2.CLINICAL ANALYSIS OF 34 CASES OF LOW -DOSE SILDENAFIL TREATMENT OF IN-TERSTITIAL CYSTITIS

Objective To investigate the treatment effect of daily low dose of sildenafil treating interstitial cystitis .Methods 68 patients with interstitial cystitis in our hospital from June , 2012 to December, 2013 were divided into observation group and con-trol group by digital randomized method, 34 cases in each group.Observation group adopted daily dose of sildenafil 0.25 g treatment every day, and control group adopted the same dose of starch placebo every day.Two groups were treated for 3 months.Curative effect, micturition number a day before and after the treatment, the maximum bladder capacity, interstitial bladder inflammation shape index and problem index score ( ICSI, ICPI) , pelvic pain and urinary frequency score ( PUF) and quality of life scores ( QOL) of two groups were compared.Results The rate of significant curative effect and total effective rate of observation group were significantly higher than that of the control group, and the difference was statistically significant (p<0.01).Micturition number every day, the maximum bladder capacity, ICSI, ICPI, PUF and QOL scores of the two groups before treatment had no statistical significant differ-ences(p>0.05).Maximum bladder capacity and QOL scores of observation group after treatment were (265.44 ±43.85) ml and (44.15 ±4.69) respectively, which were higher than the (92.45 ±25.71) ml and (22.14 ±2.97) of the control group after treat-ment and (94.12 ±23.95)ml and (22.38 ±3.18) of observation group before treatment;Urine per day, ICSI, ICPI and PUF of ob-servation group after treatment is lower than that of the observation groups after treatment and that of the control group before treat-ment, and the difference had statistically significant (p<0.05).Conclusion Daily low dose of sildenafil for treating women with in-terstitial cystitis is worthy of clinical use for it can effectively relieve clinical symptoms and improve patient's life quality.
3.The nutrition support to radial enteritis-2 case report

This lecture reports two postoperative patients with pelvic cancer radial therapy to lower abdomen with abdominal pain, diarrhea, and intestinal obstruction cause by radial therapy. Because the patient didn' t select food to eat, the diarrhea couldn' t be controlled. The nick in the abdomen cleaved to 18cm long, deep to peritoneum, and a lot of exudate leak out of the nick. We applied nutrition treatment to this two patients, and then controlled the water sample diarrhea after 2~3days, stoped TN 13 days later. After a month, the exudate out of the nick reduced, granulation grew, the weight of the patient increased. Three monthes later the nick healed.
4.Progress on relationship between occurrence, development of colon cancer and molecular mechanism of miRNA

miRNA ( microRNA) is a small molecule, which is composed of about 20 nucleotides, and has been a hot research topic in recent years.Many studies indicated that the expression level of miRNA in tumor changed, which played a role in the differentiation and proliferation of tumor cells, as a function of oncogene or tumor suppressor gene.The occurrence and development of colon cancer is related to the abnormal expression of many genes, and the data shows that the abnormal expression of miRNA is closely related to the development of colon cancer.In this paper, the influence of miRNAs on the development of colon cancer and its relevant mechanism were collected, then the colon cancer occurrence, development and miRNA molecular mechanisms were studied, which provided a theoretical basis for the molecular target treatment of colon cancer.
5.Analysis of diarrhea in 68 patients subsequent to orthotopic liver transplantation

Objective To investigate the factors in association with colorectal disorders in adult recipients of liver allograft. Methods A retrospective cohort study was carried out with clinical, microbiological and management data regarding diarrhea in 218 adult recipients of liver allograft from Jan. 2003 to Dec. 2006. Results Of the 218 patients, 68(31.2%) of them, who did not history of ulcerative colitis, were found to have diarrhea after orthotopic liver transplantation. Among these 68 patients, diarrhea was probably caused by administration of immunosuppressive agents in 35 cases (51.4%), in 12 cases (17.6%) diarrhea was antibiotics associated, and in 5 cases (7.4%) it was due to intraperitoneal infection. The other pathogenic factors included fungal infection in 3 cases (4.4%), cytomegalovirus infection in 3 cases (4.4%), Roux-en-Y choledochojejunostomy in 8 cases (11.8%) and some other unknown problem related in 2 cases (2.9%). Diarrhea occurred soon after transplantation in most cases. Of the 68 patients, the symptom of diarrhea occurred in the first 2 months in 45 cases (66.2%), and in the next 4 months in 23 cases (33.8%), i.e. the symptoms occurred 5-180 days after transplantation. Conclusion Immunosuppressive agents, antibiotics, fungal infection and cytomegalovirus infection are the top four common causes of diarrhea after orthotopic liver transplantation. The outcome is good with appropriate conservative management.
6.Fixation of fractures of the proximal humerus in elderly patients with the bone cement add plate

[Objective]To introduce the technique important points of intramedullary fixation with bone cement add the plate fixation for treated the unstable proximal humeral fractures in elderly patients,and proceed the analysis to the principle of bone cement fix.[Method]Ten cases of the unstable proximal humeral fractures were treated with open reduction,intramedullary fixation with bone cement first,then fix with the plate.Three days after operation functional trainings were began.The radiographic outcome and shoulder joint functions were observed postoperatively at 1 month,3 months,6 months and 12 months.[Result]The results were excellent in 5 cases,good in 4 cases,fair in 1 case according to the JOA function standard.The rate of excellent and good clinical results was 90%.Radiography showed that the fractures dissects to reply in 7 patients,fractures still had 3~5 mm moving in 3 cases.All plates were fixed dependable,with no loosening.All fractures had bone fusion.[Conclusion]The intramedullary fixation with bone cement add fixation with plate for the unstable proximal humeral fractures in the elderly can proceed the functional training of the earlier period,acquire the satisfactory function instauration.
7.Comparison of anterior knee pain after total knee arthroplasty using Genesis Ⅱ and PFC knee prostheses

Objective To explore relationship between anterior knee pain after total knee arthroplasty and patella resurfacing and prosthesis design,and to evaluate which femoral prosthesis is more friendly with patella:Genesis Ⅱ or PFC knee prostheses.Methods Data of 145 patients (145 knees) who had undergone primary total knee arthroplasty for treating osteoarthritis were retrospectively analyzed.Posterior-cruciate-substituting total knee prostheses were used in all patients.Among 74 patients who had undergone patella resurfacing,Genesis Ⅱ knee prosthesis was used in 32 patients and PFC knee prosthesis in 42 patients.Among 71 patients who had not undergone patella resurfacing,Genesis Ⅱ knee prosthesis was used in 38 patients and PFC knee prosthesis in 33 patients.The Hospital for Special Surgery(HSS) score,patella score,patellar function score,range of motion of knee,anterior knee pain scale,and patient satisfaction were used to evaluate clinical outcomes.At the same time,the imaging results were evaluated by X-rays.Results 144 patients were successfully followed up for 21 to 43 months (average,33 months).There was no significant difference in incidence of anterior knee pain between the group with patella resurfacing and the group without patella resurfacing,while a significant difference was found between patients receiving Genesis Ⅱ knee prosthesis and those receiving PFC knee prosthesis.Four patients who had undergone total knee arthroplasty using PFC prosthesis underwent reoperation.There were no significant differences in postoperative HSS score,range of motion of knee and patient satisfaction between the Genesis Ⅱ group and the PFC group,while there were significant differences in patella score and patellar function score between them.Conclusion Postoperative anterior knee pain was related to the prosthesis design,rather than to the patella resurfacing.Genesis Ⅱ knee prosthesis was more friendly with patella than PFC knee prosthesis.
8.Tissue-engineered bone fabricated with scaffold of demineralized tubular bone matrix

Objective: To investigate the method of using tubular demineralized tubular bone matrix scaffold composited with marrow stromal osteoblasts to fabricate tissue-engineered bone in vivo. Methods: Marrow stromal cells were harvested from illiac bone of 5 New Zealand rabbits respectively. After being cultured and multiplied in vitro, marrow stromal cells were induced to differentiate to osteoblasts with dexamethasone. The induced cells were harvested and mixed with 1.5% alginate sodium solution to generate a cell suspension. The suspension was dropped into tubular demineralized bone (1~1.5 cm in length, 0.2~0.5 cm in diameter) and then gelled with 2.5% CaCl 2. The demineralized tubular bone matrix/osteoblasts composites were placed in the dorsum of the rabbit subcutaneously as autogenous cells transplantation. Samples were investigated 6 and 12 weeks after implantation with histological and roentgenographic examination. Results: Osteoid tissue and new bone were observed in 4 weeks. In 8 weeks, more new bone formation, cells lied in the lacunae and arranged in order were found.Conclusion: Demineralized tubular bone matrix can be used as scaffold for the marrow stromal osteoblasts and alginate which was employed as three dimension carrier for cell growing and producing extral cell matrix.
9.Analysis of gene expression profiles in Tca8113/CBP nude mice by microarray technique

Objective:To study the differences of gene expression between Tca8113 and Tca8113/CBP tissues in nude mice. Methods:Tca8113 cells were injected subcutaneously in both sides of armpits of nude mice at the concentration of 5?106 cells/0.1 ml. Two weeks after injection, Carboplatin was used subcutaneously around the tumor 0.01 mg/g (weight) each day in Tca8113/CBP group while Tca8113 group was injected with physiologic saline as control. Mice were sacrificed 10 weeks after drug injection. The two kinds of tissues were investigated by human 16k cDNA v2.1 SBC-R-HC-100-21 gene chip. Results:Among the 16 000 target genes, there were 719 genes whose expression levels showed differences between the two kinds of tumor tissues. Conclusion:Microarray technique can simultaneously screen different genes from above-mentioned two kinds of tissues. Further analysis of the obtained genes will be helpful to understand the molecular mechanism of multidrug resistance.
10.Optimization of the Formula and Preparation Technology of Demethoxycurcumin Nano-structured Lipid Carriers

Objective:To screen the optimal formula and technology of demethoxycurcumin nano-structured lipid carriers. Meth-ods:The encapsulation efficiency as the main investigation index, the single factor exploration and orthogonal design were used to study the main factors affecting the quality of the nanoparticles. The optimal formula and technology were obtained. Results:The optimized parameters were as follows:the ratio of drug to lipid materials was 1 ∶40, the ratio of liquid lipid to total lipid materials was 10%, the amount of surfactants was 4% and the amount of lecithin was 2%. The prepared nanoparticles were spheric and regular. The size dis-tribution of the nanoparticles was narrow with the average particle size of 110nm and PDI of 0. 199. Conclusion:The optimized formula and technology of demethoxycurcumin nano-structured lipid carriers are stable and practicable,which provide reference for the further research of demethoxycurcumin.
