1.Diagnosis value of combined detection of CA 199 ,CA242 and DKK1 in pancreatic cancer

Objective To investigate the clinical values of CA199 ,CA242 and DKK1 in diagnosing pancreatic cancer (PCA). Methods The serum levels of CA199 ,CA242 and DKK1 were measured with ECLIA and ELISA method in 112 patients with PCA ,58 patients with benign pancreatic disease and 62 individuals undergoing physical examination.The Kruskal-Wallis single fac-tor analysis of variance (ANOVA) and Mann-Whitney rank-sum test were adopted to conduct the statistical comparison.The ROC curve was drawn by using the Logistic regression model and the area under curve (AUCROC ) was calculated.Results The levels of serum CA199 ,CA242 and DKK1 in the patients with pancreatic cancer were significantly higher than those in the benign pancreatic disease group and healthy normal control group ,the difference was statistically significant ( H=76.30 ,61.2 ,47.60 ,P<0.05);the sensitivities of CA199 ,CA242 and DKK1 for diagnosing pancreatic cancer were 76.7% ,69.6% and 85.7% respectively ;the speci-ficities were 95.0% ,96.7% and 92.5% respectively ;the accuracies were 86.2% ,83.6% and 89.2% respectively.The sensitivity , specificity and accuracy of combined detection of CA 199 ,CA242 and DKK1 by the Logistic regression equation P= 1/[1 +e-(-4.163+0.21X1+0.156X2+0.342X3) ] were 94.4% ,90.8% and 92.2% respectively ;AUC of CA199 ,CA242 ,DKK1 and pre-1 were 0.736 , 0.862 ,0.886 and 0.949 ,respectively.Conclusion Serum CA199 ,CA242 and DKK1 levels have an important value in the diagnosis of pancreatic cancer ,and their combined detection could significantly improve the sensitivity and accuracy of pancreatic cancer diag-nosis.
2.GAS/CCK-2R and function of its antagonists in treatment of the pancreatic cancer

Studies in recent years revel that there is a close relationship between gastrin/cholecystokinin-2 receptors (GAS/CCK-2R) and pancreatic cancer. CCK-2R is widely distributed in human body, but not in normal exocrine pancreas. However, CCK-2R is abnormally expressed in pancreatic carcinoma. The abnormal expression of CCK-2R in pancreatic tissues leads to the abnormal protein expression and activation, changes in acinar cells' morphology and phenotype and an increase in the sensitivity to carcinogenic substances. And some GAS/CCK-2R antagonists can be used for the early and late treatment of pancreatic cancer.
3.Cancer stem cells and drug resistance

Drug resistance is the most important obstacle in the treatment of cancer.People can know more about the mechanisms of drug resistance by study of cancer stem cells,and then identify new therapeutic targets and to develope better anticancer strategies.If we regard cancer stem cells as the therapeutic targets we can selectively kill cancer stem cells without harming normal ones,so overcome the drug resistance and avoid cancer relapse or metastasis.
4.Risk factors of recurrent ischemic stroke in elderly patients

Five hundred elderly patients with ischemic stroke admitted from January 2008 to December 2010 were enrolled in the study,including 352 recurrent cases and 148 primary cases.There was a positive correlation of stroke recurrence with age,blood pressure,blood glucose and lipid levels,smoking and alcohol consumption of patients(P <0.05 ); while the stroke recurrence was negatively correlated with physical exercise(P < 0.05).Stepwise multiple regression analysis indicated that the blood pressure,blood glucose and lipid levels were risk factors for the recurrence of ischemic stroke in elderly patients.

Objective In order to evaluate the present status of Radiation Oncology in China,the Chinese Society of Radiation Oncology performed the 6th continent prefecture of China e survey.Methods Questionnaire forms were sent by mail,e-mail,telephone and FAX.Results By August 31 st,2011,there were 1162 radiation oncology centers.There was a total of 30985 employees,including 9895 doctors,1887 physicists,11689 nurses,6103 technicians and 1411 engineers.There were 1296 linear accelerators,286 telecobalt units,81 deep x-ray machine,1040 simulators,376 CT simulators,317 brachytherapy units,1427 treatment planning system,1041 dosimeters,410 X-knife,and 230 γ-knife (122 for head only,108 for head and body).There were 56847 beds for inpatients (4 centers did not report the number of beds).More than 58000 patients were treated per day ( no report from 9 centers ),and 569056 new patients were treated annually (no report from 38 centers).Conclusions Radiation oncology was developed rapidly in the last 5 years,either in personnel,equipment,and new techniques.They are still insufficient in proportion to our population.
6.Prognostic significance of lactate and lactate clearance to critical illness

Objective To investigate the prognostic significance of serum lactate level and lactate clearance rate for critical illness patients.Methods Two hundred and eighty-six patients with hyperlactacidemia were investigated by analyzing the clinical data,laboratory data and outcomes.Comparison of mortality rate and APACHE Ⅱ score between different stratified levels of serum lactate was carried out.The blood pH,HCO3-,BE,and Lac were compared between survivors and non-survivors in terms of in-hospital death in seven days after admission.The above variables of blood gas analysis were studied in patients with severe hyperlactacidemia as well as the different lactate clearance rates and APACHE Ⅱ scores were compared between survivors and non-survivors.The mortality rates and APACHE Ⅱ scores were compared between high and low lactate-clearance rate groups.Results The mortality rates of different stratified levels of serum lactate (≥2,<4 mmol/L; ≥4,< 10mmol/L; ≥ 10 mmoL/L) were 14.04%,46.67%,78.79%,respectively.As the serum level of lactate increased,the decompensation rate of pH,APACHE Ⅱ score and mortality rate increased consequently.Compared with non-survivors,survivors had a higher lactate clearance rate (P < 0.01),and lower APACHE Ⅱ score (P < 0.01).The high-clearance group had lower mortality rate and 6-hour APACHE Ⅱ score compared with the low-clearance group (P < 0.01),but the initiate levels of serum lactate and APACHE Ⅱ scores were not noticeably different between the two groups (P > 0.05).Serum lactate level had a significant positive relationship with APACHE Ⅱ score (r =0.868,P < 0.01),but lactate clearance rate had a significant reverse relationship with APACHE Ⅱ score (r =-0.823,P < 0.01).Conclusions Both serum lactate levels and early lactate clearance rate had high prognostic value for critical illness patients,and in combination with changes in APACHE Ⅱ score,they could guide clinical treatment and give precise evaluation of the prognosis.
8.The clinical application of minimally invasive guider with CT-gulded in percutaneous iliosacral screw fixation

ObjectiveTo investigate the accuracy of percutaneous iliosacral screw fixation in sacroiliac joint fracture-dislocation with minimally invasive guider with CT-guided.MethodsFrom January 2011 to May 2011,8 patients with sacroiliac joint fracture-dislocation were treated using percutaneous iliosacral screw fixation assist with minimally invasive guider in CT-guided,which included 5 males and 3 females,with the average age of 32 years (ranged from 26 to 56 years).All patients suffered with vertically unstable pelvic fractures.Bone traction was used in femoral condyle for 6 cases which displaced more than 2cm in sacroiliac joints.Patients were prone position on the CT bed.First sacroiliac joint CT-scan was performed,then marked the needle position on affected side buttocks after measure the best position and track of needle that expected on CT computer screen.According to the data of CT-scan,the angle of the devices was adjusted,which could control 3D direction of the minimally invasive guider,then insert the Kirschner wire into sacroiliac joint guided with the front end of the sleeve of the minimally invasive guider,confirmed the track of needle was excellent with CT-scan (3D),then the guider was moved out and put the hollow screw (dia 7.3 mm Synthes) into the sacroiliac joint along the Kirschner wire.ResultsAll 8 patients were successfully insert the Kirschner wire.The operation time was from 10 to 20 min(mean,14 min).All screws were in the position expected before surgery,no cut out of bone with the CT-scan immediately after operation,the shape of sacroiliac joint was restored satisfied and the fixation was stable.No numbness and radiation-like pain appeared among the operation in the lower limb for all patients,no case had vascular and neurological complications postoperative.ConclusionThe minimally invasive guider can improve the accuracy,security and simplicity of the percutaneous iliosacral screw fixation with CT-guided,and also avoid the deviation of needle insertion angle that determined by operator himself.
9.Establishment and further improvement of D-galactosamine induced acute hepatic failure in Duroc breeding pigs

Objective To establish and improve the acute hepatic failure model in pigs induced with D-galactosamine (D-gal),and explore the feasibility of evaluating preclinical artificial liver devices.Methods Nineteen Duroc breeding pigs were divided into 4 groups.Fifteen unanesthetic Duroc breeding pigs out of 19 (5 of each group) received intravenously administration of D-gal at a dose of 1.0,1.25 and 1.5 g/kg body weight,respectively.The remaining 4 pigs which received the same volume of 5% dextrose in water served as controls. Clinical data and survival time of pigs were recorded.Blood samples were collected for dynamic testing of plasma ammonia,prothrombin time,liver and renal functions,blood glucose and L-lactate;liver tissues were sampled for pathological examination.The differences between groups were compared using t test and F test.The survival time of pigs was compared by Kaplan-Meier survival analysis and Log Rank test.Results Twelve hours after administration of D-gal,all pigs presented as acute hepatic failure characterized by progressive increases of levels of plasma ammonia,aspartate aminotransferase (AST),total bilirubin (TBil) and L-lactate,the level of blood glucose marked decreased and prothrombin time prolonged (F= 32.33,F=27.817,F=50.097,F=88.382,F=8.211,F=21.227;all P<0.01);especially in the pigs which received D-gal at a dose of 1.5 g/kg.Except 2 pigs survived for 168 h,the other 3 pigs which received D-gal at a dose of 1.0 g/kg died within 68-84 h,while all pigs which received D-gal at a dose of 1.25 and 1.5 g/kg died within 33-89 h and 23-47 h,respectively.All pigs presented coma before death and liver histopathological examination indicated massive hepatic necrosis with severe hemorrhage.Conclusions D-gal induced acute hepatic failure model in unanesthetic Duroc breeding pig appears potential reversibility and high reproducibility,which has proper therapeutic window.Thus,this model could be applied to evaluate the therapeutic effects and safety of artificial liver devices.
10.The optimal visualization of brachial plexus with utilization of nerve-sheath signal increased with inked rest-tissue rapid acquisition of relaxation imaging MR neurography and initial dignostic application

Objective To assess the optimal visiualization capacity of brachial plexus with three-dimensional nerve-sheath signal increased with inked rest-tissue rapid acquisition of relaxation imaging (3D SHINKEI), exploring the feasibility of preliminary diagnostic value on brachial plexus diseases. Methods MRI scans were performed on 24 healthy volunteers with no history of brachial plexus injury, and 46 patients whose outcomes of lesions had been verified as post-ganglionic brachial plexus injuries by surgery or clinical follows-up . The scan series consist 3D SHINKEI, STIR in the coronal plane as well as DW-MRN in the axial plane using a 3.0 T MR system. The source and post-processed images of 3D SHINKEI and DW-MRN were scored according to the optimal visibility on brachial plexus, in the meanwhile, contrast-to-noise ratio of the original images in the 3D SHINKEI and STIR sequences were calculated separately. Two radiologists blindly compared the detection rate of positive brachial plexus injuries between 3D SHINKEI and STIR in 46 patients. And then analyze the outcomes by means of Kappa test, Mann-Whitney test , independent sample t test, and Chi-square test. Results Post-ganglionic brachial plexus showed high intensity in the 3D SHINKEI sequence. In the 24 healthy volunteers, the scores by the two radiologists were 3.6 ± 0.6, 3.5 ± 0.6, 3.0 ± 0.2, 2.9 ± 0.1, respectively. There was statistical difference between the two sequences (Z=2.667,P=0.008,P<0.05). And the Kappa was 0.8 and 0.6 with favorable consistency. The CNR of 3D SHINKEI and STIR were 0.61 ± 0.07, 0.42 ± 0.03 (t=12.78, P=0.001, P<0.05). The positive detection rates of post-ganglionic brachial plexus injuries on 3D SKINKEI and STIR were, 78.3%, 52.2%(χ2=9.421, P<0.05). Conclusions 3D SHINKEI sequence demonstrates robust visibility consistently and can clearly display the structures and signals of post-ganglionic abnormality, compared with DW-MRN and STIR. This technique can be helpful to provide more complementary information to further confirm the diagnosis of brachial plexus injuries.
