosomes, as important paracrine substances of MSCs, mediate information exchange and transmission between cells in tumor microenvironment and influence the occurrence and development of tumors. Recently, conflicting findings have been reported on the effects of MSCs and their exosomes on tumors. On the one hand, MSCs and their exosomes are tumorigenic and can target specific sites to inhibit tumor growth; On the other hand, there is also evidence that MSCs could affect tumor growth and migration as part of the tumor microenvironment. In this paper, we will review the relationship between MSCs and exosomes and tumorgenesis and development, as well as how MSCs and exosomes play different roles in tumorgenesis and development, in order to provide beneficial help for tumor diagnosis, prognosis and precise treatment.

4.Potential drawbacks of and development suggestions for medical liability insurance

6.Study on the role of classical complement pathway in the development of benign lymphoepithelial lesions of lacrimal gland

AIM: To analyze the role of complement system(CS)and its classical pathway in the pathogenesis of lacrimal gland benign lymphoepithelial lesions(LGBLEL).METHODS: The tissues of patients with LGBLEL or orbital cavernous hemangioma(CH)were collected. Proteomics analysis was used for the identification of different proteins. Reverse transcription-polymerase chain reaction(RT-PCR), immunohistochemical staining(IHC)and Western Blotting were employed to verify the changes of the differential proteins in CS signal pathway, in order to identify its role in the pathogenesis of LGBLEL.RESULTS: The results of proteomic analysis showed that the expression levels of proteins C3, C5, C9 and C1q in CS signal pathway in the lacrimal gland tissues of LGBLEL patients were all changed compared with those of orbital CH patients.The results of RT-PCR showed that the mRNA expression levels of C1qA, C5 and C9 in patients with LGBLEL were significantly higher than those patients with orbital CH. The results of IHC showed that the expression levels of C1qA, C3, C5 and C9 were significantly increased in patients with LGBLEL compared with those patients with orbital CH. The results of Western Blotting showed that the protein expression levels of C1qA, C3, and C9 were significantly increased in patients with LGBLEL compared with those patients with orbital CH.CONCLUSION: The CS has been shown to participate in the pathogenesis of LGBLEL and its classical pathway may be one of the pathways which plays a role.

8. Clinical Effect of Modified Huangqi Guizhi Wuwu Tang on Patient with Lower-extremity Arterial Disease

Objective:To observe the clinical effect of modified Huangqi Guizhi Wuwu Tang on patient with lower-extremity arterial disease (LEAD) with deficiency of Qi and Yin and blood stasis syndrome, and study the antioxidant mechanism. Method:One hundred and twenty-eight patients were randomly divided into control group (64 cases) and observation group (64 cases) by random number table. Patients in control group was treated for controlling blood sugar, blood pressure and blood fat, and got probucol in the morning and evening, 0.5 g/time, 2 times/days, aspirin enteric-coated tablets for 3 months, 100 mg·d-1, and alprostadil injection diluted with 10 mL normal saline for 15 days, 10 μg·d-1, 1 time/day. A course of the treatment in control group was 15 days, and there were 4 courses. In addition to the therapy of control group, patients in observation group were additionally given modified Huangqi Guizhi Wuwu Tang, 1 dose/day, for 3 months. Before and after treatment, ankle brachial index (ABI) and toe branch index (TBI) were detected, and internal diameter of dorsal artery of foot, peak velocity and blood flow were detected by color Doppler ultrasound. Main symptoms and sign were scored. And levels of interleukin-1 (IL-1), Homocysteine (Hcy), tumor necrosis factor-α (TNF-α), high sensitive C reactive protein (hs-CRP), cystatin C (CysC), malondialdehyde (MDA), superoxide dismutase (SOD) and oxidized low density lipoprotein (OX-LDL) were detected. Result:According to the rank test, clinical effect in observation group was better than that in control group (P<0.05). And levels of ABI, TBI and SOD were higher than those in control group (P<0.01). The ameliorate of internal diameter of dorsal artery of foot, peak velocity and blood flow were better than those in control group, and scores of intermittent postscript, lower extremity pain, dorsalis pedis artery pulsation, skin color, skin temperature, numbness of limbs, swelling of extremities and the sign and levels of IL-1, Hcy TNF-α, hs-CRP, Cys-C and Ox-LDL were lower than those in control group (P<0.01). Conclusion:Modified Huangqi Guizhi Wuwu Tang can relieve symptoms and signs, lower limb vascular function and hemodynamic, with certain anti-inflammatory effect and oxidative stress. It can also reduce vascular endothelial cell injury, and relieve and postpone the progress of LEAD
10. Effects of kca3.1 on biological function and differentiation of endothelial progenitor cells

OBJECTIVE:To investigate the effect of KCa3.1 channel on the function of EPCs. METHODS: The gene expression of KCa3.1, vWF and CD31 on EPCs were detected by qRT-PCR. CCK-8 kit, cell adherent method and Matrigel were used to detect the changes of cell proliferation, adhesion and in vitro angiogenesis; cell immunofluorescence or fluorescence activated cell sorter(FACS) was used to detect the protein expression of KCa3.1, vWF, CD31, integrin β1, integrin β3 separately. RESULTS: Blocking the function of KCa3.1 or interfering with the expression of KCa3.1 can attenuated EPC function of proliferation, adhesion and angiogenesis, but it can promote the differentiation of EPCs. Overexpression or activation of KCa3.1 channel can enhance EPCs proliferation, adhesion and angiogenesis but decrease the level of differentiation. The expression of integrin β1 on EPCs was attenuated with blocking KCa3.1 channel, but the expression effect was reversible by the activator. CONCLUSION: The alteration of KCa3.1 channel function or expression affect the biological characteristics and differentiation of EPCs.
