1.Randomized Controlled Observation of Acupuncture Treatment for Knee Osteoarthritis

Objective To compare the therapeutic effects of tug-of-war needling method versus conventional electroacupuncture on knee osteoarthritis, seek a better method for treating this disease and prove the importance of needling and moxibustion techniques. Method One hundred patients meeting the criteria of knee osteoarthritis were allocated, in order of visits, to an electroacupuncture group of 50 cases and a tug-of-war needling group of 50 cases. In the tug-of-war needling group of patients, points Zusanli(ST36) and Heding(EX-LE2) were mainly selected and after the arrival of qi following needle insertion, forward pushing or backward pulling was repeatedly made like tug of war to strengthen or eliminate the needling sensation. In the electroacupuncture group, local points were mainly selected according to the routine, and after the arrival of qi, a G6805-1 type electric acupuncture therapeutic instrument was connected and sparse-dense waves were used. Reactions were recorded in the two groups of patients after two courses of treatment. Joint function was assessed using the VAS and the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). Result Both groups of patients were followed up at six months after the end of treatment. There were statistically significant pre-/post-treatment differences in the VAS score and the WOMAC sub-scores and total score in the two groups (P<0.01). The VAS score and the WOMAC sub-scores and total score decreased more in the tug-of-war needling group of patients than in the electroacupuncture group (P<0.01). The total efficacy rates in the tug-of-war needling and electroacupuncture groups were 92.0% and 90.0%, respectively, at the end of treatment and 94.0% and 92.0%, respectively, at the follow-up;there was no statistically significant difference between the two groups and between at the end of treatment and at the follow-up in the two groups (P>0.05). However, the cure and marked efficacy rates in the two groups of patients were 68.0%and 52.0%, respectively, at the end of treatment and 68.0%and 54.0%, respectively, at the follow-up. It was statistically higher in the tug-of-war needling group than in the electroacupuncture group (P<0.05). Conclusion Both tug-of-war needling and electroacupuncture have a good and very stable therapeutic effect on knee osteoarthritis. Tug-of-war needling is better than electroacupuncture in improving the clinical symptoms.
2.Clinical observation of interferon-induced mental disorder in patients with chronic hepatitis

Objective To explore clinical haracteristics, diagnosis, treatment, prognosis and prevention of mental disorder induced by interferon.Methods Thirty cases of interferon treatment for chronic hepatitis were select-ed as research subjects.The psychotic symptoms of these patients induced by interferon were analysized by clinical statistics.Results The Self-Rating Depression Scale( SDS) and the Self-Rating Anxiety Scale( SAS) scores after treatment were (47.10 ±9.47) points and (43.00 ±10.34) points,these were higher than before treatment and the difference was statistically significant (t=8.39,7.66,all P<0.01).The incidence rates of the psychotic symptoms in this group of interferon-induced were 66.7%(20/30).The occurrence rates of headaches,insomnias,depres-sions,anxieties and personality changes were respectively 36.7%,30.0%,16.7%,10.0%,10.0%.Before and after treatment,the difference was statistically significant (χ2 =15.43,P<0.01).Conclusion The symptoms of mental disorder induced by interferon have high incidence rate and serious consequences,so early detection and early treat-mentcan contribute a better prognosis.
3.Problems and countermeasures in surgical therapy of hilar cholangiocarcinoma

Surgical resection offers hilar cholangiocarcinoma patients the only chance for cure and long-term survival.Hepatectomy,extrahepatic bile duct resection,regional lymphadenectomy and hepatojejunostomy are the current treatment of choice for most patients with hilar cholangiocarcinoma.Because of the aggressive nature of hilar cholangiocarcinoma and the absence of effective adjuvant therapy,surgical therapy still remains a challenge to hepatopancreatobiliary surgeons even in the precise surgery era.Controversies exist over preoperative assessment and staging,the use of preoperative biliary drainage,preoperative portal vein embolization,the range of hepatic resection,portal vein resection and construction,hepatic arteriectomy and the range of lymphadenectomy.This article reviews the surgical management of hilar cholangiocarcinoma with a focus on these controversies.

5.Hollow screw and suture fixation for treatment of tibial intercondylar eminence fractures under arthroscopy

BACKGROUND:Intercondylar eminence fractures of the tibia are often treated by arthroscopic surgery, but the fixation methods are controversial.
OBJECTIVE:To compare curative effect of hol ow screw and suture fixation in treatment of tibial intercondylar eminence fractures under arthroscopy.
METHODS:A total of 46 patients with tibial intercondylar eminence fractures were recruited from the Third People’s Hospital of Yancheng, China from June 2010 to January 2013. According to the patient’s wil ing and physician’s suggestions, the involved patients were divided into hol ow screw group (n=25) and suture fixation group (n=21), undergoing hol ow screw and suture fixation under arthroscopy, respectively.
RESULTS AND CONCLUSION:According to Ikeuchi grade, the excellent and good rate after fixation showed no significant differences between the two groups (P>0.05), but the functional training time in hol ow screw group was significantly shorter than that in suture fixation group (P<0.05). Hol ow screw treatment under the arthroscopy is better than suture fixation in treatment of intercondylar eminence fractures of the tibia.
6.Effects of mild hypothermia on secondary cerebral vasospusm and levels of endothelin,calcitonin gene related peptide in cerebrospinal fluid and plasma in patients with severe brain injury

Objective To study the effects of mild hypothermia on secondary cerebral vasospasm and levels of endothelin (ET),calcitonin gene related peptide (CGRP) in cerebrospinal fluid (CSF)and plasma in patients with severe brain injury(SBI).Methods Sixty-five patients with SBI were randomly divided into mild hypothermia group(30 cases)and control group(35 cases).Based on the routine treating method,the patients of mild hypothermia group were treated with local mild hypothennia.The levels of ET,CGRP in CSF and plasma were detected before and after treatment.And the incidence rates of secondary cerebral vasospasm cases were compared between the two groups.Results (1)At 7,14 day after treatment,ET levels in CSF and plasma in mild hypothermia group were significantly lower than these in control group (P<0.05or<0.01).The CGRP levels in CSF and plasma were the lowest at 7 day,then gradually increased in two groups.However,the CGRP levels in mild hypothermia group at 7,14 day were higher than those in control group (P<0.05 or<0.01).(2) The rate of secondary cerebral vasospasm in mild hypothermia group was 6.67% which significantly less than that in control group (22.86%) (P<0.05).Conclusion Mild hypothermia can prevent the increasing of ET and the decreasing of CGRP in CSF and plasma,and decrease the incidence rate of secondary cerebral vasespasm in patients with SBI.

Conclusion Imagine therapy in the static state of Qigong has great clinical value.
8.TCM Sleep-regulating Technique in the Static State of Qigong
