CONCLUSIONS: Nebivolol does no effect on eNOS activity of left ventricular cadiocytes in subjects or rats without hear failure but it can inhibit eNOS activity of cadiocytes in subjects or rats with heart failure so as to exert its beneficial clinical effect.
3.Preoperative diagnostic value of ~(131)I-SPECT/CT fusion images in detecting the cervical and superior mediastinal metastases of DTC

OBJECTIVE To investigate the diagnostic value of 131I-SPECT/CT fusion images in detecting the metastases of differentiated thyroid carcinoma(DTC)in the regions of the head,neck and superior mediastinum.METHODS 131I-SPECT/CT fusion images was preformed for 30 patients with DTC,and the results of image were compared with the pathology after surgery.RESULTS There were 110 cases positive findings in 115 metastatic lesions with 131I-SPECT/CT fusion images examination(95.65%).5 cases of the metastatic lesions showed negative in 131I-SPECT/CT(4.35%).There was no false positive case.CONCLUSION 131I-SPECT/CT fusion images can detect and locate the metastasis and exclude the false positive results accurately.It is a good method to detect the cervical and superior mediastinal metastatic lesion in differentiated thyroid carcinoma.
4.Efficacy and survival analysis of triple-negative breast cancer treated with neoadjuvant chemotherapy

Objective To evaluate the efficacy and survival rate of neoadjuvant chemotherapy with docetaxe and pirarubicin in triple negative breast cancer (TNBC).Methods Total 51 breast cancer patients were divided into TNBC group (n =26,including 16 of stage Ⅱ and 10 of stage Ⅲ patients) and non-TNBC group (n =25,including 14 of stage Ⅱ and 11 of stage Ⅲ patients).All patients received a median of 4 treatment cycles with TAC regimen [docetaxe 75 mg/m2 on day 1,pirarubicin 40 mg/m2 on day 1 and cyclophosphamide (CTX) 500 mg/m2 on day 1 of each 21 day cycle].The efficacy of treatment and survival rate of two groups were evaluated.Results In TNBC group,9 out of 26 (34.62 %) patients achieved clinical complete response (cCR),and 14 (53.85 %) had partial response (cPR).Overall,88.46 % of TNBC patients had clinical response and 26.92 % (7/26) showed pathology complete response (pCR).In non-TNBC group,6 (24.00 %) patients reached cCR and 8 (32.00 %) showed cPR.The overall response rate was of 56.00 %,and 4 (16.00 %) patients achieved pCR.The overall 3-year survival rates in TNBC and non-TNBC groups were 73.08 % and 88.00 %,respectively,indicating a poorer prognosis of TNBC.The 5-year survival rates of TNBC patients with and without pCR were 88.89 % and 47.06 %,respectively.Conclusion TAC regimen improves the prognosis for locally advanced TNBC,indicating that the neoadjuvant chemotherapy is effective and safe for TNBC patients.
5.A Meta-analysis of lanthanum carbonate for hyperphosphatemia in end-stage renal disease

Objective To assess the efficacy and safety of lanthanum carbonate in treatment of hyperphosphatemia in end-stage renal disease (ESRD).Methods Randomized controlled trails of lanthanum carbonate in treatment of hyperphosphatemia in ESRD patients were searched in the database of MEDLINE,Cochrane Central Register of Controlled Trials,EMBASE,CNKI,Wanfang database.Data extracted from the literatures were analyzed with the Cochrane Collaboration's RevMan 5.1 software.Results Lanthanum carbonate group was similar with calcium carbonate group in treating hyperphosphatemia[RR =1.00,95 % CI (0.92-1.09),P =0.97],and more effective than placebo [RR=4.69,95% CI (2.63-8.39),P< 0.01] (intervention dose≤ 1500 mg) and [RR =18.92,95% CI (7.42-48.22),P < 0.01] (intervention dose > 1500 mg).In comparison with calcium carbonate group,the incidence of hypercalcinemia of lanthanum carbonate group was lower [RR =0.06,95 % CI (0.01-0.72),P =0.03],while the incidence of nausea [RR =1.80,95 % CI(0.70-4.64),P =0.22],vomiting [RR =3.94,95% CI (0.45-34.38),P=0.22] and constipation [RR=0.82,95% CI (0.49-1.37),P=0.45] were similar.The incidence of nausea and vomiting of lanthanum carbonate group were similar with placebo,with lower incidence of constipation [RR =0.19,95 % CI (0.06-0.59),P < 0.01].Conclusions The efficacy of lanthanum carbonate in treating hyperphosphatemia is similar with calcium carbonate.The incidence of hypercalcinemia of lanthanum carbonate is lower than that of calcium carbonate,and the incidence of gastrointestinal adverse effect such as nausea,vomiting and constipation are similar with calcium carbonate.
6.Expression of BTG1 protein in laryngeal squamous cell carcinoma and its clinical significance.

OBJECTIVE:
To investigate the expression of B-cell translocation gene 1 (BTG1) and to determine the relationship between BTG1 expression and clinicopathological features, biological behaviors in laryngeal squamous cell carcinoma.
METHOD:
Immunohistochemistry and Western blot were used to analyze BTG1 protein expression in 70 cases of laryngeal cancer and 35 cases of adjacent corresponding laryngeal mucosal tissues to illuminate the relationship between BTG1 expression and clinical factors.
RESULT:
The positive rate of BTG1 protein expression was 31.43% in laryngeal carcinoma tissues, significantly lower than 91.43% in the adjacent laryngeal tissues (P < 0.05). Western blot showed the relative expression of BTG1 protein between cancer lesion and adjacent tissue were 0.217 ± 0.032 and 0.918 ± 0.081, showing the difference with statistical significance (P < 0.05). The expression of protein was significantly correlated with the tumor invasion, lymph node metastasis, clinic stage and histological grade (P < 0.05 or P < 0.01), but not with sex, age and tumor location (P > 0.05) of patients with laryngeal cancer.
CONCLUSION
The expression of BTG1 protein was decreased in laryngeal squamous cell carcinoma, suggesting that BTG1 gene may be closely associated with the carcinogenesis and the degree of malignancy. Detection of BTG1 expression may be useful in diagnosis, treatment and prognosis of laryngeal carcinoma.

8.Surveillance & management of out-of-plan re-operations

A series of surveillance and management actions was taken to minimize out-of-plan re-operations. These actions include: 1) Building and completing the surveillance and management mechanism against unplanned re-operations; 2) Building a real-time surveillance and reporting mechanism; 3) Enhancing quality control and continuous improvement at the operating departments; 4) Enhancing the supervision on regulations implementation by quality control departments of the hospital; 5) Enhancing quality control for medical records; 6) Building a feedback mechanism for surveillance results; 7) Building a penalty and reward mechanism. Efforts for one year and a half on such surveillance and management proved successful, as the incidence of out-of-plan re-operations fell from 1.7% before these actions to 1.0% afterwards.

Objective To investigate the effect of angiotensin Ⅱ (Ang Ⅱ) type 1 receptor blocker (ARB) on 12-lipoxygenase (12-LO) activity and P-cadherin expression in type 2 diabetic rat glomeruli.Methods Podocytes were stimulated by 107 mol/L Ang Ⅱ for 24 hours.12(S)-HETE (1mg· kg 1 · d-1) and Ang Ⅱ (400 ng· kg-1· min-1) were infused to rats by osmotic mini-pump for 1 week and 2 weeks respectively.Rats fed with high fat diet received low dose streptozotocin (STZ) to make type 2 diabetes and divided into 2 groups:low dose STZ (DN group),low dose STZ + ARB treatment (Losartan group).Rats fed with regular chow were used as control group.All the rats were sacrificed after 6 weeks.Urine,blood,kidney cortical tissue and isolated glomeruli by sieving method were collected at the end of study respectively.ELISA,RT-PCR and Western blotting for related target were performed respectively.Results Ang Ⅱ increased 12(S)-HETE levels in podocytes and glomeruli (all P < 0.01).Ang Ⅱ levels in the glomeruli were significantly increased by 12(S)-HETE stimulation (P <0.01).Blood glucose,kidney/body weight and 24 hour urinary protein were increased in DN group compared with that in control group (all P < 0.01).However,urine protein,Kidney/body weight were decreased in Losartan group compared with DN group (all P < 0.05).Increment of 12(S)-HETE content and decrement of P-cadherin expression were observed in DN glomeruli compared with that in control group(all P < 0.01).These abnormalities were prevented by administration of the losartan (all P < 0.05).Conclusions Ang Ⅱ can down-regulate glomerular P-cadherin expression via activation of 12-LO.ARB can ameliorate the progression of DN via up-regulation of glomerular P-cadherin through inhibition of 12-LO activation in type 2 DN rats.
10. Establishment and analysis of predictive models of postoperative complications in elderly patients undergoing hip arthroplasty

Objective:
To establish a preoperative evaluation model and evaluate the accuracy of postoperative complications by analyzing the risk factors of postoperative complications in hip arthroplasty in elderly patients, and to provide a basis for grading nursing.
Methods:
From January 2012 to August 2018, 148 patients with elderly patients undergoing hip arthroplasty admitted to People′s hospital of Wanning were included in the study. Among them, 23 patients with postoperative complications were included as complications and 125 patients without complications served as a control group. Univariate analysis of the factors associated with complications, statistically significant indicators were included in the multivariate logistic regression model, and the risk factors for postoperative complications in hip arthroplasty were analyzed. Based on the results of the regression analysis, a preoperative evaluation model was established. The ROC curve and the Hosmer-Lemeshow test were used to evaluate the model prediction performance.
