1.Radial head prosthesis replacement for treatment of comminuted fracture of the radial head

Objective To evaluate the clinical efficacy of radial head prosthesis replacement in treatment of comminuted fracture of the radial head and discuss relative operative announcements.Methods There were 27 patients with comminuted fracture of the radial head who were treated with radial head prosthesis replacement. Results Of all,26 patients were followed up for averaged 14months.According to Broberg score,the post-operative function was excellent in 9 patients(35%),good in 13(50%)and fair in 4(15%),with no poor results and with excellence rate of 85%. Conclusion Radial head prosthesis replacement can better restore the stability,flexion and extension of the elbow as well as the rotational motion of the forearm and is worthy of clinical application.
2.The relationship between serum S100β, neuron-specific enolase and postoperative cognitive dysfunction in the elderly

Objective To explore the relationship between S100β, neuron-specific enolase (NSE) and postoperative cognitive dysfunction(POCD) in the elderly. Methods Cognitive function was evaluated using a battery of nine neuro-psychological tests before operation,at one week after operation. The old people were divided into POCD group(n=121) and normal group(n=439). Their S100β and NSE levels in all group were detected by enzyme linked immunesorbent assay(ELISA) before anesthesia,at the end of surgery and 6h,24h after opera-tion. Meanwhile relativity between S100βand NSE levels and factors of postoperative cognitive dysfunction were an-alyzed. Results S100βand NSE levels increased significantly at the end of surgery in both groups, but the levels in normal group were significantly lower than those in POCD group at the same stage(P<0.05). S100β and NSE levels were obviously relative to POCD. Conclusion S100β and NSE levels have close correlation with POCD in the elderly.
3.Effect of α-lipoic acid on cerebal injury induced by cardiopulmonary bypass in dogs

Objective To investigate the effect of α-lipoic acid on cerebral injury induced by cardiopulmonary bypass (CPB) in dogs. Methods Twelve adult healthy mengrel dogs of both sexes weighing 13.5-17.5 kg were randomly divided into control group (group C) and α-lopoic acid group (group L) (n = 6each). In group L α-lipoic acid 50 mg/kg was injected iv immediately before CPB. The animals were anesthetized with intraperitoneal 2.5% pentobarbital 25 mg/kg, intubated and mechanically ventilated. PaCO2 was maintained at 35-40 mm Hg. Femoral artery and vein were cannulated and Swan-Ganz catheter was inserted into pulmonary artery for MAP, CVP and CO monitoring and blood sampling. Blood samples were obtained immediately before aortic cross-clamping (T0, baseline), at 30 and 60 min after aortic cross-clamping (T1,2) and 30 and 60 min after aortic unclamping (T3,4) for measurement of plasma concentrations of TNF-α, S100β protein and neuron-specific enolase (NSE). Results In group C CPB significantly increased plasma TNF-α, S100β protein and NSE concentrations as compared with the baseline values at T0. Pretreatment with α-lipoic acid significantly attenuated CPB-induced increase in plasma TNF-α, S100β protein and NSE concentrations in group L. Plasma S100β protein and NSE levels were positively correlated with plasma TNF-α level. Conclusion Pretreatment with α-lipoic acid is effective in attenuating CPB-induced inflammatory response and cerebral injury.

5.Expression of OPG/OPGL protein and its significance in rat model of trauma-induced osteonecrosis of the femoral head

[Objective]To explore the expression of OPG/OPGL protein and its significance in rat model of trauma-induced osteonecrosis of the femoral head(ONFH). [Methods]Thirty-two SD rats about 6 months were divided randomly into experimental and control groups.The animal model of femoral head necrosis was established in 32 SD rats by removing round ligaments of femoral head.Animals were sacrificed at 1,2,4 and 6 weeks after operation,respectively.The specimens were examined through histological observation under light microscope.The other side with sham operation served as normal control group.The comparison of fat tissue with hematopoietic tissue in the cavity of bone marrow of femoral head were performed by CMIAS computer-assisted image and statistical analysis.The percentage of empty lacuna in the femoral heads was obtained.Immunohistochemistry was used to determine the expression of OPG/OPGL protein in ONFH.[Results]ONFH was confirmed in experimental group.The model in various stages was successfully duplicated.Compared with normal control group,the percentage of empty lacuna remarkable increase was found in experimental groups in different periods(P
6.Protective effect of the application of Ginkgo-damole in elderly patients with hypertension in total hip arthroplasty

Objective To clarify protective effect of the application of Ginkgo-damole in elderly patients with hypertension in total hip arthroplasty.Methods From January 2012 to December 2013,67 elderly patients with hypertension which should take total hip arthroplasty were divided into observation group and control group.Ginkgo-damole were only used in observation group during the operation.The anesthetic effect and the change of systolic blood pressure(SBP)diastolic blood pressure(DBP),heart rate(HR)and mean arterial pressure(MAP)were observed,postoperative recovery time of consciousness were recorded. Results The level of SBP,DBP,MAP and HR in both group were significantly difference when compared with before anesthesia(P<0.05 ).At the T2-4 time point,the level of SBP,DBP,MAP in observation group were significantly higher than that of control group,the level of HR was significantly lower than the control group,there had significant difference(P<0.05).At the T5 time point,the level of SBP, DBP,MAP and HR in observation group were significantly lower than of control group,there had significant difference between two groups(P<0.05 ).The postoperative recovery time in observation group was(10.41 ±2.35)min,which was lower than that in control group(15.63 ±2.42)min,there had significant difference between two groups(P<0.05 ).Conclusion Ginkgo-damole can effectively relieve the stress reactions caused by anesthesia and total hip arthroplasty,it has good protective effect on elderly patients with hypertension.
7.Expression of dihydrodiol dehydrogenase 2 in prostate cancer and its significance

Objective To study the expression of dihydrodiol dehydrogenase (DD) in prostate cancer and its significance. Methods With reference to expression of ?-actin gene,the expression level of a human dihydrodiol dehydrogenase isoform (DD2) mRNA was examined in prostate cancer tissues (11 cases) and normal prostate tissues (10 cases) by reverse transcriptive-polymerase chain reaction(RT-PCR). Quantitative determination of relevant band densities was performed using densitometry-scanning techniques. Results Strong expression of DD2 mRNA was detected in prostate cancer tissures with absorbance in the range of 0.550 to 1.018 (median,0.726),and low expression of DD2 mRNA in normal prostate tissues with absorbance in the range of 0.248 to 0.420 (median,0.333). 9.Diagnostic significance of endoscopic ultrasonography in common bile duct stones

Objective To study on the effectiveness of endoscopic ultrasonography (EUS) in diagnosing common bile duct (CBD) stones. Methods Accuracies for the diagnosis of choledocholithiasis by abdominal ultrasonography ( B-US) and computed tomography (CT) are compared with those by EUS in 45 patients. Final diagnosis is determined by endoscopic retrograde cholangiography ( ERC) with or without sphincterotomy ( EST) or operative exploration. Results Forty-three out of 45 patients with choledtx-holithia sis were diagnosed by EUS, of them 2 patients with slight dilation of common bile duct in which stones were confirmed in the proximal part of common bile duct by EST. In 2 out of 43 patients EST and operation diagnosed cholecystolithiasis instead of choledocholilhiasis. The sensitivity and the positive predictive value are both 95%. With 41 patients choledocholithiasis were confirmed by EUS, the sizes of stone were diameter ≥ 10mm in 5 patients, 6-9 mm in 10 patients and ≤5mm in 26 patients. Four out of 45 patients were suspected to have CBD stones and 2 patients had tumor in periampullar area by B-US. Three patients were diagnosed to have CBD stone and 1 patient was suspected to have tumor in peri-ampullar area by CT. Conclusions EUS appears to be the best diagnostic tool for the diagnosis of choledocholithiasis compared with other noninvasive procedures such as B-US and CT. EUS is at least as sensitive as ERC especially in diagnosis of small CBD stones.
