1.Application of hierarchical management for psychiatric nursing in psychiatric ward

Objective To know the effects of using hierarchical management for psychiatric nursing in psychiatric ward,and then to offer some rationales and theoretical support for hospital management.Methods The fine hierarchical management was implemented from Janurary 2012 in Xi'an Mental Health Center and the metal hygiene clinics of Xi'an Jiaotong University.Before and after the hierarchical management carried out,the nursing care quality was assessed among the family members of hospitalized patients by using satisfaction survey questionnaires,flight rate of patients was evaluated,and at the same time,nursing care quality and the nurse comprehensive ability was inspected.And then,the EPIDATE 3.0 and SPSS16.0 software was used to analyze and compare the satisfaction scores.The nurse comprehensive ability,and nursing care quality scores before and after implementation.Results The nurse comprehensive ability were improved.The satisfaction scores of family members of hospitalized patients was higher than that before implementation after the fine management implementation,which had statistical difference,P< 0.05.The satisfaction rate of patients' family member was 96.77%(2 543/2 628) after the fine management implementation,which was significant higher than that of before implementation,91.19%(2 382/2 612),x2=107.66,P<0.05.Flight rate of patients was reduced after the hierarchical management,which was significantly lower than that of in before,P<0.05.Nursing care quality scores were higher than those of in before,which has a statistical difference respectively,P<0.05.Conclusions The hierarchical management could improve the satisfaction of family numbers,reduce the flight rate of patients,enhance nursing care quality and improve the nurse comprehensive ability.It would reduce the incidence of medical malpractice.The fine hierarchical management is worth to be applied and generalized in nursing care of psychiatric nursing in psychiatric ward.
2.Changes in myocardial enzymes in serum of patients with primary hypothyroidism

Objective To investigate the changes in serum level of myocardial enzymes in patients with primary hypothyroidism and to evaluate the relationship between the level of those enzymes and clinical or experimental parameters.Methods We measured several myocardial enzymes in serum of 108 patients with primary hypothyroidism including 31 cases of Hashimoto's thyroiditis,28 cases of atrophic thyroiditis,24 cases of Graves' hyperthyroidism treated by 131I,and 25 cases of Graves' hyperthyroidism treated by antithyroid drugs,as well as 50 normal controls.Results All the myocardial enzymes,including creatine kinase(CK) and its isoenzyme CK-MB,aspartate aminotransferase(AST),lactate dehydrogenase(LDH) and its isoenzyme HBDH,especially CK,were elevated in serum of patients with hypothyroidism.In addition,significant negative correlations were found between CK and FT3,CK and FT4,CK-MB and FT3,CK-MB and FT4.No difference in enzyme level was found among different hypothyroidism groups.Conclusion Hypothyroidism may cause elevation of myocardial enzymes in serum of the patients.The degree of CK and CK-MB increase was associated with the severity of hypothyroidism,and thus can be regarded as indexes to determine the severity of hypothyroidism.
3.Care experience of mothers of children with nanophthalmos and congenital cataract surgery: a qualitative study

Objective:To explore the care experience of mothers of children with nanophthalmos and congenital cataract surgery.Methods:From January 2014 to December 2019, the qualitative research method was used to conduct in-depth semi-structured interview with mothers of children with nanophthalmos and congenital cataract in Ophthalmology Ward of Beijing Tongren Hospital of Capital Medical University, and carry out live recordings and transcripts. Colaizzi content analysis method was used to analyze data and refine themes.Results:A total of four themes were extracted, namely anxiety and concern about appearance, family and social relations were affected, desire to speak and be heard, and urgent need for family care skills.Conclusions:While caring for children with nanophthalmos and congenital cataract, the nursing staff should deeply understand the mother's inner experience, provide psychological support, and help the mothers of the children to establish positive coping styles and promote their physical and mental health.
4.The effect analysis of standardized nursing trainingin reducing the incidence rate of post-endoscopic retrograde cholangiopanography pancreatitis

Objective To investigate the effectiveness and feasibility of standardized nursing training in reducing the incidence rate of post-endoscopic retrograde cholangiopanography(ERCP) pancreatitis. Methods A total of 521 patients were collected from January 2015 to June 2016 in ERCP therapy as control group.The patients in this group did not have standardized nursing training.A total of 289 patients were selected from July 2016 to April 2017 in ERCP therapy as intervention group.The patients in this group were carried out the standardized nursing training. The incidence of hyperamylasemia and post-ERCP pancreatitis was observed in two groups. Results The incidence of hyperamylasemia and post-ERCP pancreatitis was 12.09%(63/521),7.87%(41/521)in control group,and 7.61% (22/289), 3.11% (9/289) in intervention group,the difference was statistically significant between two groups(χ2=3.972,7.258,P<0.05 or 0.01). Conclusions The standard nursing training and intraoperative coordination can reduce its risk factors, and to reduce the post-ERCP pancreatitis and hyperamylasemia has important meaning and function.
5.Effects of traditional Chinese medicine soothing and fixed pillow in patients after endothelial keratoplasty

Objective:To explore the effect of traditional Chinese medicine soothing and fixed pillow in patients after endothelial keratoplasty with anterior chamber gas injection.Methods:From October 2020 to May 2022, convenience sampling was used to select 60 patients who underwent endothelial keratoplasty in the Ophthalmology Ward of Beijing Tongren Hospital as the research object. The patients were divided into the intervention group and the control group by random number table method, 30 cases in each group. The control group was given routine perioperative nursing, while the intervention group was given the traditional Chinese medicine soothing and fixed pillow on the basis of the nursing of the control group. The General Information Questionnaire, General Comfort Questionnaire (GCQ) and Postural Compliance Questionnaire were used to evaluate the patients.Results:The total score and the scores of each dimension of GCQ in the intervention group were higher than those in the control group, on the day after operation, the first day after operation, and the second day after operation, the accumulative daily supine time were longer than those in the control group, and all the differences were statistically significant ( P<0.05) . Conclusions:The traditional Chinese medicine soothing and fixed pillow can improve the comfort of patients with endothelial keratoplasty in continuous supine position after operation, prolong the supine time after operation, and improve the compliance of postoperative position of patients with endothelial keratoplasty.

Hip fracture in the elderly is characterized by high incidence, high disability rate, and high mortality and has been recognized as a public health issue threatening their health. Surgery is the preferred choice for the treatment of elderly patients with hip fracture. However, lower extremity deep venous thrombosis (DVT) has an extremely high incidence rate during the perioperative period, and may significantly increase the risk of patients′ death once it progresses to pulmonary embolism. In response to this issue, the clinical guidelines and expert consensuses all emphasize active application of comprehensive preventive measures, including basic prevention, physical prevention, and pharmacological prevention. In this prevention system, basic prevention is the basis of physical and pharmacological prevention. However,there is a lack of unified and definite recommendations for basic preventive measures in clinical practice. To this end, the Orthopedic Nursing Professional Committee of the Chinese Nursing Association and Nursing Department of the Orthopedic Branch of the China International Exchange and Promotive Association for Medical and Health Care organized relevant nursing experts to formulate Expert consensus on perioperative basic prevention for lower extremity deep venous thrombosis in elderly patients with hip fracture ( version 2024) . A total of 10 recommendations were proposed, aiming to standardize the basic preventive measures for lower extremity DVT in elderly patients with hip fractures during the perioperative period and promote their subsequent rehabilitation.
