To explore the relationship between clinical findings and pathological changes of IgA nephropathy (IgAN), 37 children with IgAN were undertaken clinical classification and renal-pathological comparison including glomerulus change, renal tubule-interstitial change and immunopathology. The results showed that there were 18 (49%) cases with hematuria, 14 (38%) cases with nephrotic syndrome, 3(8%) cases with both of hematuria and proteineuria, and 2 (5%) cases with nephritic syndrome in the clinical classification. 54% of cases with glomerulus changes was as class III. No significant relationship was found between clinical classification and glomerulus changes. There were 24 cases with renal-tubule interstitial changes and 7 cases with hematuria. 43% of them were classified as class I and 57% as class II.All cases with nephrotic syn-drome developed renal tubule-interstitial change. 78% (11 cases) of them were as class II and 22% (3 cases) as class III. Besides, 2 of 3 cases with both of hematuria and proteineuria and 1 of 2 cases with nephritic syndrome also manifested renal tubule-interstitial change. There were four phenotypes were observed in immunopathology including 16 cases of them as IgA, 6 cases as IgAG, 10 cases as IgAM and 5 cases as IgAGM, respectively. 66% of cases with hematuria was found as IgA and 50% of cases with nephrotic syndrome as IgAM. It is concluded that hematuria can be a main clinical finding in both of IgAN and nephrotic syndrome. Glomerulus changes is usually not correlated with clinical classification. There is a significant renal tubule-interstitial change in cases with nephrotic syndrome. Hematuria is usually as IgA and nephrotic syndrome as IgAM in immunopathology.

Objective:To investigate the inhibitory effect of recombinant mutant human tumor necrosis factor(rmhTNF-?)combined with cisplatin(DDP)against transplanted Lewis lung carcinoma(LLC)in mice,and to explore the related mechanism.Methods:LLC cells were subcutaneously inoculated into C57BL/6 mice at the right axilla and the mice were randomly divided into 4 groups:sodium chloride(control group),DDP group,rmhTNF-? group,rmhTNF-? plus DDP group.On the 7th day after inoculation the diameter of the tumor reached 0.6 cm;the corresponding agents were intratumorally injected for 3 d.All mice were sacrificed 1 d later and the tumors were obtained,weighed,and the tumor inhibitory rate was calculated.Flow cytometric analysis was used to examine cells apoptosis and cell cycle.Expression of survivin mRNA was examined by RT-PCR.Results:(1)The tumor inhibitory rates of the DDP plus rmhTNF-? group(36.61%)was significantly higher than those of the DDP group(17.12%)and rmhTNF-? group(15.83%,P

Objective To analyze the clinical efficacy of preoperative concurrent radiotherapy and chemo-therapy and radical operation in the treatment of cervical cancer patients with locally advanced cervical cancer (IB -IIB).Methods 80 patients who were diagnosed locally advanced (stage IB -IIB)cervical cancer,were divided into observation group (preoperative radiotherapy and chemotherapy group)and control group (radical radio-therapy group).The short -term efficacy,complications and follow -up data were compared between the two groups. Results The short -term efficacy of the observation group was 87.2%,which of the control group was 95.1%,and there was no significant difference (χ2 =1.579,P =0.209).As to the early complications of the two groups,there were no significant differences in gastrointestinal tract reaction(χ2 =0.000,P =0.987),rectal reaction(χ2 =2.709, P =0.100),bladder reaction (χ2 =1.343,P =0.247)and bone marrow suppression(χ2 =1.898,P =0.168).Late complications:in the observation group,the radiation proctitis[4 cases(10.3%)vs.14 cases(34.1%)]and the radiation cystitis[3 cases(7.7%)vs.13 cases(31.7%)]were less than those of the control group,the differences were statistically significant(χ2 =6.542,P =0.011&χ2 =7.205,P =0.007).The average follow -up time was 38.6 months in the observation group,which was 44.8 months in the control group.There was no significant difference in survival rate between the two groups(χ2 =0.075,P =0.784 & χ2 =0.032,P =0.858).Conclusion Locally advanced cervical cancer (IB -IIB)patients received preoperative concurrent radiotherapy and chemotherapy has important clinical value.The treatment makes the local giant block tumor shrink significantly,which is conducive to the implementation of surgery,and reduce the incidence of late complications compared with radical radiotherapy.

Randomized control study of gemfibrozil and pantethine was made in 39 NIDDM patients to compare their effect on anti-lipid-peroxidation.The patients were divided into two groups at random. One group took gcmfibrozil 900 mg/d for 4 weeks, the ether group took pantethine 540 mg/d for 4 weeks. The results showed that there were no significant change in faslirg plasma glucose and HbA1 in both groups before and after treatment. Objective To discuss the techniques of tumor resection, pelvic reconstruction and postoperative management by reviewing the patients with malignant pelvic tumors in recent years. Methods 76 patients with primary malignant pelvic tumors were treated operatively between July 1997 and July 2003. The series comprised 47 males and 29 females. 31 cases were diagnosed as chondrosarcoma, 15 as Ewing sarcoma, 7 as osteosarcoma, 3 as lymphoma, 3 as malignant fibrohistiocytoma (MFH), 1 as hemangiopericytoma, 2 as myeloma, 13 as giant cell tumor(GCT). According to Enneking's division, the most common region of the primary pelvic tumor involved was region Ⅱ (51 cases), followed by region Ⅰ (16 cases) and region Ⅲ (9 cases). 16 cases were reconstructed with threaded pins and cement after resection of the ilium. 9 patients had local resection of pubis and ischium. 21 patients had hemipelvectomy. Among 30 patients with periacetabular tumors, 8 were reconstructed with hemipelvic prosthesis, 7 with saddle prosthesis, 6 with replantation of cauterized tumor bone and total hip arthroplasty, 9 with cemented total hip replacement after curettage of lesion. Results After 16 months to 6 years follow-up, among the 21 patients with hemipelvectomy, 4 patients had local relapse because the sacrum invaded by the tumors. 7 of 25 patients with lesions in regionⅠ or region Ⅲ had local relapse postoperatively due to the marginal incision to the acetabula. Among the 21 patients who had tumor resection and reconstruction in region Ⅱ, 4 had local relapse, 3 of which reconstructed with cauterized hemipelvis. As for functional recovery, all of the 25 patients with lesions in region Ⅰand region Ⅲcould walk postoperatively without assistance of a cane. 21 patients with acetabular reconstruction after resection of lesions in region Ⅱ could sit and stand normally and walked with a cane, some of them even had normal gait. Conclusion The major principle of treatment for primary pelvic tumor is that en bloc resection, only then the functional reconstruction of pelvic ring can be considered. Operators should be acquainted with advantages and shortcomings of different reconstructive methods of acetabulum to prevent the complications.

Objective: To establish an HPLC method for simultaneous determination of baicalein and wogonin in Baipuhuang tablets. Methods: HPLC was performed on an Agilent C 18 column(4.6 mm×250 mm, 5 μm)using the gradient acetoni trile(A)-water(B)at the 1.0 ml/min flow rate as mobile phase. The columm temperature was 30℃ and the detection wavelength was 280 nm. Results: The linear regression equation of baicalein was Y=24117X-223.95(r=0.9990);the linear range was 0.02~0.2 μg with the average recovery of 98.95% and the RSD of 0.64%(n=9). The linear regression equation of wogonin was Y=16237X-26.698(r=0.9996);the linear range was 0.02~0.2 μg with the average recovery of 99.28% and the RSD of 1.03%(n=9). The RSD values for the precision, stability and repeatability were all less than 2%. Conclusion: The established HPLC method appears to be highly sen sitive, accurate and reproducible, which could be used for the quality control of Baipuhuang tablets.

OBJECTIVETo observe the effect of total ginsenosides (TG) on monocrotaline (MCT) induced right ventricular hypertrophy rats, and to explore its correlation with calcineurin (CaN) pathway.

METHODSFifty male Sprague Dawley rats were randomly divided into the normal control group, the MCT model group, and the low, middle, high dose TG treatment groups, 10 in each group. All medication was performed by peritoneal injection for 18 days. Right ventricular peak systolic pressure (RVSP), right ventricular hypertrophy index (RVHI), and right ventricular weight/body weight (RVW/BW) were measured. Intracellular free calcium concentrations were measured by Ca2+ fluorescence indicator Fura2/AM. The atrial natriuretic factor (ANF) and CaN mRNA expression of the myocardial tissue were quantitatively analyzed by Real-time PCR. The protein expression of CaN was detected by Western blot.

RESULTSCompared with the MCT model group, preventive treatment of TG at the 3 doses could significantly reduce RVSP, RVHI, RVW/BW, and ANF mRNA expression, and decrease Ca2+ concentration in myocardial cells, CaN mRNA and protein expression in the myocardial tissue.

CONCLUSIONTG could obviously improve MCT-induced right ventricular hypertrophy, which was possibly achieved through suppressing MCT-activated CaN signal transduction.
