1.Progress of chitin nerve conduit in peripheral nerve repair

OBJECTIVE: To summarize the progress on the biomaterials of chitin nerve conduit to repair peripheral nerve injury in recent years.DATA SOURCES: A computer-based online search of literatures related to the application of the biomaterials of nerve conduit on peripheral nerve injury was performed in Medline and Springerlink by using the key words of "nerve conduit, peripheral nerve injury" between January 2000 and August 2009 and the language was limited to English. Meanwhile, we searched China Periodicals Full-text Database and Weipu database for the related Chinese articles published between January 2000 and August 2009 with the same key words in Chinese.STUDY SELECTION: Inclusive criteria: Researches about the non-biodegradable materials, biodegradable materials and biological derivative materials that were related to the peripheral nerve injury were included. Exclusive criteria: The irrelevant and repeated articles were excluded.MAIN OUTCOME MEASURES: Nerve tissue engineering, chitin nerve conduit, and preparation of nerve conduit.RESULTS: With the progress of medical materials, the neural tube of natural or synthetic materials used to bridge defect of the neural in nerve tissue engineering scaffold material with a guide and to promote nerve regeneration. In the biodegradable material, chitin neural tube could be degraded within a reasonable period of time, biological control compatibility, performance degradation, mechanical properties and porosity. And in the structure of the conduit, combined other biodegradable materials,surface modification, adding seed cells and nerve growth factor, and other aspects of the experimental study were investigated.Changing the spatial structure of renewable and micro-environment aimed to speed up the rate of nerve growth, and promote the resumption of the good nerve function. And material's surface had to be modified and improved for conduit to adapt nerve during the nerve regeneration.CONCLUSION: With the continued development of biotechnique and cross-correlation technique, the biomaterials of chitin nerve conduit will have a prosperous future on peripheral nerve tissue engineering.
2.Expression and significances of β-catenin in Barrett esophagus and esophageal adenocarcinoma

Objective To study the significance of the expression of β-catenin(β-cat) in Barrett esophagus and esophageal adenocarcinoma (EAC) and the relationship between its expression and clinicopathological factom.Methods By using SP immunohistochemistry technique,β-cat wag detected in the normal esophageal mucosa group (20 cases),Barrett esophagus with dysplasia group (26 cages) and EAC group (50 cases).Results The membranous expression of β-cat appeared in all of nomal esophageal mucosa.The rate of cytoplasmic and/or nuclear expression of β-cat in the Barrett esophagus with dysplasia group and the EAC group was 38.5% (10/26) and 60.0% (30/50) respectively,and compared with the normal esophageal mucosa group,the difference was signfficant (P<0.01).The rate of cytoplasmic and/or nuclear expression of β-cat in the EAC group with lymphatic metastasis and the EAC group exceeding deep smooth muscle was 90.0% (18/20) and 72.2%(26/36) respectively,and compared with that in the nonlymphatic metastasis and the non-exceeding deep smooth muscle patients [43-3%(13/30) and 28.6% (4/14)],the difference was significant (P<0.01).Conclusion Cytoplasmic and/or nuclear expression of β-cat might play a role in the carcinogenesis and progression of EAC and might be a predictor indicating the metastatic potential of EAC.
3.EFFECTS OF REINFORCED DECOCTION OF ANGELICAE SINENSIS FOR ENRICHING BLOOD ON THE IMMUNITY OF IMMUNOSUPPRESSED MICE

Objective To investigate the effect of reinforced Decoction of Angelicae Sinensis for enriching blood (RDAEB) on the immunity of immunosuppressed mice induced by cyclophosphamide (Cy). Methods Mice were given RDAEB through stomach perfusion for 10 d(50 mg/d). Then, RBC-C3bR rate,RBC-IC rate( as the indexes of erythrocyte immunity)and E-rosette forming rate,acidic α-naphthyl acetate esterase positive rate, lymphocyte transformation rate (as the indexes of cellular immunity) of mice were tested.Results RBC-C3Br rate, RBC-IC rate,E-rosette forming rate, acidic α-naphthyl acetate esterase positive rate and lymphocyte transformation rate in the Cy-RDAEB group were markedly higher than those in the Cy group (P<0. 01 ),and returned to the levels of normal group. Conclusion RDAEB is effective in recovering and enhancing cellular and erythrocyte immunity of immunosuppressed mice.
4.Correlation between aberrant methylation status of ras association domain family 1A and alteration of DNA methyltransferases, hepatitis B virus infection in hepatocellular carcinoma

Objective To investigate the correlation between aberrant methylation status of ras association domain family 1A (RASSF1A) , transcriptional levels of DNA methyltransferases (DNMTs) and hepatitis B virus ( HBV) infection in patients with hepatocellular carcinoma ( HCC). Methods HCC samples in 61 cases were collected. Aberrant methylation status of RASSF1A was detected using methylation specific PCR (MSP). Transcriptional levels of DNMT1, DNMT3 A and DNMT3B was measured using semi-quantitative reverse transcriptase-PCR(RT-PCR). Results RASSF1A gene with abnormal methylation in initiation zone was found in 45 cases with HCC among 61 patients (73.8%). Further analysis revealed RASSF1A methylated in 32 of 47 HBV infected cases and in 13 of 14 uninfected cases. However, there was no significant association between methylation status of RASSF1A and HBV infection (x2 =2.260, P = 0. 133). Compared with normal control, DNMTs was up-regulated in all HCC samples, HCC cell lines and HBV infected group. 5.Temporal rotation skin flap combined with cartilage plasty for correcting cryptotia.

OBJECTIVETo explore the therapeutic effect of the temporal rotation flap combined with cartilage plasty for cryptotia correction.
METHODSFrom January 2009 to June 2012, 8 cases with cryptotia (10 ears) were treated. After complete dissection of the cartilage adhesion, the cartilage was reshaped by suture to restore its appearance. Then the temporal triangular rotation flap was transferred to cover the wound.
RESULTSNo hematoma, infection or flap necrosis happened. The follow-up period ranged from 3 months to 1 year, with an average of (6.88 +/- 2.85) months. The ear appearance kept stable with no recurrence and inconspicuous scar. Satisfactory result was achieved.
CONCLUSIONTemporal rotation flap combined with cartilage plasty is a good option for correction of mild or moderate cryptotia.

6.MRI features and pathological analysis of intracranial solitary fibrous tumor

Objective To analyze MRI findings and pathological data of intracranial solitary fibroma tumor(SFT)to improve the understanding of this disease and the level of imaging diagnosis.Methods MRI images and clinical pathological data of 6 patients with intracranial SFT confirmed by surgery and pathology were analyzed retrospectively.Results All tumors were located in the supratentorium,in which 4 cases were irregularly lobulated,2 cases were oval.All lesions were clear boundaries,and size 3.0-8.6 cm (an average of 6.3 cm).The tumor was located in the right fronto-perietral area in 1 case,cross of the frontal midline in 2 cases,cross of the occipital midline in 2 cases and cross of the cerebellum in 1 case.On T1WI,6 cases of tumor parenchyma showed slight low signal.On T2WI,4 cases of tumors showed mixed signal with the"Yin and Yang"sign,and peritumoral edema was showed in 4 cases, tortuous flow void blood vessels were showed in 5 cases.On FLAIR,mixed signal was showed.On DWI,slightly higher signal was showed.On MRI enhanced study,tumor parenchyma was significantly increased,in which 3 cases of T2WI low signal areas were significantly enhanced,cystic necrosis and edema were not enhanced,and 3 cases showed"dural tail sign".On pathological study, tumor cells were spindle-shaped,bundle and braided distribution,rich in collagen fibers.Immunohistochemistry showed CD34(+), CD99(+),Vim(+).1 case was malignant,and 1 case was potentially malignant.Conclusion MRI features of intracranial SFT have certain characteristics,it is difficult to be characterized qualitatively.MRI has obvious advantages in the relationship of tumor location, shape,range and its relationship with surrounding tissue.It has important clinical application value for the clinical operation plan and the residual or recurrence of tumor after operation.
7.Effect of cristata L flavonoid on expression of bone morphogenetic protein and function of tubular reabsorption of rats with diabetes mellitus

BACKGROUND: Cristata L flavonoid is a kind of plant estrin, which possesses multiple physiological function, has no toxicity and adverse effect, and is effective in treating and preventing osteoporosis.OBJECTIVE: To study the effect of cristata L flavonoid on bone morphogenetic protein, urine inorganic salt and content of lysozyme of rats with diabetes mellitus (DM).DESIGN: Randomized grouping design and controlled study.SETTING: Pharmaceutical Laboratory of Xinxiang Medical College.MATERIALS: The experiment was completed in the Xinxiang Medical College from September to December 2003. Totally 24 health male SD rats were randomly divided into three groups: normal control group, diabetes mellitus (DM) group, DM + cristata L flavonoid group with 8 in each group.were injected intraperineally with 60 mg/kg streptozotocin, and 48-72 hours later, blood of rear caudal vein was collected to measure total blood glucose.Rats were determined as DM if the blood glucose was ≥ 16.7 mmol/L; oth erwise, 60 mg/kg streptozotocin was injected once more. After modeling,cristata L flavonoid and rats in normal control group were given the same was measured with atomic absorbency method and content of urine sodium histochemistry of bone morphogenic protein-2 (BMP2) in bone was detected with streptavidin tagged by peroxidase and immunohistochemic expression lysozyme reflected reabsorption function of renal tubule was measured with with t-test. MAIN OUTCOME MEASURES: Comparison between content of calcium, sodium, kalium in urine and lysozyme and immunohistochemic expression of BMP2 10-week intervention later.calcium and sodium in urine in DM model group were obviously higher than those in normal control group, but content of kalium in urine was obviously lower (P < 0.05-0.01). Contents of calcium in urine in DM +cristata L flavonoid group were obvioualy lower than those in DM model group and normal control group were obviously higher than those in DM model group, and content of lysozyme in urine in those groups was obviously lower than that in DM model group (P < 0.05-0.01).CONCLUSION: Expression of BMP2 of DM rats is decreased, but after supplement of cristata L flavonoid compound, the expression is increased.In addition, output of urine calcium and sodium in DM rats is decreased,and reabsorption function of renal tubule is increased.
8.Co-application of muscle-derived stem cells and FK-506 influences nerve regeneration and recovery following acellular nerve allograft implantation

BACKGROUND: Muscle-derived stem cells (MDSCs) have been accepted as seeding cells in tissue engineered artificial nerves. Tacrolimus exhibits anti-immunologic rejection and promotes nerve regeneration and recovery. Whether can combine these factors with acellular nerve to form a new bridge that can inhibit immunologic rejection and promote nerve regeneration is uncertain. OBJECTIVE: Using the freeze-thawing combined optimized acellular nerve hypotonic-chemical detergent to prepare acellular nerveallograft scaffold. To explore the effect of co-application of MDSCs and FK-506 on nerve regeneration and function recovery following acellular nerve ailograft implantation.METHODS: The sciatic nerve derived from SD rats was prepared nerve bridge after acallular disposal. Gel containing FK-506 and MDSCs was injected into acellular nerveallograft scaffold with 100 μL microsyringe to repair defects. A total of 32 SD rats were randomly divided into 4 groups, with 8 animals in each group. Agap of 10 mm of left sciatic nerve was removed. And then the defects were repaired by extracted nerve graft containing FK-506+MDSCs, MDSCs and FK-506, respectively. In the control group, only hyaluronic acid gel was injected. Sciatic nerve function index (SFI) and electrophysiological exam were performed at weeks 8 and 12 after operation. Gross observation, neurohistological and ultrastructure were observed at week 12 after operation.RESULTS AND CONCLUSION: Compared with the same time point, the SFI, recovery rate of the motor nerve conduction velocities (MNCV), and myelinated nerve fibers in grafting part and in its distal part in the FK-506+MDSCs group were superior to other groups (P < 0.05). The nerve grafts were in normal size with considerabie blood vessels and slightly connected to peripheral Ussues. Compared to other groups, the regenerated nerve fiber in the FK-506+MDSCs group was more density with well-arranged order. A great quantity of Schwann cells proliferated in grafting. The density end diameter of myelinated fiber in the middle and distal part of the grafting were all greater than that of MDSCs and FK-506 groups, and there were few connective tissues between microfascicles, which was close to normal level. The co-application of MDSCs and FK-506 enhances peripheral nerve regeneration and functional recovery in acelluler nerve allograft graft. Therefore, MDSCs and FK-506 are synergistic factors in peripheral nerve injury repair.
9.Combined laparoscopic and thoracoscopic esophagectomy for esophageal carcinoma and gastro-esophageal anastomosis in right thoracic cavity: analysis of 38 cases

Objective To investigate the feasibility of combined laparoscopic and thoracoscopic esophagectomy for esophageal carcinoma and gastro-esophageal anastomosis in right thoracic cavity.Methods We retrospectively analyzed the clinical data of 38 patients who underwent esophagectomy for esophageal carcinoma and gastro-esophageal anastomosis in right thoracic cavity from October 2011 to August 2012.To remove the stomach in laparoscopic and the esophagus in thoracoscopy.The main portion of a gastric conduit is created using three to four firings of a linear stapler(Ethicon Endo-surgery,Cincinati,OH) and jejunum stoma.Gastric conduit was pulled into the chest cavity and anastomosed to the esophagus.Results The average operative time was 280 minutes,the mean operative blood loss was 120 ml.No patient required laparotomy.No pulmonary complications or anastomotic leaks occurred.One had gastric retention,another one had chylous hydrothorax.All patients were cured,no one dead in hospital.Conclusion Combined laparoscopic and thoracoscopic esophagectomy for esophageal carcinoma and gastro-esophageal anastomosis in right thoracic cavity is technically feasible and safe,minimized trauma,less operative blood loss and quick recovery.
10.Auricular reconstruction with autologous costal cartilage grafts and delaying postauricular skin flap

Objective To summarize and explore the clinical effect of auricular reconstruction with autologous costal cartilage grafts and delaying postauricular skin flap.Methods The procedure involved three stages.The first stage was the delay procedure of postauricular skin flap.The second stage was the auricular reconstruction with autologous costal cartilage grafts and delaying postauricular skin flap about 3 weeks after the delay procedure.The third stage involved construction of tragus and conchal cavity and other local correction 6 months or longer after the second stage surgery.The clinical effect,patients' satisfaction and complication were observed.Results This method was used in 28 patients,31 ears of congenital microtia.23 cases completed auricular reconstruction with three stages,the other 8 cases completed only two stages.All auricular reconstruction were successful.The location,size and shape of reconstructive ears were good.The complications were few.The patients or their relatives were basically satisfactory.Conclusions Auricular reconstruction with autologous costal cartilage grafts and delaying postauricular skin flap is a safe and effective method.
