ObjectiveTo evaluate characteristic imaging findings of tumor attenuation in 4-phase CT between renal oncocytoma ( RO ) and clear-cell renal ceil carcinoma ( ccRCC ) of small tumor size (≤Scm).Methods Fifty-six patients with histologically confirmed renal masses (11ROs and 45 ccRCCs) were included in this study.Heterogeneous enhancement was found all tumors during the corticomedullary phase (CMP).The CT values of the normal renal cortex,the relatively high enhanced region and the relatively less-enhanced region of the tumor were measured in each phase.Statistical comparison was carried out by Chi-square test or Mann-Whitney test.ResultsIn CMP,the CT value of the relatively high enhanced region in RO [ 163.0 HU ( 141.0—178.0 HU) ] was significantly lower than that in ccRCC [ 194.0 HU ( 166.5—235.0 HU) ; Z = -2.847,P =0.004].Compared CMP with the excretory phase,the attenuation of the relatively highly enhanced region in RO [70.0 HU (41.0—86.0 HU)] were significantly lower than that in ccRCC [87.0 HU (65.0—126.5 HU) ] (Z= -2.032,P =0.042).In the excretory phase,9 of 11 ROs had a further enhancement with its relatively less-enhanced region which was significantly higher than that in ccRCC (21/45 ; x2 = 4.391,P = 0.036).ConclusionsIn CMP,the CT value of the relatively high enhanced region in RO was significantly lower than that in ccRCC.Compared with ccRCC,in the excretory phase,the RO had less attenuation of the relatively highly enhanced region with homogeneous density.

The curative effect on 124 cases with multi drug resistant pulmonary tuberculosis from January 1996 to June 1999 was reported.All the cases, with an average history of 5.84 years, were recurrent ones. They included 78 cases of infultative pulmonary tuberculosis,46 cases of chronic fibrocavitative pulmonary tuberculosis. Sputum of all the cases was cultured for Mycobacterium hominis tuberculosis with Kuang`s culture medium and drug susceptibility test was done. As a result, the drug resistant rate to 2 drugs, 3 drugs,4 drugs, 5 drugs and more were 20.16%,25.0%,20.9%,33.72% respectively.The authors added 1～2 kinds of anti TB drugs never used before in each case to KOP (Kanamycin, Ofloxacin, Sodium Aminosalicylate),with a mean 3.3 months treating course. The sputum negative conversion rate was 66.9%,foci absorption rate was 72.6%, cavity close up 50.0% . There wasn′t any serious adverse effect encounted. It suggest that KOP synergetic bacteriocidal to Mycobacterium tuberculosis either inside or outside cells, with fewer side effect, is a good regimen for multi drug tuberculosis treatment.

In order to study the curative effects of L arginine (L Arg) on high altitude pulmonary edema (HAPE),we had treated 10 patients with HAPE by intravenous L Arg at 3700m, and the result was compared with 11 patients who inhaled low concentration nitric oxide (NO) and 24 subjects with routine treatment.The findings were as follows:compared with the routine group, inhaling NO or intravenous L Arg could hasten the disappearance of moist rales and X ray shadow, and shorten the illness time( P 0.05). It suggested that intravenous L Arg is a handy way to treat HAPE, and there is no toxicity. It is worthy of further trial.

To study the surgical management of primary hyperparathyroidism, we analysed 30 patients with primary hyperparathyroidism operated in our hospital from 1981 to 1990. A low collar transverse incision was done in all patients. A single parathyroid adenoma was found in 28 patients,and in 3 cases there were ectopic glands (1 in the mediastinum 2 intrathyroid). The adenoma was removed in all patients.In 1 case there was parathyroid hyperplasia, but only one gland was involved and it was excised. 1 case was diagnosed as parathyroid carcinoma which was located in the tracheo esophageal groove and had invaded the trachea and esophagus. An en bloc resection and tracheostomy were performed. 25 cases were followed up(including 1 case with parathyroid hyperplasia and 1 case with carcinoma) from 8 months to 19 years, there was no recurrence.These results suggest surgical management is the most effective treatment for primary hyperparathyroidism. Removal of the parathyroidoma and unilateral cervical exploration are adequate for the patients with parathyroid adenoma. Bilateral exploration is necessary when parathyroid hyperplasia is found. An en bloc resection is necessary for parathyroid carcinoma.

To observe the regulative effect of electroacupuncture(EA) on plasma levels of some brain gut peptides in dogs, contents of gastric secretin(GT), vasoactive intestinal peptide(VIP), somatostatin(SS) and endothelin(ET) in plasma were simultaneously measured by radioactive immunonassay method in the dogs with or without EA stimulation at different acupoints. The results showed that in the Zusanli(st 36) group, after EA stimulation, an increase was observed in plasma contents of GT and VIP( P

M.tuberculosis strains isolated strains from sputum specimens of 134 patients were analyzed by PCR SSCP and traditional drug susceptibility tests. The results sbowed that the gene mutation rate of PZA, SM, RFP,INH and EMB resistance in those clinically isolated strains was 42 7%,71 7%,78 9%,68 6% and 43 9%, respectively. The gene mutation was in relation with drug resistance level of M. tuberculosis. The gene mutation rate was higher in high concentration resistance strains than in low concentration resistance strains.

Objective To investigate the changes of the activity and the distribution of the rat cardiac calcineurin (CaN) which is the signalling molecule of the hypertrophy induced by the external aldosterone(Ald), and to detect the mechanism of the regulation of CaN. Methods 21 male wister rats were randomly divided into 3 groups: group Ald(7 rats), treated with Ald 18?g/d, peritoneal injection (i p) for 4 weeks; group spironolactone (spiron) (7 rats), treated with Ald and with spiron 20mg?kg -1 ?d -1 oral for 4 weeks; and group control (7 rats). The plasma concentrations of Ald, angiotensin Ⅱ(Ang Ⅱ)and endothelin(ET), blood levels of NO, the tissue activity of the CaN and distribution of CaN A? in the myocardium of the left ventricle, were detected in the rats by the radioimmunoassay , by the nitrate assay, by the chromophoric assay and by the immunohistochemistry, respectively. Results The concentration of the plasma Ald increased by 1 62 times, the blood NO - 3 concentration was deceased, the ratio of Ald over NO - 3( Ald/ NO - 3) increased by 2 95 times, and the cardiac tissue levels of CaN activity was were 1 47 times higher, in the rats treated with ald than in the normotensive rats. CaN A? distributed over cardiac cytoplasm, and the positive dyeing of the local CaN A? in the cytoplasm was thick near the cell membrane in the rats treated with Ald. The treatment with spiron could decreased the blood levels of the AngII?Ald/ NO - 3?ET/NO - 3?AngII/ NO - 3 and the tissue activity of myocardial CaN, positive dyeing of the local CaN A? in the cytoplasm, and increased blood NO - 3 concentration. Conclusion The aldosterone may be the main stimuli factor which can damage the balance of the blood active factors, and then make a novel cardiac hypertrophic signalling molecule calcineurin activated and redistributed in the cytoplasm of the rat left ventricular myocardium
