1.Effects of large dose norepinephrine on the anastomotic healing after esophagectomy

Objective To explore the impact of using large dose norepinephrine on the anastomotic healing after esophagectomy.Methods Clinical data of data of 148 cases patients with esophageal cancer who were treated in Wuwei Tumor Hospital of Gansu Province with surgery from January 2014 to June 2016 were retrospectively analized.There were 7 patients who were used 25-67 mg ((32.6 ± 2.3) mg) norepinephrine because of the low blood pressure and other factors during the first 4 d postoperative as research group,and the other 141 patients who had not applied or applied the small dose were set as control group.The patients of two groups with postoperative anastomotic healing and fistula incidence were compared.Results There were 3 cases and 7 cases of postoperative patients appeared anastomotic fistula in the research group and the control group,the rate of anastomotic fistula were 42.86% (3/7) and 4.96% (7/141) respectively,the difference was significant (x2=9.78,P =0.001),and 2 cases appeared varying degrees residual gastric necrosis in the research group,all of them occurred in the patients with large dosage and long time.Conclusion There are great risk on the anastomotic fistula and residual stomach mecrosis if long time and large dose norepinephrine was used after esophagectomy,it should be caused enough attention for surgeons.
2.Diagnostic value of quantitative analysis of AgNOR,CEA and Ft for malignant serous cavities effusion

Objective: To construct the eukaryotic expression vector pSurp-EGFP regulated by the survivin gene promoter and to detect the specific expression of the promoter in human laryngeal cancer Hep-2 cells by green fluorescent protein assay.Methods: Thesurvivin gene promoter was generated by polymerase chain reaction(PCR) and the CMV promoter of the pShuttle vector replaced by the survivin gene promoter to generate the plasmid pSurp.The three plasmids pShuttle,pSurp and pEGFP-C1 were respectively double-enzyme digested so as to produce the plasmids pCMV-EGFP and pSurp-EGFP carrying the CMV or survivin promoter.The purified pCMV-EGFP and pSurp-EGFP were transfected into Hep-2 cell and vascular endothelial cell ECV304 using liposome transfection reagent and the expressions of EGFP detected by the fluorescent microscope.Results: Thesurvivin gene promoter was successfully cloned by PCR,and thesurvivin gene promoter-regulated pSurp-EGFP was constructed.Green fluorescence was observed in Hep-2 cells but not in ECV304. Conclusion: The high specific activity of the survivin gene promoter in Hep-2 cells that we successfully constructed attributes to the studies of tumor specific gene therapy.
4.Application of flexible ureteroscopy for treatment of upper urinary tract calculi in children and its complications analysis

Objective To evaluate the efficacy and complications of flexible ureteroscopy for the treatment of upper urinary tract calculi in children.Methods The clinical data of upper urinary tract calculi in 20 children including 16 male and 4 female,treated by using flexible ureteroscopy,were collected from January 2014 to May 2015 in the First Affiliated Hospital of Zhengzhou University.The mean age was 6.4 years (ranging from 2.7-15.0 years old).Among them,6 cases had upper ureteral calculi and 14 cases had renal calculi.All the calculi was found unilaterally with on the left side in 12 cases and on the right side in 8 cases.Ipsilateral mild to moderate hydronephrosis were found in all of the cases.The mean stone size was 1.2 cm (ranging from 0.5-1.8 cm).All the cases were treated through retrograde flexible ureteroscopy holmium laser lithotripsy after the invalid conservative through conservative treatment.Double J tube was left routinely in the sick side 1 week before operation to expand the internal diameter of the ureter.A double J stent was left in place for 1 month after operation as a routine.Results The flexible ureteral access sheath failed to insert into the upper ureter in 2 cases because of the narrow ureter,then the flexible ureteroscopy was inserted into ureter through wire directly.The flexible ureteral access sheath was successfully inserted into the ureter for the others and flexible ureteroscopy were inserted through flexible sheath.The mean operative time was 38 min (ranging from 20-60 min).The patients were discharged from hospital after a mean of 4.5 days (ranging from 3-7 days).The rate of stone-free in one-stage was more than 90%.There were residual small stones in the lower calices of kidney in 2 cases.The perfusion liquid volume during the operation was 500 mL(ranging from 200-1 000 mL).There was no major perioperative complication,while transient macroscopic hematuria and fever were common complications after operation.The stone search was performed successfully in the whole 14 cases.NO residual stone could be found through ultrasound and kidney ureter bladder plain and Double-J stent was removed 1 month after operation.Conclusions Flexible ureteroscopy produces high stone-free rate and clinical safety for upper urinary calculi in children.However,the complications of flexible ureteroscopy lithotripsy cannot be ignored and sufficient preoperative preparation and careful perioperative safety control should be required.

Tanshinones and phenolic acids are two major classes of effective compounds in S.miltiorrhizae Radix et Rhizoma.Accumulative levels of these compounds directly determined its quality and efficacy.Presently,for the purpose of understanding the underlying the mechanism behind the biosynthesis and regulation of effective compounds in S.miltiorrhiza,a series of progresses have been pressed ahead.Benefiting from the boost and convenience of high-throughput genomic,transcriptomic,and metabolic analysis methods,remarkable achievements have been accessed in this field.In this paper,a summary for the recent progress have been made with the provision of a reference for better understanding over the quality of S.miltiorrhiza.
6.Application and Analysis of Electronic Pharmacy Consultation System in Our Hospital

OBJECTIVE:To improve the quality of pharmacist consultation,provide real-time,precise and whole-process medi-cine guidance and to promote rational drug use. METHODS:The functions of electronic pharmacy consultation system(EPCS)in our hospital were introduced,and its effects were evaluated. RESULTS:The system had several modules as multiple dimensional knowledge assistant system,real-time prescription checking engine,electronic consultation record method,pharmacist workload and quality statistical analysis. EPCS provided convenient information access,prescription auditing,consultation record and statisti-cal analysis so as to improve consultation efficiency and accuracy. Compared with before using EPCS,EPCS had enhanced speed of inquiring information by 3.75 times,prescription auditing by 4 times,consultation record by 6 times and statistical analysis by 20 times;return visit rate to potients increased from 0.03% to 0.34%. The number of consulting questions increased month by month. The type of consultation question,the identity of the consultant and others could be classified statistically based on the sys-tem. CONCLUSIONS:EPCS helps pharmacist to build a more scientific and effective consultation platform,and promotes rational drug use in the hospital.
7.Rupture of primary hepatocellular carcinoma after transcatheter arterial chemoembolization:report ;of 8 cases

Objective To summarize the risk factors, the diagnostic and therapeutic approaches, and the outcomes of the ruptured primary hepatocellular carcinoma (PHC) occurred after transcatheter arterial chemoembolization (TACE) in order to make a further understanding of this complication. Methods The clinical data of 8 patients with ruptured PHC after TACE, who were encountered at the First Affiliated Hospital of Suzhou University during the period from Sep. 2007 to Sep. 2013, were retrospectively analyzed. Results A total of 1379 times of TACE were performed in 678 patients with PHC. Among the 678 patients, 8 developed rupture of PHC with bleeding after TACE. The overall incidence was 1.2%. The mean diameter of the tumors in the 8 patients was (11.5 ± 2.6) cm, ranging from 7.6 cm to 15.9 cm. All the lesions were located at the peripheral region close to the liver capsule, and most of them protruded outward. Five cases had coexisting portal hypertension. The average dosage of Lipiodol used in TACE was (14.9 ± 4.5) ml with a range of (8 - 20) ml. Of the 8 patients, emergency transcatheter embolization was carried out in 4 and medical management was employed in other 4. Seven patients died as all active emergency treatments failed. Only one patient, who had received emergency transcatheter embolization, survived the ruptured PHC. Conclusion Rupture of primary hepatocellular carcinoma after TACE is a rare, but very serious complication. Its occurrence may be related to lesion’s characteristics, such as large tumor size, superficial location and protrusion from the liver surface, etc. Besides, interventional management, e.g. without use of solid embolic material to enhance the embolization effect, may also be responsible for the rupture.

10.QUANTITATIVE ANALYSIS AND DEVELOPMENTAL OBSERVATIONS ON ASYMMETRY OF THE LEFT AND RIGHT PLANUM TEMPORALE OF THE FETAL AND INFANTAL BRAINS

Quantitative analysis and developmental observations on the planum temporale of 157 fetus and infantal brains have been made. The results are as follows: 1) left planum temporale of 82% of the fetal brains and 76% of the infantal brains are larger than the right. 2) Average area values of the left planum temporale are signifcantly larger than the right. 3) The earliest appearance of the planum temporale is in the sixth gestational month. 4) Rate of appearance of the left planum tempo-rale in fetuses is higher than that of the right. 5) Right/left ratio of planum temporale increases with the development of the embryo. These findings indicate that the asymmetry of the left and right planum temporale is present since the appearance of the planum temporale, and changes as the fetus develops.
