1.Effect of Different Chinese Medical Therapies on Electrocardiogram and Endothelin in Rats with Acute Myocardial Ischemia

Objective To observe the effect of therapies of activating blood and removing stasis, activating yang and dissipating phlegm, and warming and dredging meridians with aromatic herbs on relieving acute myocardial ischemia (AMI). Methods Rats were randomized into normal group,model group, Xuefu Zhuyu Decoction (XZD) groups (treated with XZD 20.5 and 6.8 gkg-1d-1 ), Gualou Xiebai Banxia Decoction (GXBD) groups (treated with GXBD 18.0 and 6.0gkg-1d-1 ), and Guanxin Suhexiang Capsules (GSC) groups (treated with GSC 0.19 and 0.06 gkg-1d-1 ). The AMI rat models were established by intravenous injection of pituitrin (Pit, 1.5U/kg) through the caudal vein. The effect of the three Chinese medical therapies on AMI rat electrocardiogram (ECG) and plasma endothelin (ET) level was evaluated. 2.Effect of donor in brain death on liver transplantation

Liver transplantation is one of the main treatments for acute and chronic liver failure currently.In recent years,with the lack of donor liver transplantation and the in creasing number of patients,the major donator of liver transplant is brain dead patients.In brain-dead state,organ functions de cline gradually.Changes in hemodynamics and immunity could lead to inflammation and alter hormone release.Organ survival and patients'prognosis can be unpredictable.The study of the above phenomenal mechanisms is still at an early stage.This article summarized the current liver transplant treatments by explai ning the pathophysiology of donor organ damage and reviewing the prognosis.The aim of this article was to provide a perspective for future study on this topic.
3.Exploration of treatment for common bile duct stones combined with cavernous transformation of the portal vein

Objective To explore more safe and effective method for the treatment of common bile duct stones combined with cavernous transformation of the portal vein (CTPV).Methods We report 2 cases of post-treated patients in this series.In order to avoid severe bleeding caused by dissection of bile duct,we applied different methods to remove common bile duct stones compared with traditional operation.We removed common bile duct stones in one patient through cystic duct approach during operation.Two years later,we performed endoscopic duodenal incision (endoscopic sphincterotomy,EST) for him because of lower common bile duct stenosis with sand like stone.Another patient,on the other hand,we conducted the duodenal papilla incision during operation and removed common bile duct stones with choledochoscope through the bottom of common bile duct.Result Of the two patients,all of the three kinds of operation had good curative effects.Conclusions For the patients with common bile duct stones combined with CTPV,we should select appropriate treatments to avoid dissection of bile duct in front of the dilated and tortuous collateral veins during the operation,which is the key to guarantee the safety and success of the operation.EST is the preferred method of the treatment.Open operation with choledochoscopy via cystic duct approach or duodenal papilla incision through distal common bile duct to perform stone extraction also appears to be effective and safe.
4.Hepatic artery reconstruction in operations for hilar cholangiocarcinaoma

Objective To investigate the feasibility and outcome of resection and reconstruction of hepatic artery in hilar cholangiocacinoma (HCC).Methods The data of 29 patients with HCC with hepatic artery reconstruction carried out from March 2009 to August 2013 in our center were retrospectively analyzed.23 right hepatic arteries and 6 common hepatic arteries were involved.In-situ anastomosis was carried out in 20 patients and a double anastomosis using gastrodoudenal artery grafts was carried out in 9 patients.Results There were no arterial thrombosis or other related complications on prolonged follow-up.Conclusion Hepatic artery resection and reconstruction should be carried out if the artery was invaded by a hilar cholangiocarcinoma to produce a high resection rate and a better outcome.
5.Surgical treatment of patients with pancreatic ductal stones: a report of 60 cases

Objective To analyze the perioperative complications and prognosis of surgical treatment of pancreatic ductal stones.Methods 60 patients diagnosed to have pancreatic ductal stones and were admitted into our hospital from January 2009 and December 2014 were enrolled into this retrospective study.There were 47 males and 13 females.The average age was (44.1 ± 12.0).Results All patients underwent surgical operation and there was no perioperative death.Perioperative complications occurred in 7 patients (11.7%), including pancreatic fistula in 4 patients, delayed gastric emptying in 1 patient, abdominal infection in 1 patient, anastomotic bleeding in 1 patient.Eight patients developed elevated amylase levels.The fasting time was 3.4 ± 1.2 days and the hospital stay was 10.0 ±4.0 days.The follow-up rate was 88.3% (53/60), at a mean follow-up of 27 months.In 51 patients who presented with abdominal pain, 43 patients (84.3%) had complete pain relief and 7 patients (13.7%) had partial pain relief.In 14 patients with preoperative diabetes mellitus, 1 patient had improvement and 2 patients recovered from diabetes.Two additional patients were diagnosed to have diabetes two years after the operation.Of the 2 patients who were shown to have pancreatic cancer on histopathological study, 1 patient was lost to follow-up while another patient died at 27 months after the operation.The patients diagnosed to have epithelial dysplasia were followed-up and none developed pancreatic cancer.Conclusions The relationship between chronic pancreatitis and pancreatic ductal stones was very close.Chronic pancreatitis also caused exocrine and endocrine dysfunction.Surgical operation plays an important role in symptomatic relief and in delaying pancreatic dysfunction.
6.Deep Fungal Infection in Patients:Clinical and Pathogenic Analysis

OBJECTIVE To investigate the postoperative morbility of deep fungal infections and the source and composition of the pathogenic fungi.METHODS Clinical data of 816 patients with post-operative deep fungal infections from Jul 2006 to Jun 2008 were reviewed and analyzed retrospectively.RESULTS The detectable rate of post-operative deep fungal infections was 24.82%,among which Candida albicans was the most common(65.69%),followed by C.tropicalis(10.57%);the rate of broad-spectrum antibiotic application in peri-operation was 96.45%;the infection site in the descending order was cardio-thorax,gastrointestinal tract,urinary tract,female reproductive system,blood and skeleton.CONCLUSIONS Operative trauma is an important factor that causes deep fungal infections in hospital,and is closely related to broad-spectrum antibiotic application.Positive prevention,timely diagnosis and effective treatment should be highly emphasized when dealing with post-operative deep fungal infections.
7.Expression of amphiregulin in human endometrium during the menstrual cycle

Objective:To investigate the expression of amphiregulin in human endometrium during the menstrual cycle.Methods: Endometrial tissues were collected from the patients undergoing hysterectomy or endometrial biopsy.Real-time RT-PCR,in situ hybridization and immunohistochemistry were used to detect the expression characteristics of amphiregulin in human endometrium in proliferative and secretory phases.Results: Real-time RT-PCR showed the expression of amphiregulin mRNA in secretory phase was 32 times that in proliferative phase.The results from in situ hybridization and immunohistochemistry showed that amphiregulin was located in the cytoplasm and mainly expressed in the gland of endometrium.The expressions of amphiregulin mRNA in proliferative and secretory phases were 0.54?0.22 and 2.96?0.47(P
8.Study of different teaching methods in directing graduated students in the department of hepatobiliary surgery

Objective To investigate the feasibility and realistic significance of different teaching methods in professional and scientific degree graduate students.Methods Twenty six graduate students (scientific 12,professional 14) were randomly selected as research object from Sep.2014 to Apr.2017,and the relationship between clinical or research outcome and type of degree,clinical or experiment time were analyzed.Results The experiment time of scientific degree graduate students was significantly longer than that of professional degree graduate students [(21.0 ± 1.6) vs.(11.0 ± 3.5) months],and professional degree graduate students paid more attention to clinical work,there was statistic difference between the two groups [(20.0 ±2.1) vs.(12.0 ±2.4) months,both P<0.05].The qualification rate of basic clinical manipulation in the professional group was higher than that in the scientific group (85.7% vs.75.0%,P < 0.05).Conclusion Different type of teaching methods based on the type of student degree were feasible and proper for graduate students in surgery,and the terminal object of our teaching should be focus on the resolution of clinical problems.
9.Epidemiology of uterine corpus cancer in some cancer registering areas of China from 2003-2007

Objective To analyze corpus uteri epidemiology in selected cancer registering areas of China during 2003-2007,and to provide scientific information for its prevention and control in China.Methods The incident and mortality data of corpus uteri cancer in 32 cancer registering areas of China with better quality during 2003-2007,which were selected according to the criteria of and provided by National Center for Cancer Registration,were analyzed.Results There were 8850 new cases and 1559 death cases of corpus uteri cancer,which accounted for 2.96% ( 8 850/299 306 ) of all female new cancer cases and 0.94% ( 1 559/166 305) of all female cancer death cases,respectively.Corpus uteri cancer was the 9th most common cancer for all new female cancer cases which world age adjusted incidence rates was 5.04/105,and 19th most common cancer for all female cancer death cases which world age adjusted mortality rate was 0.83/105 in 32 selected cancer registering areas of China during 2003-2007.Zhongshan city,Guangzhou city in Guangdong province and Beijing were the areas with the highest incidence rates in which were 14.51/105,8.51/105 and 6.69/105,respectively.Zhongshan city in Guangdong province,Dafeng city in Jiangsu province and Feicheng city in Shandong province were the areas with the highest mortality rates,in which were 4.03/105,3.19/10s and 1.65/105 respectively during 2003-2007.There were increasing trends for its incidence rates in above 32 areas during 2003-2007,its world age adjusted incidence rates increased from 3.94/105 in 2003 to 5.56/105 in 2007 ( P =0.026 ),while its urban world age adjusted incidence rates increased from 4.57/105 in 2003 to 6.18/105 in 2007 (P =0.038),and rural rates increased from 1.74/105 in 2003 to 3.01/105 in 2007 ( P =0.013 ),and the results showed that urban areas obviously higher than rural areas ( P < 0.01 ).Although there was a slow increasing trend for its world age adjusted mortality rates in above 32 areas during 2003-2007 which increased from 0.64/105 in 2003 to 0.87/105 in 2007 ( P =0.214 ),and from 0.66/105 in 2003 to 0.88/105 in 2007 in urban areas ( P =0.340 ),and from 0.57/105 in 2003 to 0.83/105 in 2007 in rural areas( P =0.070),while increasing trends without statistical significance.But mortality rates in urban areas were obviously higher than those of rural areas (P <0.01 ).Conclusions Although the world standardized incidence and mortality rates of corpus uteri cancer were at low level worldwide,there were increasing trends for its incidence rates during 2003-2007 in the 32 selected cancer registering areas of China.Moreover,its incidence and mortality rates were at high level worldwide in some areas such as Zhongshan city of Guangdong province and Dafeng city of Jiangsu province during the period,in which suggested that its prevention and control should be enhanced.
10.Protective effect of blood-activating and stasis-resolving medicines against cerebral ischemia-reperfusion injury in rats: A controlled trial to verify the time-effect and dose-effect relation

BACKGROUND: The injury of blood-brain barrier following cerebral ischemia reperfusion is a considerate pathological basis for injury caused by cerebral ischemia and reperfusion.OBJECTIVE: To investigate the effects of four most basic Chinese medicinal herbs, or safflower, peach seed, ligusticum and red peony with actions of activating blood and resolving the stasis on the contents of nitric oxide, immunoglobulins, C-reactive protein (CRP) and complements (immunological indices) of serum and cerebral homogenate, as well as the morphological and structural changes of cerebral tissue cells in rats with ischemia and reperfusion to vertify relationship between time effectiveness and quantitative effectiveness.DESIGN: A randomized controlled study, and evaluation by single blind.MATERIALS: The experiment was completed in the Laboratory of Traumatology Institute of Guangzhou Red Cross Hospital from January 2001 to De cember 2002. Safflower, peach seed, szechwan lovge rhizome and red peony are concentrated granules of single decocting pieces, the blood activating and stasis resolving decoction was prepared at 2.5 g/L according to 1:1:1:2 ratio.Totally 138 adult female SD rats were selected for the experiment,weighing 280-300 g, provided by Animal Center of Guangzhou University of TCM.INTERVENTIONS:The models rats with middle cerebral artery occlusion were set up by thread ligation(24 hours reperfusion after 2 hours middle cerebral artery occlusion).All 138 rats were randomly divided into 6 groups with 23 in each group.Sham operation group:The vessels were ligated but the middle cerebral artery was not occluded.No.1 medicated group: The BASR was by gavage given in a dose of 2 g/kg 30 minutes before operation. No. 2 medicated group: The BASR was by garage given in a dose of 2.5 g/kg 30 minutes before operation. No. 3 medicated group: The BASR was by garage given in a dose of 2 g/kg for consecutive 7 days be fore operation. No. 4 medicated group: The BASR was by gavage given in a dose of 2.5 g/kg for consecutive 7 days before operation.Control group:the same volume of saline was by gavage given for consecutive 7 days before operation. ① Scoring of dysneuria (5-score system: 0-1 score meant mild dysneuria, 2-4 scores meant severe dysneuria) for all rats were performed after consciousness following 2 hours ischemia and 24 hours reperfusion.② After 24 hours reperfusion,10 rats in each group were at random selected for assay of levels of CRP, complement 3 (C3) and complement 4 (C4) (rate nephelometry), and concentration of nitric oxide (nitrate reductase method)in both cerebral homogenate and serum.③ After 24hours reperfusion, 10 rats in each group were at random selected, and after anesthesia was completed, the brain was quickly collected through decapitation, put into a 110 ℃ drying oven till its constant weight, then the water content in brain was calculated.④ The cerebral cytomorphology in every group was observed under light microscope. ⑤ After reperfusion, 3 rats in each group were randomly selected for preparation of coronal section of cerebral tissue,the cerebral ultrastructure in each group was observed under transmission electronic microscope.MAIN OUTCOME MEASURES: ① The results of dysneuria scoring in each group. ② The levels of CRP, C3 and C4, and concentration of nitric oxide in both cerebral homogenate and serum.③ The water content in brain. ④ The cerebral cytomorphology and the cerebral ultrastructure. RESULTS: All 138 rats entered into the result analysis. ① Comparison of the extents of dysneuria of rats in each group: The ratio of severe dysneuria after 24 hours ischemic reperfusion in all medicated groups was obviously lower than that in control group(P < 0.01),and the ratio in No.4 medicated group was lower than that in No.2 medicated group(P < 0.05).② Comparison of water contents in brain of rats in each group:The water contents in sham operation group and all medicated groups were obviously lower than that in control group (P < 0.05). ③ Comparison of the nitric oxide concentration in cerebral homogenate of rats in each group: The concentration in sham operation group and all medicated groups were obviously lower than that in control group (P < 0.01). The concentration in No. 3medicated group was obviously lower than that in No. 1 medicated group (P < 0.05).The concentration in No.4 medicated group was obviously lower than that in No. 2 medicated group (P < 0.01). ④ Comparison of the nitric oxide concentration in serum of rats in each group:The concentrations in sham operation group and all medicated groups were obviously lower than that in control group(P < 0.01).The concentration in No.3 medicated group was higher than that in No. 1 medicated group (P < 0.05). The concentration in No. 4 medicated group was higher than those in No. 2 and in No. 3 medicated groups (P < 0.05). ⑤ Comparison of the levels of CRP in cerebral homogenate and serum of rats in each group: The levels in sham operation group and all medicated groups were lower than that in control group(P < 0.05-0.01).The level in No.3 medicated group was lower than that in No. 1 medicated group (P < 0.01). The level in No. 4 medicated group was lower than those in No. 2 and in No. 3 medicated groups (P <0.05-0.01).⑥ Comparison of the levels of C3 in cerebral homogenate and serum of rats in each group:The levels in sham operation group and all medicated groups were lower than that in control group (P < 0.05-0.01). The level in No.3 medicated group was lower than that in No.1 medicated group (P < 0.05). The level in No. 4 medicated group was lower than those in No. 2 and in No. 3 medicated groups (P < 0.01). ⑦ Comparison of the levels of C 4 in cerebral homogenate and serum of rats in each group:The levels in sham operation group and all medicated groups were lower than that in control group (P < 0.05-0.01). The level in No. 3 medicated group was lower than that in No. 1 medicated group (P < 0.05). The level in No.4 medicated group was lower than those in No. 2 and in No. 3 medicated groups (P < 0.01). ⑧ Comparison of the condition of cerebral edema of rats in each group:In control group there was obvious cerebral congestive edema,indicating an obvious infection;while in medicated groups the extent of cerebral edema was milder than that in control group.⑨ Changes of cerebral ultrastructure of rats in each group:The ultrastructure in sham group was normal. In control group, there obvious edema of cells, capillaries and sheaths in the marginal zone of cortex necrosis,and reduction of organelles of neuron. As well. In No. 3 and No. 4 medicated groups, the limits of cell membranes were clear, the structure was integral, the chondriosomes were rich and even in size,the medullated fibers were morphologically normal.And in No.1 and No.2 medicated groups the changes were between the twoCONCLUSION:① The scoring of dysneuria in rats was decreased after the blood-activating and stasis-resolving medicine was given,and it was lower in rats that were given a longer period of medication,indicating that the improved extent for dysneuria is related to prolonged medication.②The nitric oxide concentration of cerebral tissue in rats that recevied the blood-activating and stasis-resolving medicine was decreased, and the nitric oxide concentration of serum in the rats was increased,indicating that the blood-activating and stasis-resolving medicine can reverse the anomalies of nitric concentration in different tissues after ischemic reperfusion so as to reduce cerebralinjury.③ The levels of C3 and C4 of cerebral tissue and serum in rats that received the blood-activating andstasis-resolving medicine were obviously decreased,indicating that the medicine may reduce the cerebral injury through triggering complement system;and the CRP was also get decreased,further suggesting that the medicine can inhibiting infective reaction.④ The longer the period of medication,the milder the cerebral injury, and the dose of 2.5 g/L was better in effect.
