Endocrine therapy is the most important systemic therapy for hormone receptor positive breast cancer.However,intrinsic or acquired resistance to endocrine therapy is a challenge currently.Crosstalk between estrogen receptor pathway and other pathway is considered to be one of the underlying mechanisms.Progress has been made in reversing resistance by blocking signaling through pathways such as the phosphoinositide 3-kinasesprotein kinase B-mammalian target of rapamycin and epidermal growth factor receptor pathway.

Objective To analyze the relationship between bladder trabeculation degrees and the urodynamic findings in patients with benign prostatic hyperplasia (BPH). Methods A total of 358 patients with BPH were included.Based upon the urethrocystoscopy, the bladder trabeculation degree was divided into 4 grades:Grade 0-no bladder trabeculation;Grade 1-mild to moderate;Grade 2-severe;Grade 3-severe with formation of pseudo-diverticula.We analyzed the relationship between the bladder trabeculation found by fiber urethrocystoscopy and the urodynamic findings such as maximum flow rate (Qmax),P Qmax.Watte factor,compliance and residual urine volume.For statistical analysis AVOVA (analysis of variance) and Spearman correlation were used. Results In the 358 cases,the bladder trabeculation degrees were as follows:Grade 0,21 cases;Grade 1,228 cases;Grade 2,82 cases and Grade 3,27 cases.A significant correlation was found between bladder trabeculation and the urodynamic findings.As the degree of bladder trabeculation increased,Qmax decreased (Grades 0 to 3:11.50?1.13,10.28? 2.29,9.80?0.98,7.36?1.37 ml/s,respectively;F=22.43,P

Objective To compare HASTE-MRCP single-slice acquisition technique with ordinary MR cholangiopancreatography (OMRCP) and to assess the clinical application value of HASTE-MRCP single-slice acquisition technique.Methods 48 cases with biliary obstructive diseases and 30 cases without obstructive jaundice were studied with a 3. 0T super conductive unit(Magnetom Trio;Siemens,Germany) that used a body coil. Two acquisition techniques of MRCP were used for all patients . The accuracy of the two kinds of technique were compared. Results OMRCP , single-slice HASTE-MRCP had the same sensitivity and specificity in showing the site and feature of the diseases , the proximal dilated biliarys and distant normal cholangiopancreatic ducts .The diameter of dilated bile ducts at the same level was compared and no significant difference in them. Single-slice HASTE-MRCP had a high spatial resolution ,and shot aquisition time ,but more vessel artifact. OMRCP had high density resoultion ,but more motion artifacts.Conclusion The single-slice acquisition of HASTE-MRCP is an excellent technique in the diagnosis of biliary tract diseases.

Objective To study the influence of applied times of Batroxobin on neuroprotective effects in cerebral ischemic-reperfusion(IR) injury gerbils.Methods 45 gerbils were randomly divided into five groups:normal group;cerebral IR injury model group(IR group);three times group;five times group;seven times group.The later three groups were administered Batroxobin 8 U/kg by abdominal injection q.o.d with certain times respectively.The normal group and IR group were not given drug but isometric physiological saline.Flow cytometer was used to measure apoptotic neurons of the hippocampal CA1,meanwhile electronic microscope was used to detect the ultrastrctural change of the hippocampal CA1 region.Result The quantity of apoptotic neurons in the every Batroxobin groups were significantly less than those in the IR groups(all P0.05).The ultrastrctural changes in five times group were lighter than those in three times group,but no significant difference was found between five times group and seven times group.Conclusion Three times,five times and seven times of Batroxobin can reduce the number of apoptotic neurons after IR injury.However,applied five times and seven times of Batroxobin has stronger neuroprotective effect than three times.

Objective To study the effect of postoperative anticoagulant therapy after splenectomy in patients with cirrhosis and portal hypertension.Methods One hundred and forty patients with cirrhosis and portal hypertension receiving splenectomy and periesophagastric devascularization were divided into anticoagulant group (76 cases) and control group (64 cases) by random number table,patients in anticoagulant group received postoperative anticoagulant therapy,principally according to the platelet count,gave ligustrazine,aspirin,low molecular heparin after operation; patients in control group without postoperative anticoagulant therapy.Postoperative monitoring platelet count and D-dimer,ultrasound or CT check the presence of portal vein thrombosis.Results Platelet count,D-dimer levels in anticoagulant group and control group in 2 days after operation were significantly increased,the difference was significant compared with preoperative [anticoagulant group:(95.73 ± 28.06) × 109/L vs.(38.41 ± 11.96) × 109/L,(3.61 ± 0.18) mg/L vs.(0.42 ± 0.09) mg/L;control group:(92.56 ± 27.75) × 109/L vs.(35.13 ± 11.38) × 109/L,(3.26 ± 0.16) mg/L vs.(0.37 ± 0.09) mg/L,P ＜ 0.05].Platelet count and D-dimer levels between two groups at preoperative and postoperative in 2 days had no statistical significance (P ＞ 0.05).Ten cases of control group occurred postoperative portal vein thrombosis,anticoagulant group were 3 cases,portal vein thrombosis incidence of anticoagulant group [3.95％ (3/76)] compared with control group [15.62％(10/64)] was statistically significant (P ＜ 0.05).Conclusion Postoperative anticoagulant therapy after splenectomy in patients with cirrhosis and portal hypertension is an effective method to prevent portal vein thrombosis.

Objective To introduce the amino acid substitution for HP V16E749-57(HLA-A2-restricted CTL epitope) and to identify the novel epitopes.Me thods Quantitative method was used to evaluate the affinity of the substituted peptides.To determine the peptide candidates to be synthesized and identified,the molecular models of the HLA-A2-peptide complex and CTL epitope candidates b ound to the HLA-A2 molecule were established by computer molecular modeling.Pep tides were synthesized and purified with standard Fmoc assay,lactate dehydrogen ase (LDH) release assay was used to determine their abilities of inducing the ge neration of specific CTLs.Results Modified peptides met the requirements of H LA-A2-restricted CTL epitopes.Peptide RLHYNIVTF had the abilitiy of inducing th e generation of specific CTLs.Conclusions Compared with HPV17E749-57 the mod ified peptide RLHYNIVTF has a higher antigenicity and affinity to HLA-A2.So,pe ptide RLHYNIVTF may be used as one of the HLA-A2-restricted candidate epitopes,instead of HPV17E749-57,for peptide vaccine in the treatment of HPV infection.

Objective To investigate current status of schistosomiasis control system at the county level and make recommendations to improve the infrastructure adapting to the prevalent condition of the disease. Methods Through pertinently sampling and semi-quantitative questionnaire among the professional workers, experts and administrators in schistosomiasis control, the primary weaknesses involved in the management system and human resources were analyzed. Results 380 questionnaires were distributed, 33 out of 40 were returned from experts/administrators and 285 out of 340 were received from the professionals. In general, the participants acknowledged the effectiveness of the mechanism on anti-schistosomiasis campaign during the past 40 years. It was found that lack of funds for schistosomiasis control and the poor administrative model of anti-schistosomiasis activities were the main barriers to the control. About half of the participants agreed that the policy about merging the county level anti-schistosomiasis facilities into the Center for Disease Prevention and Control (CDPC) system. The study also showed an inbalanced distribution of the manpower for schistosomiasis control across the endemic regions. The dropout rate of human resources was high due to the poor management and welfare. Conclusion Reforms on the management model should be performed and multiform compensation system should be built in accordance with the prevalence and trends of schistosomiasis.

OBJECTIVE To evaluate the relationship between the amount of interleukin-8(IL-8) and granulocyte colony-stimulating factor(G-CSF) and Helicobacter pylori(Hp) infection in gastric mucosa.METHODS Enzyme linked immunosorbent assay(ELISA) was used to detect the amount of IL-8 and G-CSF in 56 patients including normal gastric mucosa(n=18) and H.pylori infection patients(n=38) who were performed with gastroscope with their supernate fluid of gastrostoma mucosa tissue culture,and to examine the difference between Hp infected cases and non-Hp infected ones and H.pylori infection patients before and after treatment.RESULTS The amount of IL-8 and GCSF in Hp infected cases was significantly higher than non-infected ones(P

Objective Identification of anatomic structures are essential for totally thoracoscopic anatomic pulmonary segmentectomies, however sometimes the procedure are difficulty.This study was to assess whether three-dimensional computed tomography angiography(3D-CTA) could contribute to the preoperative arrangement of thoracoscopic complex segmentectomy.Methods Between September 2012 and August 2014, 29 patients were performed thoracoscopic complex segmentectomies under the guidance of preoperative 3D-CTA.The segmentectomies pattern were based on the nodules' diameter, location,and pathology.The targeted vessels and bronchus were marked in preoperative simulated segmentectomies.Results Of the 29 cases, 9 right upper lobe segmentectomies, 13 left upper segmentectomies, and 7 bibasilar segmentectomies were achieved, among which 6 subsegmentectomies were also inclued.The mean lesion diameter, operative time and intraoperative blood loss were(1.35 ± 0.80) cm, (190.53 ± 50.83) min, and (26.90 ± 32.24) ml respectively.Under the guidance of preoperative 3D-CTA , 8(27.5％) nodules were detected accurately, moreover 2(6.9％) aberrant arteries and 1 (3.4％) aberrant bronchus were observed.According to the marked vessels and bronchus preoperatively, 27 (93.1％) arteries, 25 (86.2％)veins,and 29(100％) bronchus were identified and dissected in the operation.Three cases converted to unplanned segmentectomies.No serious complications or death occurred.Conclusion 3D-CTA is an effective tool to enhance security and efficiency in thoracoscopic complex anatomical segmentectomy.
