2.Characteristic finding of renal oncocytoma and clear-cell renal cell carcinoma with multiphase CT

ObjectiveTo evaluate characteristic imaging findings of tumor attenuation in 4-phase CT between renal oncocytoma ( RO ) and clear-cell renal ceil carcinoma ( ccRCC ) of small tumor size (≤Scm).Methods Fifty-six patients with histologically confirmed renal masses (11ROs and 45 ccRCCs) were included in this study.Heterogeneous enhancement was found all tumors during the corticomedullary phase (CMP).The CT values of the normal renal cortex,the relatively high enhanced region and the relatively less-enhanced region of the tumor were measured in each phase.Statistical comparison was carried out by Chi-square test or Mann-Whitney test.ResultsIn CMP,the CT value of the relatively high enhanced region in RO [ 163.0 HU ( 141.0—178.0 HU) ] was significantly lower than that in ccRCC [ 194.0 HU ( 166.5—235.0 HU) ; Z = -2.847,P =0.004].Compared CMP with the excretory phase,the attenuation of the relatively highly enhanced region in RO [70.0 HU (41.0—86.0 HU)] were significantly lower than that in ccRCC [87.0 HU (65.0—126.5 HU) ] (Z= -2.032,P =0.042).In the excretory phase,9 of 11 ROs had a further enhancement with its relatively less-enhanced region which was significantly higher than that in ccRCC (21/45 ; x2 = 4.391,P = 0.036).ConclusionsIn CMP,the CT value of the relatively high enhanced region in RO was significantly lower than that in ccRCC.Compared with ccRCC,in the excretory phase,the RO had less attenuation of the relatively highly enhanced region with homogeneous density.

Objective To test the accuracy of American College of Cardiology (ACC)/American Heart Association (AHA) guidelines for preoperative cardiovascular evaluation for noncardiac surgery in Chinese patients and to compare ACC/AHA guidelines with Goldman index and Lee index. Methods From January to December 2003, all patients aged ≥70 yr or patients aged 40-69 yr with a history of cardio- and cerebro-vascular disease, abnormal ECG or diabetes scheduled for noncardiovascular surgery were included in this study. A total of 1 248 patients were eligible. Their clinical data including demographic data, history of cardiovascular disease, routine physical examination and laboratory tests, the scheduled surgery and type of anesthesia were collected. The patients were then evaluated for cardiac risk and classified according to ACC/AHA guidelines (high, moderate, low and no risk), Lee index (class I -IV ) and Goldman index (class I - III ). The cardiac risk of the scheduled surgery was then stratified according to ACC/AHA guidelines. The patients were followed up until discharged from hospital. Cardiac events were defined as cardiac death, myocardial infarct, myocardial ischemia, minor myocardial cell injury, ventricular dysfunction and serious arrhythmia. Likelihood ratio of the 3 methods was calculated. Risk factors for adverse events were identified by univariate analysis and multivariate Logistic regression analysis. Results Of the 1 248 patients 694 were male and 554 female. Their age ranged from 40-102 years (mean age 65.4 yr). 44.7 % of the patients were aged ≥ 70 years. High risk operation accounted for 6.3 % and emergency operation 7.9% . One patients died of cardiac event and ten patients of other causes. Seventy-three perioperative cardiac events occurred in 53 patients. The morbidity rate was 4.2% . Objective To evaluate the right ventricular function using volumetric pulmonary artery catheter (VPAC) in patients undergoing off-pump coronary artery bypass grafting (CABG) .Methods Thirty-two patients (18 males, 14 females) aged 45-63 yrs weighing 58-74 kg undergoing CABG were studied. Their cardiac functions were graded according to NYHA classification as Ⅰ or Ⅱ. Radial artery was cannulated before induction of anesthesia for BP monitoring. Anesthesia was induced with midazolam 0.03-0.05 mg?kg-1, fentanyl 10-15 ?g?kg-1 and pipecuronium 0.1-0.15 mg?kg-1 and maintained with 1.0-1.5% isoflurane. The probe (7 MHz) of the transesophageal echocardiography (TEE, Sonos 2500, HP) was placed in esophagus after tracheal intubation for measurement of both right and left ventricular end-diastolic, end-systolic volume and ejection fraction (LVEDV, LVESV, LVEF, RVEDV, RVESV, RVEF). VPAC (type 774HF75, Edwards Life Science Co) was placed via right internal jugular vein for measurement of RVEDV, RVESV and RVEF. 6% HAES 10 ml?kg-1 was infused over 10 min. The cardiovascular parameters mentioned above were measured before and immediately after 6% HAES infusion using both TEE and VPAC, and compared.Results The RVEDV and RVESV (measured by TEE and VPAC) and LVEDV, LVESV (by TEE) were significantly increased after HAES infusion as compared with the baseline values (P

8.Study on quality standard for Puhuang Dispensing Granule

Objective: To study the quality standard for Puhuang Dispensing Granule (Pollen Typhae). Methods : Puhuang Dispensing Granule was identified by TLC and isorhamnetin-3-O-neohesperidoside and typhaneoside in Puhuang Dispensing Granule were determined by HPLC. Results : The linear relationship was at the range of 0.4~1.21?g for isorhamnetin-3-O-neohesperidoside and 0.42~1.25?g for typhaneoside, respectively. The average recovery was 98.88% for isorhamnetin-3-O-neohesperidoside and 99.68% for typhaneoside, respectively. Conclusion : The method is available with a good reproducibility and can control the quailty of Puhuang Dispensing Granule.
9.Evaluation of the detection of PML-RARα fusion gene in acute promyelocytic leukemia to monitor minimal residual disease

Objective To investigate the kinetics of PML-RARα fusion gene in acute promyelocytic leukemia(APL)to monitor minimal residual disease(MRD). Methods In induction therapy,consolidation and maintenance therapy courses, PML-RARα fusion gene was performed by RT-PCR. Results The long-term follow-up of 18 cases achieved complete remission (CR),two cases experienced molecular relapse. One case relapsed at 4 months after CR1 and achieved CR2 after induction therapy. However, molecular and hematology relapsed again at 2 months after CR2 and re-achieved CR3. The other case relapsed at 74 months after CR1 and achieved CR2 after induction treatment, who had survived for 106 months until the end of follow-up. Conclusion RT-PCR assay for detection of PML-RARα should be performed regularly during CR period so as to find molecular relapse eady. Hematological relapse could potentially be averted through treatment modification according to molecular monitoring results of PML-RARα.
10.OBSERVATION OF CURATIVE EFFECT IN THE NEAR FUTURE OF 124 CASES WITH MULTI DRUG RESISTANT PULMONARY TUBERCULOSIS

The curative effect on 124 cases with multi drug resistant pulmonary tuberculosis from January 1996 to June 1999 was reported.All the cases, with an average history of 5.84 years, were recurrent ones. They included 78 cases of infultative pulmonary tuberculosis,46 cases of chronic fibrocavitative pulmonary tuberculosis. Sputum of all the cases was cultured for Mycobacterium hominis tuberculosis with Kuang`s culture medium and drug susceptibility test was done. As a result, the drug resistant rate to 2 drugs, 3 drugs,4 drugs, 5 drugs and more were 20.16%,25.0%,20.9%,33.72% respectively.The authors added 1~2 kinds of anti TB drugs never used before in each case to KOP (Kanamycin, Ofloxacin, Sodium Aminosalicylate),with a mean 3.3 months treating course. The sputum negative conversion rate was 66.9%,foci absorption rate was 72.6%, cavity close up 50.0% . There wasn′t any serious adverse effect encounted. It suggest that KOP synergetic bacteriocidal to Mycobacterium tuberculosis either inside or outside cells, with fewer side effect, is a good regimen for multi drug tuberculosis treatment.
