1.Clinical Analysis of 83 Cases of Hyperosmolar Glucose in Prevention of Persistent Ectopic Pregnancy after Laparoscopic Salpingostomy

Objective: Medical therapy to prevent persistent ectopic pregnancy (PEP) after conservative laparoscopic operation was investigated. Methods:236 patients with ectopic pregnancy were divided into three groups according to the time of hospitalization. 83 cases were in group A, hyperosmolar glucose was injected into the fallopian tube lumen after laparoscopic salpingostomy. 81 cases were in group B, MTX was injected in the same way, 72 cases were in group C as control group. Serum β-HCG was tested for four times, before the operation, the first, third and a week after the operation respectively. Results:There were 6 patients of PEP in this study. One patient in group A, one patient in group B, the rest four patients in group C. The incidence of PEP was not statistically significant between group A and B( P>0. 05), while the difference was significant between group A and C ( P<0. 05). Serum β-HCG in three groups descended significantly in the first 24 hours after operation, but there were no differences among these groups. As for the descending of β-HCG in day 3 and 7, there was no difference between group A and B, but the difference was magnificent between group A and C( P < 0.05). Conclusions: Compared with the MTX, hyperosmolar glucose is an effective and safer method to prevent PEP.

3.Real temperature of the continuous-wave pluggers

Objective:To measure the real temperatures on the pluggers of three continuous-wave de-vices, and to provide theoretical reference to evaluate thermal damage and heat ’s influence on the filling materials.Methods:The dual channel K type thermocouple was contacted to various sizes ’ pluggers in three different continuous-wave devices (BeeFill, Elements, B&L), and the highest temperatures at dif-ferent points ( tip, and 2 mm, 5 mm, 10 mm from the tip) of the pluggers ( preset temperature was 200℃) were recorded.The measurements were performed 5 times.T-test was used to compare the real tem-peratures at the tips with that set on the display and one-way ANOVA was used to compare the tempera-tures of the pluggers in different devices , sizes and points .Results:The highest temperature was at the tip of BeeFill 40/0.03 plugger (198.7 ±7.7) ℃, but there was on statistical differences between that and the preset temperature 200℃.The temperatures of the remaining pluggers were obviously lower than 200 ℃(P<0.05).The lowest temperature of the pluggers was detected at 10 mm from the tip of BeeFill 60/0.06 plugger (69.9 ±4.0) ℃.The highest temperature of each plugger was detected at the tip or 2 mm from the tip (112.1 to 198.7℃,and the median was 140.8℃).Conclusion:The real temperature of most continuous-wave pluggers included in this study is below the set temperature 200 ℃.

5.Risk factors for hydrocephalus after moderate to severe traumatic brain injury

Objective To investigate the risk factors of posttraumatic hydrocephalus (PTH) in patients with moderate to severe traumatic brain injury (TBI).Methods Aretrospective study was conducted for 183 patients with moderate to severe TBI (125 males,58 females;6-91 years of age,mean 48.23 years).According the presence of PTH,the patients were allocated into PTH group (n =34) and non-PTH group (n =149).Risk factors of PTh were assessed by univariate and logistic regression analysis,including gender,age,injury types,injury severity,intraventricular hemorrhage,subarachnoid hemorrhage,midline shift,subdural effusion,therapeutic strategies and skull defect.Association between the boundaries of skull defect and PTH was determined.Results Between-group differences were not significant regarding age,gender,injury types and intraventricular hemorrhage (P > 0.05),but differed significantly in injury severity,subarachnoid hemorrhage,midline shift,subdural effusion,craniectomy and skull defect (P < 0.05).Further Logistic regression analysis confirmed subarachnoid hemorrhage (OR =6.169),interhemispheric subdural effusion (OR =31.743),and unilateral (OR =17.602) and bilateral (OR =30.567) skull defects were risk factors of PTH.Of the patients with unilateral skull defect following decompressive craniectomy,the inferior limit ≤ 10 mm from the zygomatic arch also played a role in the development of PTH (OR =5.500,P < 0.05).Conclusions Subarachnoid hemorrhage,interhemispheric subdural effusion and skull defect are risk factors of PTH.Unilateral skull defects with the inferior limit too close to the zygomatic arch can predispose to the development of PTH.
6.Reflection on improving the quality of the standardized training of resident in orthopaedics rotary training

We carry out the tutorial system in the standardized training of residents,promote students to grasp the clinical skills and clinical research and thinking,and promote teachers' professional theoretical level and teaching ability.In the orthopedic rotation we define the teaching focus to the trainees of different seniority:for junior trainees,the training of medical records writing and basic clinical skills should be focused,and for senior trainees,the emphasis of training is to improve their clinical capacity of diagnosis and treatment.In teaching,the teaching methods of a combination of Multidisciplinary team (MDT) of bone and soft tissue tumors and problem based learning (PBL) have been actively tried to improve students' learning enthusiasm and initiative,which helps the trainees fully grasp the diagnosis and treatment of certain diseases.
7.Association between chronotropic incompetence and metabolic equivalents in type 2 diabetic mellitus patients complicated with metabolic syndrome

Objective To investigate the association between chronotropic incompetence (CI)and motor ability in type 2 diabetic mellitus (T2DM) patients complicated with metabolic syndrome (MS).Methods One hundred and twenty patients with T2DM were enrolled in the study,including 66 cases with MS (MS group) and 54 without MS (non-MS group).The electrocardiographic exercise test was performed,the CI indexes including ratio of heart rate (rHR),heart rate reserve (HRR)were measured and the max metabolic equivalents(maxMETs) were calculated.Result The rHR(0.77 ±0.04 vs.0.81 ±0.06,t =3.41,P < 0.05),HRR (0.62 ± 0.05 vs.0.70 ± 0.07,t =5.96,P < 0.05) and maxMETs (8.8 ± 1.1 vs.9.7 ± 1.2,t =0.16,P < 0.05) in patients of MS group all lower than those in non-MS group.The maxMETs were positively correlated with rHR (r =0.81,P < 0.05) and HRR (r =0.78,P < 0.05).Conclusion The results indicate that CI as well as maxMETs and rating of perceived exertion can be used as monitoring index in type 2 diabetic patients complicated with metabolic syndrome.
8.CT features of heterotopic pancreas and its correlation with pathologic changes

Objective To analyze the CT findings of heterotopic pancreas and the correlation with pathologic changes.Methods CT findings in 1 9 patients with heterotopic pancreas proven by surgery were reviewed retrospectively,14 of whom were scanned with enhanced CT.The correlation of CT findings and pathologic results were also analyzed.Results All patients had a single lesion with sized range from 0.8 to 4.7 cm and mean diameter of (2.2±0.6)cm.All lesions with homogeneous density were showed on plain scans including 7 lesions in stomach,8 in duodenum,2 in jejunum,1 in ileum and 1 beside the cardiac lymph nodes.The border of the lesions in 5 was unclear with exudative changes,mimicking acute pancreatitis.A sign of central umbilical concave was found in 2.Pathological results confirmed 14 lesions in submucosa,3 in muscular layer,1 in serosal layer and 1 in abdominal cavity.Hetero-topic pancreas presented early obvious enhancement in 9,mimicking normal pancreas,which was composed chiefly of pancreatic aci-ni.The enhanced degree of 4 lesions was lower than normal pancreas,which were composed of pancreas ductal epithelium and few pancreatic acini.No significantly enhancement was seen in 1,mimicing liomyoma,which was composed chiefly of pancreas ductal epithelium and smooth muscle hyperplasia.Conclusion Heterotopic pancreas in most cases presents characteristic submucosal disea-ses-like pattern,and different enhanced patterns of heterotopic pancreas are related with the pathologic changes.

10.Effects of different doses of Xuebijing injection in treatment of ischemia reperfusion injury in rabbit limbs

Objective To investigate effect and mechanism of Xuebijing administration at various concentrations on ischemia reperfusion injury (IRI) of rabbit limbs.Methods Thirty New Zealand rabbits were divided into control group (n =10),Xuebijing group Ⅰ (n =10) and Xuebijing group Ⅱ(n =10) according to random number table.Rabbit models of IRI in lower extremities were established.Each group received corresponding therapy after reperfusion:rabbits in Xuebijing group Ⅰ were firstly administered 4 ml/kg Xuebijing solution and 6 ml/kg isotonic saline; rabbits in Xuebijing group Ⅱ were administered 2 ml/kg Xuebijing solution and 8 ml/kg isotonic saline; rabbits in control group were simply administered 10 ml/kg isotonic saline.Venous blood samples were collected before reperfusion and at 1 h,2 h,4 h after reperfusion to measure coagulation parameters (APTT,Fib,INR and PT) and biochemical items (ALB,LDH and CK).Results APTT in Xuebijing group Ⅰ presented obvious improvement at 1 h and 4 h after reperfusion as compared with control group (P < 0.01).PT in Xuebijing groups Ⅰ and lⅡ was significantly longer after reperfusion than that before reperfusion (P <0.05).Fib level in Xuebijing group Ⅰ was much higher at 4 h after reperfusion than that before reperfusion (P < 0.05).ALB level at 1 hour after reperfusion showed no statistical differences from that before reperfusion in Xuebijing groups Ⅰ and Ⅱ (P > 0.05).LDH and CK levels in Xuebijing group Ⅰ were much lower than those in control group after reperfusion (P < 0.05).Conclusions Xuebijing injection relieves limb IRI,with better effect in Xuebijing group Ⅰ than in Xuebijing group Ⅱ.Therapeutic mechanism may be associated with its involvement in adjusting clotting function and mitigating injury of muscle tissues.
