1.Clinical effect of Ex - press glaucoma filtration device implantation under scleral flap for traumatic glaucoma

AIM: To observe the clinical effect of Ex - press glaucoma filtration device implantation under scleral flap for traumatic glaucoma.
METHODS: Totally 19 eyes of 19 patients with traumatic glaucoma were implanted with Ex - press glaucoma filtration device under the scleral flap from December 2012 to May 2013. The postoperative visual acuity, anterior chamber depth and hyphema,intraocular pressure(IOP), filtering bleb and other complications were observed and summarized.
RESULTS: Ex - press glaucoma filtration device were implanted successfully in 19 patients. At 1d postoperatively shallow anterior chamber was occurred in 2 eyes and hyphema was occurred in 3 eyes. At 3mo postoperatively increased vision was occurred in 9 eyes (47%),functional filtering bleb was occurred in 18 eyes ( 95%), and IOP was 12. 1 ± 6. 1mmHg. At 6mo postoperatively increased vision was occurred in 11 eyes (58%),functional filtering bleb was occurred in 18 eyes (95%),and IOP was 13. 3±6. 9mmHg. Compared with the preoperative IOP ( 29. 8 ± 6. 7mmHg), the difference was significant(P<0. 05).
CONCLUSION: Ex - press glaucoma filtration device implantationfor traumatic glaucoma can improve the postoperative vision and control the postoperative IOP stably with fewer complications and high safety.
2.Nrf2: a new target for nonalcoholic fatty liver disease

The liver is an important organ of the body, which has many functions, such as metabolism and detoxification. Due to the rapid change of lifestyle and the improvement of public health, the incidence rate of non-communicable diseases has increased significantly, which fundamentally changed the disease characteristics in most parts of the world. At present, the global prevalence of non-alcoholic fatty liver disease (NAFLD) is about 25%. Moreover, about 59.10% of NAFLD patients progress to non-alcoholic steatohepatitis (NASH) within 5 years, and about 41% of NASH patients progress to fibrosis. NAFLD has become one of the most important liver diseases in the world and may become the main cause of end-stage liver disease in the next few decades. In addition, NAFLD and related cirrhosis will bring huge economic burden to patients, health care system and society. Since there are currently no medications available that have been approved by Food and Drug Administration (FDA), NAFLD is still treated mainly through lifestyle changes such as exercise and diet. Oxidative stress and inflammation are the most important pathological processes in the occurrence and development of liver diseases. Nuclear factor erythroid-2-related factor 2 (Nrf2) is a key regulator of the body's antioxidant stress system, with anti-inflammatory, antioxidant and other functions. Many studies have shown that Nrf2 pathway significantly affects the progression of liver diseases. In this review, we aimed to summarize the regulatory role of the Kelch-like ECH-associating protein 1 (Keap1)-Nrf2-antioxidant response element (ARE) signaling pathway in the pathogenesis of NAFLD, and to reveal the potential of Nrf2 as a therapeutic target for NAFLD.
3.Application of e-learning based on internet for training of minimal invasive surgery

4.Clinical application of the reformed purse-string suturing and stitching technique in procedures for prolapses and hemorrhoids(PPH)

Objective To discuss advantages of the reformed technique of purse-string sutures in PPH for the treatment of severe hemorrhoids.Methods 172 cases with severe hemorrhoids received PPH with the reformed purse-string sutures.The reformed purse-string sutures were positioned right above the anocutaneous line by 2~3cm.The chorda chirurgicalis moved from upward to downward at a tilt in the submucosal layer,and the reformed pursestring stitches were made.Results The average operation time was 17minutes,the average width of the excised mucosal ring was 3.lcm,and the average hospitalization time was 2.6 days.The prolapsed hemorrhoids went back after surgery.No fecal incontinence,recurrence of prolapsed hemorrhoids or anastomotic stenosis occurred within 1~24 weeks fonow-up.Conclusion The reformed suturing technique of purse-string stitches is safe,effective and recommendable.
7.Application of logical teaching in training of 8-year medical students' clinical thoughts
