1.The laboratory and clinical diagnosis strategy in MYH9 disorders
Chinese Journal of Laboratory Medicine 2013;(5):475-478
Myosin heavy chain 9 (MYH9)disorders are a group of ihherited thrombocytopenias resulted from the mutation of MYH9 gene, including May-Hegglin anomaly,Epstein syndrome,Fechtner syndrome and Sebastian syndrome.MYH9 disorders are very often misdiagnosed as idiopathic thrombocytopenic purpura (ITP).For better understanding of MYH9 of clinical and laboratory and getting enough attention in clinical practice, this review will focus on the pathogenesis, clinical manifestations, laboratory examination and differential diagnosis.

Objective To investigate the clinical curative effect of dorsal nerve fasciocutaneous flap in the treatment of children fingertip defect. Methods A total of 20 children (26 fingers)with fingertip defect were chosen as the research objects, and all of them were given the dorsal nerve fasciocutaneous flap surgery within the same finger. The sensory recovery, color, shape and function of the fingers were ob-served.Results All the 26 fingers were survived, and the survival rate was 100%.There were 8 cases of ambient blood circulation crisis 1 to 2 days after the surgery which were eliminated with corresponding treatment.Postoperative follow-up results showed that the sensory re-covery of the skin flap was S2 ~S3 6 months after surgery, and the two-point discrimination (2-PD)was 8 ~14 mm;the sensory recovery of the skin flap was S3 ~S3 +(19 fingers of S3 and 7 fingers of S3 +)12 months after surgery, and the two-point discrimination (2-PD)was 6 ~10 mm;the sensory recovery of the skin flap was S3 +~S4 (20 fingers of S3 +and 6 fingers of S4)12 months after surgery, and the two-point discrimination (2-PD)was 5 ~8 mm.Up to then, the finger shape and color of patients was similar with normal finger, the skin graft donor site was smooth without scar contracture, the finger joint function recovered well, and the distal interphalangeal joint function were normal. Conclusion Treatment of dorsal nerve fasciocutaneous flap repair for children fingertip defect has good clinical effect and it is wor-thy of clinical application.

Objective To investigate the role and mechanism of signal transducer and activator of transcription 4 (STAT4) in homocysteine (Hcy)-induced proliferation, migration and senescence of human aorta smooth muscle cells (VSMCs).Methods After VSMCs were incubated with Hcy (0-0.50 mmol/L) for various time periods,a CCK-8 assay was performed to determine proliferation activity.Modified Boyden chamber system (Transwell apparatus) was used to examine migration ability.A SA-β-gal staining kit was used to determine senescence status.Western blotting was performed to examine the expressions of STAT4 and phosphorylated STAT4 (p-STAT4) proteins, and ELISA was performed to examine MCP-1 expression. Results Hcy (from 0.05 mmol/L) significantly increased the proliferation of VSMCs (P < 0.05).Hcy (from 0.10 mmol/L) significantly increased the migration of VSMCs (P < 0.05).Hcy promoted the senescence of VSMC in a concentration-dependent manner, and significantly increased VSMC senescence when the concentration of Hcy reached 0.10 mmol/L (P < 0.05).Hcy (from 0.10 mmol/L) treatment significantly increased the expression of p-STAT4 (P < 0.05) and MCP-1 (P < 0.05).Conclusions Hcy increases the proliferation, migration and senescence of VSMCs,probably by the activation of STAT4 pathway.

To obtain the aptamers with high affinity and specificity for cyclosporin A(CsA),a synthesized 78 nt single stranded DNA(ssDNA)random library containing 35 random sequences flanked by invariant primer was subjected to 1 1 rounds of selection against CsA by SELEX protocol.Magnetic beads were used for target immobilization and the biotin-streptavidin-horseradish peroxidase system was employed for determining the binding affinity between the aptamers and CsA. After ten rounds of selection and amplification, with an increasing affinity for each round, the selected aptamers were cloned, sequenced and analyzed for their primaryand secondary structures. The 19 aptamers were divided into five groups based on primary sequence homology.Hairpin loop is the main motif in the predicted secondary structure and is supposed to be the binding part of the aptamers to CsA.The CsA-specific aptamers will be useful for enzyme-linked assays or immunofluorescence asses of CsA.

The basic medicine teaching must meet the clinical application, while the clinical practice should be timely feedback to basic medical teaching, which is the embodiment of the idea in the translational medicine. The experimental teaching is an important part of modern pathology teaching. We actively improve the experiment course and deepen the basic concept of translational medicine in the medical education development by adopting such approaches as using the problem-based learning method(PBL) to promote the link between basic courses and clinical pathology, integrating and optimizing the teaching content to promote the knowledge mastery and conducting molecular pathology research to promote the implementation of translational medicine.

Objective:To investigate different incidences of recent complications after thoracoscopic lobectomy at different operation schedules in patients with early stage non-small cell lung cancer(NSCLC).Methods:A total of 730 patients with early stage NSCLC who underwent thoracoscopic lobectomies in the Department of Thoracic Surgery, Peking University People's Hospital from June 2013 to October 2017 were enrolled. Clinical data were collected retrospectively to calculate the incidence of postoperative complications. Meanwhile, the effects of different operation time periods(before 4pm or after 4pm, working day or weekend) on complications were compared, and the propensity score matching method was used to eliminate the confusion factors that may cause result bias. Univariate and multivariate logistic regression methods were used to analyze the independent risk factors of postoperative complications.Results:162 patients(22.2%) had complications within one month after thoracoscopic surgeries, and 53(7.3%) of them were major complications. After the propensity scores of the preoperative clinical factors were matched, there was no significant difference in the incidence of complications between weekdays and weekends(18.7% vs 23.9%, P=0.267). There was also no significant difference in the effect of complications between the group before 4pm and group after 4pm on surgery(22.1% vs 27.9%, P=0.337). Conclusion:The incidence of recent complications in patients with early stage NSCLC after thoracoscopic lobectomy was as acceptable as 22.2%. The choice of operation schedules did not affect the postoperative complications.
