1.Exploration for the rela tionship of glutamate and nitric oxide with hypoxic-ischemia encephalopathy in ne wborn infants

To explore the role of cerebro-spinal flu id(CSF)glutamate and nitric oxide (NO) in the pathogenesis of neonatal hypoxic- ischemia encephalopathy (HIE), glutamate and NO in CSF were determined in 24 new born infants with HIE and 8 normal control infants, respectively. The results sh owed that the levels of glutamate and NO in CSF were significantly higher in the neonates with moderate and severe HIE than those in both of the control group an d neonates with mild HIE.There was an obviously positive relationship between le v els of glutamate and NO. It is concluded that glutamate and NO play an important role in the pathogenesis of neonatal HIE.
2.Comparison between radial head resection and open reduction and internal fixation in clinical treatment of Mason type- Ⅲ comminuted fractures of radial head

Objective To compare the clinical results of radial head resection with those of open reduction and internal fixation in treatment of Mason type- Ⅲ comminuted fracture of radial head. Methods 58 cases of Mason type- Ⅲ radial head fracture were treated in our department from January, 1983 to July, 2005. 28 cases had resection of the fractured redial head, while the other 30 cases had open reduction and internal fixation. The outcomes were assessed on the basis of pain, motion, strength, radiographic findings, and elbow functional rating score. Results The resection group had a mean follow- up of 5.5 years (2 to 6), while the internal fixation group had 10 years (3 to 13). The average pain VAS (visual analog scale) score was 18.7 (11 to 25) for the resection group and 23.5 (17 to 25) for the fixation group with a P value of 0.0023. 3.Treatment of acute osteochondral fractures of the talus using autologous osteochondral transplantation

[Objective]To study the clinical results of treatment of local osteochondral fracture lesions on the talus dome associated with ankle fracture using autologous osteochondral transplantation.[Method]There were 18 patients with osteochondral fracture defects of the talus dome associated with ankle fracture, male 10, female 8, with the average age 33.5 years (18~49 years). Transplantation procedure was osteochondral cylinders from non-weight bearing surface of the ipsilateral knee and implant the donor graft into the recipient holes of talus cartilaginous defects with press-fit technique. The outcomes were assessed on the basis of ankle functional rating score, radiographic findings and MRI findings.[Result]The patients were followed up for 17 to 40 months (mean, 28 months). The ankle functional rating score was measured by the Baird and Jackson functional rating score system. The averaged scores was 96.3?3.5,excellent in 14 cases, good in 3 cases and fair in 1 case. The rate of being excellent and good was 94.4%.The rate of posttraumatic arthritis was 5.6% (1/18) and osteochondritis dissecans was 5.6% (1/18).[Conclusion]Autogenous osteochondral transplantation using ipsilateral knee osteochondral grafts is an effective surgical procedure on treating acute local osteochondral fracture lesions of the talus associated with ankle fracture.
4.Investigation of resection and reconstruction procedure of high-sacrum tumors

Objective To investigate the way of resection of high-sacrum tumors and the reconstruction way of the sacrum. Methods From October 2001 to October 2005,7 patients with high-sacrum tumors were enrolled. After resection, the pelviclring were recormtmcted with Chinese Great Wall pedicle screw system and fibulae graft,corresponding chemotherapy and radiotherapy were given after operation. Results The short-term results were satisfactory with the lumbosacral pain reduced and the neurological function improved in different degrees, however dysuria occurred in 1 ease and 1 case cerebrospinal fluid leakageand 1 case postoperative infection and delayedunion among the 7 eases in this group. In the follow-up period of 6 months to 3 years,4 eases died for tumor recurred or metastasis. Conclusions Surgical procedure,reconstruction of the sacrum and postoperative comprehensive treatment have important effects on the prognosis. Meanwhile,it is operative key to lessen operative hemorrhage,reserve the function of caudal equine and rebuild weight high post function of the pelvis after superior sacrum tumor is removed.
5.Factors Influencing of Outcome in Cauda Equina Syndrome

Objective: We examined the cause、level of pathology、onset of symptoms、time taken to treatment and outcome of 14 patients with cauda equina syndrome(CES) to determine how these factors influenced the eventual outcome.Methods:14 patients with CES treated at our hospital between 1991 and 1997 underwent laminectomy followed by bilateral decompression.The mean time of followup was 35 months(range 22~48months).Logistical regression analysis was used to manage the data.Results:Out of 14 patients,11 had satisfactory recovery at 2 years postdecompression;3 patients were left with some residual dysfunction.Conclusion:There was a clear correlation between delayed decompression and the poor overall outcome,as well as the presence of perineal anaesthesia and significant sphicter dysfuncation.
6.Non-operative and operative treatments of senile comminuted distal radial fractures:a comparative research of their clinical results

Objective To compare the clinical results of operation and manual reduction and plaster immobilization in management of senile comminuted distal radial fractures.Methods Retrospective analysis of the clinical data was done for the 178 senile patients with comminuted distal radial fracture who were treated from January 2000 to September 2004 in our department.102 cases were treated with manual reduction and plaster immobilization,46 cases with external fixator and limited internal fixation,and 30 cases with T-plate internal fixation.Results Follow-up duration averaged 27 months(13 to 52 months).According to Dienst criteria,the excellent rate of functional outcome was 93.5%for external fixator combined with limited internal fixation,78.4%for manual reduction and plaster immobilization and 80.0%for T-plate internal fixation.The differences were statistically significant(P
7.Myogenetic potential of rabbit adipose-derived stromal cells

BACKGROUND: Myogenetic seed cells are the fundamental for construction of tissue-engineered smooth muscle, and optimizing the amplification of seed cells is the key in further clinical application. OBJECTIVE: To analyze the myogenetic potential of rabbit adipose-derived stromal cells induced with the combination of MyoD, transforming growth factor β1 and 5-azacytidine, and to investigate a new way to induce cells. METHODS: The rabbit abdominal fats were isolated and then the adipose-derived stromal cells were separated by col agenase digestion method for myogenetic induction with different methods: 5-azacytidine induction group, MyoD+transforming growth factor β1 induction group, 5-azacytidine+MyoD+transforming growth factor β1 combination induction group. The blank control group was set. The morphological characteristics of cells were observed at 1, 4, 8, 12, 16, 20, 24 and 28 days after induction, and the col agen type Ⅲ level were detected with 4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide assay, flow cytometry and immunohistochemistry. RESULTS AND CONCLUSION: Compared with the other groups, the cel activity was higher in the combination induction group and reached peak at 16 days after induction, the number and volume of myotubes were increased at 20 days with regular arrangement, and desmin showed strong expression. Cel cycle showed the ratio of adipose-derived stromal cells in the DNA replication phase was increased in the combination induction group, the ratio of cells in the clearance period was decreased; the level of col agen type Ⅲ was increased significantly, and the difference was significant. The results indicate that 5-azacytidine combined with multiple factors can promote the differentiation of adipose-derived stromal cells into myoblasts with significant cel proliferation, which is considered as the ideal method to in vitro induction of myoblasts.
8.The establishment and application of double TaqMan real-time fluorescence nested PCR method for rapid detection of α-thalassemia SEA deletion

Objective To establish a double TaqMan real-time fluorescence nested PCR method for rapid detection of α-thalassemia SEA deletion.Methods One hundred blood samples for thalassemia screening were collected from May to July of 2010 in the Tianhe Maternal and Child Health Hospital of Guangzhou.Seven fetal specimens for prenatal diagnosis were collected from December 2010 to February 2011 in Dongguan TungWah Hospital(2 villi and 5 amniotic fluid specimens).Fifty samples of α-thalassemia SEA deletion with genotyping results were selected from the sample bank of our laboratory.The double TaqMan real-time fluorescence nested PCR was applied to detect the truncated sequence of SEA deletion and the normal sequence within deletion range simultaneously for all these samples with the same detecting system.The genotype of α-thalassemia SEA deletion was accurately acquired according to the positive result of fluorescent PCR combined with the Ct value difference.Meanwhile,the accuracy and feasibility of this method were verified and analyzed by parallely detecting these samples with routine gapPCR for α-thalassemia SEA deletion.The genotype could be obtained according to PCR amplification and agarose gel electrophoresis.Results Two amplification efficiencies of the optimized dual TaqMan real-time fluorescence nested PCR system established in this study were both close to 100% with the slops of -3.153 and -3.182,respectively.The results of 50 samples of α-thalassemia SEA deletion with genotyping results showed that this method could not only realize rapid diagnosis,but also effectively avoid false negative or false-positive misdiagnosis by accurately determine the external contamination in the sample.Among 100 blood samples,eleven samples with SEA deletion were detected respectively and the diagnosis coincidence rate of these two methods was 100%,3 samples with SEA deletion were detected by gap-PCR,but 2 samples with SEA deletion and 1 villi sample with normal genotype but contaminated by SEA were detected by this method among 7 fetal samples.Conclusions A double TaqMan real-time fluorescence nested PCR method for α-thalassemia SEA deletion was developed.The method is a rapid and reliable test with simple operation,and is suitable for large-scale population screening and routine molecular diagnosis.
9.Carboxy-methyl-chitosan inhibits interleukin-1β-induced NF-kappa B activation in rat chondrocytes

Objective To investigate the efffect of carboxy-methyl-chitosan (CM-chitosan) on NF-kB activation and iNOS expression in IL-1β-induced rat chondrocytes, and the mechanism is explored. The statistical treatment was ANOVA. Methods The chondrocytes derived from knee joint of rats were cultured in vitro. 10 ng/ml IL-1β with or without CM-chitosan of varied concentrations (100, 200, 300 μg/ml) was added into the culture medium. Inducible NO synthase (iNOS) was examined by semi-quantitative reverse transcription-polymerase chain reaction (RT-PCR) and Western blotting. The expression of NF-κBn and NFκBc as well as I-κB were examined by Western blotting. Results CM-chitosan could significantly decrease iNOS expression of IL-1β-induced chondrocytes. Degradation of I-κB and NF-κB nuclear translocation were reversed by carbox-ymethyl-chitosan. Conclusion The study has demonstrated that CM-chitosan can inhibit the synthesis of inflammatory mediators in rat chondrocytes when stimulated with IL-1β through a NF-κB-dependent mechanism.
10.Study on status and influencing factors of psychological capital of nursing administrative personnel

Objective To investigate the level of psychological capital of nursing administrative personnel and analyze its influencing factors.Methods A total of 302 nursing administrative personnel of Anhui Province were investigated with Chinese Version of Nurses Psychological Capital Questionnaire (PCQ-24) and a self-designed question-naire about demographic information.Results The overall score of psychological capital was (106.23±10.67) points,and the dimension scores from high to low were selfefficacy (26.79±3.40) points,resilience (26.60±3.25) points,hope(26.56±3.55) points,and optimism(26.27±3.34) points.Multiple regression analysis showed that health?level and positions entered the regression equation,t=2.904,1.838,P<0.05.Conclusions The overall nursing administrative personnel psychological capital is in the middle level,health level and positions were influencing factors of nurses psychological capital.It is suggested that the psychological capital should be introduced into nursing administrative field and take diversified measured to improve their psychological capital.
