AIM: To investigate the mRNA changes of L-type Ca~(2+) channel ?_(1C) subunit (CaL-?_(1C)) and Na~+-Ca~(2+) exchanger (NCX) in the atria of renovascular hypertensive rats. METHODS: Two-kidney one-clip Goldblatt hypertensive Sprague-Dawley rats were divided into three groups 1 week after operation, HD group was treated with 250 mg/d diltiazem, LD group was treated with 50 mg/d diltiazem, control group (C group) was treated with vehicle. After 4 weeks treatment, semiquantitative RT-PCR was used to estimate the mRNA changes of CaL-?_(1C) and NCX, and GAPDH was used as internal control. RESULTS: Systolic blood pressure in LD group was comparable with C group, and that in HD group was decreased to normal level after diltiazem treatment. In C group, CaL-?_(1C) mRNA level were 2.5, 2.4 and 2.1 times of S, HD and LD group, and NCX mRNA level were 1.9, 1.6 and 2.1 times of S, HD and LD group. There were no significant difference in the mRNA level of CaL-?_(1C) and NCX among S, HD and LD group. CONCLUSION: The mRNA levels of CaL-?_(1C) and NCX are upregulated in the atria of hypertensive rats. Ca~(2+) antagonist inhibits their upregulation independent of blood pressure.

Objective To study the accuracy and practical value in the clinical diagnosis of Budd-Chiari syndrome by B ultrasonography and postcaval vein contrast examination.Methods 42 patients received B ultrasonographic scanning of the postcaval vein and the liver before the use of angiography.Angiography included Seldinger single-direction postcaval intubation(14 cases) and double-direction intubation(28 cases),and quick photographic observation.Results B ultrasonography showed that 18 cases had postcaval segmental or membranous obstruction 24 cases had postcaval stricture at its opening,and 20 cases complicated with single right hepatic stricture,8 cases with left vein stricture and 12 cases with central hepatic caval stricture.Communicating branched vessels were formed among 16 cases between hepatic vessels were formed among 8 cases between hepatic left-middle,and middle-right hepatic veins each.Reticular communicating vessels were formed in 8 cases between left middle and right.5 cases had slight postcaval stricture and their blood flowed non-obstructedly postcaval veinography showed 12 cases had postcaval stricture at proximal end,28 cases total segmental or membranous obstruction.There were 8 cases respectively for hepatic central vein and right hepatic vein development each.The remaining 26 cases had no hepatic venous development.There were 30 cases who had obvious branched circulaltion formation,and even their branched circular vessels had tumous-like expansion.Conclusion B ultrasonography,as a way of easy,and no-trauma examination,is the first-choice means to screen the patients.Postcaval angiography is a means to diagnose Budd-Chiari syndrome,which is neccessary for the cases with total segmental or membranous obstruction to have double-direction postcaval angiography,especially for the cases to have interventive therapy.

Objective In order to investigate the synaptic plasticity in dentate gyrus after seizures and axonal and dendrtic sprout- ing induced by KA administration. Methods The density of synapses, the curvature forms of synaptic interface were studied under electron microscope. Results 1 .The density of the synapse is decreased obviously 3 days after KA injection, while the density of synapses is increased to control level 7 days after KA injection. 2. Compare with the curvation forms of synaptic interface of control animals and 3days after KA administration animals, the amount of smile synapses is significantly decresed and the amount of frown synapses is significantly increased in the moleculous layer of dentate gyrus 7 days after KA injection. Conclusions 1 .This result demonstrates that the axonal and dendritic sprouting of dentate granule cells is functional. 2. The increase of frown synapse is related to the release of glutamate of sprouting mossy fibers.

Objective:To evaluate the clinical efficacy of Huzhang Polou Decoction combined with acupoint application in the treatment of children with mycoplasma pneumoniae pneumonia (MPP) complicated with atelectasis.Methods:A total of 105 MPP children with atelectasis and phlegm-heat obstructing lung syndrome in our hospital from August 2019 to March 2022, who met the inclusion criteria, were divided into control group (53 cases) and observation group (52 cases), by random number table method. The control group was given azithromycin sequential therapy on the basis of conventional western medicine treatment, and the observation group was given Huzhang Polou Decoction and acupoint application on the basis of the control group. Both groups were treated for 19 days. TCM syndromes were scored before and after treatment. The FEV1 and peak expiratory flow rate (PEF) were measured by pulmonary function measurement instrument. The CRP was measured by immunoturbidimetry and procalcitonin (PCT) was measured by ELISA. The recovery time of fever, disappearance time of cough and sputum, disappearance time of lung rales, lung recruitment time, and adverse reactions during treatment were recorded, and the clinical efficacy was evaluated.Results:The total effective rate was 96.15% (50/52) in the observation group and 84.91% (45/53) in the control group, and the difference between the two groups was statistically significant ( χ2=3.85, P=0.050). After treatment, the TCM syndrome score of the observation group was significantly lower than that of the control group ( t=22.78, P<0.01). FEV1 [(1.87±0.29) L vs. (1.54±0.28) L, t=5.93] and PEF [(79.82±6.29) L/min vs. (74.32±6.30) L/min, t=4.48] were significantly higher than those in the control group ( P<0.01). The levels of serum CRP [(9.18±2.98) mg/L vs. (12.34±3.00) mg/L, t=5.42] and PCT [(0.60±0.15) ng/L vs. (0.96±0.21) ng/L, t=9.93] were significantly lower than those in the control group ( P<0.01). The recovery time of fever, disappearance time of cough and sputum, disappearance time of lung rale and lung recruitment time in the observation group were significantly earlier than those in the control group ( t=7.27, 6.84, 3.76, 5.87, all Ps<0.01). During treatment, the incidence of adverse reactions was 3.77% (2/53) in the control group and 1.92% (1/52) in the observation group, and there was no significant difference between the two groups ( χ2=0.32, P=0.569). Conclusion:The Huzhang Polou Decoction combined with acupoint application can improve the pulmonary function of children with MPP complicated with atelectasis, reduce the level of serum inflammatory cytokines, improve the clinical efficacy safely.

Objective To evaluate clinical features,therapeutic effects and outcomes of patients with non-human immunodeficiency virus(HIV)-infected cryptococcal meningitis treated with fluconazole or fluconazole and flucytosine.Methods Twenty-four cases of non-HIV-infected cryptococcal meningitis(fluconazole with or without flucytosine as initial therapy)in Huashan Hospital,Fudan University from 1997 to 2007 were retrospectively reviewed.Clinical manifestations,therapeutic effects and outcomes of the patients were collected.Results Fluconazole was administered with median dosage of 400 mg/d,for a median duration of 20.5 days.After fluconazole initial therapy for 2 weeks,16.7% showed partial response,83.3% showed no response,and the overall response rate was 16.7%.After 10 weeks,33.3% showed partial response,29.2% showed complete response,16.7% showed no response,and the overall response rate was 62.5%.Mortality at week 10 was 20.8%.Twenty-two patients who failed to respond to initial therapy were switched to other antifungal drugs(amphotericin B,amphotericin B colloidal dispersion,itraconazole)or other fluconazole containing combined therapy.Eleven out of the 24 patients died during one-year follow-up,8 of whom died of eryptococcal meningitis,and 3 died of other diseases.Conclusions The initial therapy of fluconazole with or without flucytosine is inefficient,and most of the patients need other antifungal drugs because of initial therapy failure.Therefore,fluconazole might not be appropriate for initial therapy in non-HIV-infected cryptococcal meningitis.

ObjectiveTo evaluate the feasibility of determining the chemical composition of kidney stones using gemstone spectral imaging ( GSI ).Methods One hundred and sixty eight extracted human kidney stones immersed in a 10 cm deep water tank underwent CT (Discovery CT750 HD) scans with GSI mode and conventional polychromatic imaging ( CPI,120 kVp) mode.All GSI data were transferred to Workstation AW 4.4 to acquire monochromatic images of 50 keY,effective atomic number (Zeff) mapping images,water (calcium)-based images and calcium (Water)-based images with GSI Viewer.CT numbers of stones were measured and compared at 50 keV monochromatic images and 120 kVp polychromatic images,the mean Zeff,calcium density and water density were measured at Zeff mapping images,Calcium (Water) -based images and Water (Calcium)-based images,respectively.The mean Zeff,spectral HU curve slope and calcium water ratio (CWR) were compared with ANOVA and Wilcoxon test.The composition of kidney stones was determined by infrared spectrometer after CT examination.According to the result of stone composition determined by infrared spectroscopy,108 pure kidney stones were divided into five groups:Uric acid stones ( UA,n = 13 ),struvite stones ( STR,n = 24),cystine stones ( CYS,n = 14),calcium phosphate stones ( CaP,n = 18),and calcium oxalate stones ( COX,n = 39).ResultsThe mean Zeff,CWR,the mean CT numbers at 50 keV images,120 kVp images and spectral HU curve slope of each group were listed as the following:UA [ 7.4 ± 0.4,0.0085 ± 0.0021,( 503 ± 168 ) HU,(495 ± 106 ) HU and - 0.77 ] ; STR [ 11.8 ± 0.9,0.1743 ± 0.0677,( 1056 ± 290 ) HU,( 799 ± 165 ) HU and 18.72 ] ; CYS [ 11.2 ± 0.6,0.1253 ± 0.0297,( 740 ± 172 ) HU,( 565 ± 129 ) HU and 12.79 ] ; CaP [ 16.0 ± 0.4,0.6781 ± 0.0952,( 2567 ±178 ) HU,( 1602 ± 200 ) HU and 37.14 ] ; COX [ 15.4 ± 0.4,0.5683 ± 0.0759,( 2267 ± 385 ) HU,( 1489 ±284) HU and 36.36 ],there were significant differences among groups ( P ＜ 0.01 ).The differences in the mean Zeff,CRW,spectral HU curve slope were statistically significant among the five groups ( P ＜ 0.05 ).Conclusion Spectral CT imaging provides a new method to characterize the kidney stones with the information orovided by mean Zeff,CRW and the CT numbers at 50 keV.

Objective:To understand the pain knowledge, belief and behavior level of cancer pain patients, analyze the influencing factors.Methods:A total of 176 patients with a convenient sampling method selection on November 20, 2021 in Ningxia medical institutions that have created cancer pain in the treatment of cancer pain in Ningxia as the research object of cancer pain in medical institutions, and use the "Cancer Pain Pain Incarring Credit Council Cancer Question Question" that is designed and examined by self-examination.Results:The pain knowledge dimension score of cancer pain patients in Ningxia was 5.00 (4.00, 6.00), the belief dimension score was 3.00 (-3.00, 10.00), and the behavior dimension score was 26.00 (24.00, 28.00). The results of multiple linear regression showed that financial burden and burst pain were the factors affecting the pain belief and perception of cancer pain patients ( t=-2.96, -3.00, both P<0.05). The type of medical insurance and the duration of pain were the factors affecting the behavior of pain ( t=-2.49, 3.21, both P<0.05). Conclusions:Cancer pain patients have a low level of pain knowledge, an upper-middle level of negative pain beliefs and perceptions, and a moderate level of pain behavior. It is recommended that nursing personnel use the discharge plan service as the main line to build a full-process management model of cancer pain. During the hospitalization period, pay attention to the outbreak of pain and psychological conditions, and gradually improve the quality of health education through nursing quality evaluation. After discharge, pay attention to the practicality and long-term development of continuity health education, and promote the full management of patients with cancer pain.

Objective To study the serum laminin γ 2 monomer(LG2m),human circulating Gastrin(hPG80)in patients with hepatocellular carcinoma(HCC)and their clinical value.Methods 128 HCC patients admitted to the First People's Hospital of Shuangliu District from January 2016 to January 2018 were selected as the HCC group,70 patients with benign liver lesions diagnosed and treated at the same time were selected as the benign lesion group,and 70 healthy individuals who underwent physical examinations during the same time were selected as the control group.Use enzyme-linked immunosorbent assay to detect serum LG2m and hPG80 levels.The evaluation value of serum LG2m and hPG80 in predicting the prognosis of HCC patients were analyzed by receiver operating characteristic curve.Kaplan-Meier survival analysis was used to compare the survival prognosis of HCC patients with different serum LG2m and hPG80 levels.COX regression analysis was used to analyze the prognostic factors of HCC patients.Results The levels of serum LG2m(28.14±3.10 ng/L),hPG80(84.83±11.39 ng/L)in the HCC group were higher than those in the benign lesion group(9.18±1.74 ng/L,25.10±4.11 ng/L)and the control group(8.24±1.65 ng/L,23.15±3.26 ng/L),and the differences were statistically significant(t=68.240～76.635,all P＜0.05).The levels of serum LG2m and hPG80 in patients with TNM stage III,low differentiation degree,serum AFP＞400 μg/L were higher than those in TNM stage I to II,high differentiation,AFP≤400 μg/L,the differences were statistically significant(t=3.216～13.552,all P＜0.05).The area under the curve(95%CI)of the combined evaluation of serum LG2m and hPG80 for the prognosis of HCC patients was 0.934(0.889～0.961),which was greater than the single indicator of 0.813(0.774～0.849)and 0.896(0.840～0.937).The overall 5-year survival rates of HCC patients with high and low serum LG2m expression were 33.33%(21/63)and 67.69%(44/65),respectively.Compared with HCC patients with low expression of serum LG2m,HCC patients with high expression of serum LG2m had a lower 5-year cumulative survival rate(Log-Rankχ2=19.522,P＜0.05).The overall 5-year survival rates of HCC patients with high and low serum hPG80 expression were 35.48%(22/62)and 65.15%(43/66),respectively.Compared with HCC patients with low serum hPG80 expression,HCC patients with high serum hPG80 expression had a lower 5-year cumulative survival rate(Log-Rankχ2=12.546,P＜0.05).TNM stage III(OR=1.487,P＜0.05),low differentiation degree(OR=1.611,P＜0.05),serum AFP≥400μg/L(OR=1.416,P＜0.05),high expression of serum LG2m(OR=1.838,P＜0.05),and high expression of hPG80(OR=1.735,P＜0.05)were independent risk factors for poor prognosis in HCC patients.Conclusion The serum levels of LG2m and hPG80 in HCC patients increase,which are related to TNM staging,tumor differentiation,and serum AFP levels.They are serum biomarkers for evaluating the survival and prognosis of HCC patients.
