AIM: To study on the differences of chemical compos i tion between extracts of Bushen recipe prepared by decocting together and single , with fingerprint technology. METHODS: The extracts of Bushen recipe were prepared by decocting together and single under the same process. The comparison was done under the different wavelength s and different brand columns, HPLC method determined the extract's fingerprint within the range of optimized conditions. RESULTS: The ratio of The compositions and its main chemical substa nces are the different between the two extracts, and the extract rate of the main chemical substances are higher in extrat decocted single than in extract decocted togethe r. CONCLUSION: There are some chemical substances and composition's ra tio differences in the extracts of Bushen recipe made by decocting together and single.
2.Effect of health management on quality of life of hypertensive patients living in underdeveloped rural regions

Objective To evaluate the effectiveness of health management on quality of life of hypertensive patients living in underdeveloped rural regions. Methods Minqin County of Gansu Province was taken as research field, and health education covered all the population. Individual follow-up was adopted by quasi-experiment,and SF-8 scale was used to evaluate the change of scores of quality of life at baseline and the end of the study. Results The score of various dimension of quality life of interventive group showed a significant decrease at the end of follow-up ( P < 0. 05) , and the net score of general health status was 10. 92,the net score of impact to social role exerted by physical function was 9. 59,and the net score of social function was 4.61. Moreover, there was statistical difference between the intervention group and the control group for their quality of life(P <0. 05) , which showed in detail that each dimension of quality of life of the intervention group had higher score than that of the control group, after adjusting baseline difference by analysis of covariance. Conclusions All these results suggest that the active screening, following up and health education, conducted by the primary health care staff of township hospitals, under the idea of health management, can improve the quality of life of hypertension patients effectively in the rural area of underdeveloped region.
3.Clinical significance and histological origin of glomerular epithelial proliferative lesion in patients with focal segmental glomerulosclerosis

ObjectiveTo investigate the clinical significance and histological origin of glomerular epithelial proliferative lesion in patients with focal segmental glomerulosclerosis (FSGS). MethodsSeventy-four patients with idiopathic FSGS hospitalized in Peking University First Hospital from Jan. 2000 to Dec.2005 were enrolled in this study. Patients were classified into two groups according to with or without glomerular epithelial proliferative lesion. Estimation of active and chronic pathological scores was carried out using a semi-quantitative grade system by two pathologists. Clinical and pathological characteristics were compared between two groups. Immunohistochemical studies were performed to analyze the histological origin of glomerular epithelial proliferative lesion. ResultsThirty-one patients with glomerular epithelial proliferative lesion showed shorter interval from presentation to biopsy (P<0.05), higher percentage of nephrotic syndrome (NS) (P<0.05), higher frequency of segmental glomerulosclerosis(P<0.05), higher pathological active scores (P<0.05) and lower pathological chronic scores (P<0.05)as compared to 43 patients without glomerular epithelial proliferative lesion. Twenty-nine patients were followed up and renal survival rate in patients with glomerular epithelial proliferative lesion (39.7%) was significantly lower than that in patients without glomerular epithelial proliferative lesion (83.3%) (P=0.049). The frequency of glomerular epithelial proliferative lesion and the serum creatinine (Scr) level at biopsy were independent predictors of ESRD (OR value was 1.204, 1.008 respectively ). Glomerular epithelial proliferative lesion did not express mature podocyte markers including WT-1 and pedocalyxin, but stained positive for PCNA, PAX-2 and CK-8. ConclusionsGlomerular epithelial proliferative lesion represents the pathological change of acute stage and active lesion of FSGS, and also may be the pathological marker of severe clinical presentation and worse renal survival. Glomerular epithelial proliferative lesion may be derived from proliferation of parietal epithelial proliferation or de-differentiated podocytes.
4.Spectral marker construction for evaluating antipyretic effects of 4 herbs

AIM: To construct suitable quality standard of evaluating antipyretic effect of 4 herbs,Bupleurum,Flos Lonicerae Japonicae,Radix Isatidis and Fructus Forsythiae. METHODS: Extracts,essential oils and its mixtures of extract and oil of above herbs,plus Ibuprofen Suspension,were used for antipyretic experiment on rats.Principal Component Analysis and Canonical Correlation Analysis were adopted as evaluation tools to examine the correlation between pharmacological effect and UV spectra of herb's chemicals,from which spectral marker of potential reliability for evaluating antipyretic effects of samples could be constructed. RESULTS: Antipyretic effect sequencing of UV spectral canonical variables was consistent with that of the four herbs. CONCLUSION: The first canonical variable U1 expressed by principal component scores of UV spectra of herbs could be used as spectral marker for evaluating antipyretic effects of the four herbs.
5.Purification of total flavones from Bushen Recipe with macroporous resin

AIM: To study the absorption performance and purification process of total flavones from Bushen Recipe(Radix polygoni multiflori;Herba epimedii;Rhizoma drynariae;etc) with macroporous resin. METHODS: The contents of total flavones,icariin,tetrahydroxystilbene and naringin were worked as indexes,the adsorptive capacity of D101,DA201 and DM301 resins were investigated,and the static adsorption curve,dynamic leakage curve and elution curve were used for the selection of macroporous resin. RESULTS: The elution parameters of selected DM301 consisted of the 0.25 BV/h feed rate solution ratio of 4 BV water to elute resin and 4 BV 70% alcohol to be collected as elution solvent,and the elution rate of 2.4 BV/h was established. CONCLUSION: Macroporous resin method is a better candidate method for purifying flavones in Bushen Recipe.
6.Effects of Different Extraction Methods on Chemical Component Contents and Pharmacological Actions of Yangxue Ruanjian Prescription

Objective To establish a proper extraction method for Yangxue Ruanjian Prescription (YRP) by comparing the chemical component contents and analgesic and anti - inflammatory effect of extracts extracted from YRP by different methods. Methods Water - decocting, alcohol - refluxing and alcohol - percolating methods were adopted and component indexes of paeoniflorin and gentiopicrin were determined by HPLC. The anti - inflammatory effect of the extracts were observed on mice with xylol - induced auricular swelling and analgesic effect by hot - plate method. Results The content of gentiopicrin is similar in the extracts extracted by three methods and the content of paeoniflorin is higher in alcohol - percolating extract than in alcohol - refluxing extract, lowest in water - extraction. The analgesic effect of water - decocting extract is better than that of alcohol - refluxing extract, and that of alcohol - percolating extract is weakest; the anti - inflammatory effect of the three extracts is almost the same. Conclusion Water - decocting extraction is suitable to YRP, and this is determined by the characteristics of Chinese compound prescription.
7.The changes and significance of the soluble B cell-activating factor in the peripheral blood of patients with chronic human immunodeficiency virus infection

Objective To elaborate the changes of the soluble B cell-activating factor of the tumor necrosis factor family (BAFF) in the peripheral blood of chronic human immunodeficiency virus (HIV)-infected patients ,and to study the correlation between the soluble BAFF in HIV-infected patients and the progressions of acquired immune deficiency syndrome (AIDS).Methods Fifty untreated HIV outpatients and 30 healthy controls were recruited .According to the counts of CD4+T lymphocytes ,HIV-infected patients were divided into three groups ,< 200 cells/μL group , (200 - 350 ) cells/μL group and >350 cells/μL group .B cell counts and the BAFF levels were compared among the three groups and the healthy controls .The correlation analysis was conducted for the levels of BAFF ,the counts of CD4+T lymphocytes and B cells ,and viral load in HIV-infected patients .The value of BAFF in staging of HIV disease was identified by receiver operating characteristic (ROC) curve.Results The B cell counts were (90.3 ± 43.1)cells/μL in <200 cells/μL group ,(114 .4 ± 28 .8) cells/μL in (200 -350) cells/μL group ,and (162 .1 ± 29 .5) cells/μL in >350 cells/μL group and (307.1 ± 97 .0) cells/μL in healthy controls ,which was significantly different among the four groups (F=47.92 ,P<0.05).The concentrations of BAFF in the four groups were (1 737.5 ± 719.7) ,(962.8 ± 341.1) ,(859.8 ± 270.4) ,and (456.9 ± 163.7) ng/L ,with significant difference among the groups (F=36.72 ,P<0.05).The level of BAFF was negatively correlated with both B cell counts and CD4+T lymphocyte counts (r= -0.722 and -0.568 ,respectively ;both P<0.05) ,and positively correlated with viral load (r=0.607 ,P<0 .05).The area under the ROC curve was 0 .881.If the level of BAFF was 1 281.5 ng/L ,the sensitivity and specificity to predict the period of AIDS were 74 .1% and 87.0%,respectively .Conclusion The levels of soluble BAFF in HIV-infected patients are significantly increased and related with the reduction of B cell counts and disease progression.
8.Correlation between CD169 expression of peripheral blood monocytes and disease progression in human immunodeficiency virus-infected patients

Objective To study the correlation between CD169 expression of monocytes and disease progression in human immunodeficiency virus (HIV )-infected patients .Methods Sixty HIV-infected patients and 30 healthy controls were recruited .According to the CD4 + T lymphocyte counts ,HIV-infected patients were divided into three groups including < 200 cells/μL ,200 — 350 cells/μL and > 350 cells/μL groups . The differences in monocytes counts ,the proportions of CD16 + and CD169 + monocytes were analyzed among the three groups and healthy controls .The correlations between proportion of CD169 + monocytes and CD4 + T lymphocyte counts ,viral load ,and proportion of CD16 + monocytes were analyzed .Results The monocyte counts in CD4 + T lymphocytes < 200 cells/μL group , (200 — 350 ) cells/μL group , >350 cells/μL group and healthy control group were (342 ± 99) ,(396 ± 145) ,(365 ± 80) ,and (404 ± 106)/μL ,respectively ,which were not significantly different (F= 2 .55 , P > 0 .05) .The proportions of CD16 + monocytes in the four groups were (19 .8 ± 8 .8)% ,(14 .3 ± 2 .8)% ,(9 .7 ± 2 .0)% and (4 .0 ± 0 .8)% ,respectively ,which were significantly different ( F = 30 .90 , P < 0 .05 ) . The proportions of CD169 + monocytes in the four groups were (72 .6 ± 11 .4)% ,(59 .4 ± 14 .7)% ,(33 .3 ± 14 .5)% and (2 .6 ± 0 .8)% ,respectively ,which were significantly different (F = 152 .40 , P< 0 .05) .The proportion of CD169 + monocytes was negatively correlated with CD4 + T lymphocyte counts (r = 0 .792 , P< 0 .05) , while positively correlated with both viral load (r= 0 .485 ,P< 0 .05) and proportion of CD16 + monocytes (r= 0 .395 , P< 0 .05) .Conclusions The CD169 expressions of monocytes in HIV-infected patients are significantly increased and correlated with both monocyte activation and disease progression .
9.Clinicopathological features and renal outcomes of IgA nephropathy with acute tubulointerstitial nephropathy

Objective To evaluate the clinicopathological characteristics and outcomes of IgA nephropathy (IgAN) with acute tubulointerstitial nephropathy (ATIN).Methods Patients who were diagnosed as IgAN with ATIN and IgAN without ATIN by renal biopsy in Peking University First Hospital were enrolled.There were 74 cases of IgAN with ATIN,and seventy-four cases of IgAN without ATIN were enrolled based on stratified sampling (chosen by 1∶ 1).The two groups were well matched with age,gender,follow-up time,mesangial hypercellularity(M),endocapillaryhypercellularity (E),segmental glomerulosclerosis(S),tubular atrophy/interstitial fibrosis(T) and cellular/fibrocellular crescent(C).The clinicopathological characteristics and outcomes of two groups were retrospectively analyzed.A composite end point,defined as 30% or 50% estimated glomerular filtration rate (eGFR)decline and end stage renal disease (ESRD) was used.Renal function and proteinuria during follow-up were observed.Renal survival was calculated by Kaplan-Meier survival analysis and risk factors of progression were analyzed by using univariate and multivariate Cox regression models.Results Seventy-four cases of IgAN with ATIN and seventy-four cases of IgAN without ATIN were enrolled.Serum creatinine [(185.6±83.2) μmol/L vs (146.3 ±69.2) μmol/L,P=0.010] and incidence of acute kidney disease (AKD) (31.1% vs 5.4%,P < 0.001) were higher in IgAN with ATIN group than those in IgAN without ATIN group.Patients in ATIN group received more immunosuppressive treatment (86.5%vs 58.1%,P< 0.001).During 1 year after biopsy,mean eGFR increased significantly in IgAN with ATIN group [(39.7+ 14.6) ml· min-1· (1.73 m2)-1 vs (47.2+ 19.9) ml· min-1· (1.73 m2)-1,P=0.017],but mean eGFR was not statistic different in IgAN without ATIN group [(60.0±30.5) ml· min-1· (1.73 m2)-1 vs (59.0±31.7) ml· min-1· (1.73 m2)-1,P=0.567].Median follow-up was 23.0 months in IgAN with ATIN group,and Median follow-up was 30.0 months in IgAN without ATIN group.Incidence of composite end point had no significant differences between two groups.IgAN with ATIN was not the independent risk factor for end point.IgAN patients with ATIN were divided into two groups (with AKD and without AKD),then renal survival rate was higher (Log-rank test,x2=5.293,P=0.021) and the risk for composite end point decreased by 79.2% (HR=0.208,95%CI 0.046-0.939,P=0.041) in the group with AKD.Conclusions In IgAN,there is a subgroup of patients with the specific pathological phenotype combined with ATIN.Compared with those without AKD,the risk for composite end point of IgAN patients with ATIN and AKD showed a 79.2% decrease.
